Clients who do not have the ability to change their position and weight bearing independently need a 24 hour postural care programme. Assessment Service JCM offer a free, no obligation, assessment service undertaken by experienced Product Specialists. Our national team can work alongside you to help find the best solution, as well as offering degrees of product customisation. Workshop and Training Packages at JCM The workshops provide an opportunity to enhance the clinical reasoning skills for determining the optimal seating and positioning equipment. It applies evidence-based knowledge to seating and postural equipment. Participants also have the opportunity to reflect on their own experience and to develop the practical skills to complete a comprehensive and holistic assessment and informed equipment choices. In addition, those taking part can also gain practical knowledge with seating and positioning with a hands on approach to develop skills. Workshops may be tailored for parent and carer groups, nursery and school staff.

Postural management and JCM At JCM we work on the premise that human posture can be defined as the position of one or more body segments in relation to one another and their orientation in space. The head, trunk, pelvis, lower limbs and feet are known as body ‘segments’, while the spinal joints, hips, knees, ankles and shoulder joints are considered the body ‘linkages’. This means all have to be thought about and accommodated, and we know that when considering posture it should be seen as an active and dynamic process which underpins movement and function, as well as our health and well being. Posture and movement are inextricably linked, they work together! Our posture at any one time is a temporary rested movement, which is in a constant state of change. It is never static and our equipment is designed to respond to these ever changing snapshots of posture.

Equipment needs to achieve a balance with all these factors whilst also considering its environment and practical image of the equipment in everyday situations. By considering as many factors as possible we are able to work towards our goal of true 24 hour postural care. What does 24 hour postural management equipment need to do? True holistic solutions should consider all of the following:-

This can be facilitation, correction or maintaining the posture. For example, optimum positioning enables the pelvis, trunk and shoulder girdle to remain stable so that the hands are free to function.

Good posture can help develop many skills, for example perception: an upright posture helps develop three dimensional depth and distance awareness, as well as general communication and levels of concentration.

Social Inclusion: When development is delayed, independent sitting, standing or walking may not be achievable. Postural equipment becomes a vitally important tool socially - as most people interact in different positions in different environments and different times.

Functional and Environmental Goals: Any individual of any age with any illness or disability may benefit from some type of postural management support to improve their quality of life, and interact with their wider environment, family, friends and carers in a positive way.

How do we do this? Where does good posture start? Postural management equipment aims to address general clinical goals by providing optimum:

Good posture begins with the pelvis - the most important feature of any postural management equipment or mobility system is its ability to align or accommodate a range of pelvic positions. This gives the best possible base of support.

Why? A focus on pelvic control maintains an upright stable pelvis in its neutral position. In this position the spine, shoulder girdle, trunk, head and neck can function in their most stable position. Our spines respond to the correct pelvic angle which means that the spine too can be active in its natural upright curves. Pelvic stability – good positioning at the hips means our limbs are encouraged to lie in a mid-range position which

can minimise abnormal tone. This can help prevent further complications such as muscle contracture or hip pain and subluxation which can require surgical intervention. Options in sacral positioning - optimal positioning at the sacrum should be used to accommodate specific positions - we are all different. For example this maybe used to minimise posterior tilt and allow the extensor spasm to occur without the user’s pelvis sliding out of alignment, or accommodate spinal rotation by rotating and contouring with the hips and spine.


Trunk and head alignment, upper limb The aim is for stability and symmetry. When we have a good level of stability at the trunk and head this contributes to the stability of the pelvis and facilitates arm and hand function, concentration and social interaction. Good trunk control helps retraction of the shoulder girdle into the neutral position which means that the upper limb is able to get on with tasks and fine motor activities can be facilitated allowing activities such as meals, washing and dressing, writing, computer work, hobbies and playing games. Daily activities are more easily performed and engaged in a context with family, carers, teachers and peers more readily.

Lower limb, legs and feet: A focus on the lower limb adds to a more stable symmetrical and functional position. For example tightness in the hamstrings and unsupported feet can contribute to instability of the pelvis, and therefore the alignment of the trunk and head, impacting overall posture. Scissoring or wind sweeping can be accommodated as well as

real or apparent leg length discrepancies. The ability to accommodate wind sweeping and spinal rotation means that the user can be fully supported while the trunk and head face forward, improving opportunities for social interaction.

Dynamix as a clinical tool and outcome measure This dynamix system is ideal for a user with a strong extensor spasm and a varying muscle tone. The system allows the extensor spasm to occur in a totally controlled fashion, eliminating any unwanted feed back or stimulation from uncontrolled movement of the seating system. The system facilitates smooth movement and return in the same way in which the body moves naturally: as the air pressure mechanism ensures that any movement in the back is smooth and controlled. By using a variable pressure piston you are able to manipulate two factors: firstly you must set the strength of the resistance of the back, secondly the speed of its return to its original position – thus you have two clinical variables which are able to be manipulated and measured directly as clinical outcome measures in the context of evidence based practice, and used as part of a therapeutic tool kit in 24 hour postural management plans.

Superior Dynamics The unique dynamic back is easily adjusted to control the level of resistance the user feels as they extend in the chair. This mechanism is not a spring action, as commonly used on dynamic chairs, but is air pressure controlled and the recoil is also adjustable so the back moves in harmony with the user rather than pushing them forward as they relax. These features, unique to Triton, give the chair a controlled, gentle movement with the flexibility to adjust the system as the user grows or presents differently. The dynamic mechanism works independently to the chair’s reclining back. A locking function can be simply engaged for periods of feeding, transferring and transportation.

