For many patients with macular degeneration and other vision-related conditions, the loss of central visual detail also signals the end to one of the last bastions of independence, driving.

Colorado optometrist Dr. Robert Stamm is using miniaturized telescopes which are mounted in glasses to help people who have lost vision from macular degeneration and other eye conditions.

Becoming Santa, becoming a better man

H

aving a full Santa beard can have unexpected consequences on your life.

For instance, a Life After 50 reporter may stop you with a copy of the May issue saying “You look like you might know this man on the front cover!”

Or perhaps another Santa look-alike may stop you with “Nice beard,” which may ultimately change your life.

For Jay Alcenius, the accidental meeting seven years ago with Billy Gooch at Sheldon’s Diner not only changed his life, but that of a friend, also a bearded Santa lookalike.

Billy Gooch was not only a well-known Santa character at the North Pole up Highway 24, but was also a Santa recruiter. Gooch, now deceased, had also served one Christmas at the White House.

While the meeting did not recruit Alcenius at the time, he referred another friend to Gooch, who did enter the Santa world. Last year, that friend returned the favor, by referring Alcenius to his Santa recruiter. Thus, another Santa was created. And as he will tell you, it made him a better man.

“You can’t be Santa through Christmas, keep the beard all year long; everybody makes comments, so you kinda have to be Santa year-round. You can’t have road rage and yell at somebody, get in a bar fight, cause you’re Santa year-round even if you’re not getting paid for it,” Alcenius said. “You live it.

It’s fun. And there are lots worse characters to be mistaken for. Nobody wants to hear, “Aren’t you the Charlie Manson guy?” Nobody wants to be mistaken for him.”

As I learned more about Alcenius, I began to realize how neatly he fits the true Santa role.

First of all, Alcenius is a retired master sergeant (E-7), having served 24 years in the U.S. Air Force.

This shaped him in many ways. Although he served in Florida, California, Northern Maine (186 miles from the nearest 7-11!) and Colorado Springs, the deployments that affected him most deeply was the one in Korea.

It was there he learned to eat with chopsticks, a feat he still practices today, as I witnessed his facility with them at Bhan Thai over Cashew Chicken. It was much more than that, though, that left impressions which challenged his thinking of how things should be.

“It’s interesting when you go to another country and see another culture and see how they have a totally different logic than we have. Here, if you are in a taxi and the taxi gets in an accident, it’s the taxi driver’s fault. But there, if you’re in a taxi and get into an accident, it’s your fault, cause you told him where to go and otherwise he wouldn’t have been there. Totally different logic.

You see that there’s more than one way of seeing something.”

After retiring in 2003, Alcenius spent seven years with Boeing, writing maintenance manuals on the GPS ground antenna for Spacecraft support. He was looking around for something interesting to do

can’t deliver live puppies anymore. Last year I had a puppy on the sleigh, and he barked, scaring the reindeer who jumped to another roof, so I can’t deliver live puppies anymore.”

If the parent has nodded yes, he adds, “I have a special service that may be able to deliver a live puppy.”

When I asked where he learned his ability to think on his feet, he told me about his NCO training. One of the trainings involved picking a card of - Jay Alcenius topics from a batch, and being given 15 minutes to prepare a speech either for or against, even if you knew nothing about the topic. You needed an introduction, at least three facts to cover, transitions, and when the friend he had referred to Gooch contacted conclusions and at the end you were given five him about filling in as a Santa.

This usually requires about six weeks during the minutes to give your speech.

If you weren’t finished, you were cut off Christmas holiday season, so it becomes hard to work at another job and request that amount of time anyway. He said being a grandfather to a three-year old granddaughter and six-year old grandson had off. At loose ends, Alcenius accepted and was sent to the Merced Mall in Merced, California. Since they live with him and his wife, he has year-round training on which programs children watch, who the main characters are and their names. This comes in handy when he is acting Santa.

Last month, 450 Santas gathered in Denver for the International Brotherhood of Real Bearded Santas. There were workshops on Santa characters, the history of Santa, what to say and do and not do or say. “You never promise unless the kid’s mom nods.” Other trainings included “You never reach for a child, you let them come to you. There were trainings on what to look for like signs of abuse, legalities, trends, sexual harassment and molestation.

