Trigger finger, a common disorder of the hand, is a much easier name to remember than the technical term stenosing tenosynovitis which means inflammation and thickening of the tendon and its lining. For unknown reasons, the lining of a finger’s long flexor tendon becomes inflamed, and the tendon thickens into a localized oval mass just before it enters the base of the finger—still another sheath or tunnel through which the tendon must slide.

If the tendon grows larger than the sheath’s entrance, it can’t slide easily into it when the finger position changes from flexion to extension . occur. In some cases, pain at the base of the finger may be the only complaint, the trigger effect coming on later. By the trigger effect, I mean a flexed finger that one cannot extend easily and, when applying enough force to straighten it, seems to pop suddenly into extension.

The fingers most commonly subject to this condition are the middle and ring fingers and the thumb, although I have seen it in index and pinky fingers as well. Treatment in the form of antiinflammatory medications, injections of refined steroid, and stretching exercises will usually help. If the problem persists, outpatient surgery to release the tunnel (under local anesthesia) and allow the thickened tendon to slide easily is & Insured generally very successful. –itis) where the long (metacarpal) bone at the base of the thumb meets one of the carpal bones, the trapezium. This disorder is very common, especially in women forty years of age and older.

Pain, usually directly over the joint, with radiation into the thumb, occurs with pinching action, as happens when grasping a key to turn it in a lock, turning a tight doorknob, or opening a jar. As the arthritic condition progresses, swelling and deformity may appear, and, in the worst cases, the base of the thumb may start to slide outward over a flattened trapezium, moving the rest of thumb’s metacarpal bone closer to the index finger.

For some patients, the pain, deformity, or both can be quite debilitating despite conservative management by medication and cortisone injections. When all else fails, surgery can be considered to replace the trapezium with a rolled-up tendon or fusion of the metacarpal bone to the trapezium. Other procedures are available and what works best in your physician's hands is the best course of action.

Mallet Finger Mallet finger is the result of a rupture

of the extensor tendon that straightens the finger’s outermost joint, the distal interpahalangeal (DIP) joint. The typical injury happens when one jams one’s finger downward, causing the tip of the finger to droop in a deformity that looks like a small hammer, hence the word mallet. Sometimes, only the tendon ruptures, but occasionally a piece of bone is pulled off with it. Appropriate treatment calls for a splint to keep the finger extended for six weeks. If the pulled-off piece of bone is large enough, it may cause the DIP joint to become unstable and crooked, necessitating a surgical repair.

Many of these injuries go unrecognized because initial swelling masks the deformity, but as the swelling subsides, the deformity becomes more obvious. Little functional disability accompanies an untreated mallet finger, though a drooping of the joint may be a nuisance for a while until the patient becomes used to it. I know a hand surgeon who has two fingers with such a deformity, neither one ever having been treated. As an internationally known surgeon specializing in the shoulder, arm, and the hand, he gets along very well in spite of these pesky conditions.

Malone Electrical Solutions, LLC today announced Lorraine Carter as the new vice president of operations for the company.

Carter joins Malone after working at the Culpeper Chamber of Commerce for five years, where she served as marketing/ communications coordinator.

“I’m very excited about this new adventure and I wish future success for the Culpeper Chamber of Commerce in their new direction,” Carter said.

Carter resides in the White Shop area of Culpeper County with her husband John, her daughter Katie and son John Jr. She is active with the Culpeper County Republican Committee, and fosters felines.

Fauquier Hospital Acknowledged in Virginia Living’s Top Hospitals 2019

Culpeper Medical Center part of flu restrictions Out of concern for the health and well-being of our patients, Novant Health UVA Health System is asking the community to keep visitors age 12 and under out of all acute care facilities, unless seeking treatment, due to the widespread outbreak of the flu virus throughout the area. Restrictions are effective beginning at 7 a.m. on Wednesday, Jan. 8, and last until further notice.

Restrictions apply to the following Novant Health UVA Health System hospitals: Novant Health UVA Health System Culpeper Medical Center Novant Health UVA Health System Haymarket Medical Center Novant Health UVA Health System Prince William Medical Center “The flu virus can be extremely dangerous to people who have compromised immune systems, cancer, kidney disease and other chronic conditions,” said Jon D'Souza, MD, Chief Medical Officer, Novant Health UVA Health System Culpeper Medical Center. “Visitor restrictions protect our patients and reduce the likelihood that children who are visiting might encounter the flu.”

