As we moved past the height of COVID-19 closures and back to in-person meals, evidence-based courses, wellness classes, and events, we also continued to look for ways to offer additional information, resources, and support for improving quality of life and maintaining independence to those in our community who need it most. Over the past year, our team has grown to include additional care managers and options counselors so that LifeStream can continue to meet the needs of our growing aging population. Additionally, LifeStream was pleased to launch our STEP In Caregiver Programs, a comprehensive way to educate and support family caregivers, care partners, and people with care needs. More individuals than ever are choosing to age in place and at home, which for many would not be possible without the support of caregivers. These caregivers do so much on behalf of their loved ones, and LifeStream recognizes that they, too, need education and support. This is where our STEP In program is a much needed resource for these important people and offers help with support, transitions, education, and planning. This Annual Impact Report shows the dedication and passion we have for serving older adults, individuals with disabilities, and family caregivers as well as our vision of seeing those in the region we serve living lives of quality, choice, and independence.

Connecting Care Managers and Partners with Duett In March 2023, LifeStream began using the Duett web platform to more effectively and efficiently link care management individuals with available service providers. Care managers can post specific individuals’ requests for services, and quickly receive responses from appropriate providers who are able to meet these needs. Duett ensures that the client’s freedom of choice remains a top priority, while helping to streamline this important process for LifeStream care managers.

In-Home Services & Support Case Management Awarded 3-Year NCQA Re-Accreditation LifeStream was awarded a 3-year NCQA (National Committee for Quality Assurance) Accreditation for case management long-term services and supports in March 2023. NCQA CM-LTSS Accreditation standards are designed to help healthcare organizations navigate care coordination, integration of health equity principles, and patient protections into care for vulnerable populations. STEP In Caregiver Programs LifeStream Hosts Caregiver Workshop with Teepa Snow LifeStream Services hosted Teepa Snow for a full-day educational workshop for professionals and caregivers at Purdue Polytechnic Center in Anderson. Teepa Snow is one of the world’s leading advocates and educators for anyone living with dementia or other forms of brain change. She is an Occupational Therapist with over forty years of rich and varied clinical and academic experience. Teepa spent the day educating 160 attendees on two important topics: Brain Changes in Dementia, and Using a Positive Approach to Dementia Care. Sessions improved the learner’s understanding of the brain changes that occur during the process of dementia with the goal of providing knowledge, skills, and attitudes that will change daily outcomes of dementia care.

Utilizing specialized gear that simulates dementia-specific sensory changes, this safe experience is a powerful and effective tool to strengthen the participant’s understanding of, and empathy for, people living with dementia. Gaining a firsthand understanding of anxiety, isolation, frustration, confusion, and more, Dementia Live® uniquely walked 253 participants from the “AHA” moment to empowering participants with tools to improve communication, care processes, relationship building, and the environment.
LifeStream Services Rolls Out STEP In Branding for Caregiver Programs In September 2022, LifeStream Services proudly rolled out the new branding of our caregiver programs. LifeStream’s STEP In Caregiver Programs are a comprehensive way to educate and support family caregivers, care partners, and people with care needs. Caregiver Memory Kits, resource binders, support groups, and evidence-based programs are just a few of the ways LifeStream is here to help you STEP In to caregiving.
Care Team Resource Binder LifeStream Services is also thrilled to provide caregivers and care partners with a resource binder that is an organizational tool used to prepare in the case that someone else may need to step in and provide care – either temporarily or permanently – and to empower caregivers who help with care needs. Individuals who may need to step in could include another family member, a professional caregiver, a neighbor, or a friend. All of these care partners make up your care team.

534 volunteers provided 18,622 hours of service equal in value to $530,526
With the assistance of Ball State University students and Foisters Flowers, 100 Delaware County seniors received bud vases to help brighten their day. This small gesture is just one of the ways volunteers allow us to provide essential services to seniors, such as delivering meals and commodities, serving at our senior café sites, providing companionship, and other important tasks. Volunteers enhance many of our programs by offering additional support, energy, and enthusiasm. They provide a valuable service to the community by giving their time and skills to support our mission. 13
Local Volunteers Honored with Golden Hoosier Award Christie Bunner of Randolph County and Ruth Ann Ratz of Franklin County were the recipients of the LifeStream Golden Hoosier Award and Tom Farris, Delaware County; John Clark, Fayette County; and Carolyn Anderson, Wayne County were recognized as runners-up for the LifeStream Golden Hoosier Award.
