I have great pleasure in presenting the April 2014 ISHLINK newsletter and informing you on the activities of the institute.

Welcomes

I am extremely happy to inform you that Prof. R. Rangasayee (Ex-Director and Professor, AYJNIHH, NIOH, NIMH, CRC-Bhopal and Ahmedabad) has joined the Institute as Director Technical from February 2014. Prof. Rangasayee with an illustrious career in speech and hearing in India, brings with him a wealth of experience as head of several National Institutes in the country. Dr. Madhuri Gore is our Principal and also Deputy Director- Technical. I congratulate Dr. Madhuri Gore, for receiving the Padmashree Prof. S. Kameswaran Oration award from the Indian Speech and Hearing Association.

New Trustee

We were very happy to conduct the 4th Graduation Day of our Sunaad School for children with hearing impairment. Students of class X after completing 13 years of education from preschool to class X were getting ready to step into a different world of educational excellence. It was a great occasion that showed new ways and a turning point in their career to march, ahead and far, in their lives. We wish them all the very best and assure them that we will be there to support them in all their future pursuits.

I am happy to note that the Students Association of Speech & Hearing (SASH) had an active quarter with several team building activities, sports week and the talents day – Srishti-2014. Students and staff also participated in different seminars, workshops and conferences and presented their research work.

Our Founder Late Dr. S. R. Chandrasekhar’s very humble initiative with great thought, vision and dream, with unstinted efforts and hard work, with goals and objectives, has taken us through this great journey of accomplishments in service of society. We continue to strive to achieve greater heights.

I hope the readers would enjoy reading this issue. As always, we look forward for feedback and suggestions from all readers. Please feel free to pen down your thoughts and send them across to dr.srcish@gmail.com.

Bangalore Speech and Hearing Trust Meeting

The Bangalore Speech and Hearing Trust held their meeting on 26th February 2014. Trustees discussed issues related to development activities, allocation of funds and grants for various activities of the institute, events related to college, institute, and hostels. The Trustees took appropriate decisions on policy matters and monitoring aspects. The Trust extends its grateful thanks to benevolent donors for release of funds for various activities and for their voluntary unstinted support and patronage.

Governing Council Meeting

The Governing Council of the Dr. S. R. Chandrasekhar Institute of Speech and Hearing met on 23rd January, 2014 with Sr. Genevieve as the chairperson. The Governing Council reviewed the academic activities of the institute.

Local Inquiry Committee of Bangalore University visits the Institute

The Local Inquiry Committee (LIC) of Bangalore University inspected the Institute on 28th January, 2014 for granting affiliation to BSLPA, MASLP, M.Sc. (SLP) and M.Sc. (Aud) courses for the academic year 2014-15. The members visited all the departments including the newly inaugurated College Block on the 3rd Floor of the Institute building. Report from the Committee is awaited.

16-01-2014 Homage to Lion. A.P. Naidu Lion. A.P. Naidu, the Founder Trustee has rendered excellent services to the Trust for more than three decades. He always came forward with good suggestions and made invaluable contribution to the Trust. Lion Naidu served as the Treasurer for nearly three decades and meticulously maintained the finances of the Trust. In his demise, the Trust has lost a benevolent supporter.

We pray for his soul to rest in peace.

Bangalore Speech and Hearing Trust is happy to inform the appointment of Prof. R. Rangasayee as Director (Technical) of the Institute. Dr. (Mrs.) Madhuri Gore, Principal has been promoted as Deputy Director (Technical) and Principal. Prof. R. Rangasayee joined as the Director (Technical) at the Institute on 17th February, 2014. Prof.

Rangasayee brings with him a wealth of professional experience spanning over 38 years in the area of speech and hearing.

