Dr. Wayne Sotile, the keynote speaker for our VIP Conference, is an internationally known speaker on Physician Burnout. Fortunately, Dr. Sotile is still planning on speaking at the event on May 7, 2021 (details to follow). In the interim, if you are dealing with burnout, the Knoxville Academy of Medicine is working in conjunction with the Tennessee Medical Foundation to provide completely anonymous counseling and resources to help address the stressors that physicians are facing.

We still plan to have a legislative event in November 2020 and are exploring ways in which we can host a virtual event with our

legislators. The annual legislative dinner is an important function in which KAM members interact with our legislators and discuss issues which are important to the practice of medicine. The legislators really do want to hear from us.

Please watch for more information on how you can participate in this event and further advocate for our profession and our patients.

This year has certainly had its challenges but hopefully Knox County is turning the corner on Covid-19. As of press time, Knox County currently has a daily case rate of 18.3 cases per 100,000 residents which is down significantly from just a couple of weeks ago. Please know that your Knoxville Academy of Medicine stands ready to serve our physicians members and their practices during this time. We have and will continue to supply PPE at cost to our KAM Members. We have a variety of PPE products that we have been able to obtain including N95 and KN95 masks, face shields, disposable gowns, and much more. ACADEMY NEWS Update from the Tennessee Board of Medical Examiners By: Reeves Johnson, MD

Changes to Initial and Renewal Applications Questions In 2017, the AAFP published a position paper about family physician burnout, well-being and professional satisfaction noting over 50% of all physicians and 63% of family physicians reported at least one symptom of physician burnout.1 Then in the spring of 2018, the Federation of State Medical Boards, “FSMB” adopted a policy regarding physician wellness and burnout, following the recommendations of an FSMB workgroup which was convened in April of 2016 to identify resources and strategies to address physician burnout. It was discovered that many physicians are reluctant to seek help for burnout for fear that they will be perceived as weak or unfit to practice medicine by their colleagues or employers, or because they believe that seeking such help may have a detrimental effect on their ability to renew or retain their state medical license.2 Some of the recommendations from the FSMB policy to state medical boards include the following: 1) initial and renewal application questions should focus on current impairment rather than illness, diagnosis, or previous treatment; 2) initial and renewal application questions should not distinguish between mental health and physical health; and, 3) state medical boards should consider offering

the option of “safe haven non-reporting” to applicants for licensure who are receiving appropriate treatment for mental health or addiction and when treatment received is commensurate with the illness being treated and has a reasonable chance of avoiding any resultant impairment.

Several state medical boards have begun changing their application questions to reflect these recommendations. In West Virginia, after they made these changes, their professional health program saw a 1000% increase in referrals. Realizing that making these changes encourages physicians who need help to get help, your state medical board created a task force to evaluate their initial and renewal application questions. With the help of Dr. Michael Baron, former board member and the current medical director of the Tennessee Medical Foundation, “TMF,” the task force’s recommendations were adopted by your state medical board at their January 2020 meeting. The revised initial and renewal applications reflect the noted recommendations by the FSMB. Where do I go for help?

The TMF has been around for decades providing professional and confidential help for physicians dealing with emotional and mental issues including, but not limited to, burnout, depression and substance abuse. Your state medical board relies heavily on their expertise as they seek to insure the people of Tennessee are being treated by competent physicians. Earlier this year, the TMF in partnership with your state medical board, TMA, SVMIC and others launched an online resource to address these issues among licensed professionals called the Tennessee Professional Screening Questionnaire (TN PSQ). This service is available to all Tennessee licensed health professionals including MDs, DOs, residents, interns, medical students, PAs, x-ray technicians and others along with those in training. This is a non-crisis service offering free, voluntary, anonymous and confidential online medical health screening and can result in referrals to appropriate mental health resources and an optional interaction with a program therapist. Beyond Retirement: What About Your Other Goals?

Let’s say that at the age of 25, you earned $35,000. If your salary increased at the average historical rate, you’d have earned nearly $2 million in total by the time you were 65.1,2 That might sound like a lot -- until you begin thinking about all the financial goals you’ll need to juggle in a lifetime, including buying a home and paying for your child’s education, while funding your own retirement.

If managed wisely, your money could potentially go a long way. It’s really all about putting a plan in place and sticking to it. These tips may help get you started.

Get a Jump on All Your Goals You’ve read before about the value of starting early on retirement savings, even if you can only invest a little each month. The same goes for college savings and other goals.

Even a $100 a month investment for college could potentially leave you with about $16,470 in 10 years, assuming an average annual return of 6% -- a good start that you can build on as your income grows.1 Set Aside a Slice of Pay Hikes As your income rises over the course of your career, it’s easy to slip into a pattern of “living up” to your means; that is, spending that extra pay you didn’t have before on daily living expenses. Instead, consider

setting a quota for yourself: Earmark a predetermined portion of every pay hike for your savings goals. You may want to apply the same rule to other windfalls, like an unexpected bonus or tax return.

Use the Right Tools for the Job Just as your employer-sponsored retirement plan offers a tax-advantaged opportunity to set aside money for your later years, certain vehicles, such as 529 college savings plans, provide potentially attractive tax breaks for college savers. Minimizing the taxes you have to pay up front on investments and earnings gives you the chance to make the most of compounding over time.

Finally, whatever your particular financial goals may be, keep in mind that minimizing debt is a timeless, indispensable strategy for establishing personal financial balance.

Telemedicine: Where Are We Now?

