BE THE SURGEON GENERAL OF CALIFORNIA

Dr. Diana Ramos of Laguna Beach became the state’s second-ever surgeon general in August.

At

first, I thought it was a joke. I had received an email saying, “Are you interested in being nominated for the position?” I ignored it. Finally, I got a text saying, “Diana, we’ve been trying to contact you and we want to know if you will be considered.” In looking back at my career, hindsight is 20/20. I see that everything I’ve done has prepared me for this moment

I have been living in Orange County since 2001, but I was born and raised in Los Angeles in a single-mom household. My mom would work up to three jobs at a time to afford to send me to the best school she could. She told me, “When I die, I’m not going to leave you rich with money, but I will leave you with something no one can take away from you, and that’s an education.” I would take the bus to my high school in West L.A. even though we lived in South Central. I studied really hard, and I always wanted to be a doctor. I always wanted to be able to help somebody. I loved to teach. And helping people and teaching is what a doctor does. I always say, “Try to teach your patient.”

I went to USC for undergrad and USC Medical School. I did my residency in OB-GYN and went on to get a master’s in public health at UCLA. And most recently, I received my MBA from the Paul Merage School of Business at UC Irvine. Along the way, I got married and ended up in Laguna Beach. I was also formerly president of the Orange County Medical Association. I still live here because COVID-19 demonstrated that we can work remotely, and the governor realizes that Southern California has some of the state’s biggest movers and shakers. It’s wonderful to be able to mostly stay here in person and be a local resource.

My undergrad was in communications. So it’s great that this role allows me to communicate to the public what’s important for health. I’ve been doing a lot of Spanishspeaking media, and I look forward to being that voice for the Hispanic community.

I’ve been in public health for more than 17 years, and I understand what the priorities are. Mental health is the common denominator for many of the challenges Californians are being faced with. Folks are dealing with depression and anxiety. They medicate with food and that can lead to obesity and diabetes. They medicate with substances and that can lead to substance abuse. Negative stressors take a toll on our children especially. And as an OB-GYN, reproductive health is also really important to

Many times, California is a role model for the country. There are only a few other states with surgeons general. That says a lot about the gov-

ernor, that he recognizes you need a leader to promote health. My mother, my husband, and my son all flew up to Sacramento when I was appointed, and we stood next to the bear statue right outside the governor’s office. My mom held the California Constitution, and it was a dream come true. Looking back to where I started and where I am now, I tell people I feel like Cinderella.

BATTLE CANCER WITH A BOXER’S SPIRIT

Istarted boxing when I was 21. I always wanted to be able to protect myself. I loved to box, I loved the skills, I loved being calm in the ring when someone is throwing a punch at you. Little did I know I would need it for cancer—everything I learned from boxing, I used for my cancer journey.

I was diagnosed in 2019. I started getting symptoms about two months before. It started out with throwing up, and I thought I had food poisoning. Eventually I ended up in the hospital, and they did an endoscopy with an ultrasound. When I woke up, the doctor was holding my hand and my parents were at the foot of my bed. I was told that I had pancreatic cancer and that I was going to get a Whipple procedure (to remove parts of the gastrointestinal tract).

When they did the surgery, I got really lucky because they got all the cancer out. The cancer had spread to my lymph nodes, but they were able to get it out of my lymph nodes. I was very grateful.

After my surgery, I had to do six months of five different chemos. It was intense. Jason Parillo, my boxing coach, would come and visit me every week when I went (for) chemo. He helped me get mentally prepared. He’s like, “Keep your head up, roll with the punches, you know how to do this, Laurie.” It was like a boxing match: Sometimes you win a round and sometimes you lose a round, but you can pick yourself up and push through, which I did.

I came through the chemo, and I was cancer free. I was like “Woo-hoo!” And then exactly a year later, my tumor markers were going up. It ended up I had cancer in my stomach again and in my liver. Luckily, when I was diagnosed, Hoag had gotten this new radiation machine. It’s the (ViewRay) MRIdian, an MRI with a laser in it. Usually when you have cancer in the stomach where I had it, radiation wouldn’t be able to go there. But with this radiation machine, it would. And instead of getting 30 zaps of radiation like the old one, you’d only get five.

As soon as I was cancer-free for the second time, two weeks later I was on a plane to Italy. I spent my summer there. My parents were born in Italy, and my dad rebuilt the house that he was born in and we’d go every year.

I came back and my tumor markers went up again. I was like, “Oh, no; not again.” My oncologist (Dr. Tara Seery) is a superstar. I call her my five-star general. I put it in her hands and say, “OK, tell me the game plan.” And she lets me make adjustments. She said, “You know, Laurie, we can

do that radiation again.” With the old kind of radiation, I wouldn’t be able to get radiation in my liver because it goes to the whole liver. But because this is laser-directed, it only affects just that spot, so I was able to get radiation again.

Six months later, I got another test and I was cancer-free. Again. The third time. I was so excited, and two weeks later, I was in Italy. Luckily, I was able

UC Irvine English professor Jonathan Alexander survived an eye stroke that led him to look deeply at his health history as a gay man and write “Stroke Book: The Diary of a Blindspot.”

