This year is well underway and we are continuing to experience growth in many areas. This growth is a reflection of Madison Health serving you better and allowing individuals and families to stay close to home for their healthcare needs. For example, our newest satellite location, Madison Health Mount Sterling, opened in late March to bring patient-centered services such as laboratory, physical therapy and primary care to southern Madison County.

adison Health is proud to offer fast, easy and reliable outpatient lab services at our clinics in West Jefferson and Mount Sterling, as well as the Keny Boulevard and Park Avenue Medical Building locations in London.

“Our aim in opening the labs is to not only bring these services closer to our community members and reduce their need to travel, but to also reduce the time our patients and their families have to wait for results,” said Cyndi Roberts, Director of Laboratory Services. “It is also driven by the fact that a medical issue doesn’t just impact the patient; it impacts their family members as well. If we can provide the information to them and their physicians sooner, that can equate to better care for our patients.”

Madison Health Diagnostic Center in West Jefferson opened in late 2015 and Madison Health Mount Sterling opened in March of this year.

The fully-accredited laboratories and staff of medical professionals have the clinical expertise to process specimens for everything from routine to specialized testing, with next-day results available.

When a medical emergency is at hand, Madison Health’s STAT service can make lab results available in about an hour for samples collected on-site at our main facility or Park Avenue location.

The expansion of outpatient laboratory services has resulted in an increase of patients, as well as a decrease in wait time because of a more efficient registration process.

“Last year, we saw more than 11,000 patients in the lab. So far this year, we have seen a 47 percent increase over last year,” said Roberts. “We are also seeing a decrease in waiting times, with the average patient in and out of the lab in 12 minutes or less. That’s time saved by everyone, but especially our patients and their family members who often accompany them on lab visits.”

Each outpatient laboratory offers convenient hours to accommodate busy schedules and close parking is available for all patients. The inpatient laboratory in the hospital is open 24 hours a day, seven days a week, including holidays.

Understanding

Lymphedema Signs, symptoms and treatments

A

fter undergoing a bilateral mastectomy and axillary lymph node removal as part of her treatment for breast cancer in 2014, Esther Weisman knew she was at a higher risk of developing a condition called lymphedema.

Commonly seen in patients with cancer who have had malignant lymph nodes removed to prevent the further spread of their disease, lymphedema is characterized as an abnormal accumulation of lymph fluid under the skin and is usually seen in the limbs. It can also occur in other areas of the body like the abdomen, head, neck, face and genitals.

While still an inpatient at Madison Health, Weisman was referred by her surgeon, Dr. Donald Hura, to the organization’s Rehabilitation Services team where she met physical therapist and certified lymphedema therapist Myah Walker to learn more about the condition – which she had not yet developed.

“As part of our proactive, patient-centered approach, I met with Esther and educated her about lymphedema and told her what signs and symptoms to watch for when she went home,” said Walker, who is also the Director of Rehabilitation Services at Madison Health.

After surgery such as Weisman’s, some patients may experience a delayed onset of swelling. Depending on how the body’s lymphatic system reacts to overcome the removal of nodes, symptoms can present years later.

“Lymphedema can be classified as either primary or secondary,” explained Walker. “Primary lymphedema is caused by congenital or genetic abnormalities. Secondary lymphedema occurs due to injury or removal of the lymph nodes during any surgery.

It’s incidence in the U.S. occurs most commonly with cancer treatment, but can also result from trauma, radiation and infection.”

Regular checkups with a physician are recommended for patients who are at higher risk of lymphedema.

“Patients who are predisposed to lymphedema need to take some precautions like keeping their skin clean, being careful not to scratch or cut their skin, avoiding temperature extremes such as frostbite or sunburns,” said Stone.

Lymphedema should not be confused with edema, which causes an accumulation of excess fluids in the tissues due to conditions that upset the fluid balance in the body like congestive heart failure, chronic venous insufficiency (CVI), immobility and pregnancy. Although CVI – an advanced stage of venous disease that causes blood to pool in the legs and feet – has been shown to respond to some of the same modalities used in lymphedema treatment.

The following is a list of symptoms of lymphedema: n Swelling in the arms or legs that lasts longer than

Changes in the skin like thickening or wounds that

are difficult to heal “It’s important to receive treatment to reduce swelling and prevent the condition from worsening,” said Courtney Stone, physical therapy assistant and certified lymphedema therapist at Madison Health. “Complete Decongestive Therapy (CDT) is often recommended for lymphedema as well as patients with CVI and may include manual lymph drainage massage, compression bandaging and/or garments, exercise and skin care.”

“Since there is no cure for lymphedema learning how to manage it can greatly improve a person’s quality of life,” said Walker.

Weisman agrees. When she started to notice swelling in her hands, she made an appointment with her oncologist at the Madison Health Cancer Center, Dr. Shabana Dewani. As a member of Weisman’s multidisciplinary team, Dr. Dewani ordered outpatient treatment for her lymphedema.

One such type of infection, known as cellulitis, is caused by bacteria that gets under the skin as the result of a cut or tear.

While some of the risks of lymphedema are out of a person’s control, other factors can be managed.

“It’s important to get regular exercise and use the affected body part to help promote drainage,” said Stone.