People who are seeking treatment at hospitals are not subject to the restrictions. However, individuals age 13 and older who are experiencing runny nose, sore throat, fever or cough are also encouraged not to visit patients being treated at Novant Health UVA Health System facilities.

My dream vacation is to visit Costa Rica. Growing up, I loved the summer, going to the beach, going swimming, and exploring anywhere tropical. Costa Rica is known for it’s misty waterfalls, white beaches and dense jungles. It also has a wide variety of wildlife such as ocelots, toucans, sea turtles, giant manta rays, and tree frogs. In fact, Costa Rica is known as one of the happiest countries in the world. I look forward to exploring this country and all it has to offer.

I would also love to visit Italy. It would be intriguing to learn about it’s culture and history. I am interested in visiting different cities within Italy to examine what the lifestyle is like. There are several places to explore in Italy, such as castles, the Colosseum, and the city of Rome. Italy seems like a classy, eccentric country which I would love to discover.

Commissioner of the Revenue’s Office achieves accreditation ➤ Yowell honored at 100th Annual Conferece of Commissoners of Revenue Contributed report Culpeper County Commissioner of the Revenue Terry Yowell was recently honored at the 100th Annual Conference of the Commissioners of the Revenue Association of Virginia by becoming one of the first localities in the Commonwealth to achieve office accreditation.

Accreditation is awarded only to offices which have met the rigorous standards set forth by the Commissioner’s Association.

The 12 standards set forth by the Association include education requirements for the Commissioner and staff, customer service, personnel and ethics policies as well as assessment methodology.

Each office receiving accreditation has undergone an audit to ensure compliance and will

be required to demonstrate they continue to meet these standards annually.

“Commissioner Yowell has demonstrated a commitment to Culpeper County and to good governance. This is an honor that was earned through hard work and leadership. It signifies that the Culpeper County’s Commissioner of the Revenue office has met the highest standards of professionalism as set forth by the Commissioners of the Revenue Association of Virginia,” stated The Honorable Thomas Blackwell, 2019-2020 President of the Association.

“The Commissioners of the Revenue Association of Virginia has always been at the forefront in seeking to improve professional skills and overall performance for the good of the Commonwealth and it’s localities,” Blackwell added.

“In this, our centennial year, commissioners from around the state have worked to attain Office Accreditation in order to ensure greater accountability and increased uniformity with respect to office procedures for all our taxpayers.”

The Association launched a Career Development program in 2000 to encourage professionalism and promote consistency in the administration of assessment throughout the Commonwealth.

In addition to Commissioner Yowell, four out of seven full time staff have achieved Master Deputy designation and three are currently working toward their

master designation.

When absolutely everything in your life leads you in a specific direction, you should pay attention and follow that path. John Bosworth did just that, taking all the ingredients through the years, stirring them in a pot and developing a recipe to educate, inspire and create opportunities for youth.

Bosworth, 24, is the founder and CEO of Love Learning Music, a non-profit organization based out of Nashville, Tennessee that utilizes music to engage and improve lives of individuals and communities.

Bosworth, born and raised in Orange, Virginia, has always been musically inclined. He has played piano since age seven and as a result of his “creative spirit” he taught himself the flute, drum, guitar, digeridoo and numerous other instruments over the years. With a desire to leave Orange and spread his wings, Bosworth travelled to Tennessee to attend college and it was then that his focus became clear.

In 2012 Bosworth did his thesis project on the difference that music education makes in the development of children.

“Everything in my life had led to this; my defining moment. I found out that music is the best element to improve the development of children,” said Bosworth. Beaming with enthusiasm he shared how he met Jesus that same year.

“I met Him in person. I began composing music immediately after and dove head first into music.”

Love Learning Music emerged in 2016 though Bosworth had been developing it since 2012 while working with an after-school music program at the Boys and

Girls Club in Orange. Equipped with two full time staff, eight teachers and fifty volunteers, LLM provides lessons, music therapy and classroom instruction. They provide these outreach services in schools, homes and other requested sites. “It’s about meeting people where they are, physically and mentally,” said Bosworth.

While most of their services are tailored for children and youth with special needs, the organization does provide music lessons to adults. “We can train adults and they can train their kids and then that effect spreads,” said Bosworth.

LLM has a music education app and Bosworth said they are looking forward to expanding their video resources. Working with many children and youth with special needs, they will soon include children who have experienced trauma.

