After decades of setbacks, we now have FDA-approved treatments that can slow Alzheimer’s progression, revolutionary blood-based diagnostics and unprecedented scientific momentum that is transforming how we understand and approach dementia.

At the KU Alzheimer’s Disease Research Center, we have the privilege of contributing to this transformation from dementia to brain health. None of this work would be possible without our dedicated team members and the courageous study volunteers who partner with us in advancing science.

Together, we are developing new ways of understanding Alzheimer’s disease through our mitochondrial research— shifting focus from traditional markers to energy metabolism and cellular function. This work is opening pathways to treatments we never thought possible.

We are also honored to help lead the development of new diagnostics and deployment strategies. Our Brain Health Care Accelerator is focused on early detection and new models of care—empowering primary care providers and ensuring everyone has access to new technology. We are working to advance early detection and unlock the promise of prevention while creating blueprints for how these innovations can reach every community—from rural Kansas to urban centers.

THE KU ADRC: TWO DECADES OF ADVANCEMENT IN ALZHEIMER’S RESEARCH

The KU Alzheimer’s Disease Research Center (KU ADRC) was established to advance research, care and education in Alzheimer’s and brain aging. Recognized for its excellence, the KU ADRC received its initial National Institute on Aging (NIA) designation in 2011, becoming one of a select group of federally funded ADRCs nationwide. Since then, it has continued to renewed its NIA designation, securing continued funding to expand groundbreaking research, innovative clinical trials and community outreach. The KU ADRC has grown into a national leader in Alzheimer’s research, focusing on dementia prevention, mitochondrial function and early detection, while strengthening partnerships to accelerate discovery and care.

Dementia is a public health crisis Alzheimer’s disease presents a significant and growing public health crisis, this is due primarily to our aging population. According to the US Census Bureau, older adults will outnumber children for first time in U.S. history in 2035.

11.4% of Kansans aged 45 and older report experiencing subjective cognitive decline—a selfreported worsening or more frequent confusion or memory loss, which may indicate early changes in cognitive function.

There is estimated to be a 12.7% increase of Alzheimer’s cases in Kansas from 55,000 to 62,000 from 2020 to 2025.

Caregiving Impact 89,000 family caregivers in Kansas provide an estimated 125 million hours of unpaid care annually, valued at $1.9 billion.

Nationally, rural patients travel an average of 88-100 miles to reach the nearest neurologist, compared to just 9 miles for urban residents.

Powering progress: Our response in numbers As a national leader in Alzheimer’s research, the KU ADRC continues to expand its reach, supporting investigators, patients, caregivers and clinicians across Kansas and beyond. Our research enterprise drives discovery, our training programs shape the next generation of leaders and our community outreach ensures that the benefits of cutting-edge science extend to rural and underserved populations.

Community education

Cognitive Care Network (CCN)

In 2025, our 46 Weekly Webinar sessions welcomed 2,397 live attendees—and reached 6,389 views on YouTube. We hosted 52 community education programs with 1,199 people attending

Our team of dementia trained social workers provided support to 4,500 individuals including 254 from rural areas. 631 were newly diagnosed with MCI or early dementia. Additionally, a monthly average of 172 people attended our dementia support groups.

Provider Training The KU ADRC equips care providers with evidence-based tools to improve dementia care—partnering with 128 primary care providers across 17 counties to offer in-office support. Our educational outreach included 16 CME-eligible Pocket Talks reaching 550 clinicians and a full-day Managing Dementia Workshop attended by 60 regional providers.

Mitochondria:

Powering the future of Azheimer’s research

At the KU Alzheimer’s Disease Research Center, we are pioneering research into the role of mitochondria in Alzheimer’s disease. These organelles, often called the “powerhouses of the cell,” fuel nearly every function in the human body, including the brain’s energy demands.

Growing evidence suggests that mitochondrial dysfunction is not just a byproduct of Alzheimer’s but a fundamental driver of the disease itself.

Why mitochondria matter in Alzheimer’s disease

The DNA of the cell is shown in blue. Using specialized dyes, we can examine how mitochondria function and how much energy they are producing.