Trustees of Bangalore Speech and Hearing Trust along with the staff and students of the institute gathered together for a brief function to welcome Prof. Rangasayee and Dr. Madhuri Gore. Sri M.S. Venkatesh, Director and Chairman - Bangalore Speech and Hearing Trust welcomed Prof. Rangasayee to the ISH family and gave a brief background on the activities of the institute. Dr. Prathibha Karanth introduced Prof. R. Rangasayee to the staff and students highlighting his immense experience and expertise in the field of speech and hearing. Dr. Rashmi Bhat, Vice Principal, highlighted Dr. Madhuri Gore’s contribution to the institute over the past 32 years. CA D.L. Suresh Babu - Secretary, Sri V.S. Venkatesh We are very proud to announce that the Public Relations Council of India conferred the coveted National level ‘CHANAKYA’ Award for Empowerment of special children on our Chairman Rtn. M.S. Venkatesh in recognition of services to community rendered through Sunaad School for Hearing Impaired and Dr. S. R. Chandrasekhar Institute of Speech and Hearing. The readers may kindly recall the proud moment when the ‘CHANAKYA’ Award was conferred on our Founder Late Dr. S. R. Chandrasekhar in the year 2007 as NGO of the year. We are very proud to announce that Dr. Madhuri Gore, our beloved Principal received the Padmashree Prof. S. Kameswaran Oration Award from Indian Speech and Hearing Association at the 46th Annual conference of the association at Kochi from 7th to 9th February, 2014.

Dr. Madhuri Gore delivered her oration titled “Understanding audition through cochlear implants: a personal journey”.

The award recognizes Dr. Madhuri Gore’s outstanding clinical excellence and exemplary leadership in the area of Audiology in her long career spanning more than thirty two years. After training at the M/S Cochlear, Melbourne, Australia in 1997, Dr. Madhuri Gore successfully established a habilitation program for cochlear implant recipients at the Institute which was one of the earliest in the country. She spear headed the program, for the benefit of children from within the country and neighbouring countries as well. Her strength as an adept clinician and passion to continually update her knowledge and skills have made her a much sought after expert in the area of habilitation of cochlear implant recipients in India.

Seminars/Workshops/Conferences at the Institute

One day professional seminar on “Listening skills, Listening Challenges and Auditory Processing Disorders”.

Prof. Lois Kam Heymann visited the institute and conducted a one day professional seminar on “Listening skills, Listening Challenges and Auditory Processing Disorders” on 14th February, 2014 from 10.00 am to 4.30 pm at Smt. K.S. Bramarambha & Dr. K.S. Sriram Auditorium. Ms. Heymann is an accomplished Speech Language Pathologist with nearly 40 years of experience working with children with listening and hearing difficulties and their parents.

She is the author of the acclaimed book “The Sound of Hope”. Ms. Heymann, a former professor at State University of New York, is currently the Director, Shelley and Steven Einhorn Communication Center and Auditory Processing Center at the Center for Hearing and Communication, New York, USA. She is also the mentor for the Children’s Hospital of Philadelphia Cochlear Implant Program.

Prof. Heymann presented a series of lectures describing the process of identification of children with listening challenges and their management.

Students and staff of the institute benefited immensely by participation in the seminar. Prof. Heymann’s visit to India is being coordinated by Advanced Bionics India Pvt. Ltd., Aditi Mallaya International School and Communication-DEALL Trust, Bangalore. We thank Dr. Prathibha Karanth, Trustee, Bangalore Speech and Hearing Trust and Director-Communication-DEALL, who was instrumental in organizing the visit of Prof. Heymann and seminar at the institute.

We organized several talks and seminars as part of the Student Improvement Program during this quarter. Ms. Sowmya Menon, Director-Services, FiVE Pediatric Therapy Services, spoke to students on skills needed to become a successful entrepreneur in Speech, Language intervention on 23rd January, 2014. Ms. Sowmya is an alumnus of institute. Prior to founding FiVE, she had worked with the Child Development Centre and Department for School Services, National Health Services, London, UK and as visiting consultant at Hope Centre for Cognitive Rehabilitation, London. FiVE Pediatric Therapy Services has multiple intervention centers in Bangalore and Chennai and provides multidisciplinary services to children with communication difficulties. Students participated enthusiastically in the interactive session with Sowmya.

Mr. Nagarjun, Clinical Audiologist, Widex India Pvt. Ltd., gave a talk on the cross hearing aid products from Widex to all the staff in the Department of Hearing Studies on 21st February, 2014.

Starkey Laboratories India Pvt. Ltd., Bangalore organized a session on ‘Recent Advances in Starkey Hearing Aid Technology’ at the Institute on 22nd February, 2014. The talk was delivered by Mr. Sai Kiran S., Audiologist & Product Trainer and was attended by III year B.Sc., I and II M.Sc. (Audiology) and MASLP students.