All Santas must have background checks and insurance. There are two categories, mall Santas and independent Santas, who may be contracted out for small parties, family appearances. I’m a mall Santa. This year I will be the alternate Santa at the North Pole.” His friend whom he referred to the recruiter seven years ago will be the full-time Santa, and Alcenius will be the alternate.

It’s a good thing he’s the alternate, because this Santa has many interests. He is an accomplished musician, playing first solo clarinet in the New Horizon Symphonic Band of Colorado Springs, which has 85 pieces and performs two concerts a year. He also plays in a community band and in a clarinet choir.

In addition to his music, he enjoys long distance motorcycle rides on his “BMW K1200 LT with power adjustable windshield, six-disc CD changer, heated seat and handgrips, and hydraulic center stand. It’s just missing two wheels of being a car!” he grins. He is currently participating in a 3K “Run for the Wall” to arrive in D.C. Memorial Day to bring awareness to the POWs and MIAs. This is the 30th year of the run, and 1,600 participants will travel three routes to get there.

Alcenius has been in the Springs for 21 years, where he is also a ham radio operator, coordinating with the Pikes Peak Hill Climb for emergencies, and reports weather conditions to NOAA in Pueblo. They coordinate storm conditions with the 300-500 ham operators in the Pikes Peak region. This information helps refine weather reporting for the future.

Photography is another interest of this Renaissance guy. He and his wife enjoyed square dancing for many years. His father-in-law was a caller, so date nights were frequent and free! He also played competitive darts for 16 years and ranked 320th in the nation.

You see, this Santa was accomplished in many areas but was unprepared for some aspects of being a Santa. “I wasn’t prepared for the counseling aspect of being a Santa, cause I’d been pretty well-isolated from that kind of life.”

Homelessness, kids who were unkempt, unmatched shoes, separated from their parents. “One little girl who came in with two men, asked if I knew where her dad was. No, I said, where is he? He’s in jail, she said. That’s when I knew the men were her foster parents.”

“It’s hard to stay in character and not shed a tear when you hear some of the stories. I didn’t succeed. I cried right along with them. But it’s interesting to see the lives of people. Some people bring in 17 family members and everything seems happy.” This is often when they have a family picture taken, all 17 in one photograph. “The kids all like Santa Claus, and the sparkle in their eyes when they realize someone is talking with them not to them.”

Some kids are real curious. “’Where are your reindeer,’ they ask, and I tell them reindeer are kind of messy animals and we don’t want to have that kind of mess on the floor and the Department of Health wouldn’t allow it.”

This father’s day, I extend a Happy Father’s Day to Jay Alcenius and all fathers. I salute his service in the military, applaud his music, avoid his darts, smile for his photos and shelter during the storms he helps supervise. And for being a Santa all year I silently add my own wish list: world peace, a new car (selfishly, I admit), and happy holidays for children and parents everywhere. By the way, his other sleigh is a 1997 candy-apple red Jaguar convertible with white leather seats and bird’s eye maple dash. Wonder if we could trade my 1997 Ford Escort Station Wagon with five on the floor with his Jag?

Couple comes out of retirement to build senior apartments

Max and Georgene Nazarenus recently completed construction of their newest senior apartment building - Park Manor East.

ax and Georgene Nazarenus may finally retire (for real this time) with the completion of their new senior apartment building – Park Manor East.

The couple has retired a few times prior, but always seem to find a new venture or project that keeps them busy.

“Our friends said, “You’re 83.

Why don’t you kick back and enjoy your retirement,’” Georgene said.

“The challenge always beckons.”

Georgene’s mother Viola Stalzle owned and operated the Park Manor Apartments, a senior 55-plus building located at the end of Monument Street just west of Cascade Avenue, for 40 years. When she passed away 10 years ago, Georgene inherited half of the property and purchased her sister’s half.

The property included vacant land, located just east of Park Manor.

Max and Georgene owned and operated two successful caramel candy stores in malls located in Cheyenne and Casper, Wyoming.

“We did those two stores for 10 years. It was very, very good for us,” Georgene said. “It’s a tremendous amount of work because you’re manufacturing everything.”

The two ventured into that business after Max retired from the Army, where he was an aviator. The couple moved around quite a bit during Max’s military career.

