Letter from the President DEAR NEIGHBORS, Spring is here and with the change of the season comes many exciting announcements from Bayhealth. We are in the process of launching graduate and undergraduate medical education programs that will move Bayhealth toward becoming teaching hospitals. The announcement is an important one for the community and health system, as this residency training program will assist in bringing more doctors to the area, enhance the culture of learning at Bayhealth and foster improvements to patient care and the patient experience. Head to page 4 to learn more.

Other big changes are blooming as we expand many of our services at Bayhealth. Turn to page 6 to learn more about our growing Neurosciences service line.

This growth will allow greater access to neurosurgery and neurology for Bayhealth patients. We are also expanding within our Division of Orthopaedics.

But sometimes those changes are hard to make.

Instead of expecting big changes at the start of each year, we believe each and every Monday gives each of us a chance to start fresh and hit the reset button.

I’d be remiss not to mention our new Bayhealth Sussex Campus. We moved to the facility that’s just off of Route 1 in Milford on Feb. 5, 2019. Our youngest community members were engaged in the process too — showing their excitement for the move in essays and through art pieces as part of our Art and Essay Contest. Mentoring our future physicians BAYHEALTH RESIDENCY PROGRAMS ARE COMING JULY 2021 Bayhealth is pleased to announce that it has begun the process to launch residency teaching programs for doctors who have recently graduated from medical school — called Graduate Medical Education (GME). The announcement is an exciting one for the community and health system, as this residency training program will assist in bringing more doctors to the area, enhance the culture of learning at Bayhealth, and foster improvements to patient care and the patient experience.

“Residency programs create the opportunity to provide more physicians to serve the community.

Once the physicians have completed their residencies, there is a higher chance they will stay in the community where they trained, resulting in greater physician retention,” said Bayhealth President and CEO Terry Murphy, FACHE. “Developing the next generation of physicians to serve our communities is a priority for Bayhealth.”

Bayhealth Surgeon and GME Chair Assar Rather, MD, FACS, FASCRS; Senior Vice President, Chief Medical Officer and GME Designated Institutional Official Gary Siegelman, MD; administrative leaders; and a group of more than 20 physicians have worked for the past year to establish the GME.

A draft assessment and full implementation plan was developed and outlines a proposed start date of July 2021 for family medicine and internal medicine programs, with general surgery and emergency medicine to follow in subsequent years.

“This is truly an exciting time for Bayhealth as we look toward our future,” said Dr. Siegelman.

“These medical education programs are an investment not only in Bayhealth’s future, but in southern Delaware’s communities as well.”

“Residency programs are a great way to recruit new physicians to the area and support growth within a hospital system,” said Dr. Rather. “I’m excited to assist Bayhealth in recruiting new physicians to care for the present and many future generations of central and southern Delaware to come.”

“This will be a great opportunity, and it will allow us to educate the physicians of the future to meet the needs of our community. I am excited to be a part of developing this program,” said Primary Care Physician, UME Committee Chair and GME Committee Member Joseph F. Rubacky, III, DO, FAAFP. ■

In an effort to broaden medical education offerings, Bayhealth is also partnering with area medical schools to host medical students for a portion of their Undergraduate Medical Education (UME) clinical rotations. Upon graduating medical school, these students may later apply for one of Bayhealth’s residency programs.

Growing the Bayhealth braintrust in Neurosciences It’s been eight years since Bayhealth introduced its neurosurgery program, the first of its kind in central and southern Delaware. The Neurosciences service line is comprised of the Department of Neurosurgery and the Department of Neurology, including Primary Stroke Centers at both Bayhealth Hospitals, Kent and Sussex Campuses. With significant growth over the past year, the service line is poised to meet the complex needs of area patients and is expanding its offerings.

“A larger team has allowed us to advance each of our Neurosciences specialties and improve access,” explained Senior Director of the Bayhealth Neurosciences Service Line Tom Cairo, MBA. Neurosurgery Medical Director Dawn Tartaglione, DO, FACOS, joined Bayhealth in November and “brings extensive knowledge and experience to strategize as we take the department to the next level,” said Cairo.

