Driver’s license

Arizona law requires new residents to obtain an Arizona driver’s license; there is no grace period.

To obtain a license or permit, you need:

• a previous license or permit and one other form of identification; if between ages 16 and 18, two identification documents, such as birth certificate and Social Security card

• to pass all required examinations

• to pay the required fee

• to provide a registered road test

• a vehicle that meets insurance requirements

◆ DEFINITIONS

Adult day care

Adult day care, the supervised care of older adults, provides activities such as meals and socializing one or more days a week during specified daytime hours.

Participants – who primarily have physical and/or cognitive limitations and need physical and/or psychological assistance and a chance to socialize – return to their homes each evening. Adult day care is often used to provide a respite to family members caring for an older person who cannot safely be left alone in the home.

Home-health services

Home-health services, medically related services provided to patients in a home setting rather than in a medical facility (such as a hospital or a primary health care center), include medical or psychological assessment, wound care, medication teaching, pain management, disease education and management, physical therapy, speech therapy or occupational therapy.

Practitioners providing homehealth care aim to help patients increase their ability to perform the daily activities of life. Home-health care can be covered under most insurance programs, but one should always verify the coverage before signing up for this service.

Nonmedical home care

Nonmedical home care helps with the day-to-day tasks of living and provides some companionship. Such care helps both seniors and their families by taking some of the stress out of daily caregiving.

Nonmedical senior care companies focus on companionship, but also assist with daily needs, such as meal preparation, running errands, medication reminders, light housecleaning, incidental transportation to doctor visits and other nonmedical tasks.

This is currently an unregulated business, so potential clients should vet companies thoroughly before signing up for their services.

Medicare, other insurance

The Medicare alphabet can confuse the savviest consumer. Medicare Part A is hospital/acute care insurance. Medicare Part B covers ancillary services and physician visits. Medicare Part D is a prescription benefit. In addition, there is Medigap (providers include AARP, United Health Care, Aetna, etc.), a type of private insurance that helps cover some costs that Medicare doesn’t pay. In addition, because Medicare doesn’t cover care for chronic conditions, disabilities or other conditions over an extended period, seniors and their families should consider long-term care insurance to cover such needs should they arise.

Senior centers are community agencies for older adults. These centers offer nutritional, recreational, educational, health and legal services. For a listing of senior centers in Phoenix, visit phoenix.gov/ humanservices.

◆ RESOURCES Arizona Health Care Cost Containment System (AHCCCS)

Arizona’s Medicaid agency, known as AHCCCS (Arizona Health Care Cost Containment System), provides the Arizona Long Term Care System (ALTCS) for individuals who are 65 or older, blind or disabled and need ongoing services at a nursing facility level of care. However, program participants do not have to reside in a nursing home.

AHCCCS (pronounced “access”) uses several contractors to provide long-term care services under ALTCS. An ALTCS contractor works like a health maintenance organization (HMO). The program

contractor works with doctors, nursing homes, assisted-living facilities, hospitals, pharmacies, specialists, etc., to provide care. Each enrollee is assigned a case manager who will coordinate their care. Many ALTCS participants live in their own homes or an assisted-living facility and receive needed in-home services. care

There are several agencies in the Valley that provide services for Alzheimer’s care. Community-based services, such as Banner Alzheimer’s Institute and the Alzheimer’s Association, provide resources and research to the community. There are also many places throughout the Valley that offer full-time care for a loved one who is suffering from various forms of dementia.

Alzheimer’s Special Care Units (SCUs) exist to better meet dementia residents’ needs and to protect residents without dementia in nursing homes and residential-care facilities. Special care unit growth has become a challenge for Alzheimer’s advocates concerned about the quality of long-term care.

Nearly 60% of nursing home residents have Alzheimer’s or another form of dementia. In recent years, the number of nursing homes that have developed SCUs designed to serve residents with Alzheimer’s disease has proliferated. Special care dementia units now represent one of the fastestgrowing parts of the nursing home business.

Until recently, facilities did not have to meet any specific standards, beyond the nursing home regulations and standards to which all nursing homes are held, in order to promote themselves as being special-care units.

Many facilities did not offer the same services or a comparable level of care, making it impossible for families of Alzheimer’s patients to determine a correct placement for their loved one. Often, the term special care unit was used as a marketing scheme where the

only distinguishing factor was that the unit was locked.