“We bounced all around. We lived in nine different states; some of them two or three different times,” Georgene said. “We were overseas for eight years – Thailand, Vietnam, Korea and Germany.”

The couple had just retired for a second time, after selling the stores, and moved to south Texas.

“We went down there and stayed there for 10 years,” Georgene said.

“We fished and played golf every day.

But then, we were homesick for Colorado.

Georgene grew up in Rocky Ford, and Max grew up just a few hours down the road east in Holly. The two didn’t meet, however, until they were at the University of Colorado in Boulder.

Once the couple found themselves back in Colorado, it wasn’t long before Max got restless with retirement. He decided to build 10 townhomes down on Tabor Place near Eighth Street. Then when Georgene acquired the Park Manor property, the vacant land started to bother Max. About three years ago, Max decided to do something with the land.

“It was something to do,” he said.

“I thought, ‘Well, we ought to do it because it didn’t make sense to leave (the land) there doing nothing.’”

With the aid of architect David Barber and Art Klein Construction, the Max and Georgene broke ground on the land. Now, a few years later, the building is completed and Park Manor East is now accepting applications for residents.

Senior citizen property tax exemption program application deadline July 16 For Life After 50 Senior Property Tax Exemption Program funding is reviewed annually by the Colorado General Assembly. If the program is not funded, but you meet the qualifications, you are encouraged to apply and be placed in the system for future funding.

If you applied and received notification of approval previously, there is no need to reapply - you will remain on the approval list. If approved and additional information is needed, our office will contact you.

In November 2000, Colorado voters amended the State Constitution with the adoption of a property tax reduction (also known as the Senior Homestead Exemption Act.) Specifically, this grants exemption up to 50 percent of the first $200,000 in actual value on residential property, resulting in substantial savings in property taxes to qualifying seniors.

In order to qualify, the following criteria must be met: • The qualifying senior must be at least 65 years of age or older, as of January 1st in the year of application; and • Must be the “Owner of Record” of the property for at least 10 consecutive years as of January 1st in the year of application; and • Must occupy the property as the primary residence for 10 consecutive years prior to January 1st in the year of application.

To receive the reduction, the senior taxpayer must have applied and been approved prior to the year the exemption is effective. If you meet the above criteria, or are the surviving spouse of a senior who would have qualified, please contact the El Paso County Officer of the County Assessor at 719-520-6600 to obtain additional information or to receive an application.

While Park Manor East was quite the endeavor for Max and Georgene,

there was some joy in the process.

“It was a challenge. You feel a feeling of accomplishment,” Georgene added. “At times it was fun. There’s satisfaction in it.”

But, yes, the Nazarenuses are really retired now.

Disabled veteran tax exemption program application deadline is July 2

In order to qualify, the following criteria must be met: • The qualifying veteran sustained a service-connected disability while serving on active duty in the Armed Forces of the United States. This includes members of the National Guard and Reserves who sustained their injury during a period in which they were called to active duty; and • The veteran was honorably discharged; and • The U.S. Department of Veterans Affairs has rated the veteran’s service-connected disability as 100 percent permanent and total; and • The veteran must be the “Owner of Record” and occupy the property as his or her primary residence prior to Jan. 1 of the year of application.

• The exemption is available to

surviving spouses of disabled veterans who had the disabled veteran exemption and who passed away prior to Jan. 1 of the current year. The property must be owner occupied and used as the primary residence of an owner-occupier who is the surviving spouse of a qualifying disabled veteran.

The application deadline for 2017 is July 2. The application is available from the Division of Veterans Affairs or the county assessor of the county in which the property is located, and must be returned to the Colorado Department of Military & Veteran’s Affairs.

Couples (1992), Langer (1985, 1993) and Singh (2000) are all past Masters champions.

Couples, 58, has played in four U.S. Senior Opens. He was the runner-up in 2010 at Sahalee Country Club, in Sammamish, Wash. and tied for fourth last year at Salem Country Club, in Peabody, Mass.

Langer, 60, won the 2010 U.S.

Senior Open by three strokes over Couples and was the runner-up in 2012 at Indianwood Golf and Country Club in Lake Orion, Mich.

Langer has recorded six top-10 finishes in the 10 Senior Opens he has competed.