Bayhealth Neurosurgery has grown from one to three neurosurgeons supported by five advanced practice providers. The neurosurgeons perform surgeries at Bayhealth Hospital, Kent Campus to treat cranial and spine conditions, brain or spine trauma and hemorrhagic stroke, among other conditions. They are increasing the use of endoscopy, a sophisticated minimally invasive technique to access parts of the brain, often through the nose or mouth, to remove tumors without an incision. For this and other procedures, the Bayhealth team uses advanced image-guided navigation, which provides real-time images of what’s shown in a patient’s MRI, optimizing precision and patient safety.

Bayhealth’s Department of Neurology now has several neurologists and advanced practice providers on staff providing specialized care on an inpatient

and outpatient basis, and a certified Stroke Program that has won national awards through the American Heart Association and the American Stroke Association. Medical Director for Stroke and Neurology Sumeet Multani, MD, says the ability of their team to diagnose and manage a myriad of neurological conditions, including but not limited to migraines, dizziness, Alzheimer’s, Parkinson’s, multiple sclerosis, and several recent cases of rare cerebrovascular diseases, speaks to the strength of the program. “Our outpatient stroke clinics are keeping our community healthier by providing follow-up care and education to help patients with their conditions and prevent reoccurrence of stroke and other medical issues,” said Dr. Multani.

Continuity of care is an important aspect that has been facilitated by an expanded team. The doctor that a Bayhealth patient would see in the hospital for neurological or neurosurgical care would be the same one they’d see for appointments after discharge. This is an important benefit, particularly to those who have been through serious neurological trauma or complex operations, as it develops trust and builds a strong patient-doctor relationship. There are times when care between neurology and neurosurgery overlaps.

Bayhealth’s multidisciplinary approach, where the two specialties work collaboratively and in coordination with the emergency department and other departments, provides the best outcomes for patients, explained Dr. Multani.

Increasing their outpatient presence, Bayhealth Neurosciences is able to serve more patients in our community. Appointments are available in Dover daily and at the clinic in Lewes at least twice a month. Cairo says they’ll gradually ramp up scheduling in Lewes, based on patient needs, and have recently hired another physician assistant to meet demands.

What to expect at a sleep study Over 65 million American adults — more than onethird of the population — have trouble sleeping, and over 100 sleep disorders have been identified. One of the most common disorders is sleep apnea, with roughly 90 percent of those affected undiagnosed.

Symptoms include loud snoring, not feeling refreshed after a night’s sleep and daytime sleepiness.

If this sounds familiar, talk to your doctor. He or she may examine you, ask about your medical history and have you fill out a sleep questionnaire.

If you are at moderate or high risk for sleep apnea, your doctor may order a comprehensive sleep study, also known as an in-lab study, an overnight study or a polysomnography (PSG), which is performed in a sleep center. A PSG monitors a number of bodily functions during your sleep.

A less comprehensive sleep study, also referred to as a home sleep study or home sleep apnea test (HSAT), may be ordered if you have no underlying chronic or latter-stage medical conditions.

DETAILS OF A SLEEP STUDY • Physiological functions including brain waves, muscle and eye movement, oxygen levels, and heart rhythms are monitored and tracked during a PSG.

• Most patients sleep well in the sleep center and are able to move around in the bed, and may even get out of bed when necessary, perhaps to use the restroom.

• Sleep technologists carefully review the data and compile a report with the results and send it to the referring doctor; the technologists are not allowed to disclose test results to patients.

• The referring doctor will present the study results to the patient, unless the referring physician requested a follow-up consultation by a sleep specialist.

At Bayhealth Sleep Centers, patients undergo testing in a private room with a homelike environment that includes a bed as well as a recliner and flat-screen TV. Bayhealth Sleep Centers, which are accredited by the American Academy of Sleep Medicine, offer PSGs, HSATs and other types of sleep studies.

• Breathing, abdominal and chest movement and snoring are monitored.

• All data are collected using painless, noninvasive electrodes and sensors, and in the case of a PSG, also via images from cameras in the testing room.

• PSGs are conducted and monitored by licensed sleep technologists, who are highly trained sleep study technicians that will help ensure a conclusive sleep study.

• It takes about 45 minutes to an hour for the sleep technologist to hook you up to the monitoring equipment. Once the technologist has made all the proper connections, he or she will go to their monitoring station in another room.