Area Agency on Aging

The Area Agency on Aging is part of a national network of organizations established to respond to the needs of older adults. As a private nonprofit corporation, the Area Agency on Aging plans, coordinates, develops and delivers services for seniors (60 years and older), persons of any age who are HIV positive, adults (18 to 59 years) with disabilities and longterm care needs and family caregivers of older adults.

Hospice provides good-quality, compassionate care for people facing a life-limiting illness or injury. Hospice care uses a team-oriented approach to provide medical care, pain management and emotional and spiritual support expressly tailored to the patient’s needs and wishes. Support is provided to the patient’s loved ones as well.

HUD housing (Low-income housing)

Looking for housing options for yourself, an aging parent, relative or friend? Do some research first to determine what kind of assistance or living arrangement is needed, and what health insurance might cover. Then, check the website of HUD (the U.S. Department of Housing and Urban Development) for financial assistance resources and guides for making the right choice. HUD counselors are available to answer any questions. HUD Housing options include apartment living and staying in your home. Staying in the home: Options include reverse mortgages for seniors, federal housing programs, housing counseling and rural housing loans.

Apartments: Options include apartments for the elderly and people with disabilities, finding affordable rents, public housing, housing choice vouchers (Section 8) and rural rental help.

Meal programs

There are several different meal programs available for seniors in the Valley. The Area Agency on Aging is an excellent resource to find the mealdelivery service in your area. Several private-pay meal-delivery services are available as well. Many of the meal programs ask for a donation or a nominal fee.

Social Security

How survivor benefits work: Currently, Social Security retirement benefits terminate at death. However, if two people have been married for at least nine months at the date of death (except in the case of an accident, which would provide another process through pre-existing insurance policies), a survivor benefit is available to the widow/er. Survivor benefits are based on two factors:

1. The age at which the deceased spouse originally claimed his or her benefit.

a. If the deceased claimed the benefit before full retirement age (FRA), the survivor benefit will be limited to the higher person’s actual benefit or 82.5 percent of the FRA benefit.

b. If the deceased person died before FRA without taking his or her own retirement benefits, the survivor benefit will be equal to the deceased person’s FRA benefit.

c. If the deceased person lived past the FRA or claims after FRA, the survivor benefit will include all the maximized benefits accrued.

2. The age at which the surviving person claims the survivor benefit.

a. If the surviving person claims before FRA, the survivor benefit will be reduced.

Some CILs also offer travel training, where seniors can learn the overall transit system and how to use public transportation so that they can be more independent in the community. Seniors may be required to pay for transportation services; however, travel vouchers might be available to help defray costs. The Arizona Statewide Independent Living Council lists the CILs in the state (including Arizona Bridge to Independent Living, see listing below) on its website, azsilc.org. Contact the CIL in your area about the availability of transportation services.

VA benefits

The U.S. Department of Veterans Affairs (VA) provides payments to help veterans and their survivors who require the “aid and attendance” of another person or who are housebound.

Veterans and survivors who are eligible for a VA pension may be eligible for these benefits, which are paid in addition to a monthly pension. They are not available to those who are ineligible for a VA pension.

Since the Aid and Attendance and

Housebound allowances increase the pension amount, people who are not eligible for a basic pension because they exceed the income limit for a pension may be eligible for a pension at these increased rates. A veteran or surviving spouse may not receive Aid and Attendance benefits and Housebound benefits at the same time.

The Aid and Attendance (A&A) increased monthly pension amount may be added to a monthly pension if the applicant meets one of the following conditions:

• Requires the aid of another person in order to perform personal functions required in everyday living, such as bathing, feeding, dressing, attending to the wants of nature, adjusting prosthetic devices or protecting himself or herself from the hazards of their daily environment.

• Is bedridden, in that the disability or disabilities require that the applicant remain in bed apart from any prescribed course of convalescence or treatment.

• Is a patient in a nursing home due to mental or physical incapacity.

• Has eyesight limited to a corrected 5/200 visual acuity or less in both eyes; or concentric contraction of the visual field to 5 degrees or less.

Housebound

This increased monthly pension amount may be added to a pension when the applicant is substantially confined to his or her immediate premises because of permanent disability.