Singh, 55, tied for seventh last year in his third U.S. Senior Open appearance. His best finish came in 2014 when he tied for fifth at Oak Tree National, in Edmond, Okla.

President Trump’s new tax law, which will result in a $1.5 trillion tax cut, is seen by some as a belated Christmas present to most American taxpayers. Critics of the law, however, suggest that the Grinch will have stolen Christmas from some seniors.

Older Americans have some important issues to consider in terms of how the new tax law will affect their health care costs.

“For starters, the tax reform eliminates the tax deduction for medical expenses, and although a relatively small percentage of the population claim this deduction, it had a coalition of opposition to the largest tax reform proposal in a generation,” said senior care advocate Chris Orestis, author of the books, “Help on the Way” and “A Survival Guide to Aging.”

According to the IRS, about 8.8 million households filed 2015 tax returns that claimed the medical expenses deduction. The American Association of Retired Persons’ (AARP) Public Policy Institute says 69 percent of those taking the deduction had income of under $75,000, and 49 percent were below $50,000.

“The irony of the new tax law is that it penalizes people who are covering their own costs instead of relying on taxpayer-funded coverage through Medicaid,” Orestis said.

Orestis narrows down how three pieces of the new tax law could adversely impact seniors and their health care: • Eliminates medical expense deduction. Under the previous tax law, you could deduct family medical expenses if the cost exceeded 10 percent of your adjusted gross income.

The new law ended this deduction and will particularly impact seniors paying

for long-term care.

“This tax deduction was especially beneficial for seniors who are paying for long-term care costs not covered by Medicare or private insurance,” Orestis said. “It provided a critical tax break for seniors in this category, and the elimination of this deduction will have a disproportionately negative impact on seniors paying for the expensive costs of skilled nursing homes and long-term care.”

• Eliminates Obamacare’s insurance mandate. The AARP opposed the repeal of the insurance mandate because of the spikes in insurance premiums that will result.

“De-incentivizing the young and healthy to get insurance or basically allowing them to drop coverage will make coverage for the old and sick more expensive,” Orestis said.

• Cuts to Medicare and Medicaid.

One reason for the failed efforts to “repeal and replace” the Affordable Care Act was the likelihood that millions would have lost coverage due to Medicaid cuts. The new tax law triggered a $25 billion slice to Medicare in 2018 to help begin accounting for the $1.5 trillion tax cut.

Medicaid covers 60 percent of those in nursing homes.

“It’s important to recognize the fact that Medicaid is the nation’s primary payer of long-term care services, in the same way that Medicare is the nation’s payer of hospital and doctor visits for anyone over the age of 65,” Orestis said.

Orestis said a more sensible approach to tax reform is to look for ways to use the tax code to create incentives for people to take personal responsibility, whenever possible, over relying on taxpayer-funded government programs such as Medicaid.

Be not conformed

And do not be conformed to this world, but be transformed by the renewing of your mind, that you may prove what is that good and acceptable and perfect will of God.” Romans 12:1-2

I

t is so easy to conform to the things around us and so very difficult to go against the tide of people and circumstances in our life, but that is exactly what the Lord calls us to do.

We are not called to be like the caterpillar that crawls on the earth and conforms to the earth. We are called to be the butterfly that soars high above the earth and is carried about by the wind of the Spirit.

It is said that the greatest struggle in the life of the butterfly is breaking out of its cocoon. However, without

the great struggle, the butterfly would not have the strength to use its huge wings.

There are many struggles facing each of us today; some are in the arena of health, finances or other struggles might be with your family situation.

The struggles may vary in kind and strength but they are calling us to give in and conform to their pressures.

The only solution is to renew our minds to the Word of God and be transformed to Christ’s way of thinking not our own.

This is the time to obey Romans 12: 1-2. The Greek word that is translated “transformed” in this text is actually the word that we derive our English word “metamorphosis” from, which is the process that a caterpillar goes through to become a butterfly. So wherever you are in the process—it is ok.

We are all being transformed from the cocoon to a beautiful butterfly when we renew our minds to the acceptable and perfect will of God. Now is the time to “fly.”

