New to Medicare? Here’s an overview By Jason Alderman
I
f your 65th birthday is around the corner or you’re anxious about the Medicare open enrollment period, it’s a good time to start focusing on your options. Health care choice has become a bigger factor in the lives of pre-retirees since the Affordable Care Act. Though a separate federal health insurance system with no connection to Obamacare or its online marketplaces, Medicare is going through its own evolution in terms of plan offerings and customer access.
Here’s a basic primer for future Medicare enrollees: What is Medicare? Medicare is a government-
care” because if you choose to take this option, you’ll sign up for them as a pair. You’ll get to go to any doctor, hospital or facility that’s enrolled in Medicare and accepting new Medicare patients. You’ll end up paying a premium for Part B, which can change from year to year while Part A is usually free.
• Part C (Medicare Advantage). Call this one-stop shopping for your hospital, outpatient (Part A and B) and usually your prescription coverage (see Part D below). Medicare Advantage policies are sold by private insurers. Medicare pays their share and you pay your premium. They may include different assortments of services, including vision, prescription drug, dental or hearing. But keep in mind, different Medicare Advantage plans have different rules for services and can charge different out-of-network costs.

What does it cost? Though you’ve likely paid taxes into the Medicare system your entire career, Medicare isn’t a completely free program. You’ll pay premiums deducted from your Social Security checks for some portions of your benefits. There may be copays and deductibles for certain services. When do I get to join? You can apply for Medicare through your nearest Social Security office up to three months before your 65th birthday. If you wait longer than three months after your 65th birthday to apply, you may face a late enrollment penalty.
WHAT DOES COVERAGE LOOK LIKE? • Parts A (Hospital) and B (Outpatient). Part A covers inpatient hospital care and Part B covers outpatient medical care and procedures. You’ll hear Parts A and B referred to as “Original Medi-
What’s Medigap insurance? If you go with Original Medicare (Part A and B), you’ll find some gaps in your Medicare coverage. Medigap is the nickname for Medicare Supplement Insurance, which you purchase privately. If you’re on Medicare Advantage, it’s unlikely you’ll need separate Medigap coverage but it’s important to check. What if you plan to keep working with company health benefits after you turn 65? Confer with your financial advisor and Medicare before you make any decisions, but you may qualify for a special enrollment period. People who continue to work past 65 may enroll in Part A but bypass Part B and D if they’re on a company plan.
Are your doctors on the plan? You’ve probably had some experience with this, but continue to check to see whether your physicians are in network on Medicare Advantage.
What can your state do for you? If you find you’re not getting answers on specific Medicare subjects from Medicare itself, then contact SHIP (State Health Insurance Assistance Program). SHIP offers one-on-one counseling on Medicare.

Your Balance and Mobility After 50 By Mike Zimmerman & Jessica Migala, AARP
W
ill my body get weaker as I age? That’s up to you; you have control over that, says Chad Landers, founder of Push Private Fitness in Los Angeles, who has trained older clients for 26 years. “The difference between people who work out and those who don’t isn’t that big when you’re 20 or 30. But in your 50s and beyond? It’s huge.” Any exercise is worthwhile, though Landers advises basic strength training as the bedrock for every older adult.
Am I too out of shape to exert myself? “I say you’re too out of shape not to exert yourself,” Landers adds. One study of more than 1,200 cases of sudden cardiac arrest in people ages 35 to 65 found that only 5 percent happened during physical activity. The bigger risk, experts agree, is lack of exercise. A massive study — of 334,000 people — found lack of fitness to be responsible for twice as many early deaths as obesity, and even introducing moderate activity can reduce your risk by 16 to 30 percent. “The oldest client I ever had started with me at age 87, and she increased her strength well into her 90s,” Landers says. “Too out of shape isn’t a valid excuse for anything.”
walking lunges for each leg. Detour your route to include hills or stairs. Or sub out one walk per week for a swim in the local pool. And as mentioned, strength training is key as you age; can you mix in some fitness for above your hips? Do at least two days a week of resistance training, including pushing and pulling exercises for both the upper and lower body.
Weight lifting is a total turnoff. What should I do instead? You don’t have to think of strength training as lifting weights in a gym. “All you need to do is add resistance to movements, or simply work against gravity during your day,” Landers says. Carrying a laundry basket up stairs, walking to the end of the driveway and back carrying full gallon jugs and squatting to clean under furniture all count as strength training. Using stretch bands at home is another low-cost, highly effective way to train. And you can do a full-body workout with push-ups, sit-ups, squats and lunges — no weights required.
Is my daily walk enough exercise? Yes, if you’re walking briskly. The American Heart Association recommends 150 minutes of moderate-intensity exercise per week; five brisk 30-minute walks makes the cut. But why stop there? “Your body can do more than just walk,” Landers notes. “Add little challenges that won’t take up extra time.” Every five minutes, do five