Self-care

for parents

When you’re on a plane with your child or loved one during an emergency, flight attendants instruct you to place your own oxygen mask and then assist the person next to you.

The reason for this is simple — if you don’t take care of yourself, you can’t help others. Bayhealth Primary Care Physician Lisa Reid, MD, says the same is true for parents and caregivers of young and old.

“There’s an expectation for some parents that they have to be perfect at everything — their job, doing everything with their kids, maintaining the house, doing laundry. We need to remember it’s OK to say no. It’s OK to take time for yourself,” Dr. Reid said.

How can parents become better about taking care of themselves? Dr. Reid recommends the following self-care tips to her patients.

MAKE A COMMITMENT TO SELF-CARE Self-care doesn’t mean you take an entire day to yourself; maybe it’s one hour or merely five minutes. Either way, make time for yourself.

ASK FOR HELP Ask someone — family members or a local college student — to watch your children for a couple of hours, whether it’s to unwind or focus on cooking a meal. We all need help sometimes and it’s OK to ask for it.

TAKE CARE OF YOUR HEALTH This includes eating right, practicing good sleep habits and exercising. Living a healthy life reduces stress and makes you feel better.

FIND THREE THINGS TO LOOK FORWARD TO Living the same day over and over again can lead to burnout. Find three things to look forward to, to get out of mundane schedules. These could include a vacation, a trip to the mall or a visit to the spa.

FIND A HOBBY Pick a small activity that you like to do and make time for it. It could be painting, meditating, reading, scrapbooking, or anything else that you enjoy.

GO ON DATE NIGHTS It’s important for couples to make time for each other and to continue nurturing their relationship.

TAKE A BREAK FROM SOCIAL MEDIA Social media is a great way to stay connected to people, but can lead to unrealistic expectations and comparisons. Periodic breaks from social media can help a person’s mental state.

UNDERSTAND KIDS GO THROUGH PHASES Whenever you’re overwhelmed with your children, take some deep breaths and remember that “this too shall pass.” They’re going to grow up and move out so it’s important to enjoy the time you have.

Along with these self-care tips, Dr. Reid also tells her patients to let the little things go. “Let go of the stress of not getting everything you want done and the need for perfection. Worry can lead to more serious health issues,” Dr. Reid said. ■ Talking to your doctor about self-care can help you get back on track. STEPS program becomes PACE Bayhealth launched its STEPS to Healthy Aging Program (STEPS) over 20 years ago. “At first, it was mostly a walking program, where people met at the Blue Hen Mall,” explained Bayhealth Clinical Educator Terry Towne, MSN, RN-BC, NE-BC. Over the years, blood pressure monitoring clinics and lectures covering a variety of health topics were added.

Last fall, Bayhealth’s Education department decided to revamp and rename STEPS to better reflect what the program is about. The new PACE Program — PACE stands for Promoting Active Community Engagement — began in January. It includes monthly clinics with individualized counseling from a registered nurse based on each participant’s health problems, medications and blood pressure, as well as quarterly lectures.

Unlike STEPS, which required participants to register and was for those age 50 and older, PACE is available to anyone interested in living a healthier life. However, as Towne explained, a top goal is to involve more of the underserved and homeless populations, as well as those with limited or no healthcare options. That’s why monthly clinics at Dover Interfaith were added.

“PACE targets people with chronic conditions such as diabetes, obesity, COPD, and depression, those at

risk for heart attack and stroke, and those dealing with tobacco, alcohol and drug abuse,” said Towne. “The program is designed to better educate and engage them in their health to promote more self-care, which will ultimately help lower readmission rates.”

Currently, each PACE clinic is staffed with two educators. Plans are underway to put participants’ charts into the electronic health record system, Epic, which will save time and make it easier for staff to track changes and for providers to better communicate, as they all can access the charts.

Towne says while participants may go to different clinic locations, for the best outcomes they’re encouraged to attend one location.

