Dear Friends of LifeStream,

As we reflect on the past year, we are grateful for the continued support that allows us to serve our community in meaningful ways. At LifeStream Services, our commitment to providing essential resources and support for older adults, individuals with disabilities, and caregivers remains stronger than ever. This year has been one of change, innovation, and renewed connections, and we are excited to share some of our key accomplishments with you:

· To better support individuals and families affected by dementia, we welcomed Dementia Outreach Specialists to our staff. These specialists focus on outreach, education, and ensuring greater access to critical resources, helping us better serve those navigating the challenges of living with dementia.

· We also prioritized professional development by increasing and enhancing staff training opportunities. Investing in our team strengthens our ability to provide high-quality programs and services, and ensures we remain knowledgeable and adaptable in an ever-changing landscape of community needs.

· As part of this effort, we implemented the DISC assessment as a staff development and training tool. This assessment helped us better understand communication styles and team dynamics, fostering stronger collaboration and improving overall effectiveness within our organization.

· Another highlight of the year was the return of our Fall Festival in September 2023, marking the first time we hosted this beloved event since the COVID-19 pandemic. It was a joy to reconnect with the community in person, celebrating the resilience and spirit that bring us all together.

· Finally, we partnered with a consultant, Tanorria Askew, to strengthen our outreach and communication efforts, particularly in underserved and underrepresented populations. This initiative is vital to ensuring that all individuals, regardless of background or circumstances, have access to the services and supports they need.

These milestones reflect our ongoing dedication to serving our community through our values of respect, compassion, collaboration, and excellence. Thank you for being a part of our mission. We look forward to continuing this important work together in the years ahead.

AGING & DISABILITY

Quality Improvement

LifeStream remains committed to quality improvement and professional development. Each year, individuals receiving home and communitybased services participate in Quality Improvement Surveys (QIP) to provide feedback on their services. This year, 98% of respondents reported being “always” or “usually” satisfied with their care management services, and 96% felt actively involved in their assessment and service planning process. Additionally, LifeStream’s Aging and Disability Resource Center (ADRC) provided extensive training opportunities for staff throughout the year, including the 2nd Annual ADRC Resource Fair, which featured 64 agencies and community resources. Staff received training on a variety of topics including low vision services, heart disease management, and hospice care, accumulating a total of 2,307.5 training hours among all staff. These efforts ensure that LifeStream continues to deliver high-quality, person-centered care to those we serve.

ABC Care Coach Program

The ABC Care Coach Program offers support to caregivers of individuals with dementia, helping them manage stress and improve their ability to care for their loved ones. Care Coaches assess how caregivers are coping with the challenges of dementia-related care and work with them to create personalized coaching plans with the goal of reducing stress. These plans incorporate evidencebased strategies to manage cognitive, functional, behavioral, and psychological difficulties. Stress levels are reassessed by the Care Coach every three months to measure progress and ensure caregivers receive the ongoing guidance they need to prevent burnout and provide quality care.

Behavioral Health Care Managers

LifeStream Services has strengthened its support for individuals experiencing mental health challenges through grants from the Division of Mental Health and Addiction and the Division of Aging. These funds have allowed LifeStream to designate Behavioral Health Care Managers who provide specialized assistance to individuals referred by In-Home Care Managers. Behavioral Health Care Managers work with individuals to access local mental health professionals, connect with community resources, and communicate with other health professionals using a team-based approach.

STEP In CAREGIVER PROGRAMS

Dementia Outreach Specialists

Thanks to Indiana House Bill 1422, LifeStream Services has expanded our team to include Dementia Outreach Specialists in both of our areas, providing dementia resources and education across our 12-county footprint.

House Bill 1422 provides that an area agency on aging, such as LifeStream, may establish a dementia care specialist program and allows an area agency to designate at least one individual as a dementia care specialist to administer the program.

These dementia specialists joined the LifeStream team last spring and have helped expand valuable information and resources in the communities we serve.

Care Team Resource Binder

LifeStream Services is thrilled to provide caregivers and care partners with a resource binder that is an organizational tool used to prepare in the case that someone else may need to step in and provide care—either temporarily or permanently—and to empower caregivers who help with care needs.