In the summer of 2019, I was on vacation in Colorado with my husband when I woke up in the morning and realized I couldn’t see clearly out of my right eye. Part of the vision was grayed out or blurred out. I thought at first I was having a detached retina.

We found an ophthalmologist who did a series of examinations and realized that I was not having a detached retina. He could actually see a little globule of cholesterol that had lodged itself in a branch artery of my retina. And there’s nothing they can do for that. There are no instruments small enough to go in and remove that. He was very concerned, given my blood pressure was very, very high at that time, that this was the beginning of what he called “a cerebral event.” The retina is part of the nervous system. That’s why they consider this a stroke.

I was immediately admitted to the hospital for observation and put on medication for my blood pressure. I was lying in that hospital bed away from home—at the time I was 52—and thinking, “OK, I’m not going to get that vision back. It’s not treatable. What they can do is try to prevent other events from happening.” I was thinking, “How did I get to this point?” So I started this journal that eventually became “Stroke Book.”

I’ve always been a pretty high-strung individual. I would say my pulse rate and blood pressure have always been a little higher than the norm. I was under the assumption that we had been treating that and controlling it. But sometimes you don’t always know about what’s going on in your body. So that question, “How did I get to this point?” led me to start journaling. I guess as a writer, it’s your bad habit. I thought, “OK, where did this come from? Can I trace this?”

Things that kept coming to me had a lot to do with having grown up in the ’70s and early ’80s in the Deep South and encountering quite a bit of homophobia. From a fairly young age, I was readily identifiable as a gay kid, a queer kid. At the time, the South was a tough place to live as a queer kid. I was subject to quite a bit of bullying. Also entering into late puberty and early adulthood and going off to college at the time of the AIDS crisis in the ’80s, when everyone was telling me that AIDS was God’s punishment for homosexuality. That was a toxic environment to grow up in as a queer kid, and it gave me a set of anxieties, even a kind of self-hatred that took decades to undo.

What I tried to suggest in “Stroke Book” is that it’s

hard for me not to think about this later-life health crisis as at least connected to this five-decade experience of growing up at a time when the cultural environment, the social environment, and even the political environment was extremely toxic. I think that has consequences, even if I can’t piece it together as A plus B results in eye stroke, there’s this larger sense that living for prolonged periods of time in an environment that says “We don’t value your life” can create the kinds of anxieties, bad habits, and ways of living that can manifest in ill health.

I don’t think there has been as much research as is needed. It’s taken the medical establishment a long time to pay attention to the particularities of

the health of gay people. We do know that gay men have lower life expectancy than non-gay men, and not just (because of) things like HIV.

I’ve been really pleased when physicians have commented on the book and said they’ve wanted to teach it in medical schools. That’s gratifying because it lets young doctors-intraining know about the health needs and sensitives of LGBT people. In my life, I’ve only ever had one gay doctor. So the vast majority of us LGBT people are not going to have physicians who have lived experience of our lives, and we have particular health needs and challenges that I think are important for the medical establishment to know and pay attention to.

DISCUSS MENTAL HEALTH AT THE WHITE HOUSE

Iwork with a mental health nonprofit, Asian Mental Health Collective, which matches therapists to patients based on cultural compatibility and also houses an online support community. The group was one of the partners for the first-ever Youth Mental Health Action Forum hosted by the White House and MTV Entertainment, and they asked if any volunteers were interested in applying. They were looking for 30 young adults from really diverse backgrounds. I was in shock when I was chosen. I was the only one selected from Orange County, and I was one of two doctoral students who were there. So that really speaks to the fact that there were people from so many different backgrounds.

I was born and raised in Orange County, and I studied psychology and integrated educational studies at Chapman University. After graduating in three years, I wanted to stay in O.C. because I have a huge family here. I ended up joining UCI’s social ecology program and figured out over the last two years here where my passion in research lies, which is digital mental health and, in particular, looking at digital mental health through a cultural lens.

There is a cultural stigma around mental health and seeking help, especially in the Asian community. A lot of young people are unable to seek help because of the family dynamics and cultures they’re in, which may not be supportive. So digital mental health—online peer support communities, social media, streaming services—is a supplement to in-person therapy.

For the forum, they flew us to Washington, D.C., in May and we had a full weekend of events, including forming groups and giving presentations about groundbreaking mental health action ideas. My group had an idea for a podcast by Gen Z and for Gen Z with a call to action, which would allow people to create their own digital mental health toolkit. And that has since been picked up by Spotify.

We got to meet Selena Gomez, who talked about her Rare Beauty Initiative. And we also got to meet members of the Biden administration. I had an in-depth conversation with Dr. Jill Biden about the importance of young people being mentored by people in power, especially women. She told me about a college mentorship program she started at the community college where she teaches

Then the president surprised us. He just walked in very casually and spoke to everyone. That was really incredible. He was exactly how you’d think. He told a lot of stories and he talked about having a friend with PTSD and about his first exposures to mental health issues. He was there

SPREAD AWARENESS FOR A RARE DISEASE

My brother, Trevor, was 17 when he woke up one morning in 2004 with a ton of chest pain and went to urgent care. They thought maybe he had a collapsed lung, but he got sent to a hospital and they found that it was an aortic dissection. He ended up having two major surgeries to replace his aorta.