PACE is free, registration is not required, and participants may invite others to join them. Clinics are held monthly from 9 to 11 a.m. on designated dates in Dover, Lincoln and Milford. First-timers receive a full health assessment that covers lifestyle habits, as well as weight, blood pressure and pulse rate/oximetry. Lectures are held quarterly. Moving a hospital Following years of planning, building and preparation, the Bayhealth Hospital, Sussex Campus opened to patients on Feb. 5, 2019. The Bayhealth Outpatient Center, Sussex Campus quickly followed on Feb. 6, 2019. In addition to opening the new campus, leaders at Bayhealth also had to plan for the patient move from Bayhealth Milford Memorial to the new location. These plans were carefully coordinated by hundreds of employees who all took part in the planning and implementation.

Bayhealth first announced plans to build the new health campus in late 2014. At the announcement, Bayhealth’s President and CEO Terry Murphy, FACHE, explained that “healthcare is changing, and at the same time so are the needs of our community.

After evaluating the direction of healthcare and conducting detailed research, we realized the community needs more than a hospital.”

The new location was selected because of its visibility, access to Route 1 and space to grow.

“From the beginning, we shared our vision that the parcel of land that was purchased would eventually turn into a thriving healthcare campus,” said Vice President of Operations/Administrator, Bayhealth Sussex Campus Michael Ashton, FACHE. “We challenged the architects and construction partners to show us what this would look like built out to ensure this would be a successful project.”

The project involved careful coordination on many levels that involved staff and provider input throughout the process. Approximately 500 employees and medical staff members took part in designing the hospital and outpatient center.

These team members also developed new work flows that would be needed in the new facility.

The community was also consulted. More than 300 community members took part in the “cardboard city” tours. These tours allowed community members to walk through cardboard mock-ups of key areas that would be built in the hospital. There were also town halls in various communities and a furniture demonstration. The community continued to support Bayhealth during the open house events once the hospital was built. More than 8,000 people toured the new facility.

“Our guiding principles drove the project,” said Ashton. “Everything we did was viewed through the lens of the patient experience. Second to that, we focused on the provider and staff experience.

The team did a phenomenal job and took careful consideration at each stage of the process.”

The preparation for the patient move involved staff from nearly every department in the organization.

A comprehensive patient move plan was developed with each department or service area responsible for specific components of the move.

“Planning for the patient move created an incredible opportunity to collaborate,” said Senior Director of Patient Care Services, Sussex Campus and Magnet Program Director Andrea Holecek, EdD, MSN, MBA, RN, NE-BC, FACHE. “In addition to our plans on paper, we also practiced in real time. We held a full-scale physical mock move to ensure every system and every individual plan was ready.

We were able to gather feedback from this drill that helped shape the final plan and our approach to the patient and family experience.”

From infrastructure to patient care to food preparation, all team members from all facets of the health system played a pivotal role in the

patient move process. In total, more than 60 patients were transported from Bayhealth Milford Memorial to the Bayhealth Hospital, Sussex Campus on move day.

“The process of planning the patient move was the most incredible teamwork I’ve ever seen,” said Holecek. “Our team members gave 110 percent to ensure the patient move was a success.”

In addition to the patient move, there were also dozens of other individual and departmental moves to be considered throughout the two-week period surrounding the opening. “Everyone knew we had a responsibility to this community and to our patients,” said Ashton. “It was our responsibility and our mission to complete the patient move in the safest, best way for everyone.”

Most team members involved in the move describe the process as a once-in-a-lifetime experience.

The collaboration and coordination among different departments and team members were on full display throughout the planning and implementation of the moves.

Student artwork and essays provide fresh perspective The once-imagined Bayhealth Sussex Campus became a reality when it opened its doors in February 2019. The community had a front-row seat to the transformation of healthcare, which yielded a perspective on the historic project from a very important audience: our youth.

The Sussex Campus Art and Essay Contest invited students from surrounding elementary schools and middle schools to submit a work of art or short essay in response to one of the following prompts:

• Why do you think Bayhealth Hospital, Sussex Campus is an important part of our community?

• What does it mean to you or your family to have a hospital close to home?

Bayhealth received over 150 submissions from a mix of public, private and home schools. After each was carefully reviewed and scored, winners were determined for each grade, in each category, and prizes were awarded throughout the following weeks.

The contest was an overwhelming success thanks to the students’ willingness to share their stories and ideas. They creatively expressed gratitude, imagination and ambition in ways that only kids can, and contributed an untapped perspective on the Bayhealth Sussex Campus.