Mitochondria generate the energy that keeps brain cells functioning, but as we age, their efficiency declines. Research has shown that disruptions in mitochondrial function can impair the brain’s ability to produce and use energy, leading to oxidative stress, inflammation and neurodegeneration—key features of Alzheimer’s disease. Studies led by the KU ADRC’s Dr. Russell Swerdlow support the mitochondrial cascade hypothesis, which suggests that changes in mitochondrial function may occur long before traditional markers of Alzheimer’s appear.

In recognition of his pioneering work, Dr. Swerdlow was recently awarded a prestigious grant from the National Institutes of Health (NIH) to explore mitochondria-based interventions. This funding underscores the KU ADRC’s leadership in reframing Alzheimer’s research, shifting the focus from amyloid plaques and tau tangles to the role of energy metabolism in cognitive decline.

A new frontier: developing new biomarkers and models

Building on decades of research, KUADRC has developed new mitochondrial biomarkers allowing measurements of mitochondrial function in blood. These biomarkers allow us to understand how age-related mitochondrial changes contribute to cognitive decline and whether interventions targeting mitochondria can slow or prevent Alzheimer’s progression. These novel biomarkers are being measured in our clinical cohort, the

Down Syndrome Cohort and clinical trials.

A crucial part of our work is developing stronger research models to study the impact of mitochondrial dysfunction in Alzheimer’s disease. A recent study from our team demonstrated that inhibiting mitochondrial protein production altered key biological pathways linked to Alzheimer’s, reinforcing the idea that energy metabolism plays a direct role in disease mechanisms. These findings are helping establish a new foundation for how Alzheimer’s is studied and treated.

As a result, the KU ADRC has become a leading center for mitochondrial research in Alzheimer’s, attracting collaborations with top universities and biopharmaceutical companies, including those developing GLP-1 weight-loss drugs, which have shown potential links to brain metabolism and neuroprotection.

Expanding the horizon for Alzheimer’s treatment With continued investment in mitochondrial research, the KU ADRC is not only generating new discoveries but shaping the future of Alzheimer’s treatment. By uncovering how energy metabolism impacts brain function, we are laying the groundwork for new diagnostic tools, innovative therapies and novel drug development strategies.

As this field advances, the KU ADRC remains at the forefront, bringing together scientists, clinicians and industry leaders to drive meaningful progress against Alzheimer’s disease.

Finding new ways to deliver care: Transforming how we identify, prevent and treat Alzheimer’s disease

Through initiatives like the Davos Alzheimer’s Collaborative (DAC) and Brain Health Care Accelerator (BHCA), the KU ADRC is leading the transformation in how Alzheimer’s is detected, diagnosed and treated—ensuring faster, more equitable care for all.

As one of only four U.S. sites selected for Davos Alzheimer’s Collaborative, KU ADRC is at the forefront of implementing bloodbased biomarkers for early detection. Through this research which we call the Brain Health Care Accelerator, we’re validating the most efficient and effective workflows, trainings and partnerships to ensure this groundbreaking technology is available to everyone. With funding from Gates Ventures, we are building a cutting-edge data platform to bridge research and clinical applications, ensuring patients receive timely, evidence-based care.

Transformative trials: Where discovery meets possibility

COMET - strength training & brain health The COMbined Exercise Trial (COMET) is the largest study to evaluate how aerobic and strength training impact cognitive function in older adults. Led by the KU ADRC and funded by the NIH, COMET enrolls cognitively normal adults ages 65–80 in a 52-week structured exercise program. By analyzing cognitive performance, brain volume and biomarkers, this trial will determine whether strength training, aerobic exercise, or both offer the most protection against cognitive decline. COMET’s findings will help shape future exercise guidelines for maintaining brain health in aging populations.

FIGHT-AD Study – heat & brain metabolics fight-ad

The FIGHT-AD Study explores the potential benefits of passive heat therapy on metabolic and brain health in older adults at risk for Alzheimer’s disease. Participants engage in a 10-week intervention involving hot tub sessions to assess whether heat therapy can mimic exercise benefits, potentially offering an alternative for individuals unable to engage in regular physical activity.

MENO-COG Study menopause & cognition Little is known about how common symptoms of menopause are inter-related. This study will examine the cognitive changes reported by women during their menopausal transition and how they are affected by sleep, physical activity, hot flashes and night sweats.