Ms. Shruthi Gulwadi, Clinical Specialist at MEDEL India Pvt. Ltd. conducted a one day orientation program on different aspects of cochlear implants on 15th March, 2014. Staff and students of third year BSLPA, first and second year M.Sc. (Audiology) and MASLP students participated. The program included lectures on introduction to cochlear implants, products of MEDEL implants, levels of intervention and indication for the different implants, fine hearing technology and speech coding strategies and ended with hands-on training for the staffs and students.

Siemens Hearing Instruments Pvt. Ltd. conducted a hands-on training on recent advancements in hearing products by Siemens for the staff in Department of Hearing Sciences on 19th March 2014.

Mr. Kushal Raj, Speech Language Pathologist practicing at Chicago, Illinois, USA gave a talk on “Early identification and early intervention practices in USA” to undergraduate and post graduate students and staff on 25th March 2014. The interactive and informative session was well received by the audience.

Mr. Pradeep Pai H., Chief Creative Officer & CEO, Sudhee Networks Pvt. Ltd., an E-Media company based in Bangalore and their representative Ms. Ujwala Viswanath visited the Institute on 3rd January 2014 and requested the association of the Institute for developing a video for creating public awareness on noise pollution.

Video recording of sound measuring devices such as the sound level meter and a demonstration of traffic noise measurement using the sound level meter was done for their short film.

Participation in Seminars/ Conferences and Training Programs

Mrs. Sujatha, clinical supervisor in department of speech language studies attended a two day workshop on “Cleft Speech Therapy” organized by Smile Train at Bhagwan Mahaveer Jain Hospital, Bangalore on 23rd and 24th January, 2014.

Dr. B.S. Premalatha, Professor and Head- Department of Speech Language Studies conducted a one day workshop on “Dysphagia-Current Status and Management” on 19th February, 2014 at Dr. M.V. Shetty College of Speech and Hearing, Mangalore. BASLP students from other training institutes as well including the Marthoma College, Father Muller’s College of Speech and Hearing and the NITTE Institute of Speech and Hearing participated in the workshop.

We are happy to announce that a team from Amplifon India Pvt. Ltd., Bangalore conducted a campus recruitment drive at the Institute on 28th March, 2014. All final year post graduate students appeared for the interview.

Farewells and Goodbyes !

Staff and students of ISH bid farewell to Mr. Jijo Joy Oommen, Clinical Supervisor, Department of Hearing Studies and Mrs. Sindhu, Lecturer, Department of Speech Language Studies during this quarter. We wish them well and success in all their future endeavors.

From the SASH Desk

The January to March quarter is the most awaited quarter for the students and the Students Association of Speech and Hearing, (SASH) as it’s the time for the annual sports week and the talent day. These indeed bring fun and frolic into the hectic academic lives of all students.

An inter-college cricket tournament was organized by the Students Association in the month of January that strengthened the ties between the three training institutes conducting speech and hearing courses in Bangalore, Dr. SRCISH, Naseema Institute of Speech and Hearing and Samvaad Institute of Speech and Hearing. The team from Naseema institute won a thrilling final match with Dr. SRCISH.

Sports Meet 2014

SASH conducted the sports week from 24th February to 1st March. A variety of indoor events like sudoku, chess, carom and table tennis were conducted. Badminton, girls cricket, boys cricket, football, track games, dodge ball, food gathering, dog and the bone were some of the outdoor events that were conducted in the college campus and Campus Crusade ground. Students from BSLPA, M.Sc., and Diploma program took part enthusiastically in all the events.

The 3rd year BSLPA students won the first place in the sports championship. The 2nd year BSLPA group won the second place. The third place was a tie between the 1st year BSLPA and the M.Sc., combined group.

The “Sportsperson of the Year” title went to Ms. Gemi of 2nd BSLPA and Ms. Prakruti of M.Sc. (Audiology) for contributing highest individual score to their respective teams.