Bayhealth would like to congratulate all of the winners on a job well done, and would especially like to recognize the contest’s grand prize recipients, Natalie Lewis and Alli Parisi. Natalie, a 7th-grade home school student, earned the middle school grand prize with her outstanding artwork, and Alli,

also an artist, was awarded the elementary school grand prize as a 4th-grader at Lulu Ross Elementary.

Your child’s go-to when they need orthopaedic care Bayhealth Division of Orthopaedics is proud to now have two orthopaedic surgeons who specialize in pediatric orthopaedics. This is helping to meet a need in the local area for pediatric subspecialists.

In addition to taking patient appointments in their respective clinics, Justin Connor, MD, and Andrew Gambone, MD, each perform surgery and provide inpatient and outpatient care at Bayhealth Hospital, Kent Campus. They also support Bayhealth Hospital, Sussex Campus and the Emergency Department in Smyrna, providing patient consultations and emergency fracture care when needed.

Both doctors attribute their specialty expertise to top-notch children’s hospital orthopaedic programs and working alongside world-renowned pediatric orthopaedic surgeons. Dr. Gambone completed a fellowship in pediatric orthopaedic surgery at Children’s Hospital of Philadelphia followed by an orthopaedic sports medicine fellowship at Penn State University, which has strongly

equipped him to see patients of any age with a multitude of conditions.

Dr. Connor spent much of his career in Delaware, staying on with Alfred I. duPont Hospital for Children in Wilmington after his pediatric orthopaedic surgery fellowship there. He gained valuable experience as a surgeon in their neuromuscular orthopaedic division and in their spina bifida program. He also trained and served in their accredited Gait Lab, one of only seven in the country that focus on the study of children’s movement and walking patterns to diagnose and better treat the conditions that impact mobility.

Both Dr. Connor and Dr. Gambone are accustomed to treating broken bones, sprains and common orthopaedic issues in young patients. Both surgeons are also skilled in handling trauma and complex cases. But each has a unique area of clinical interest.

With his specialized orthopaedic sports medicine training, Dr. Gambone treats a lot of sports-related

injuries. He also does cartilage restoration procedures, a minimally invasive technique to stimulate growth of healthy cartilage, which has been known to delay or prevent joint replacement in a patient’s future. Dr. Connor works extensively with patients who have neuromuscular conditions, such as spina bifida, cerebral palsy and complex abnormalities that affect their ability to move or walk.

“It’s advantageous to go to someone who not only has additional and specialized training but also the broad experience of having treated many children and adolescents,” said Dr. Gambone. It’s a common misconception that children are just small adults with smaller-scale variations of bones and other body parts that can be treated similarly. Their growing and developing bodies, at all stages, present special considerations when it comes to orthopaedic care. These include congenital abnormalities and growth plate disturbances that can affect future development and pose problems later in life if not addressed properly.

Having pediatric orthopaedic specialists locally means that the vast majority of acute injuries, elective surgeries and hospitalizations can be handled at Bayhealth without having to refer pediatric patients to a facility further away.

“Not having to travel so far leads to better follow-up, and therefore better care,” said Dr. Gambone. It also ensures expedited care when accidents happen. “Because of the nature of their rapidly changing bodies, children should always be seen emergently or urgently after injury for the best outcome.

#Motivated Monday Challenge The Bayhealth Motivated Monday Challenge is a fun and engaging way to promote a healthy lifestyle. We believe starting healthy lifestyle changes shouldn’t be relegated to the month of January each year.

Instead we believe Mondays give each of us a chance to start fresh and hit the reset button. This is why Bayhealth is running a year-long healthy living campaign called #Motivated.

All of us are working on improving some part of our health.

We may be drinking more water, exercising more or getting our stress under control. Making these changes is hard; achieving them takes time and maintaining them takes commitment.

It’s not uncommon to take a few steps forward, followed by a few steps back. But that’s OK as long as you continue to work on those improvements.

That’s what our #Motivated campaign is all about — focusing on the positives and being nice to ourselves. At the end of the day it’s about making progress, not being perfect.

Healthy eating simplified Healthy eating habits don’t need to be complicated.

Bayhealth Outpatient Registered Dietitian Jinette Fellows, RD, LDN, offers tips on how to get started.