Weisman is now back to enjoying life again and is grateful to the team who helped her get there.

“My healthcare team at Madison Health – from my Cancer Center nurses and physician to my physical therapists – has been very compassionate and shown care to my family and me,” she said. “They were always available to listen.”

Colonoscopies W

hen London resident Juanita Stedding began to experience pain in her abdomen that wouldn’t subside, she was referred to general surgeon, Dr. Mark Coate, for a colonoscopy.

offer the option of a fecal immunochemical test (FIT) which checks for microscopic traces of blood in the stool. This test has to be done yearly and if blood is found, I will send the patient for a colonoscopy.”

Although it’s recommended that people have a colonoscopy screening starting at age 50, Stedding had avoided one because of all of the unpleasant stories she had heard.

Most people who have had a colonoscopy will attest that the preparation is usually the worst part.

“I heard how uncomfortable having a colonoscopy can be, which contributed to my anxiety about having the procedure,” said Stedding.

Dr. Michelle Khoury of Madison Health Primary Care is all too familiar with patients like Stedding. As a family practitioner, she is the one making the referrals for patients to have colonoscopies. Colonoscopies use a thin, flexible tube to look at the inner lining of the large intestine for areas of inflammation, bleeding, ulcers, tumors, and polyps (pre-cancerous growths).

“I start screening my patients for colon cancer at age 50, unless they have a family history in a first degree relative and then we screen sooner,” she said. “For those without risk factors, we

According to Steve Smith, Director of Surgery at Madison Health, “The procedure itself starts the day before with a clear liquid diet and later in the evening some form of medication to help clear the bowels of any stool so there is good visualization of the colon.”

Colonoscopies are performed in the endoscopy suite at Madison Health. Patients are given anesthesia through an IV which puts them in a twilight sleep state (conscious sedation) that allows them to respond to commands and questions.

“On the day of the colonoscopy, patients can expect to be here for about 2 hours from start to finish,” said Smith. “The actual procedure only takes about 15 minutes to complete.”

“Colonoscopies are the gold standard screening tool for colon cancer because, unlike other tests, tissue samples can be taken and tested and polyps can be removed,” said Dr. Khoury.

“There was no hesitation about where I would have my operation. Madison Health has become my home away from home in a good way.”

“If a colonoscopy comes back clean a patient won’t need another one for 10 years. If polyps or anything concerning is found, I recommend another screening in 3 to 5 years.”

Stedding, who came to find out it was a hernia causing her pain, will undergo surgery to repair it in the near future and will once again choose Madison Health.

“There was no hesitation about where I would have my operation,” said Stedding. “Madison Health has become my home away from home in a good way.”

Her positive experience with Madison Health has convinced her daughter, who suffers from Nonalcoholic Steatohepatitis (a liver disease) and has to undergo regular colonoscopies, to stay close to home instead of driving to Columbus for care.

“I would recommend having an outpatient procedure to surgery at Madison Health to anyone,” she continued. “It’s not common to find their level of care and expertise. Each person I encountered was extraordinary. They worked together in sequence and always had a smile on their face.”

Stedding also appreciated the little touches shown to her while she was a patient at Madison Health.

“In recovery I received a thank you card signed by each of my care team members and a flower,” she said. “To this day, I look at that card at home and think of the wonderful experience I had.”

“All of our outpatient surgery patients receive a thank you card and flower as our way of thanking them for choosing ‘us’ as their provider of choice,” explained Smith.

For family practitioners, like Dr. Khoury, having good relationships with the specialists in the community translates into good care for her patients.

“We are a rural community and we want to keep our patients close to home for their care,” she said. “It’s important that we all work together to decide the best course of care for our patients.”

Colonoscopies are covered by most insurance plans as a diagnostics screening for patients over 50. Some insurance companies require a referral from a physician.

Regardless of your age, you should make an appointment with your doctor if you are experiencing any of the following: a change in bowel habits (diarrhea or constipation), blood in the stool or rectal bleeding, persistent abdominal pain, fatigue or weakness, or unexplained weight loss.

“Colon cancer is preventable with early detection and treatment,” said Dr. Khoury.

Seeking applicants to ensure a healthier tomorrow The Madison Health Auxiliary is seeking scholarship applicants to help ensure that well-trained healthcare professionals are prepared to serve the community. Any resident of Madison County, or employee of Madison Health, who is seeking a professional healthcare career and is currently enrolled for training at an accredited school may apply.

Dr. Gary Erdy to the Mount Sterling community Madison Health is proud to welcome Gary Erdy, MD, a primary care practitioner, to its family of physicians and further meet the healthcare needs of the Mount Sterling and surrounding communities. “Throughout my career and even within the changing climate of medicine, my passion for Family Medicine has only become stronger as I see an even greater need for caring and compassionate family physicians,� he said.

Dr. Erdy brings more than 30 years of experience to Madison Health, having served as both an emergency room and primary care physician.

He received a Bachelor of Science in Biochemistry and Doctor of Medicine from The Ohio State University. He completed both his family practice internship and residency at Mount Carmel Medical Center and is experienced in using the Epic system. He also has more than 15 years of clinical research experience, serving as Principal Investigator for many studies.