Air Pressure Controlled Adjustable Works in harmony with the user

Controlled Pelvic Positioning In conjunction with the hip supports, the wide range of adjustment in the sacral pad allows you to position the pelvis precisely. As well as accommodating adjustment for anterior and posterior positioning, the sacral pad can be adjusted to accommodate rotation. This can be used to support a fixed position or assist with de-rotating the pelvis.

Dynamic Footplate Four independent springs allow the footplate to move in any direction. This feature absorbs forces through the feet, protecting vulnerable joints and reducing footplate failure. In relation to the reclining and dynamic back functions the sacral pad works independently. This helps maintain a good position at the pelvis as the backrest moves.

To determine the belt size required, the recommended method for measuring hip width is from trochanter to trochanter (with the user seated). If this is not possible, measure from ASIS to ASIS and add a couple of inches. If the hip width falls between sizes on the chart, consider future factors such as growth, weight changes and clothing. The Trekker is designed for children and adults who do not have the functional gait skills or postural control to safely and independently use a standard walker. Moonlite sleep system offers continuing support complimenting the day time seating systems from JCM. The system has a light, easy to clean plastic sub frame that can be flexed to profile the shape you require, supporting your client from head to toe. Supine or Side Lying Supine or side lying can be accommodated through a modular and flexible range of components that can be utilised independently or in conjunction with Moonlite. This provides a solution-led approach to comfortable and posturally supportive sleeping. The knee cup system helps maintain alignment of the legs in a supine lying position – providing midline symmetry and preventing windsweeping through secure support of the knees. The knee cup system can be finely adjusted to ensure the correct degree of abduction or adduction.

If side lying is indicated, a leg support and side lying table helps position and abduct the legs. FollowMeMOMO for independent cycling or for being pulled along After the one-off assembly, FollowMeMOMO can remain permanently attached to an adult’s bicycle thanks to its low weight of only 5.1 kilograms. To assemble for the first time, the rear wheel axle of the leading bicycle (26” or 28”) merely has to be removed, and the special parent-child tandem coupling axle included in delivery has to be fitted. Proximat® – a practitioner developed, evidence based solution which provides increased accuracy and greater flexibility of use for the measurement of hip range movement in children with cerebral palsy. Programmes and interventions for children with cerebral palsy aim to reduce deformity, improve comfort and enhance functional skills. To monitor the effectiveness of these interventions, accurate measurements are needed using simple tools in daily clinical practice. Although using x-rays is the gold standard, it is important to measure hip range movement in between these assessments. Traditionally, these inter-x-ray assessments have been performed using a goniometer, but this handheld device can be difficult to use and provide inaccurate measurements. It was during a particularly awkward assessment that Patsy Pott, a physiotherapist working within Manchester PCT, had the idea for Proximat®. Soon afterwards the idea progressed to a protractor drawn on to a piece of vinyl and colleague, Andrea Selley brought the skills to conduct a reliability study within the local community -

The study provided evidence that Proximat®, as well as being easier and simpler in use than a goniometer, proved to be a reliable tool for measuring range of movement. Through analysis of test/retest (testing on separate days) results showed that error with Proximat was only 3° to 12°. This compares very favourably with previously published results for goniometry of error of 15° to 28°. Proximat® as a product is the result of an NHS-industry collaboration, facilitated by TrusTECH® (NHS Innovations North West) The Proximat® is child friendly, lightweight and versatile. Intended for use in either a clinic on a mat or plinth or in the home on a carpet, portability and flexibility are built in.

PERMABLOK3 ® is a proven vinyl protective coating that has been designed to create a tough, effective barrier against three major problems in the education, healthcare and hospitality environments – germs, abrasion and stains. PERMABLOK3 ® not only guards against stains, abrasion damage and the surface growth of fungus, mould and mildew spores – it also provides superior antimicrobial and antibacterial protection by resisting gram negative and gram positive bacteria (including MRSA), filamentous fungi and yeast.

Cleaning & Care Clean using a damp, soapy cloth or sponge, and then rinse with cold water and wipe dry. Spills and accidents require immediate attention for best results. In many cases, stains such as coffee, tea, cocoa, ketchup and blood, may be cleaned simply with warm water alone. Stains from chemical colourings such as ballpoint pens, dyes and food colourings should cleaned off as soon as possible to avoid long-term staining. If the stain is allowed to set, more concentrated cleaners may be required. Generally speaking, always start with the mildest cleaning agents first. Never use harsh powdered abrasive cleansers or steel wool.

Cleaning & Care Regularly wipe over with a damp soapy cloth. Clean stains immediately to avoid absorption into the vinyl. Strong detergents, solvents, polish, wax or patent chemical cleaners must not be used as they could damage the material.

Wipe frequently with a damp cloth to maintain the appearance of the fabric. Most stains can be removed with cold/warm water and a clean cloth. Use cold water when treating stains caused by bodily fluids. If soap based cleaners are used, all soap residues must be rinsed from the fabric. Can be cleaned with mild hypochlorite solutions (half a cup of household bleach with 5 litres of water) or mineral spirit. Can be steam cleaned in situ.
