Accommodations and Services At our community, you will find both private and semi-private rooms, along with shared spaces such as dining rooms, lounge/common areas, activity/program rooms, and outdoor spaces. Private rooms have their own bathroom, while semi-private rooms have a shared bathroom and accommodation for two people. Air conditioning is available in common areas and all resident rooms. Regardless of your accommodation type, you will receive the same care, attention, and services provided to all who live here. Each person’s room is furnished with a bed, mattress, bedside table, chair, and wardrobe/closet. We also provide pillows, bed linens, washcloths, towels, and some toiletries/personal hygiene supplies. Mechanical lifts are provided as required. Laundry and housekeeping services are also provided. We welcome you to personalize your space with furniture, decorations, and other items as space allows, while noting for safety reasons that the room cannot be overcrowded. All rooms have the capability for television and private phone lines, which can also be used for computers. We can provide you with all the information you need to arrange accounts and setup, to be billed directly to you/your designate via the provider(s) you choose.

Care Around-the-clock nursing and personal care is provided. Rooms and washrooms are equipped with call bells/alert system, which when activated; send a signal to one of our team members for assistance. As partners in care, our team will seek your input to learn about your unique needs and preferences to ensure care and services that are tailored just for you, and will work with you to develop care options that best suit your needs You may choose to keep your own Doctor or Nurse Practitioner, or work with our community’s Physicians.
Recreation and Leisure Our community provides many experiences designed to suit a variety of tastes and interests, with social and interactive programs available on a daily basis. Recreation schedules are posted throughout the community, and our team members are available to help connect you with programs and experiences that you will enjoy. We respect individual preferences, and you will have the opportunity to express your interests, which we will do our best to support. You have the freedom to choose how you will spend your time. We are proud of our multicultural and diverse community, and we will accommodate your requirements to the best of our ability to ensure your spiritual, cultural, and personal needs are met. We will support you in participating in religious and spiritual services and activities that are important to you.
Dining All meals, snacks, and beverages are provided by the community, including therapeutic diets, dietary supplements, diabetic-friendly choices, and others as needed. Menus are posted outside the dining room and are refreshed weekly. You will have a choice of selections for three nutritious and appetizing meals daily (breakfast, lunch, and dinner), along with a variety of hot and cold beverages, and snacks throughout the day. These are planned by our culinary experts in collaboration with those living here. If you require a therapeutic diet, our Registered Dietitian will work with you to create a meal plan, including any dietary supplements and assistive devices you may need. 7 | Victoria Manor Welcome to our Community
Before you move in, we will work with you to accommodate any dietary preference you may have such as vegetarian, kosher, halal, and others. You are welcome to invite your family and friends to dine with you in the community. Meal tickets can be arranged for a fee. You and your family/friends may wish to bring your own food into the community and you are welcome to do so. Please check with us if you would like to do this so we can explain the steps needed to ensure safety.
Community Living It is important to understand that a long-term care community is home to a number of individuals with a variety of social, spiritual, cultural, and healthcare needs. It is different from a hospital, which generally provides short-term care, whereas our communities become a new home for those who live here. Some people living in longterm care experience difficulty with verbal communication and may use behaviours to communicate. These behaviours may include things like making unexpected and/or loud noises, agitation or aggression, wandering or restlessness, paranoia, and others. We call these responsive behaviours or personal expressions, and our teams are educated to support and respond to people expressing their needs in this way. We understand that both you and your loved ones may experience many different feelings when faced with such behaviours and in general about entering a community living setting. We are here to help you in any way we can, and we welcome you to reach out to us with any questions or concerns.

How Does Long-Term Care Work? The Role of Long-Term Care Long-term care communities provide 24/7 nursing care and supervision, primary medical care, help with daily activities and interests, and a safe, caring home environment. The majority of people who live in long-term care have some form of cognitive impairment and/or physical disability, along with chronic health conditions that have seriously compromised their health. As part of the health care system, long-term care communities are licensed and funded by the Ministry of Long-Term Care to provide care for individuals who need a level of support beyond what is possible at home. People living in long-term care also pay a fee for their accommodation.
How Long-Term Care Works In Ontario, long-term care communities are regulated and funded by the provincial government. Government agencies determine who is eligible to be admitted to longterm care, and manage the wait lists. Each community is granted a license to operate by the provincial government.

