New to Medicare? Here’s an overview

I

f your 65th birthday is around the corner or you’re anxious about the Medicare open enrollment period, it’s a good time to start focusing on your options.

Health care choice has become a bigger factor in the lives of pre-retirees since the Affordable Care Act. Though a separate federal health insurance system with no connection to Obamacare or its online marketplaces, Medicare is going through its own evolution in terms of plan offerings and customer access.

Here’s a basic primer for future Medicare enrollees: What is Medicare? Medicare is a governmentprovided health and hospitalization insurance program for people 65 and older and for some people under age 65, based on disability or particular forms of illness.

What does it cost? Though you’ve likely paid taxes into the Medicare system your entire career, Medicare isn’t a completely free program. You’ll pay premiums deducted from your Social Security checks for some portions of your benefits. There may be copays and deductibles for certain services.

When do I get to join? You can apply for Medicare through your nearest Social Security office up to three months before your 65th birthday. If you wait longer than three months after your 65th birthday to apply, you may face a late enrollment penalty.

WHAT DOES COVERAGE LOOK LIKE? • Parts A (Hospital) and B (Outpatient). Part A covers inpatient hospital care and Part B covers outpatient medical care and procedures. You’ll hear Parts A and B referred to as “Original Medi-

care” because if you choose to take this option, you’ll sign up for them as a pair. You’ll get to go to any doctor, hospital or facility that’s enrolled in Medicare and accepting new Medicare patients. You’ll end up paying a premium for Part B, which can change from year to year while Part A is usually free.

• Part C (Medicare Advantage). Call this one-stop shopping for your hospital, outpatient (Part A and B) and usually your prescription coverage (see Part D below).

Medicare Advantage policies are sold by private insurers. Medicare pays their share and you pay your premium. They may include different assortments of services, including vision, prescription drug, dental or hearing.

But keep in mind, different Medicare Advantage plans have different rules for services and can charge different out-of-network costs.

• Part D (Prescription drug coverage). Part D added prescription drug coverage to Original Medicare but may be folded into Medicare Advantage coverage if you elect to take it.

What’s Medigap insurance? If you go with Original Medicare (Part A and B), you’ll find some gaps in your Medicare coverage. Medigap is the nickname for Medicare Supplement Insurance, which you purchase privately. If you’re on Medicare Advantage, it’s unlikely you’ll need separate Medigap coverage but it’s important to check.

What if you plan to keep working with company health benefits after you turn 65? Confer with your financial advisor and Medicare before you make any decisions, but you may qualify for a special enrollment period. People who continue to work past 65 may enroll in Part A but bypass Part B and D if they’re on a company plan.

Are your doctors on the plan? You’ve probably had some experience with this, but continue to check to see whether your physicians are in network on Medicare Advantage.

What can your state do for you? If you find you’re not getting answers on specific Medicare subjects from Medicare itself, then contact SHIP (State Health Insurance Assistance Program). SHIP offers one-on-one counseling on Medicare.

Staying active helps reduce cognitive decline

M

ost of us don’t get enough exercise, and we have any number of excuses. Too tired.

No time. Too expensive. Lack of willpower. And, 25 percent of us say we’re simply satisfied being sedentary.

Does this sound familiar? If so, you’re far from alone. The majority of us age 40-plus are not logging the recommended 2.5 hours per week of moderate to vigorous physical activity that’s good for us, according to AARP’s latest healthy aging survey.

It would strain credulity to claim we don’t know any better. We’re saturated with messages promising the well-documented benefits of exercise: better sleep, better weight, better heart, better mood. Yet, most of us persist with the excuses.

What if I told you that the stakes of not exercising are higher than you might have thought? What if I told you that physical exercise is good not only for the way you look and feel, it’s also good for your brain — good for raising the odds that you’ll stay mentally sharp and reduce the risk of cognitive decline?

It’s true. Staying active is key to maintaining our brains. Getting regular exercise can even change our brain structure and improve its functioning.