Bosworth is in awe when he shares how much LLM has grown since its inception in 2016. With locations in Nashville and Murfreesboro, Tennessee the organization is spreading into the Richmond area soon as well as other communities in Virginia.

Bosworth said this is the season to build.

“We are currently building on the foundation to franchise,” he said.

LLM has begun outreach training and initiated an International department this year. “We already have academies in place in West Africa, Central Africa and in Haiti, Nicaragua and El Salvador. We are equipping communities to thrive on their own, so they aren’t just dependent on us over here,” said Bosworth.

A sudden winter storm gave Culpeper it's first snowfall of 2020 Tuesday as approximately four inches fell throughout the county beginning around 10 a.m. The snow quickly accumulated leading to multiple accidents in the town and county.

In Fauquier County, a crash involving a Fauquier County Public School bus led to a fatality, according to Virginia State Police. No children were in the bus at the time of the crash.

FILM • “Musical Highlights from The Smothers Brothers Comedy Hour and The Hollywood Palace” (1964-1970) This compilation of musical guests on “The Smothers Brothers Comedy Hour” and “The Hollywood Palace” was curated from the Library of Congress Television Collections to be a good representation of the British Invasion, Motown, 1960’s Pop, Folk and R&B and the San Francisco Psychedelic Rock scene. “The Smothers Brothers Comedy Hour” aired on CBS from 1967 to 1969 and was a somewhat "hip" version of the typical comedy-variety show of its era. The series show cased new musical artists that other comedyvariety shows rarely gave airtime to, due to the nature of their music or their political affiliations. The hour-long weekly variety series “The Hollywood Palace” was broadcast on ABC from 1964 to 1970 and unlike similar programs such as “The Ed Sullivan Show,” the series used a different host each week. A number of popular music performers got their start on “The Hollywood Palace,” among them The Rolling Stones, who made their first US television appearance on the episode that aired on June 6, 1964.

VIEWS Special Needs Trust – An Estate Planning 'Superpower' ESTATE STEWARDSHIP

Estate planning “superpowers!”

Yes, they do exist.

One of them is Katherine Charapich a Special Needs Trust (SNT). In the world of estate planning, a SNT is an imperative consideration for those who are recipients of needsbased government benefits that are means-tested.

Think of a SNT as an avenue to provide supplemental support to an individual who has a disability and who is a recipient of government benefits that are only awarded as long as her assets are valued at certain amount.

For example, if an individual has a disability that falls within an area defined as deserving of government assistance, but that has her own resources with an asset value above the set amount, or she inherits assets that would increase her asset value above the set amount, both precluding her from being a recipient of the needsbased government assets, a SNT may be a consideration.

The Code of Virginia, § 64.2-779.10.

Trust for beneficiary with disability, defines the parameters of a SNT and the respective beneficiary. “Beneficiary with a disability" means a beneficiary of a first trust who the special-needs fiduciary believes may qualify for governmental benefits based on disability, whether or not the beneficiary currently receives those benefits or is an individual who has been determined to be an incapacitated person . . . "Specialneeds trust" means a trust the trustee believes would not be considered a resource for purposes of determining whether a beneficiary with a disability is eligible for governmental benefits.

The SNT is a trust designed to provide for supplemental needs that are beyond the very basic needs met by the government-based assistance.

The assets in a SNT may assist with enhancing the quality of life of the

beneficiary, allowing for expenditures such as transportation, recreation, dental care, and communication services.

SNTs are complex trusts and require careful drafting and administration.

There are two types of SNTs, self-settled and third-party. Self-settled (first-party) SNTs are funded with the beneficiary’s own assets, whether through resources she already has, assets inherited when a SNT was not considered, or through a settlement awarded in the event of a personal injury.

This article focuses on third-party SNTs. Third-party SNTs may be used as part of the estate planning of individuals other than the individual with the disability who is a recipient of needs-based government assets and who would be the primary beneficiary of the SNT, such as the parents or relatives of the individual with the disability. Thus, if the individual is a recipient of Medicaid or Supplement Security Income (SSI), the assets in the SNT would provide additional resources that are not met by the public benefits, without precluding the receipt of such public benefits.

The funding of a third-party SNT may occur during the life of the donor(s) or upon the death of the donor(s).

When a SNT is established during the life of the donor, the benefit is twofold. A parent, relative, or friend may convey gifts to the SNT in order for the beneficiary to receive immediate or timely benefits. Creation of the SNT during the donor’s lifetime also allows for the existence of the SNT to be communicated to relatives. This enables the presentation of inter vivos gifts to the beneficiary or the conveyance of assets through a trust or last will and testament; the asset can be transferred to the trustee of the SNT for the benefit of the beneficiary without causing a preclusion from government benefits.