Brain health training program Shaping the future of ADRD research and clinical care

The KU Brain Health Training Program (BHTP) is a comprehensive research and clinical training ecosystem developing the next generation of Alzheimer’s and dementia specialists. Since 2016, it has grown from a single fellow to supporting dozens of trainees annually, becoming a nationally recognized hub for Alzheimer’s Disease and Related Dementias (ADRD) workforce development.

BHTP’s collaborative culture fosters interdisciplinary learning in clinical trials, biomarker research, neuroimaging, metabolism and brain aging.

The progressive mentorship model ensures seasoned investigators guide trainees, while senior trainees mentor early-stage researchers.

This pipeline approach creates a seamless transition from learning to leadership, positioning KU ADRC at the forefront of addressing the growing dementia crisis affecting millions of Americans.

The program’s pioneering Brain Health Fellowship for Advanced Practice Providers (APPs) - the first of its kind nationwide - offers an intensive six-month program where fellows work alongside physician mentors in the Memory Care Clinic while receiving specialized training in diagnosis, treatment and research methodologies. For physicians, the Clinical and Research Fellowship in Dementia and the United Council for Neurologic Subspecialties-accredited Cognitive and Behavioral Neurology Fellowship provide advanced pathways for clinicianscientists with flexible one or two-year tracks.

“Being a postdoc could be very isolating, but the purposeful and supportive community that BHTP creates has really propelled me forward. After completing my PhD in neuroscience, I was wondering what to do next and knew I needed more specialized training. When I connected with the BHTP program through my mentor, I was excited about the opportunity to build skills in implementing lifestyle clinical trials with older adults while incorporating neuroimaging.

What makes BHTP special is our cohort model—we’re in it together, learning from each other and sharing expertise. We help push each other forward, whether it’s refining a grant application or developing a specific hypothesis. It’s team science at its best.

It’s easy to get stuck in your own research silo, but this program intentionally brings us out of isolation. With mentors in different specialties, I’m exposed to diverse collaborations that truly advance the science. This is exactly the support system earlystage researchers need to succeed.”

As far as grad school goes, I’m in the best spot—I have creative freedom but guidance on how to pursue it in a scientifically rigorous way.

I especially value the connection between research and clinical care. This “cells to systems approach” advances patient care while preparing us for competitive careers. The Lifestyle Empowerment for Alzheimer’s Prevention (LEAP!) program has been empowering our community for a decade. With dementia cases expected to triple by 2050, prevention strategies are essential. The good news? Research shows that lifestyle changes could prevent or delay 40-60% of cases.

Cognitive Care Network For too many Kansans, a dementia diagnosis means a long drive to a specialist, a months-long wait and navigating a complex system largely on their own. The Cognitive Care Network (CCN) was built to change that. By partnering with primary care practices across the state — from Kansas City to rural communities in western Kansas — CCN embeds dementia expertise directly into the places Kansans already go for care. Through training, navigation and ongoing support, CCN’s team of specialized social workers is expanding access to early diagnosis, connecting families to resources and ensuring no one faces this disease without guidance.

Brain health & Down syndrome

Pioneering hope for those at greatest risk At the KU Alzheimer’s Disease Research Center, our Brain Health and Down Syndrome program addresses one of the most urgent yet underserved populations in dementia research. Adults with Down syndrome face an exceptionally high risk of developing Alzheimer’s disease, with up to 90% affected in their lifetime.

Pioneering clinical trials The KU ADRC serves as a clinical trial site for two landmark anti-amyloid studies—ABATE and HERO—the first therapeutic trials designed exclusively for adults with Down syndrome. We have also joined the ALADDIN trial, investigating donanemab as a potential treatment.

We are also national leaders in lifestyle-based research, conducting some of the only studies in the country exploring how diet and exercise modifications can reduce Alzheimer’s risk in adults with Down syndrome.

National collaboration In 2023, the KU ADRC joined the Alzheimer’s Biomarker Consortium– Down Syndrome (ABC-DS) as the 10th site in this groundbreaking longitudinal study. As the largest consortium of its kind in the U.S., ABC-DS tracks Alzheimer’s progression through advanced biomarker analysis.

Supporting caregivers We’ve expanded resources for caregivers of adults with Down syndrome showing signs of dementia, providing education, planning support and wellness resources.

Through groundbreaking clinical trials, innovative lifestyle interventions and genuine partnership with the Down syndrome community, the KU ADRC is working to transform the future for those who face Alzheimer’s greatest threat.
