Hospital Discharge

Unsafe at Home

After a fall or other health crisis, a senior is discharged from the hospital to skilled nursing or undergoes therapies to return to their living situation and lifestyle. Sometimes the normal routine and living situation must change to accommodate recovery time and the long-term health needs. See the services that are in the Aging in Place category.

Most older adults desire to remain in their home for as long as possible. At some point the individual needs services to help them remain safely at home. This may include adult day services, nonmedical home care, home health care, and home modifications like grab bars and stair lifts. See the services that are in the Aging in Place category.

The rate of Alzheimer’s disease and other forms of dementia is rising. Those with memory issues have specialized needs, and depending on the level of cognitive impairment, there are numerous products and services to enhance quality of life, mitigate the effects of dementia, and keep the individual safe. Below are suggestions for mapping of care. Please note that the below information is intended for educational purposes only, and decisions should be made only after discussing your situation with a qualified professional. Have Questions? If you do not need care in the imminent future it is still wise to have a plan in place should you need additional care. It is recommended that you continue to stay active, engage in social activities and have your financial and legal matters in order. Be sure to discuss your end-of-life wishes with your family. Having a well defined plan of care will ease the burden on your family.

Having a plan of care, working with a doctor and forming a strategy to address the memory deterioration may slow the advancement of Alzheimer’s and dementia. Sometimes individuals are able to remain home safely with the help of reliable family members and other caregivers. This includes having a caregiver assisting with home help, companionship and personal care tasks. It could also include attending an adult day care center or an all-inclusive care program. For individuals who value social interaction, it is important to continue attending family, volunteer, church, and other events as you are able. Family caregivers should also seek caregiver support groups.

Individuals with sharp minds but needing some assistance around the house benefit from home care. They can receive weekly hours of service proportional to their needs and direct the caregiver to help where necessary, including transportation to doctors or other appointments, running errands, personal care, light housekeeping and more. These individuals may also consider adult day programs that provide transportation to and from the adult day centers. All-inclusive care is also an option. Family caregivers also benefit from the assistance by receiving respite and personal time.

Those looking to relocate may benefit from the wellrounded offerings of senior and independent living facilities and personal care homes / assisted living facilities. These communities are age-friendly, accessible and offer options for group outings and other activities. Those who are mobile may prefer a senior or independent living community since they are not in need of the support services offered by the personal care homes / assisted living facilities. For those who need assistance with activities of daily living, a personal care home / assisted living facility would provide the services necessary to fit their needs.

Individuals in the early stages of Alzheimer’s or dementia will want to consider senior / independent living as long as they are accompanied by a spouse or family caregiver, or supported with caregiving services. Another option is a personal care or assisted living facility. Those in more advanced stages of Alzheimer’s or dementia should seek information on personal care or assisted living facilities that have services on-site, including secured dementia units and trained staff.

Financial Matters

One aspect that each individual will need to assess in all care outcomes is the cost of each level of care. One must consider their financial resources and work with a qualified professional to understand which options they can afford. In some cases, the individual will pay from their own resources because government programs, insurances, and other outside funding programs may not suffice.

Adult Day Centers provide a coordinated program of professional and compassionate services for adults in a community-based group setting. Services are designed to provide social and some health services for adults who need supervised care in a safe place outside the home during the day. They also afford caregivers respite from the demanding responsibilities of caregiving. Adult day care facilities for the elderly are licensed and inspected by the Department of Aging. Costs for Adult Day Centers range from $60/day to $100+/day on average in the Lehigh Valley. The costs are paid for with private funds or may be partially or fully covered by long-term care insurance, Veterans Assistance, or Medicaid through the county’s Aging Waiver Program.

Preventative health care not only improves your loved one’s health but also their quality of life. Vaccinations reduce your risk of getting lifethreatening illnesses like the flu and COVID-19. Screenings identify diseases in their early, most treatable stages. Chances are your father and mother might benefit from your help in managing preventive health care, he says. Juggling multiple providers and appointments can be confusing for anyone but even more so for many older adults. You many need to remind them to schedule needed health care, such as their annual physical. Some older adults also need assistance making calls or getting to the doctor’s office. Preventive care can have significant benefits,” he adds. “For example,

routine blood work can identify a health condition that can be treated easily. By identifying and treating the problem, your loved one will likely feel better. In addition, we can often reverse or slow the progression of the disease,” he adds. Now, with COVID-19 infections again on the rise, make sure they have received the COVID vaccine and booster. Encourage them to social distance and mask if they must go out and avoid large, in-door gatherings when possible. Keeping up to date on vaccines and boosters will reduce their risk and lower the severity of illness should they have a breakthrough case. When discussing your parent’s health, Dr. Mira suggests approaching the topic delicately. Offer your help while allowing your mother and father to make decisions as they are able. Have a list of screenings and vaccinations in hand so you can discuss them together. Finally, if unsure what preventive health care your loved one has received, see if you can join in a conversation with your parent and the

All adults should consider getting an annual flu shot, but as you grow older, your risk of having serious complications from the flu increase, particularly if you have poor health. In fact, about 85% of people who die from influenza are 65 and older. 2. High blood pressure screening. All older adults should have regular blood pressure screenings. The prevalence of high blood pressure increases with age and is a leading indicator of diabetes and heart disease. 3. Cholesterol screening: Men age 35 and women age 45 who are at risk for coronary heart disease should be tested. Risk factors include high blood pressure, being overweight or obese and having personal or a family history of heart disease. 4. Diabetes screening. Nearly one in