During its most recent session, the Tennessee General Assembly was unable to pass telemedicine legislation supported by the Tennessee Medical Association. Under current state law, health insurance companies are not required to pay total reimbursement for a telehealth encounter, including the cost associated with the use of telehealth equipment, in an amount that exceeds the amount that would be paid for the same service provided by a healthcare services provider in an in-person encounter. Legislation that would have eliminated this disparity and established pay parity for telemedicine did not pass the Tennessee Senate before the session was adjourned. So, what happens next? Although new Tennessee legislation is unlikely until at least 2021, a shift toward more telemedicine appears to be picking up momentum due in large part to the COVID-19 pandemic.

At the federal level, as part of its early response to the pandemic, the Centers for Medicare & Medicaid Services (CMS) broadened access to Medicare telehealth services to allow Medicare beneficiaries to receive services from their physicians without having to travel to a healthcare facility. Effective March 6, Medicare began to pay for office, hospital, and other visits furnished via

telehealth across the country (including in the patient’s home). Prior to this waiver, Medicare could only pay for telehealth for rural patients receiving the service from a clinic, hospital, or certain other types of medical facilities. In a related change, the HHS Office for Civil Rights (OCR) waived applicable HIPAA prohibitions to allow health care providers to serve patients in good faith through everyday communications technologies, such as FaceTime or Skype (though not for “public” sites such as Facebook Live). On May 29, the Federal Trade Commission wrote a 19-page letter urging CMS to continue expansion of telehealth after the pandemic subsides, including supervision requirements under Medicare’s “Incident To” billing rules. In late June, CMS issued a proposed rule to make certain temporary, COVID-19 related changes to the use of telehealth in home health services permanent beginning January 1, 2021.

At the state level, TennCare expanded telehealth services beginning in March and each of its managed care organizations followed by issuing additional guidance on telemedicine services. On March 19, Tennessee Governor Bill Lee’s Executive Order No. 15 urged health insurance carriers to provide coverage “for the delivery of clinically appropriate, medically necessary covered services via telemedicine to all providers, irrespective of network status or originating site.”

Subsequent executive orders both extended and expanded permissible telehealth and telemedicine services during the pandemic.

Much like the rest of the economy, the healthcare sector continues to sail through uncharted waters as a result of the global pandemic. In many cases, whether changes made in response to COVID-19 prove to be temporary or permanent is still very much unresolved. What is certain, however, is that the additional flexibilities permitted at both the state and federal level in response to the pandemic, as well as the subsequent shift in the adoption and utilization of telemedicine, have created an environment much more likely to support the continued expansion of telemedicine into the future.

TECHNOLOGY NEWS Digital Contact Information Required in NPPES by: Leigh Sterling, Executive Director East Tennessee Health Information Network

The ability for providers to input their digital contact information in the National Plan and Provider Enumeration System (NPPES) has been available since June 2018 in order to improve interoperability and the efficiency and effectiveness of electronic health information exchange. While providing digital contact information is not required to receive an NPI number, CMS will soon begin public reporting of providers who have not provided their digital contact information in NPPES. The recently passed CMS Interoperability and Patient Access Rule mandates a centralized directory of provider electronic addresses for data exchange published by the National Plan & Provider Enumeration System (NPPES) as part of the requirements to promote interoperability. Providers must input their digital contact information into NPPES by 6/30/2020, or they will be publicly reported as non-compliant during the second half of 2020.

Direct secure messaging is a CMS-approved method of transport that is permitted for use in the NPPES directory. etHIN Direct is an inexpensive, HIPAA-compliant healthcare-only email messaging system that allows providers to securely send and receive referrals, patient PHI, and have secure one-to-one communication with other healthcare providers. In addition, etHIN uses Direct as one of several transport methods for participants using our Alerting services.

It has been reported that some providers may have used their work or personal email address in the NPPES directory. It is important to know that CMS emphasizes the importance of NOT using your work or personal email address as your digital contact information in NPPES. The contact information in NPPES will be used by hospitals and other providers to send event notifications, PHI, and referrals if they are using NPPES as a resource to obtain your digital contact information. Protection of PHI is a top priority for etHIN, and using etHIN Direct will provide peace of mind, knowing that your confidential communications are securely encrypted and that you have found a simple, inexpensive way to comply with the CMS requirement to provide appropriate digital contact information.

The friendships that I have made in KAMA are very special and that is the sentiment that will carry forward in our theme this year. Our theme this year will be focusing on our friendships in KAMA. KAMA is a non-profit organization established in partnership with the Knoxville Academy of Medicine that promotes impactful health education programs within our community and encourages friendships among its membership. An example of encouragement in the midst of the COVID-19 outbreak, KAMA Cares Committee led by Nancy Sewell made phone calls to all of our KAMA members. With each phone call there was the assurance that we were there to help and support them if need be. Pursuant to our desire to connect with our members, Jennifer Ollis has been facilitating and organizing our zoom meeting book club. These book club meetings

have been enjoyed by several of our members.

COVID-19 has unequivocally changed the flow of our lives. As a result, we, as officers and committee chairs, have worked diligently this to adapt our agenda for 2021 accordingly. Tina Callicut and Aprakon Walker, our 1st Vice-Presidents, have carefully selected meeting venues. We are indebted to Cynthia Gash, our 2nd Vice President, for once again updating our KAMA yearbook. A big thank to Judy Akers, our Philanthropic Chair, for organizing our philanthropic board to move forward with our patron fund, our Holiday sharing card and our exciting virtual fundraiser “Comedy and Cocktails” in January 2021.