This national service organization, headquartered in Kansas City, Missouri, and their main sponsorship is helping the hearing impaired These five Sertoma clubs (Austin Bluffs, Colorado Springs, Gleneagle, Legacy and Woodmen Valley) have provided hearing aids to those individuals in El Paso County who might otherwise been unable to afford them.

Brigitte was lifting very heavy weights at the time, even though she started later in life. She was her coach for many years, and then starting working out on her own. Now she prefers training by herself because she gets to come and go when she wants, and stay as long as she wants.

Brigitte trains for 2-2 ½ hours a day.

Dead Duck

A woman rushed into a veterinarian’s office with a limp duck in her hands. As she laid her pet on the table, the vet pulled out his stethoscope and listened to the bird’s chest.

After a moment or two, the vet shook his head and said, “I’m sorry.

Cuddles has passed away.”

The distressed woman wailed, “Are you sure?”

“Yes, I am sure. Your duck is dead,” the vet replied.

“How can you be so sure?” she protested. “You haven’t done any testing on him or anything. He might just be in a coma or something.”

The vet rolled his eyes, turned around and left the room. He returned a few minutes later with a black Labrador. With Cuddles’ owner looking on in amazement, the dog stood on his hind legs, put his front paws on the examination table and sniffed the duck from top to bottom.

He looked up at the vet with sad eyes and shook his head.

The vet patted the dog on the head and took it out of the room. A few minutes later, he returned with a cat.

The cat jumped on the table and delicately sniffed the bird from head to foot. The cat sat back on its haunches, shook its head, meowed softly and strolled out of the room.

The vet looked at the woman and said, “I’m sorry, but this duck is certifiably dead.”

He turned to his computer, hit a few keys and produced a bill, which he handed the woman. The duck’s owner took the bill and cried, “$150? $150 just to tell me my duck is dead?!”

The vet shrugged, “If you had just taken my word for it, the bill would have been $20, but with the Lab Report and the Cat Scan, it’s now $150.”

A Golfer’s Problem

Sex isn’t just for healthy bodies.

Handicapped people who’ve been willing to talk about their sex lives have taught us how creative sex can be. Body parts that don’t cooperate don’t have to limit us. Remember, the mind is the controlling factor. A man with paraplegia knows where his partner’s hands are, even without feeling.

It’s really important to know how flexible sex can be. It so easily gets categorized as the Big “O” experience when there are many other ways to partake. We can be honest about our preferences.

From the babies we were to the elders we will become, we can embrace sex in our own way now.

hen my mother was 83, she troubled me with the way she used — or rather misused — her pillbox. I set it up for her each Sunday evening, carefully placing mounds of tablets of different hues and sizes in the appropriate daily slots.

But I’d often return later in the week to find she had somehow taken the wrong pills from the wrong slots.

It worried me that she might not be getting the medicine she needed for her high blood pressure, mild dementia and pain. Worse, I was concerned she was taking too much medication at times and could suffer adverse effects.

It should have been clear to me that my mother could no longer safely take her pills on her own. And yet I hesitated for months to completely take over the task.

In my mind, she was still the most compulsively organized woman I’d ever known. During her working years, she had been a teacher, an accountant, an insurance broker and a condo board president. (The “Condo Queen,” we had called her teasingly.)

Would she feel angry at me if I spoon-fed medicine to her as if she were an unruly child?

When I finally went ahead and told her that from now on, I or someone else would literally hand her the right pills (and then observe to make sure she swallowed them), she winced but hardly protested. I worried

When a parent’s judgment is impaired, the most loving thing a child can do is to speak up and make prudent decisions on their behalf.

Few adult children relish taking over. It takes more hands-on work and emotional duress. No one wants to be known as the power-mad usurper of an aging parent’s dignity. As a consequence, many of us watch and wait — and wait and wait.

But while a parent is declining physically or cognitively, we have constant fears about medication errors, frequent falls, poor hygiene, financial mismanagement, errant driving and other threats to their well-being.

Ultimately, it is not a question of if they need more help, but what type and when.

Timing is everything. If we act too soon, we risk babying a parent and

fostering unhappy dependence. If we act too late, we risk a catastrophic event that we could have prevented.

Here are some ideas for finding the right balance.