They were questioning why this 17-year-old had an aortic dissection. It’s not a normal thing. So they flew him to the Mayo Clinic. They were able to find VEDS (vascular Ehlers-Danlos syndrome) through genetic testing.

We had never heard of VEDS, even though my family had already suffered complications. At the time, there was no one to guide us on what to do, how to monitor. Eventually, we got our whole family tested, and we traced it up the family line that my mom and both of her sisters had it, and that it came from my grandmother. Then I have two cousins who have it and one of the children.

I got diagnosed in 2014 officially. My mother died of breast cancer. She just had so much guilt about passing this gene onto my brother. I assumed that I had it, so I waited until she passed to get tested.

About one in 50,000 (in the U.S.) have this disease. It’s a connective tissue disease that stems from a mutation in the gene that makes type III collagen, which is important for the elasticity of connective tissue. If your vessels don’t have this proper collagen, then they’re more prone to tear and dissect and rupture. You can have bowel separation, uterine ruptures during pregnancy—all of these can be lifethreatening emergencies, and so the life expectancy for people with VEDS is 51 years.

My brother had his second surgery in 2010 and was fine. He was living a good life. He chose to go through IVF with his wife, so they had a daughter who did not have the disease. She was born on March 26, 2021, and then in October, we were on a family vacation in Alabama. My brother and his wife were in an Uber, on their way to do some sightseeing, and out of nowhere an artery ruptured in his lungs. They had to pull over and call an ambulance. By the time they arrived at the hospital, they were unable to save him.

I try to focus on the positive. He wasn’t driving when it happened, so he didn’t put my sister-in-law in danger. He wasn’t with the baby when it happened. I’m grateful that

my dad and I were in the area, so we were able to at least say goodbye to his body.

After my diagnosis, I was spearheading the search to find a specialist who knew more about VEDS who could guide us. Ultimately, thankfully, we were able to find Dr. (David H.) Liang. I was searching for five years until Dr. Liang opened the (Hoag Marfan Syndrome and Related Conditions Program) clinic. Now my whole family is being treated by him.

In this moment, I am alive and well and I have so many people in my life whom I love and who love me. My niece is now almost 2, and she is the biggest joy in our lives. I have an amazing job as a physician assistant where I feel like I’m able to give back

to the community working with cancer patients. I’m engaged to an incredible man.

Living with a rare disease is tough because there’s not enough awareness or support. I have been through more tragedy than most people know in a lifetime, and I’m only 32. It’s scary to know that my life may be cut short, but I do try to live with the motto that my mother passed on to my brother and me: Choose joy. At the end of the day, I want to stay hopeful that we can get that awareness out there, get more interest in our rare disease, and get more fundraising and donors to help fund the research that could really save lives.

CURATE ART FOR CITY OF HOPE

Iwas asked by City of Hope Orange County President Annette Walker if I would be interested in curating the rotating gallery of Orange County artists. The concept was for living, breathing artists with living, breathing work that could keep the space alive in an area that could be scary for people to be within during a scary time in their life.

I had just lost my younger sister to brain cancer, which was a shock to me. She was as healthy as anybody in our family and had just spent the summer months out here. She left in August to get her family back to school at home in Dallas. One morning, she woke up and her speech was garbled. She was not able to put a sentence together, and they thought she had a stroke. After multiple tests, they realized she had a glioblastoma tumor that was inoperable. Within a year, she was dead. It was just so shocking to all of us. So when this opportunity arose, I said yes immediately because I understand how cancer can impact lives.

The rotating gallery really keeps the facility alive and doesn’t make it feel like a hospital. The art had to be from artists who live or work in Orange County, and it had to be about familiarity and about comfort so that people felt like they were walking into their living room. It’s meant to remind patients of places they love, to make them go, “Oh, I know that place! That reminds me of Crystal Cove,” or “That reminds me of when I was a teenager!” or whatever their connection to the art might be.

My background in the art world goes back to the ’80s when I had to find a way to provide for my three children by myself. I wanted to stay in the art world, so I got another degree in secondary art education. I went to work as an art teacher and then the opportunity came up for me to be one of the founding directors for the visual arts department at Orange County School of the Arts. My career was 18 years long there. I eventually taught elsewhere, founded an art conservatory along the way, and I still do some consulting work at Santa Margarita Catholic High School today. I also said yes because I love the arts, and because it’s my personality to say yes. That sometimes gets me in trouble because I get overloaded, but I also think it makes life exciting. You can say yes sometimes and think, “That was one of the best things I ever did.”

whether it was a family member, themselves, or a loved one. They all immediately said yes when they were chosen to be a part of the initial show we put together. They were so proud to be part of the gallery, and they each said it several times at our opening in August: “I’m so proud to be a part of this project that is impacting lives daily.”

The feedback from doctors and nurses has been positive. They all have favorite walls and favorite pieces, and when we told them this art would be (rotated), they were excited. It will all be constantly changing, living, and breathing with the patients who come through here.
