IHDC commenced operations on 1st Feb 2018. Prior to that, the Centre was known as SPICE. IHDC packages are comprehensive and provide personalised support to seniors with multiple care needs, to maintain their independence and quality of life in the community for as long as possible.

Our IHDC Centre provide both day care setting for seniors who require supervision while their caregivers are at work and also home care.  Day Care Centre aims to improve physical and social well-being of seniors through therapy and also take part in recreational activities.  Home Cares service includes personal hygiene such as bathing, nail and skin care, meals delivery and medical escort / transport service for medical appointments.

Since inception, attendance at our Centre has continued to increase steadily and our client capacity has been raised to 50. Our healthcare staff are equipped with the skills and knowledge, which are essential to cope with the demands and challenges of caring for those with multiple medical problems.

The primary objective of the IHDC Programme is to enable frail individuals who have multiple nursing care needs to delay or reduce institutional care and remain in the community for as long as possible.

The secondary objective is to improve their quality of life with care arrangements as measured by satisfactory results obtained from the survey on services provided by IHDC.

The IHDC Programme has 3 packages catering to the three levels of care:

 Package 1 caters to clients who require minimum to moderate assistance.  Package 2 and 3 are for clients requiring moderate to maximum assistance. The difference between Package 2 and 3 is that the latter deliver more services in the client’s home.

The main source of referrals comes from Agency of Integrated Care (AIC) through hospitals. Clients who are identified suitable for IHDC Programme must be referred through AIC’S Electronic Integrated Referral Management System (IRMS).

The service areas that IHDC serves include Admiralty, AMK, Lentor, Marsiling, Sembawang, Sin Ming, Springleaf, Upper Thomson, Woodlands, Yishun, Canberra Dr and Yio Chu Kang. We have a multi-disciplinary team approach which comprise of Doctors, Senior Manager, Nurse Manager, Nurses, Social worker, Health Care Assistants, Therapists (OT&PT) and Therapy aides who discuss with clients and their family members to assess, develop and carry out a customised and individual care plan for each client.

IHDC support clients to continue living with their families at home as this enable them to have a better quality of life and maintain their dignity.

Mr.Chan aged 81yrs old was admitted to SWAMI IHDC programme since 11 March 2014.He has been staying with his wife who is frail elderly, together with their second daughter Jocelyn. Mr.Chan has been wheel chair bound and he requires moderate to maximum assistance in most of his Activities of Daily Living. (ADL).

However, he is still able to transfer and walk with close supervision and assistance. When client attends Day Care Centre from Monday to Saturday, whereby his care needs are attended to by care staff in the Centre, his main caregiver Ms Jocelyn is at work in the office. She is committed to taking care of both elderly parents after work and during weekends. He has one son and two daughters. He has limited social support from his children since two of them are married and live apart from him.

He is often sick due to his present medical conditions and has been in and out of hospital. Client’s eldest daughter accompanies him to Polyclinic/Hospital appointment’s etc. However his wife was unable to care for him due to her old age /multiple medical conditions. SWAMI IHDC Day Care centre assist him for personal hygiene, provide medical escort and transport, offer meals delivery, help him to change diaper at the Centre and takes care of him during this Covid pandemic.

Now, Mr.Chan is happy with full of smiles and satisfied with the care provided since he does not have to worry about who will take care of him. When he was first admitted to Centre, he was depressed, moody and unable to walk/transfer. Now, he is comfortable and look forward to coming here every day to meet his Centre friends.

I enjoy these beautiful moments with them. I like to listen music, watch movie, enjoy singing, play bingo and other activities. I am thankful to God for providing the doctor, nursing care staff, therapists and other professional staff to look after me here.”

Physiotherapy and Occupational Therapy Exercises

The Centre is also concerned for the psychosocial needs of our clients. We conduct social, recreational activities such as bingo, Karaoke sessions, colouring and art therapy as well as horticultural therapy for clients to enhance their mental and social well-being through interaction with each other in the Centre.

Patient’s care plans include both Centre and home care. IHDC Team also looks after patient’s care needs at their home, including recommending safety aids, personal hygiene, wound care, change nasogastric tube and urinary catheter, provide meals delivery and medical escort services for client’s medical appointments. With this service in place, clients are able to stay comfortably in their homes and maintain their dignity, while being cared for by our healthcare professionals.

In order to support the higher care needs of our clients , our healthcare staff attend training courses conducted by AICand KTPH to develop their caregiving skills and work competencies.

Our additional services include Comprehensive Nursing and Medical services at the Centre and at home as follows:

 Our doctor review client’s general condition, prescribe medications that the client needs and also provide health education to client and caregiver pertaining to the client’s medical condition. He also change the intra drainage catheter, SPC catheter and tracheostomy tube for difficult clients.  Provision of Medical care such as a Geriatrician, Pre-packing Medication, supervision and Insulin Administration.  Personal hygiene include eye bath, nail, skin care, perineal care etc.  Caregiver counselling, training and group support.  Maintenance of Physiotherapy and Occupational therapy

HOME HELP SERVICES

Community care starts from Home Help care. This concept aims to empower seniors to age with grace and dignity in their own familiar environment with support from institutional care to enable the frail elderly to continue coping with daily living and socialising in their familiar community.

The core components of the Home Help service include Meals-on-Wheels Programme (MOW) and Medical Escort Service (MET).

For the past 21 years, Meals on Wheels has played a crucial role nationally in supporting the health and wellbeing of the elderly living in the northern region of Singapore. Meals on wheels refer to meals delivered daily to the housebound elderly, including public holidays, who are unable to cook or purchase his or her meals.

During the COVID-19 pandemic lockdown, our dedicated staff with the support of our caring volunteers continue to deliver meals to the homebound elderly who would otherwise have gone hungry. We also brought cheer and emotional support to our lonely seniors when we visit their homes. Meals On Wheels

Medical Escort and Transport (MET) Service provide transport and a trained Health Care staff to accompany the elderly and persons with disability to hospitals and polyclinics for their medical appoinments. Upon receiving referrals from hospitals, polyclinics and other Social Service Agencies requesting for medical escort service, we assess each client’s case to understand their needs and help needed.

Those with no care givers and living alone are assisted by a Health Care Assistant to and from hospitals.Those with caregivers are encouraged to bring them along for medical appoinments so that they can be aware of the medical conditions and needs of their loved ones.

The subsidy of MET service will depend on client’s National Means Testing.

Volunteers

Volunteering is the act of an individual or a group of people who freely devote their time and labour for community service.

We have some dedicated and caring volunteers who continued to support SWAMI Home in delivery of our Meals on Wheels to the community during the COVID-19 lockdown period. Some of our volunteers are family members who encourage their children to volunteer at a young age.