As physician and author Atul Gawande points out in his best-selling bookBeing Mortal, keeping our parents safe is crucial but shouldn’t negate every other consideration. We must take into account a parent’s preferences and needs for independence before we swathe them in bubble wrap and order 24-hour supervision.

Most of us weigh the risks and benefits of any given activity. If Mom still wants to cook, then we ask ourselves if she can safely use the oven, perhaps with some prompting. If

she wants to go to the bathroom on her own, then we ponder if she can physically manage with a commode seat and grab bars. We exercise reasonable caution but are also prepared to tolerate some degree of risk.

Talk early and often

Adult children and their parents should start talking about developing contingency plans for adapting to aging-induced changes long before an older adult is absentminded or tottering. (See AARP’s The Other Talk

by Tim Prosch.) That’s not being negative or disrespectful; it’s being realistic and proactive.

Choosing the right home health care agency W

hen illness or injury strikes, home health care is oftentimes not only preferred, but statistically shown to rapidly improve healing and give peace of mind.

So, how do you sift through the many choices?

To begin the process, make a list of what you think your loved one will need. Next, ask for referrals from your loved one’s physician or a family member or friend.

If your loved one has been hospitalized, ask a social worker or nurse at the hospital. Another good place to look is the Internet.

Once you’ve identified the type of care needed, narrow the search to a few agencies. Here are some important questions to ask:

Services

• Service listing. Do their services cover your loved one’s needs? Is someone available to provide information about services, eligibility requirements and funding sources?

• Training and supervision. What formal training programs and certifications does the agency require of its direct care providers? What level of professional supervises the care? How often and how do the supervisors oversee the caregivers on location to ensure proper care?

• Documentation and coordination. Are visits and treatments documented? Do family members have easy access? Does the agency coordinate with the physician?

Direct Care Providers

• Length of visits. How often does the care provider visit, and how long do they stay? Do they provide care on weekends and evenings? Do the visits allow time to get to know the patient?

• Rotation of care. Is care provided consistently by the same caregiver, or is it rotated among different

• Special fees. Are there fees for special or extended services? Who is responsible for them? Are payment plans available for any out-of-pocket services? Is there someone at the agency who can assist in exploring all payment options?

Finally, remember that agencies that embrace current technology work efficiently and seamlessly and often provide the best patient care.

Unless they experience some untoward medical event, aging parents usually change slowly. Closely gauge their functioning and help them out incrementally. In other words, don’t take over all at once. Tailor what you do to only provide as much support as they clearly need, but no more. For example, a parent with dimming eyesight may need to be driven by others after dark but not during the day.

Overrule, if necessary

When a parent’s judgment is impaired by dementia or blinded by pride, then the most loving thing a child can do is to speak up firmly and make prudent decisions on his or her behalf. That parent may fume or pout.

You have to calmly respond with empathy and understanding, but do what you know is right for all involved.

Ten facts you need to know about hospice Is hospice care the right option for you or a loved one?

n interesting thing happens when Dawn Gross brings up hospice to patients or their families: “Oh, no, we don’t want that!” they often say.

“OK,” says Gross, a hospice and palliative care physician in San Francisco. “Tell me exactly what you don’t want, so we’re sure not to give you that.”

Going off to some facility, they tell her. Losing control of care. Being knocked out by morphine. Or — the clincher — giving up. When Gross assures them that hospice isn’t at all like that — that two-thirds of hospice care takes place in the person’s home or a long-term care facility, that the patient can still receive medical care, and that Medicare and most private health insurers pay for it in full — they often change their minds.

In 2011, about a million people died in hospice — about 42 percent of all those who died, according to the National Hospice and Palliative Care Organization in Alexandria, Va. — and its use is growing.

Still, misconceptions about this end-of-life service abound. As a result, many who might benefit from hospice don’t sign up until the very end: About a third of hospice users enroll for less than a week,

and the median time is 18 days.

So how can you tell whether hospice is the right choice for you or your loved one? The answer depends on what you believe hospice is, your current goals, and what you think it can — or can’t — do for you.

Here’s what you need to know.

Hospice is a philosophy of care, not a brickand-mortar location. Most people say they want to die at home, but only about 1 in 4 end up doing so.