The provincial government provides funding for all the staff and supplies related to nursing and personal care, social and recreational programs and support services, and food (used to make meals). In addition, long-term care communities receive other government funding for specific needs, such as falls prevention equipment. You pay an accommodation fee to the long-term care community that is used to pay for expenses such as non-care staff, utilities and mortgages, as well as building maintenance and major capital repairs (like a new roof). The government sets the rate for these fees and provides subsidies as needed.
Long-Term Care Communities Account for Every Dollar As part of their agreement with the government, all funding provided to communities by the province must be rigorously accounted for, and can only be used for its specified purpose. Any unspent funds related to care are returned to the government. Funding from accommodation fees is also accounted for, and the community uses these funds to pay for accommodation operating expenses, building and equipment repairs and maintenance, property taxes, insurance, and financing costs. Every long-term care community produces an audited annual report documenting the community’s spending, which is then reviewed and audited again by the government. The final report is presented to each community’s Residents’ Council and Family Council and posted publicly in the community.
Ministry of Long-Term Care Long-term care communities are licensed, regulated, and funded by the Ministry of Long-Term Care (MLTC). The Ministry of Long-Term Care conducts unannounced inspections of all long-term care homes throughout the province and publishes its reports online. This information is also posted within the community along with a statement of residents’ rights. Moving Into Long-Term Care Application Process All long-term care applications are submitted through your local Ontario Health atHome office. Applicants must be over 18, possess a valid Ontario Health Card, and have care needs that can be met in a long-term care community.
When You Accept an Offer Ontario Health atHome will recommend that you contact your chosen long-term care community to make arrangements for a move-in day. You will receive information from the community on what you should bring with you, and what to expect in your first days of settling in. The community team will review all of your needs with you and welcome you to share any important information about your wishes and requirements.

Transition and Getting Settled Once you’ve been assessed and officially offered a room, you are welcome to move-in on the pre-arranged date. We provide a bed, bedside table, dresser, and closet or bureau. We encourage you to make the space your own – consider bringing small personal items, a TV, photographs and pictures for your wall, etc. – we want your new space to feel like home.
Spousal Reunification Process Spousal Reunification is a special provision that allows your spouse or partner to come and live with you in your long-term care home. If this is something you are interested in, please speak to your Ontario Health atHome Coordinator or to our Manager of Resident and Family Services for more information and to find out if you may be eligible.
Here is a quick step-by-step guide to the process: 1. Complete an application for long-term care through your local Ontario Health atHome team (visit ontariohealthathome.ca or call 310-2222 no area code required). They will work with you to assess your needs and determine your eligibility. 2. Decide what type of accommodation you would like to apply for – basic, semiprivate, or private. 3. Ontario Health atHome will ask you to provide them with a list of up to five longterm care communities based on your preference. Visit ontariohealthathome.ca/long-term-care/selecting-a-home/ to learn more about choosing the right location for you. 4. If you can, tour your chosen long-term care communities before making your final decision. 5. Ontario Health atHome will send applications to your chosen list of long-term care communities. 6. Once a community accepts an application, Ontario Health atHome will notify you that you have been added to that community’s wait list. times vary depending on where you are located – your Ontario Health atHome Coordinator can help you understand the timelines for your area. 7. You will be notified by Ontario Health atHome as soon as accommodation becomes available – which could be at any of the long-term care communities on your list – and you will have 24 hours to make a decision to accept or decline an offer, with up to 5 days from the date of your acceptance to move-in. 8. If you decline the offer, you will need to reapply for long-term care again, and will be placed back on the waiting list. Costs and Financial Supports You Pay a Portion of Long-Term Care Long-term care is part of the province’s health care system and publicly funded on a cost-shared basis with those residing here. The government does not pay the full cost of long-term care and expects you to pay a portion of your “room and board” to the long-term care community. This accommodation fee or co-payment is what long-term care communities use to fund the ongoing maintenance and operation of the home. The provincial government sets the rates for all long-term care in Ontario. This is done once a year, and includes a review of accommodations, meals, nursing, personal care, and services such as laundry. The government pays the “care portion” of this directly to us and you pay the “copayment” amount/accommodation fee to cover room costs. This is also set by the government, standardized across the province, and based on pension rates. Your payments are made monthly and set up as pre-authorized payments from a bank account.

