Our approach Individualized Programs. We realize that everyone is different and so are their needs. Our therapists work to develop an individualized rehabilitation program that takes into account those differences. Southgate offers inpatient and outpatient programs. Our trained staff will determine the program best suited for you after an extensive evaluation of your needs and goals.

Free Weights

OUR REHAB FACILITY Our facility is designed to teach independent living skills and our full-time therapy staff is supported by the latest therapy equipment.

Apartment Retraining Center Our Apartment Retraining Center is designed to teach independent living skills for your safe return home. Independent living skills include: cooking, cleaning, vacuuming, doing laundry, making a bed, and any other activity required to live independently. Our apartment includes "home like" furnishings with an attached bathroom and access to a full kitchenette.

Kitchen retraining We are one of the few facilities in the area that offers a kitchen retraining area as part of our rehab program. Our therapy staff will teach safe meal preparation, as well as assess ability to safely maneuver around a kitchen environment. Prior to discharge, recommendations will be made for a safe transition back home. These recommendations could include the use of a timer, adapting the location of cooking supplies for safe access, and visual aids to assist with memory.

We utilize free weights to increase your muscle strength and range of motion. The advantages of free weights over exercise machines are: •

Free weights allow the patients to strengthen specific muscles and/or muscle groups while not limiting movement. When using exercise machines you are restricted by what machines allow you to do. • Free weights provide more of a challenge for patients because the patient must stabilize their body while doing an exercise that allows full range of motion. • After in-house therapy, patients will receive a home exercise program utilizing free weights.

Electrical Stimulation and Ultrasound Electrical Stimulation and Ultrasound treatment programs are integrated into our physical and occupational therapy treatment sessions. These programs are designed to address the complex needs of each patient and enhance the recovery process. Both electrical stimulation and ultrasound provide noninvasive treatments to assist with pain management, wound healing, increasing circulation, relaxing muscle spasms, decreasing joint stiffness, and maximizing muscle contractibility and strength.

Our Services Physical Therapy Our Physical Therapist provides treatment to individuals who have experienced a decline in physical functioning such as difficulty walking, muscle weakness, fractures, or tight joints. Using specialized equipment and techniques we teach you how to address problems that cannot be managed without skilled Physical Therapy intervention such as: • Stretching, strengthening and conditioning exercises • Walking, balance and fall prevention • Pain management • Wound care • Assessment of equipment needs

Occupational Therapy The Occupational Therapist provides treatment to individuals who have difficulties with functional daily living skills following an illness or injury. We teach you how to simplify the work and conserve your energy while... • Making the bed • Getting in and out of chairs • Going to the restroom independently • Bathing • Dressing • Cleaning • Cooking and planning meals • Opening jars

To ensure a safe and successful return home our licensed therapists will make a “Safe Home Visit”, prior to discharge and follow-up with additional visits after discharge if necessary. We will evaluate the safety of the home and make suggestions regarding household management. We realize everyone’s home is different and we feel this is an important element of our program. Our goal is a safe environment where you can continue your independent living…your “home”.

Speech Therapy Our Speech/Language Pathologist provides therapy for individuals who are experiencing difficulty with speech, hearing and/or swallowing. Our Speech Therapy program assists you in regaining your ability to communicate and swallow. We will also design a program to help with memory and thinking skills.