One big reason: It’s often just too hard. “Trying to care for someone with a serious illness, especially at home, without hospice is like trying to have surgery without anesthesia,” says Ira Byock, the executive director of the Providence Institute for Human Caring.

Hospices bring everything you might need to the home — hospital bed, bedside commode, medications, bandages, expert consults — tailored to your needs.

But if you’re daunted by home care, or simply don’t want a loved one to die in your home, hospice care also is available in facilities and hospitals.

Signing up doesn’t mean giving up all medical care. Transitioning to hospice means shifting from one set of goals (how to get longer life through a cure) to another (how to get the best quality of life out of whatever time is left).

“When people say, ‘I don’t want to give up,’ the key is to understand what they think they’re giving up,” Gross says. Even when a cure is no longer viable, therapies that improve symptoms and raise

comfort can continue. “I deliver very aggressive care in hospice,” she adds.

If, however, you feel that you have not exhausted all of your treatment options in search of a cure, hospice may not be for you. Medicare hospice rules require forgoing curative treatments.

That may soon change, though. In July, Medicare announced the expansion of a five-year pilot program to 141 hospices in 40 states to allow patients to continue pursuing curative treatments while under hospice care.

You have to qualify for hospice, but you can opt out at any time. To qualify for hospice benefits, either through Medicare or private insurance, two physicians must certify that you have a life-altering condition with an expected prognosis of six months or less. This time frame is arbitrary, however; there’s no biological or scientific basis for knowing how long you have left, Gross says.

If you start hospice and realize it’s not for you, you can quit. How can you know when to try hospice? This should be part of ongoing discussions with your health care team, Byock says — “ongoing” because goals and needs evolve.

You may live longer during the time you have left. Hospice recipients live longer, on average, than those receiving standard care, research shows. A 2010 study of lung cancer patients found they lived nearly three months longer; another study, looking at the most common terminal diagnoses, found the

You can still see your regular doctor. Multidisciplinary by intention, a basic hospice team consists of a physician and nurse (both on call 24 hours a day); a social worker, counselor or chaplain; and a volunteer.

Many hospices offer added services: psychologists, psychiatrists, home health aides, art or pet therapists, nutritionists, and occupational, speech, massage or physical therapists. You may also continue to see your regular doctor. And you remain in charge of your medical decisions.

More on Hospices As hospice has evolved from a grassroots movement to a booming industry, a few bad apples have inevitably emerged. What should you look for when evaluating a hospice? “There’s no easy shortcut to evaluating quality care,” says Joe Rotella, M.D., chief medical director of the American Academy of Hospice and Palliative Medicine. But here are some questions to ask.

Is the hospice accredited? “Hospices aren’t required to be certified or accredited, but those that seek outside accreditation are making a special commitment to quality care,” Rotella says.

The Joint Commission, Community Health Accreditation Partner, the National Hospice and Palliative Care Organization and other groups have created standards of excellence that participating hospices must meet. Some state agencies also certify hospices.

Has the hospice been surveyed by a state or federal oversight organization? If so, when? What were the results? The Affordable Care Act now requires hospices that accept Medicare — almost all of them — to complete surveys and provide data about several quality measures, including how well they manage patients’ pain.

Eventually the results will be publicly available online. For now, ask if a survey has been done and what the results show.

Is the medical director board-certified? Board certification is not a requisite, “but having a medical director who is certified by a medical board as a palliative medicine specialist offers one more assurance of training, experience and overall quality,” Rotella says.

How many patients does the hospice program care for? Hospices come in many sizes. Although smaller ones may provide more personalized care, larger ones (those serving at least 100 patients) have more resources.

What is the typical caseload?

Ideally, a hospice nurse or nurse practitioner should manage a caseload with no more than 12 patients.

For various reasons, older people prefer to receive medical care at home, whether it be in their own home or their children’s home. Census data show a 67 percent jump between 2000 and 2007 in the number of parents who have moved in with their adult children.

More than 8 million seniors and people with disabilities enjoy the benefits of medical care at home. Services and equipment that enable people to receive care at home include oxygen therapy, power wheelchairs, hospital beds and diabetic supplies. Congress and the White House will consider the value of homecare for patients and taxpayers as they explore avenues for healthcare reform.