REPLACE REFINED GRAINS WITH WHOLE GRAINS “This is one of the easiest changes you can make and, therefore, is a great place to start,” said Fellows. “For example, instead of choosing refined grains, opt for whole grains when choosing breads, tortillas, buns, crackers, bagels, and pastas.” The benefit of this tip for healthy eating is an increase in the amount of fiber you consume, which helps with weight management, satiety, gut health, and lowering cholesterol. Being brown doesn’t make bread whole wheat. The only way to know is to read the nutrition facts panel to make sure whole-wheat flour is the first ingredient. Experiment with other whole grains such as barley, buckwheat, quinoa, or oatmeal.

UP YOUR INTAKE OF FRUITS AND VEGGIES Strive for five or more servings a day. Fellows says fresh, canned or frozen count. “Frozen and canned are a great option as long as they don’t have added sodium, sugar or fat,” she explained. “I also recommend trying different cooking methods.

If you’re getting tired of steamed vegetables, for example, try roasting them with some olive oil and cracked black pepper instead, and make them the star of your meal.”

“To ensure you are getting a wide variety of nutrients, you should eat fruits and vegetables from all of the different color groups, such as dark leafy greens like spinach, and orange veggies and fruits like bell peppers or oranges,” said Fellows.

“Try using non-animal sources of protein such as nuts, seeds, beans, and soy. People are often not in the mood or have no time to dice vegetables first thing in the morning. If you’re someone who likes to have omelets, try dicing peppers and onions ahead of time so they can be a quick addition.” Fellows also recommends getting the whole family involved in meal planning so everyone is on board and more likely to stick with healthy eating.

PLAN YOUR SNACKS “This will help you fill in food groups you’re missing,” she said. “For example, a lot of people don’t eat fruit with their meals. So a great snack would be apple slices with a tablespoon of peanut butter. Pairing a little bit of protein with other foods can also help tide you over until the next meal.”

MAKE A GROCERY LIST “Related to planning for meals and snacks, I tell people to take an inventory of the food items in their cabinets to avoid buying items they already have on hand, and this can be a good starting point to planning your meals for the week.”

DON’T FORGET YOUR FIBER Women should aim for 25 grams of fiber per day, while men should aim for 38 grams. ■ Bayhealth has both inpatient and outpatient dietitians.

A unique way to make monetary donations Holly Pfeifer, the widow of Donald Pfeifer, MD, is a longtime donor to Bayhealth. Most recently, she contributed to the Bayhealth Sussex Campus project, including a gift to dedicate the Medical Staff Lounge in the new hospital in Dr. Pfeifer’s memory. An orthopaedic surgeon, Dr. Pfeifer joined the medical practice of the late Edward F. “Terry” Quinn, III, MD, founder of The Dickinson Medical Group and former chief of surgery at Milford Memorial, when he and Holly moved to Milford from Baltimore in 1977.

they became friends. They had a lot in common, including their sense of humor. For example, they named their cadaver Napoleon Pick A Bonaparte,” she laughed. Holly says they soon connected with others in Delaware who shared their sense of humor.

“The Dickinson Medical Group had a Christmas party every year, and I remember Tom Draper coming in a tux and wearing sneakers,” she said.

A former home design and residential architecture freelance consultant, Holly also shared her excitement for the rare opportunity to freelance with architectural firm French & Ryan on the design of the third building for The Dickinson Medical Group — now known as Bayhealth’s Quinn Medical Offices.

The Pfeifers lived in Milford until Dr. Pfeifer passed away in 2001. At that time, Holly moved to Dover to be closer to her son and her church.

Although she has been donating to Bayhealth for a number of years, Holly recently changed her contribution method, and now makes regular donations through her Individual Retirement Account (IRA).

“If you have an IRA, once you turn 70½ years old, you have to do what is called a required minimum distribution, or RMD,” she explained. “If you don’t want to incur a penalty, you either need to transfer the RMD to another account or donate it. I also read that if you donate from your IRA directly, it’s not included as part of your gross income. I knew this was how I wanted to make my donations to Bayhealth going forward. I also can now donate one-third more than I could before.”

This giving option is great for those who can afford to donate their IRA’s RMD because it’s an easy way to give back to the community and is a win-win for all. ■