Additional Charges Additional services are available for a fee, should you require them. These include: • Transportation to appointments • Dental, hearing, and vision care • Medications and treatments not covered under the Ontario Drug Benefit Plan • Telephone, cable TV, internet • Newspaper delivery • Tuck shop purchases • Hair salon and barber services We would also be happy to help you coordinate the purchase of equipment such as wheelchairs, toilet aids, and other self-help items through any of our providers. Please note that you are not required to purchase care, services, programs, or goods through the community. You may purchase these things from other providers if you wish (note that some restrictions may apply with respect to the supply of medications).
Financial Supports and Rate Reductions You may be eligible for a co-payment/accommodation fee rate reduction for basic accommodation. People living in preferred or private accommodation are not eligible for this co-payment rate reduction unless they meet the criteria for exceptional circumstances. In situations where one spouse/partner moves into a community and the other requires financial assistance to remain in their own home, an application for exceptional circumstances can be made. To apply for exceptional circumstances, please complete an Application for Reduction in the Long-Term Care Home Accommodation Fees. Preparing to Move While waiting list times vary across the province, the time between being notified of a vacancy and accepting a room in a long-term care community may be very short. There are some steps you can take to begin preparing to help make the transition easier.
The Importance of Advance Care Planning One of the most important ways you can prepare for moving into long-term care is to discuss your wishes for healthcare, both for general treatments and for end-of-life, with the important people in your life – your family, friends, healthcare provider and Substitute Decision Maker or Power of Attorney for Care. Talk about your values and goals when it comes to decisions such as allowing a natural death. You may want to consider questions such as: • What makes my life meaningful? What does a good day look like? What brings me happiness? • Do I want everything done to prolong my life even if I do not have a good quality of life? What outcomes are unacceptable to me? What am I willing to sacrifice – or not? • Do I have cultural, religious, or spiritual values that influence the decisions made about my care? • What fears or worries do I have about my health changing or about dying? • What would be important to me if I was nearing death? 17 | Victoria Manor Welcome to our Community
These conversations can bring up strong emotions, but it’s important that our team understands what you would want should you be unable to express your needs or decisions. Your advance care plan will enable us to provide the best care and services throughout your time with us. When you move in, one of the first discussions our team will have with you and/or your substitute decision maker will centre on understanding your wishes in these areas. Any time decisions must be made related to your care, we will reach out to you and/or your substitute decision maker to discuss your goals as related to your care and/or treatment. Identifying your wishes and preferences now will help support these conversations down the road to ensure we are proceeding as you would wish.
Before You Move: • Check in with your family doctor to confirm whether they will continue to provide care after your move, or whether you will transition care to one of the physicians associated with the community. • Tour the community (or communities) you have chosen to apply to. It will help you determine what you would like to bring with you. • Start thinking and planning for what you want to bring with you. Begin labelling your personal items (e.g. eye glasses, hearing aids, dentures, radio, clock, etc.). • Make a list of people to inform about your change of address. • Talk with family, friends, and your healthcare provider about designating your Substitute Decision Maker/Power of Attorney for Care and Power of Attorney for Property. • Check to see if you qualify for financial benefits and have your most recent Income Tax Notice of Assessment available if you are applying for an income subsidy. Reach out to your Ontario Health atHome Coordinator or to our team with questions and for support with getting ready.

