Staffing Update for CBHR SW Support
We wanted to notify the CBHR clinic that Jessica Reyes, SW located at CBHR, is out of the office on leave and not expected to return until August 1st We know many team members and providers at CBHR rely on Jessica for support with transportation, medication vouchers, crisis calls and resource needs for patients If you need transportation or a medication voucher for your patient, please notify the PAR team at CBHR
We have a Microsoft Team set up for voucher and transport requests with all clinic PARs and all Outpatient SWs/LPC,s so the PARs can put the request in this Teams chat and it will get completed Please remember that the CDC COVID Transportation Grant ended on May 31st, so we only have transportation funds for our PEC Overflow patients (the patients sent out to a community psych hospital on contract from JPS), and patients participating in PHP, IOP, BH Day Rehab, and START MAT

If you have case management/resource needs, please remember that we now have REF250-Referral to BH Transitions and Care Management as an additional support for patients When you put in this referral, a Transition Coordinator (SW) will contact the patient to support them with whatever case management needs you identify in the referral
Our Embedded BH Specialists (who support community health providers with BH needs in several JPS Medical Homes) are also willing to help if you have a social services related question or need for a patient in Jessica’s absence You can reach any of these team members via Microsoft Teams: Maria Albertson, Kaelen Barclay, Dalila Campos, Marquita Martin, and Courtney Wilson

Maria is currently covering the BH virtual guidance line for Jessica so if community health providers call the virtual line requesting BH medication guidance, Maria will be reaching out to the providers on Open Access via Teams chat to get assistance