The alternative to establishing a SNT during the life of the donor is to include SNT language in the donor’s trust or last will and testament. The effect is that a SNT is only established

if the beneficiary who is a recipient of needs-based government benefits that are means-tested survives the decedent.

In such a case, the SNT may be funded from sources such as: life insurance proceeds, retirement plans, assets that pass by probate, or assets distributed from a trust.

Transfers to a SNT are completed gifts; it is wise for a donor to consult with her Certified Public Accountant (CPA) prior to the conveyance of a gift.

There are times when third-party SNTs are funded only with “seed money,” and receive additional funds at the death of a parent or relative. Following the initial funding of the third-party SNT, the beneficiary should be given notice of the existence of the SNT, and the Social Security Administration should be notified.

Prior to substantial funding of the SNT, it may be appropriate for a parent or family member to serve as trustee. However, in addition to avoiding conflicts of interest between family members and the beneficiary, the administration of SNTs is complex; therefore, consider naming a corporate trustee as a successor trustee. In addition to taking special care in the naming a trustee, the services of a CPA who is familiar with SNTs should be engaged.

As you review your own estate planning, ask yourself the following questions. Is your loved one, like a child – whether a minor or an adult child, a recipient of or may become a recipient of needs-based government benefits that are means-tested? Would you like to gift assets to her while you are alive? Do you intend to leave assets to her upon your death? Does she depend on her government benefits? If you have answered “yes” to any of these questions, perhaps the estate planning “superpower” of a SNT may be worth your consideration.

Culpeper Minute Men Chapter of SAR celebrates victory at Great Bridge We have important dates in history such as Independence Day July 4, 1776. We still celebrate it. Let me give you another important date, Dec. 9, 1775. This date should ring in this community’s ears. This was the date of the Battle of Great Bridge.

The Patriot victory at Great Bridge had an impact on the course of the American Revolution that was out of proportion to its size. It made the British position in Virginia untenable. It resulted in Virginia being free of any organized British presence for five critical years - free to provide massive amounts of men and supplies to the Continental army with virtually no enemy interference. It was as Colonel Woodford said in his letter to the Virginia Convention on Dec. 10, 1775, “ a second Bunker’s Hill affair, in miniature - with the difference being “ we kept our post”. There were 300 Militia, with the main force being the Culpeper Minutemen.

possess North America’s largest shipyard and a wonderful base of operation for the Royal Navy. Washington would not have been able to shift forces between the northern and southern campaigns.

The produce of Virginia and a great source of soldiers would have been denied by the British. What Impact would this have had on General Washington. Great Bridge was one of many small, but critical, battles leading to our Independence The Culpeper Minutemen existed for a little more than a year, but left their mark on history. Many Minutemen saw later fame, General Edward Stevens commanded the Virginia Militia at Guilford Court House.; Col Abraham Buford led a failed attempt to relieve the siege of Charleston; Lt. Col. John Jameson participated in the capture of Major Andre in New York and Lt. John Marshall, a future chief justice.

Each year on the 1st Saturday in December, the victory of the Culpeper Minutemen and other Americans at Great Bridge is celebrated at the site of the battle in a ceremony conducted jointly by the SAR and DAR. They saved Virginia for the struggle for Independence. This is also a day for Culpeper and surrounding counties to reflect and be proud.

LEGALS Public Notice Rappahannock Rapidan Community Services (RRCS) in Culpeper, Virginia provides transportation to/from RRCS programs and community locations for older adults and individuals with developmental and other disabilities who live in Culpeper, Fauquier, Madison, Orange and Rappahannock counties.

RRCS is requesting funds from the Commonwealth of Virginia to purchase six 15-passenger body-on-chassis vehicles with wheelchair lifts and one modified minivan with a wheelchair ramp through the FTA Section 5310 Program to continue this transportation service. These vehicles will be used to transport wheelchair and ambulatory participants to and from residential, day support and senior nutrition programs as well as medical appointments between 7:00 a.m. and 6:00 p.m. Monday – Friday and other times as needed. RRCS invites any interested public or private transit or paratransit operators within Planning District 9 to comment on the proposed vehicle replacements by sending written notice by February 1, 2020 to: Virginia Department of Rail and Public Transportation—Public Transportation Division, 600 E.