We encourage you to reach out to people who have already gone through it – to answer questions and make recommendations – or even just to talk about what you are going through. Every resident has the right to meet privately with their spouse or another person in a room that assures privacy. Every resident has the right to pursue social, cultural, religious, spiritual and other interests, to develop their potential and to be given reasonable assistance by the licensee to pursue these interests and to develop their potential. Every resident has the right to live in a safe and clean environment. Every resident has the right to be given access to protected outdoor areas in order to enjoy outdoor activity unless the physical setting makes this impossible. Every resident has the right to keep and display personal possessions, pictures and furnishings in their room subject to safety requirements and the rights of other residents. Every resident has the right to manage their own financial affairs unless the resident lacks the legal capacity to do so. Every resident has the right to exercise the rights of a citizen.

Every resident has the right to proper accommodation, nutrition, care and services consistent with their needs. Every resident has the right to be told both who is responsible for and who is providing the resident’s direct care. Every resident has the right to be afforded privacy in treatment and in caring for their personal needs. Every resident has the right to, i. participate fully in the development, implement. participate fully in the development, implementation, review and revision of their plan of care ii. give or refuse consent to any treatment, care or services for which their consent is required by law and to be informed of the consequences of giving or refusing consent, iii. participate fully in making any decision concerning any aspect of their care, including any decision concerning their admission, discharge or transfer to or from a long-term care home and to obtain an independent opinion with regard to any of those matters, and iv. have their personal health information within the meaning of the Personal Health Information Protection Act, 2004 kept confidential in accordance with that Act, and to have access to their records of personal health information, including their plan of care, in accordance with that Act. Every resident has a right to ongoing and safe support from their caregivers to support their physical, mental, social and emotional wellbeing and their quality of life and to assistance in contacting a caregiver or other person to support their needs. Every resident has the right to have any friend, family member, caregiver or other person of importance to the resident attend any meeting with the licensee or the staff of the home. Every resident has the right to designate a person to receive information concerning any transfer or any hospitalization of the resident and to have that person receive that information immediately. Every resident has the right to receive care and assistance towards independence based on a restorative care philosophy to maximize independence to the greatest extent possible.

Every resident has the right not to be restrained, except in the limited circumstances provided for under this Act and subject to the requirements provided for under this Act. Note: On a day to be named by proclamation of the Lieutenant Governor, paragraph 24 of subsection 3 (1) of the Act is amended by striking out “restrained” and substituting “restrained or confined”. (See: 2021, c. 39, Sched. 1, s. 203 (3)) 25. Every resident has the right to be provided with care and services based on a palliative care philosophy. 26. Every resident who is dying or who is very ill has the right to have family and friends present 24 hours per day.

Every resident has the right to be informed in writing of any law, rule or policy affecting services provided to the resident and of the procedures for initiating complaints. Every resident has the right to participate in the Residents’ Council. Every resident has the right to raise concerns or recommend changes in policies and services on behalf of themself or others to the following persons and organizations without interference and without fear of coercion, discrimination or reprisal, whether directed at the resident or anyone else: i. the Residents’ Council. ii. the Family Council. iii. the licensee, and, if the licensee is a corporation, the directors and officers of the corporation, and, in the case of a home approved under Part IX, a member of the iv. committee of management for the home under section 135 or of the board of management for the home under section 128 or 132. v. staff members. vi. government officials. vii.any other person inside or outside the long- term care home.

Further Guide to Interpretation (2) Without restricting the generality of the fundamental principle, the following are to be interpreted so as to advance the objective that a resident’s rights set out in subsection (1) are respected: 1. 2. 3.
This Act and the regulations. Any agreement entered into between a licensee and the Crown or an agent of the Crown. Any agreement entered into between a licensee and a resident or the resident’s substitute decision-maker.
Enforcement by the Resident (3) A resident may enforce the Residents’ Bill of Rights against the licensee as though the resident and the licensee had entered into a contract under which the licensee had agreed to fully respect and promote all of the rights set out in the Residents’ Bill of Rights.