of motion, protects knee cartilage and reduces the stress you place on the knee, says Richard Willy, an assistant professor of physical therapy at the University of Montana School of Physical Therapy and Rehabilitation Sciences. Get in the habit of doing squats and lunges twice a week, making sure that your knees stay above your feet and don’t extend in front of your toes. After age 50, avoid squatting below a 90-degree angle (with your hips dropping below your knees), because deep squats increase pressure on the knees, Cardone warns. 4. Perfect your posture. “People often slouch or get more stooped over as they get older,” Cardone says. This is problematic because poor posture changes your body’s center of gravity, placing added stress on your knees and hips. So make a point of standing tall, with your head in line with your shoulders, your shoulders directly over your hips, your hips aligned with your knees, and your knees aligned with your feet. 5. Choose the right shoes. Wearing supportive, comfortable shoes promotes proper alignment of the joints in your lower extremities as you move, explains Barton Branam, M.D., an assistant professor of orthopaedic surgery at the University of Cincinnati. For exercise, choose kicks that suit your activity — say, running shoes for running — and that are appropriate for your gait and foot (whether you pronate or supinate, for example), Branam advises. Believe it or not, research suggests that postural abnormalities of the feet can contribute to knee osteoarthritis, which is why it’s important to wear shoes that help prevent inward or outward rolling of the feet during movement. If you’re in the market for new exercise footwear, it’s a good idea to get professionally evaluated and fitted by someone at a top-notch running or sporting goods store, Branam suggests.
If you’ve walked, jogged and hiked this far without any major knee trouble, there’s more you can do than thank your lucky stars. A few basic steps can help you protect your knees as you age. Besides being the largest joint in the human body, “the knees are unique in that the motion involved is very complex,” says Sanjeev Bhatia, an orthopedic sports medicine surgeon and codirector of the Northwestern Medicine Hip and Knee Joint Preservation Center at Central DuPage Hospital in Winfield, Illinois. With the passage of time, a certain amount of wear and tear on your joints is inevitable. But it doesn’t have to affect the way your knees feel, function or move. Otherwise, to minimize your risk of experiencing pain, stiffness and inflammation in your knees, take these steps to protect these essential joints. 1. Maintain a healthy weight. Toting around excess weight places tremendous pressure on your knees. “For every pound of weight you put on, the knees will have four more pounds of force on them and even more than that when you go up or down stairs,” says John-Paul Rue, an orthopedic sports medicine surgeon at Mercy Medical Center in Baltimore. The flip side is true, as well. For every pound of excess weight you lose, you’ll spare your knees from having to cope with four additional pounds of extra force. 2. Keep moving. Regular physical activity helps maintain joint function, including strength and range of motion in the knees, which “means less force gets applied to the knee,” Bhatia explains. Although it used to be believed that high-impact activities such as running are bad for the knees, the latest evidence shows that’s not necessarily true. But there is a sweet spot for runners: A meta-analysis of 17 studies, published in the Journal of Orthopaedic & Sports Physical Therapy, found that recreational runners had a much lower risk of developing osteoarthritis of the knee than competitive runners and sedentary people did. It’s also wise to mix up your workout routines; so if you run three times a week, do something low-impact, like bicycling, Pilates, swimming or using the elliptical machine, on the in-between days. 3. Strengthen the muscles that support your knees. Developing strong thigh muscles — especially the quadriceps, hamstrings and abductors — improves range

Give your knee the RICE treatment — rest, ice, compression and elevation — and take an anti-inflammatory medication such as ibuprofen, Rue advises.