Reforming healthcare, especially Medicare, is a top issue in Washington, D.C. Medicare provides health insurance to approximately 43 million Americans aged 65 and older, and to people with permanent disabilities. The spending of Medicare is projected to grow annually by 7.5 percent over the next decade.

Medicare trustees project that Medicare’s Trust Fund will be exhausted in 2019.

While total Medicare spending skyrockets, the portion devoted to home medical care and equipment remains less than 2 percent. At the same time, homecare holds down costs better than other healthcare segments. Two years of home oxygen therapy costs less than the average Medicare cost for a single day in the hospital, which is more than $5,500.

Providing care to seniors in their homes requires services. Homecare providers serve clients after hours and over weekends to ensure that their patients stay safe – and out of emergency rooms. Also, homecare providers help vulnerable seniors during emergencies such as ice storms and hurricanes.

As the President and Congress work toward solutions regarding the uninsured and the rising costs of care, the role of home medical care and equipment is likely to be considered as one of the key solutions that will help sustain Medicare and Medicaid.

Tyler J. Wilson, president of the American Association for Homecare, notes, “Homecare will continue to be safe and cost-effective only as long as policymakers in Washington remember that homecare requires a human touch, including services and personal attention.”

Finding respite care: Where to look for a substitute caregiver R

espite breaks for the caregiver are vital to one’s health.

Caregivers have a lot of reasons for not arranging respite for themselves. Among them: guilt, money, being too busy in the present to plan for the near future and reluctance to change their loved one’s routine.

Expert advice: Let yourself go. Respite care is made for you — and your loved one. If you’re among those caregivers who need convincing, you should know: • Respite is good for you and your loved one.

• Being too busy to track down a respite care provider is a sign that you need a break.

• A break now can ward off burnout later.

• Respite care is often covered by insurance or an agency such as the Department of Veterans Affairs or Medicare. And volunteer help is available.

Becoming a family caregiver often means making less and spending more than you’d like, which can make you feel respite care is an indulgence you can’t consider. Do it anyway. Every caregiver needs a caregiver — someone who will care for your loved one for a few hours, days or weeks, so you can take care of yourself. Respite is now recognized as an essential part of the job.

Volunteer groups; faith-based organizations; local, state and federal agencies; and some insurance plans are making it easier to afford respite care.

Step by Step: Finding (and Paying for) Respite Care

Step 1: Get information Click on the National Association of Area Agencies on Aging (AAA) to find a contact for your local agency, which can connect you with visiting

companions, hourly in-home respite care, adult day care and overnight respite providers. It also can tell you if there are local no-cost or low-cost programs, if your state Medicare program will cover some of the cost, and if financial assistance is available.

Step 2: Expand your search Contact faith-based caregiving organizations, including your local branches of Faith in Action or Interfaith Caregivers. Many have friendly visitor programs that will set up regular two- or three-hour social visits with your loved one, giving you time to spend on yourself.

Elder Helpers prescreens and posts pictures and bios of local volunteers who want to visit and help older people by doing basic chores. There is no charge for visits or services.

Corporation for National and Community Service Senior Corps sponsors Senior Companions, a federal, community-based program that matches volunteers over age 55 with aging people who need help with daily life activities. Volunteers visit weekly, offering assistance with tasks and friendship.

Adult day care offers supervised, planned activities, social interaction, meals and limited health services for as little as a half day and as many as five days a week. Adult Day Care Health Services is for aging people who have qualifying intellectual, physical or mental challenges or are close to needing nursing-home level care, and includes intensive therapies and health and social services.

Rules, regulations and costs vary from state to state, so you will want to look at the national picture before you find a local program. Adult day care can cost $25 to more than $100 per day, depending on

the area and program. If your loved one is taking part in Programs of All-Inclusive Care for the Elderly (PACE), adult day care is free. State Medicaid waiver programs approved by the Centers for Medicare & Medicaid Services (CMS) may pay for adult day services or adult day health services according to a person-centered plan of care for beneficiaries who have nursing facility or institutional level-of-care requirements or are at risk of institutionalization.

• Inpatient Respite Care. Many nursing homes and care facilities offer short-term respite care. If your loved one qualifies for the hospice benefit, Medicare will pay for most of the Medicare-approved amount for respite care provided by an approved inpatient facility.
