Hearing

Are you in need of a hearing aid or a hearing aid upgrade? The Local 1014 Health Plan has partnered with EPIC the premier provider of hearing care to provide members with exciting new bene ts including:

• $2500 allowance per year

• Name brand hearing aids at a signi cantly reduced prices

• 45- day trial period with a complimentary 3-year warranty and a 1-year supply of batteries

Dental

Do you need a crown, root canal, or llings? The Local 1014 Health Plan has made it easier to get your dental expenses reimbursed. This bene t is available regardless of the underlying dental plan in which you are enrolled. Receive up to $1,500 in excess dental care each year and receive a separate lifetime $2,500 allowance for dental implants.

CHIROPRACTIC/ACUPUNCTURE/ OCCUPATIONAL THERAPY

Have a bad back or knee pain? The Local 1014 Health Plan has doubled the amount of chiropractic, acupuncture and occupational therapy visits allowed each year. You can now have 30 chiropractic and 30 acupuncture visits each year for a total of 60 visits per year. That’s more than one per week. We have also doubled our occupational therapy visits allowed to 12 visits per year.

THIS EXAMPLE EVENT INCLUDES SERVICES LIKE:

Primary care physician o ce visits (including disease education)

Diagnostic tests (blood work)

Prescription drugs

Durable medical equipment (glucose meter)

Weight Loss

Did you know the loss of as few as 20 pounds can help the management of heart disease, hypertension and diabetes? The Local 1014 Health Plan has removed the dollar-limits for medical weight loss programs. We have also eased the requirements to qualify allowing more members access to these programs.

INTRAUTERINE INSEMINATION (IUI)

Looking to start a family? The Local 1014 Health Plan now covers IUI for all member couples. The Local 1014 Health Plan now provides a $10,000 lifetime bene t for diagnosis and treatment and a $10,000 lifetime bene t for drug therapy.

Body Scan

Did you know Fire ghters are at a higher risk that the general public for developing illness such as heart disease and lung cancer? We are pleased to announce that the Local 1014 Health Plan has extended the Body Scan bene t, now available until August 31, 2020.

Hospice

Looking into end-of-life care options for yourself or a loved one? We have removed the dollar-limit for Hospice Services in order to provide support during the most dif cult time for members and their families.
