CVS CaremarkTM is the prescription provider for the Medial Mutual Preferred Choice PPO plan. You will receive a separate CVS Caremark ID card when enrolling for the first time. The program provides a retail card for purchasing acute, immediate prescriptions and a mandatory mail-order program for long-term prescriptions. After receiving two maintenance medication fills at a retail pharmacy for a 30-day supply, the next refill will need to be at the 90-day supply, which can be mailed to your home or picked up at CVS® or Target® pharmacies.
The table below outlines your copays for prescription drugs under all three tiers, using in-network retail pharmacies or mail-order fulfillment.
