NCAA & PBA COMPLIANCE FORMS

Fall Sports - all NCAA 6 PBA compliance forms are due by July 25, 2022

All other sports - all NCAA 6 PBA compliance forms are due by August 1, 2022

PHYSICALS - SIGNED OFF BY MD, DO, OR NURSE PRACTITIONER

Must be done before you report back to campus in August 2022.

Must be dated June 1, 2022 or later. Physicals dated earlier will not be accepted.

If compliance forms and physicals are not submitted by/before July 25 (fall sports) or August 1 (spring sports), the student-athlete will sit out, at minimum, the first week of practice.