Srishti 2014

Srishti-2014, the annual talent’s day was organized on the 27th March, 2014. Several events including hair styling and make up, cold cookery, movie making, music without instruments, photography, treasure hunt, duet dance, group song, fashion show, comedy hungama and group dance. All the classes took part enthusiastically in all the events. The overall trophy was won by M.Sc./MASLP students (a.k.a The Eagles). The second place went to 3rdyear BSLPA students (a.k.a Aslansilagos) and the third place went to 2nd year BSLPA students (a.k.a Desi Bails). The duet dance performance by Ms. Javeed and Ms. Kavana of 3rd BSLPA was judged as the best performance of the day.

The “Talent of the Year” Award went to Mr. Nithin of 2nd BSLPA and Mr. Suman of MASLP. Kudos to the outgoing SASH team for the academic year 2013-14 for all their hard work in conducting the events and keeping SASH an active and energetic group.

The center reopened on 2nd January after a week of Christmas holidays. Children began preparations for the festival of Makara Sankranti with a visit to the market. Children were introduced sugarcane, turmeric stem, sweet potatoes, cow gram, groundnuts, ellu-bella etc., being sold at the market during the festival. Children enjoyed purchasing some of these items for the ensuing celebrations at the center. The children were explained of the change in the direction of the movement of the Sun northward and the importance of Makara Sankranti (the Harvest Festival). Everyone enjoyed the sweet pongal (rice pudding) prepared by mothers and the ellu-bella (sesame seed and jiggery mixed laddu).

Children visited the flower show at Lalbagh, Bangalore on 24th January and were thrilled to see the models of fruits and vegetables made by flowers in the show. The snake like snake gourd and the strawberries were especially interesting for the children. The center observed the birth anniversary of Netaji Subhash Chandra Bose on 23rd January by garlanding the photo and offering floral tributes. Mahatma Gandhi’s death anniversary on 30th January was observed as the Martyr’s day.

Children sang meera bhajan and paid their respects to Mahatma Gandhiji with floral tributes. The festival of Maha Shivaratri was celebrated on 26th February. The story related to Maha Shivaratri was narrated to the children emphasizing once again to mothers on the use of incidental language to train their children.

Ms. Aswini, audiologist from cochlear India Pvt. Ltd., visited the center on 11th March and gave a talk on audiological aspects and mapping after cochlear implantation to all parents attending the center. We are happy to announce that two of our children, Baby Namana and Baby Tanmay have re-joined the center after successful cochlear implantation. Children are showing rapid progress after the surgery.

Occupational therapist Mr. Achyuth Bhandari leading the OT unit at our institute spoke to parents on the motor and sensory development of children from birth to 5 years of age. He gave insights into typical developmental milestones and identification of delays in gross and fine motor skills. Parents interacted actively with Mr. Bhandari and appreciated the talk. The month of March is the month for assessments. Children’s progress is assessed through oral and written formats. We discussed each child’s progress with their parents and provided guidance about their future lesson plans. The center will close on 15th April, 2014 and reopen on 9th May, 2014.

Sunaad School for Children with Hearing Impairment

Parent Meetings: A class-wise parent-teacher meeting was conducted on 15th February, 2014 to discuss each student’s progress in different subjects. The importance of punctuality, regularity, neatness and preparations for final exams to be conducted in March was reviewed in the meeting. A meeting was conducted for the parents of Xth standard on 24th February, 2014 to inform the parents about the examination dates. As per the request of the parents, children who were weak in some subjects were given extra revision in school. A parents meeting was arranged on 8th March, 2014 to discuss about the annual exam timetable, project work, and proposed revision of lessons completed which will be carried out at the school.

Visitors to the School: Students undergoing various training courses across India visited Sunaad School as part of their course requirements to visit special schools for children with hearing impairment. These included the M.Sc. Rehabilitation Science students from St. Anne’s Degree College for Women, Bangalore (30th January, 2014), students from Mobility India, Bangalore (30th January, 2014), DTY-DHH students from National Institute of Speech and Hearing, Kerala (19th March, 2014) and D.Ed. students from National Institute for Empowerment of Persons with Multiple Disability, Chennai (19th March, 2014). An inter-house competition on memory games was held on 28th January, 2014. Sarawathi Pooja was conducted at the school on 7th March, 2014 prior to the examination. Sweet boondhi was distributed to the children and staff. All the staff of the institute participated in the Pooja.

