OFFICIAL RULES Important: Please read these rules before entering these skill-based contests (herein referred to as “contests”).

By participating in this contest, you agree to be bound by these Official Rules and represent that you satisfy all of the eligibility requirements below.

Places, Dates and Sponsor LifeWave, Inc. (the “Sponsoring Company”) will hold a monthly evaluation of testimonials submitted by Brand Partners and award prizes to the contest winners.

Eligibility These contests are open to all LifeWave Brand Partners based in USA only, who are active and in good standing (as defined in the Policies and Procedures) and are at least age 18 (or the age of majority under applicable law) at the time of the contest.

If you do not meet any of these requirements, or any other eligibility requirements in these Official Rules, you are not eligible to win a prize.

Terms The contests will take place monthly or as Sponsoring Company otherwise chooses.

How to Enter Every Brand Partner entering a product testimonial through Testimonial Form link is automatically entered into the Contest.

Winner Selection A panel of judges consisting of employees of the Sponsoring Company shall select the winners.

The criteria are a qualitative evaluation of the effectiveness of the testimonial, and compliance with LifeWave’s published Advertising Guidelines for product claims. The decisions of the panel of judges are final. Three winners shall be selected.

Compliance by Winners By accepting the prize, the winner confirms that they comply with the rules of eligibility and the liability and publicity terms below. In the event of non-compliance with these Official Rules, or if the prize is unclaimed or undeliverable, the prize will be forfeited and awarded to an alternate winner determined by Sponsoring Company.

Prizes The prizes are as follows: Prizes will be delivered upon verification of each winner within 4 weeks of the date the prize winners are announced. Each eligible participant may win no more than one (1) prize per contest.

ALL TAXES ASSESSED AGAINST THE WINNER FOR THE PRIZE ARE THE SOLE RESPONSIBILITY OF THE WINNER. Winners and/or the Sponsoring Company may be required to report the prize value to the tax authority. If a prize is not claimed within ten (10) days after delivery of notification, the prize will be forfeited and awarded to an alternate winner selected by random drawing. Prizes are not transferable, are not eligible for refund, and may not be used with any other promotional discount. No substitutions or exchanges (including for cash) of the prize will be permitted, except that the Sponsoring Company reserves the right to substitute a prize of comparable or greater value for the prize. Each prize is awarded "AS IS" and WITHOUT WARRANTY OF ANY KIND, express or implied, (including, without limitation, any implied warranty of merchantability or fitness for a particular purpose).

Odds of Winning Actual odds of winning each contest depend on the total number of entries received for the contest.

Use of Winner’s Name, Likeness, etc.

Except where prohibited by law, entry into the contest constitutes permission to use winner's name, Facebook and other social media names, likeness, persona, hometown, and/or prize information in all media for up to 5 years for all purposes the Sponsoring Company deems appropriate — including, without limitation, for promotional and publicity purposes — without further permission or compensation.

General Release By entering the contest, the participant releases the Sponsoring Company and its parent, subsidiary, and affiliated LifeWave companies, and their respective officers, directors, employees and agents (the “Released Parties”) from any liability whatsoever, and waive any and all causes of action for any claims, costs, injuries, losses or damages of any kind arising out of or in connection with the contest or delivery, mis-delivery, acceptance, possession, or use of or inability to use any prize (including, without limitation, claims, costs, injuries, losses and damages related to personal injuries, death, damage to or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light, whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory.

Affidavit and Release As a condition of being awarded any prize, winner may (in the Sponsoring Company’s sole discretion) be required to execute and deliver to the Sponsoring Company a signed affidavit of eligibility, acceptance of these Official Rules, release of liability, and any other legal, regulatory or tax-related documents required by the Sponsoring Company in its sole discretion.

Right to Cancel or Modify the Random Drawing Each Sponsoring Company reserves the right to cancel or modify the contest if fraud or technical problems question the integrity of the contest or otherwise it chooses to do so, in its sole discretion.

Disclaimer All applicable national, state and local laws and regulations of the Sponsoring Company apply.

List of Winners Winners will be notified by email at the address on file. A list of the winners’ names will be added to the Back Office under the Testimonials section. BO>Resources>Testimonial section Miscellaneous These Official Rules and the contest are governed by the laws of the jurisdiction of the Sponsoring Company. Entrants agree to be bound by these Official Rules and by the decisions of the Sponsoring Company, which are final and binding in all respects. The Sponsoring Company reserves the right to change these Official Rules at any time, in its sole discretion, and to suspend or cancel the contest or any entrant's participation in the contest should viruses, bugs, unauthorized human intervention, or other causes beyond its control affect the administration, security or proper play of the contest. Notwithstanding the foregoing, the Sponsoring Company reserves the right to amend, modify or cancel the contest at any time without notice. Entrants who violate these Official Rules; violate any law, rule or regulation in connection with participation in the contest; tamper with the operation of the contest; or engage in any conduct that is detrimental or unfair to Jeunesse, the contest, or any other entrant (in each case as determined in the Sponsoring Company’s sole discretion) are subject to disqualification from entry into the contest. Each Sponsoring Company reserves the right to lock out persons whose eligibility is in question or who have been disqualified or are otherwise ineligible to enter the contest. Any provision of these Official Rules deemed unenforceable will be enforced to the extent permissible, and the remainder of these Official Rules will remain in effect.