How to apply

Apply for Aid and Attendance or Housebound benefits by writing to the VA regional office where the original claim for pension benefits was filed. If the regional office of jurisdiction is not known, the request may be filed with any VA regional office. Copies of any evidence, preferably a report from an attending physician validating the need for Aid and Attendance or Housebound care should be included.

The report should be in sufficient detail to determine whether there is disease or injury producing physical

or mental impairment, loss of coordination or conditions affecting the ability to dress and undress, to feed oneself, to attend to sanitary needs and to keep oneself ordinarily clean and presentable.

Whether the claim is for Aid and Attendance or Housebound, the report should indicate how well the applicant gets around, where the applicant goes, and what he or she is able to do during a typical day. In addition, it is necessary to determine whether the claimant is confined to the home or immediate premises.

Senior housing (Continuing Care Retirement Communities)

Continuing Care Retirement Communities (CCRC) offer residents the ability to “age in place” with the knowledge that they won’t need to move to other facilities for health reasons. Whereas most retirement communities offer independent living and assisted living only, Continuing Care Retirement Communities provide a continuum of care that includes independent and assisted living, secured Alzheimer’s care and skilled-nursing care.

Each level of care is provided in a separate building and/or area of a campus and nearly all residents move into the community while living independently (without the need for assisted-living or skilled-nursing services).

Contracts

CCRCs may offer three basic types of contracts: life care (Type A), modified (Type B), and non-traditional (Type C). The primary difference between the first two is the amount of skilled-nursing care received, without a change in the monthly fee. In the third type, the resident is financially responsible for all care.

Life Care Contracts (Type A)

Life care contracts include unlimited enriched-housing/assisted-living care (if offered by the community) and unlimited skilled-nursing-facility services, along with independent housing and residential services and amenities. The resident’s monthly fee cannot change due to a change in the level of covered health care needed by the resident (except for normal operating costs and inflation adjustments).

Modified Contracts (Type B)

Modified contracts include independent housing and residential services but include only a limited number of days in a skilled-nursing facility. The number of skilled-nursing days covered in a modified contract may vary, but the contract must include at least 60 days of skilled-nursing-facility coverage, exclusive of Medicare days. As in the life care contract, the resident’s monthly fee cannot change due to a change in the level of covered health care needed during the skilled-nursing-

Modified contracts may also include an enriched-housing/assisted-living benefit (if this service is offered by the community). If included in the contract, the resident’s monthly fee cannot change due to a need for enrichedhousing/assisted-living services during this covered benefit period. When the contract’s health-care-facility days have been exhausted, the resident must then pay a market or per diem rate if additional skilled-nursing-care days (or enriched-housing/assisted-living days) are needed.

Non-traditional, fee-for-service contracts (Type C)

Non-traditional, fee-for-service continuing-care contracts include independent housing, residential amenities and social activities, and access to a continuum of long-termcare services. The long-term-care services, enriched-housing/assistedliving and skilled-nursing-facility care are available on a fee-for-service or per diem basis. There is no long-term-care benefit included in the contract; the resident pays for long-term care if and when needed.

Senior housing (Assisted Living)

Assisted living is an alternative to the CCRC independent-living concept. It is not appropriate for people who need 24-hour nursing care. Instead, assisted living is more of a philosophy of care that promotes the residents’ dignity.

These facilities provide supervision and assistance with activities of daily living, coordination of services by outside health-care providers and monitoring of resident activities that ensure their health, safety and well-being.

Senior housing (Skilled-nursing/ Long-term Care)

A nursing home is normally the highest level of care for older adults outside of a hospital. Nursing homes provide what is called custodial care and help with their activities of daily living, including getting in and out of bed, and providing assistance with

feeding, bathing and dressing.

However, nursing homes differ from other senior housing options in that they also provide a high level of medical care. A licensed physician supervises each patient’s care and a nurse or other medical professional is almost always on the premises. Skilled-nursing care is available on site, usually 24 hours a day. Other medical professionals, such as occupational or physical therapists, are also available. This allows the delivery of medical procedures and therapies on site that would not be possible in other housing.

Skilled nursing is usually used to describe a Medicare stay for an acute medical condition, such as a broken hip or stroke. Once a resident has completed the therapy for standard recovery covered by Medicare, the resident either graduates and goes home or moves into long-term-care living at the nursing home.