Dental Screening Camp: The V.S. Dental College of V.V. Puram, Bangalore arranged a dental camp in the school on 4th March, 2014. A comprehensive dental checkup was carried out for all children and children with any dental complaints were referred to the Dental College for further treatment. The doctors also explained the importance of oral health and hygiene to all children and teachers.

Graduation and farewell to students of 10th standard: We were happy to organize the 4th graduation ceremony of students of class X at the Sunaad School on 7th March, 2014 at the Institute Auditorium. Sri M.S. Venkatesh, Chairman, Prof. R. Rangasayee, Director-Technical and Dr. Madhuri Gore, Dy. Director (Technical)/Principal were Guests of Honour. All the staff of Institute and school joined to bid farewell to the students appearing for the 10th standard.

Parents and students of VIIIth and IXth standard were also invited. All students appearing for standard X examination were distributed a scroll, certificate and momentos by the guests of honour. Parents and outgoing students shared their memories at the school. All wished the students all the very best for their examination. Refreshments and soft drinks were served to all invitees.

“Hic!” You’ve just hiccuped for what seems like the tenth time since you finished your big dinner. Wonder where these funny noises are coming from? The part to blame is your diaphragm (say: die-uh-fram). This is a domeshaped muscle at the bottom of your chest, and all hiccups start here.

A hiccup (also spelled Hiccough) is an involuntary contraction (myoclonic jerk) of the diaphragm that may repeat several times per minute. In medicine it is known as Synchronous Diaphragmatic Flutter (SDF), or singultus, Latin for the act of catching one’s breath while sobbing. The hiccup is an involuntary action involving a reflex arc. Once triggered, the reflex causes a strong contraction of the diaphragm followed about 0.25 seconds later by closure of the vocal cords, which results in the classic “hic” sound. At the same time, the normal peristalsis of the esophagus is suppressed.

Hiccups may occur individually, or they may occur in bouts. The rhythm of the hiccup, or the time between hiccups, tends to be relatively constant.

A bout of hiccups, in general, resolves itself without intervention, although many home remedies are often used to attempt to shorten the duration. Medical treatment is occasionally necessary in cases of chronic hiccups.

Consultant ENT Surgeon • consuming very hot or very cold foods • a sudden change in air temperature • excitement or emotional stress • aerophagia (swallowing too much air) Hiccups that last longer than 48 hours are categorized by the type of irritant that caused the episode. In some cases, episodes can last for weeks. Hiccups that last for more than two months are considered intractable.

The majority of persistent hiccups are caused by injury or irritation to either the vagus or phrenic nerve. The vagus and phrenic nerves control the movement of your diaphragm.

Eighty-two percent of persistent or intractable episodes occur in men. An organic cause can be identified in 93% of men and in 8% of women, resulting in an overall organic incidence of 80%; the remaining 20% of cases are considered psychogenic in origin. Psychogenic conditions associated with hiccups include the following: • Hysteria • Shock • Fear • Personality disorders • Conversion disorders • Malingering Central nervous system (CNS)-related causes of hiccups include the following: • Structural lesions - Congenital malformations, malignancies, or multiple sclerosis • Vascular lesions • Infection • Trauma

Hiccups is an annoying symptom but it usually lasts only for a few seconds or minutes. They usually can occur spontaneoulsy but more often are precipitated by some combination of eating, drinking or laughing. Most cases also resolve spontaneously, and do not require any treatment unless prolonged or severe.

Hiccups in Indian, Nepalese and Arabic folklore are similarly said to occur when the person experiencing them is being thought of by somebody close.

Etiology

Numerous causes of hiccups have been identified.

However, there is no definitive list of triggers. Hiccups often come and go for no apparent reason.

