CONFIDENTIALITY AGREEMENT
I understand that as a result of my volunteer position, internship, or contracted work with Kokua Services, I may become aware of confidential information concerning individuals supported by Kokua’s programs and services. I understand that I am forbidden to divulge, publish, or otherwise make known to the public or any unauthorized individual any of the confidential information known to me. I realize that the unauthorized release of confidential information will subject me to disciplinary action, up to and including termination, and may be subject me to civil liability.
Please initial by each procedure to acknowledge your understanding of our expectations regarding confidentiality for individuals supported by Kokua Services.
___ Confidential information can only be disclosed to an individual or an organization when there is a signed release of information giving permission for the release.
___ No documentation or any material containing first and last names or photos will ever be stored on a personal computer or device. This would include cameras, phones, tablets, or personal computers.
___ Rules covering confidentiality extend to use of social media. It is strictly forbidden to post pictures, names, or other personal information on your personal social media. You may, however, share a post from Kokua’s social media platforms.
