Health Reimbursement Arrangement (HRA) HMO Plan What is it?
A fund that helps bridge the gap between the 2025 Annual Deductible and the Annual Deductible in 2026.
Who funds it?
$2,000 (Deductible)
Kokua covers the $1,000 difference between your out-of-pocket costs of $1,000 and the $2,000 Deductible.
How do I use it?
Once you have reached $1,000 in out-of-pocket Qualified Medical Expenses (QMEs), any additional QMEs can be submitted for reimbursement, up to $1,000 in total.

What and where do I submit?
Send your EOB (Explanation of Benefits) that has the QMEs you want to be reimbursed for to Heather Van Slageren (heatherv@fournierinsurance.com) at Fournier for review. Confirmation of qualifying expenses will be sent to Kokua administration for reimbursement on your next regular paycheck.
Is this an HSA?
No. An HSA accumulates funds to pay for QMEs. An HRA reimburses you after you’ve paid for the QMEs.