We’ve long suspected that exercise was an important component of brain health, and it’s been a point of focus of AARP’s work with Age UK and the recently launched Global Council on Brain Health (GCBH). An early yield of that collaboration is a new evidence-based consensus report by GCBH, saying definitively that brisk walking, cycling, strength training and group exercise classes can provide benefits to our brain health, as well as our overall health.

Based on the scientific evidence, the GCBH concluded that: • Physical activity has a positive impact on brain health.

• People can change their behavior to become

more physically active at any age.

• People who participate in purposeful exercise show beneficial changes in brain structure and function.

• People who lead a physically active lifestyle have a lower risk of cognitive decline.

Although we do not yet have sufficient evidence that physical activity can reduce the risk of brain diseases that cause dementia, including Alzheimer’s disease, other brain benefits are clear-cut.

People who exercised on a regular basis ranked their brain health higher than those who didn’t exercise. In fact, adults who exercised regularly reported higher levels of improvements in their ability to solve problems, manage stress, learn new things, pay attention and remember things in the past five years compared with people who did not regularly exercise. Many adults (67 percent) reporting problems remembering things do not get the recommended amounts of exercise.

For the sake of your brain — in the interest of staying mentally as well as physically sharp — let’s make an extra effort to get moving!

Heart attacks in women: Gender-specific knowledge could save your life

ccording to the Center for Disease Control, heart disease is the leading cause of death in women in the U.S. One in four women will die of coronary disease, and almost two-thirds of these deaths happen with no previous warnings or symptoms. Many subtle symptoms can persist for days or weeks before a heart attack. Would you know if you were having one?

Because of their high tolerance for pain, women will often ignore the early warning signs, calling it an anxiety attack or indigestion. They can miss the opportunity to stop it before it becomes life threatening.

Recognize the warning signs

“Women have a higher tolerance for visceral [internal organ] pain because of childbirth,” said Dr. Caroline Dorman of St. Mary’s Family Medicine Center. “Men think ‘I’m having a heart attack.’ Women tend to downplay it.”

Additionally, people with diabetes and the elderly may have mild symptoms even when a heart attack is in process, which makes them even more prone to overlook warning signs.

“I once had a woman present to me with pain in her thumb,” said Dorman.

There was no sign of anything wrong with it, so Dorman asked her to describe her pain.

“She pointed with her finger, running it from her thumb up her arm to her chest. ‘It goes all the way up here, and it’s kind of crushing.’ So she was having a heart attack and calling it pain in her thumb!” Dorman said.

Another common sign is pain in the back. Many women, especially those with chronic low back pain, will dismiss pain that is just a bit higher along the spine.

“I have women who will tell me they are having an anxiety attack,” Dorman said. “I ask if they’ve ever had one before, and they’ll say no. That’s usually a sign that it’s not an anxiety attack.”

When you suspect it’s a heart attack

Call 911. You may only have minutes before you lose consciousness. The Mayo Clinic guidelines suggest chewing and swallowing an aspirin if you’re not allergic. Take nitroglycerin, but only if it has been previously prescribed by a physician. If you have someone drive you to the emergency room, have him or her stay as your advocate and insist that you are seen within 10 minutes of arrival. Tell the emergency room staff, “I think I’m having a heart attack.” Don’t be afraid to be a bother.

The long-term outcome after a heart attack, according to Pittsburgh cardiologist Dr. Richard Fogoros, largely depends on how quickly you get medical care. A heart attack, or myocardial infarction, stops blood flow to the heart, causing death of the heart muscle. The first few hours after a heart attack are crucial. Increasing blood flow to the heart minimizes the amount of heart muscle affected by the heart attack, which in turn, increases the probability of recovery and the extent of long-term implications.

Even with early recognition and expert medical care, a heart attack means that there has been some damage to your heart. Eating well and exercising can add to your quality of life after a heart attack. Doing these before a heart attack is an even better idea.