Individuals who may need to step in could include another family member, a professional caregiver, a neighbor, or a friend. All of these care partners make up your care team. The binder is meant to be a helpful and important resource, not to add to your full plate.

ANNUAL EVENTS & OUTREACH

Annual Golf Outing 2023

The Annual Golf Outing took place on August 17th, 2023, at the Players Club in Yorktown. Individual Support Services was the presenting sponsor and Silver Birch of Muncie was the lunch sponsor for the day. We had 120 golfers join us along with 8 businesses who were cart and hole sponsors, and several volunteers who all contributed to the success of the event. American Pest Professionals took home the John R. Craddock Cup for the win!

Fall Fest 2023

After a 3-year pause, LifeStream was excited to invite the community back to our annual Fall Festival at Treaty Line Pioneer Village in Liberty, IN in September 2023! It was a day of interactive activities, informational programs and demonstrations, pontoon boat rides, Bingo, a car show, musical entertainment by Sean Lamb Janet Miller Band, a health fair, and more.

Aging Well 19th Annual Conference

Aging Well hosted its 19th annual conference at the Horizon Convention Center with Tanorria Askew as our keynote speaker. Tanorria is a chef, podcast host, and cookbook author who enlightened the group with her knowledge and experience. This one-day conference was a great day for community members, partners, wellness enthusiasts, and caregivers to join together to educate themselves on various topics as well as networking with several exhibitors to learn of the many opportunities that are offered within the communities we serve.

Wayne County Foundation Grant

Wayne County Foundation awarded LifeStream a grant of $13,200 during its third program support grant cycle of 2023. This award helped LifeStream to provide vital supplemental nutrition to homebound older adults and people with disabilities who otherwise would not be able to access it themselves. Through LifeStream’s partnership with Gleaners Food Bank, this program will be able to serve and positively impact the community by providing necessary supplemental nutrition to seniors. Mandy Williams, Vice President of Programs at LifeStream, said, “We are grateful for the generous support of the Wayne County Foundation, which will allow us to extend our partnership with Gleaners in Wayne County to provide supplemental foods and nutrition to some of our community’s most vulnerable residents. With the help of our volunteer partners, LifeStream will be able to continue delivering fresh food and pantry items directly to the doorsteps of our homebound community members.”

Herb Gardens

Folks at our café meal sites made their own herb gardens for Nutrition Month! Herb gardens were sent out to some of our café meal sites, and our community members got to plant things like cilantro, lavender, parsley, and more. Herbs are an important part of nutritional balance and health—they can help reduce inflammation, lower cholesterol, and support your immune system.

Healthy Aging Expo 2024

In May, we Shook, Rattled and Rolled while hosting our annual Healthy Aging Expo. The theme was a ‘50s Sock Hop with live music from Kicks 96, rroot beer floats, antique car displays, and community exhibitors, as well as demonstrations for our older population.

LifeStream’s first JOY Games North was held April 9–11, 2024, at the Muncie YMCA. There were 32 participants who spent time socializing and staying active with group fitness, 3-point-throw, football toss, giant Jenga, chair volleyball, Bingo, and Euchre. A few folks walked away as medalists and many walked away with prizes!

Golden Hoosiers 2024

Randy Davis of Redkey, Indiana, founder of Brianna’s Hope, and Lonnie Snow of Liberty, Indiana, volunteer for the Special Olympics Indiana Wayne County, each received the LifeStream Golden Hoosier Award. Randy Davis was then selected as the recipient of the Golden Hoosier Award, the state’s highest volunteer distinction!

Crafters from all over our service area shared their talents to create gifts for our community including craft blankets, summer hats, placemats for our home-delivered meal recipients, putting together our new Care Team Resource Binders, and many other items for our AngelWish program.

Tornado Rides

LifeStream had the privilege of transporting residents from The Lodge at Summers Point in Winchester to a local salon to get their hair styled. After some hardships following the tornadoes in the area, residents got some much needed and deserved pampering!

LifeStream was excited to announce the arrival of our brand-new fleet of buses over the summer! Our buses are designed with comfort and accessibility, ensuring our communities have access to safe and reliable transportation. Whether it’s a ride to the grocery store, a trip to the doctor, or a ride to the park, LifeStream Services can get you where you need to be in East Central Indiana.