The most common causes of short-term hiccups include: • overeating • eating spicy food • consuming alcohol • drinking carbonated beverages, such as sodas

Conditions that can give rise to diaphragmatic irritation causing hiccups are as follows: • Hiatal hernia • Subphrenic abscess or collection • MI • Pericarditis Conditions associated with irritation of the following branches of the vagus nerve irritation can cause hiccups: • Meningeal branches – Meningitis or glaucoma • Auricular branches - Foreign body or hairs • Pharyngeal branches - Pharyngitis • Recurrent laryngeal nerve - Mass lesions in the neck, goiter, or laryngitis • Thoracic branches - Infection, tumors, esophagitis (ie, reflux), myocardial infarction (MI), asthma, trauma, thoracic aortic aneurysm, or pacemaker lead complications • Abdominal branches - Tumors, gastric distention, peptic ulcer, abdominal aortic aneurysm, infection, organ enlargement, or inflammation (eg, appendicitis, cholecystitis, pancreatitis, or inflammatory bowel disease)

Procedure-or anesthesia-related causes of hiccups include the following : • Hyperextension of the neck - Stretching the phrenic nerve roots • Manipulation of the diaphragm or stomach • Laparotomy • Thoracotomy • Craniotomy Metabolic causes of hiccups include the following: • Hyponatremia • Hypokalemia • Hypocalcemia • Hyperglycemia • Uremia • Hypocarbia • Fever Drugs associated with hiccups include the following: • Benzodiazepines • Short-acting barbiturates • Dexamethasone • Alpha-methyldopa

Prognosis

In general, hiccups are self-limited, and the prognosis is excellent. The prognosis of protracted hiccups is related to that of the underlying etiology.

Protracted hiccups often are associated with underlying organic disease and often induce social and emotional distress. Therapy must address causative and complicating factors of protracted hiccups. Complications of hiccups may include the following: • Arrhythmias • Gastroesophageal reflux • In prolonged cases, weight loss and sleep disturbance

Physical Examination

In view of the wide range of differentials, a full physical examination is necessary. A complete and focused physical examination may yield evidence of the following: • Head (including ears, eyes, and the entire scalp) – Foreign body or aberrant hair adjacent to tympanic membrane; glaucoma • Mouth -Pharyngitis • Neck – Inflammation (including laryngitis); mass lesions; goiter; voice abnormalities (recurrent laryngeal nerve); stiffness (possibly indicating tumors or infection) • Chest – Tumors; pneumonia; asthma • Cardiovascular system – Arrhythmias; myocardial infarction (MI); pericarditis; unequal pulses (thoracic aortic aneurysm) • Abdomen – Gastric atony (succussion splash); organomegaly; subphrenic abscess; cholecystitis; appendicitis; abdominal aortic aneurysm (AAA);pancreatitis; peritonitis • Rectum – Mass lesions • Nervous system – Focal lesions; disordered higher mental function; indications of multiple sclerosis

Laboratory Studies

A wide variety of conditions may contribute to hiccups.

The possibilities are narrowed down by the findings from the history and physical examination. Laboratory testing is directed toward suspected abnormalities.

• Electrolytes - Hyponatremia can be either the cause (including Addison disease) or the effect (drinking water as a cure) of hiccups; other electrolyte-related causes are hypokalemia, hypocalcemia, and hyperglycemia • Renal function tests - Uremia • Liver function tests - Hepatitis • Amylase and lipase levels - Pancreatitis • White blood cell (WBC) count Certain infectious diseases may cause a patient to hiccup.

Appropriate testing for such infections may include the following: • Urine • Sputum • Cerebrospinal fluid (CSF)

Radiography, CT, and MRI

Imaging modalities that may be helpful in the workup include the following: • Chest radiography to identify tumors, detect infection, or assess the thoracic aorta • Fluoroscopy of diaphragmatic movement to confirm the diagnosis if malingering is suspected or to determine if the diaphragmatic abnormality is unilateral or bilateral before invasive therapy • Computed tomography (CT) of the head, thorax, and abdomen to identify tumors, infection, or structural lesions • Magnetic resonance imaging (MRI) to detect multiple sclerosis or evaluate vascular relations to the vagus and phrenic nerves

Other Studies

Other studies that may be helpful include the following: • Electrocardiography to detect MI, pericarditis, or dysrhythmias • Before invasive therapy, nerve conduction studies to confirm the diagnosis and to confirm the presence of unilateral or bilateral involvement • Endoscopy or bronchoscopy • Before corrective surgery, an esophageal acid perfusion test to determine if gastroesophageal reflux is a causative factor.

Pharmacologic Therapy

Various agents have been reported to cure hiccups. Of these, chlorpromazine is the most thoroughly studied and appears to be the drug of choice. Regimens in the range of 25-50 mg intravenously (IV) or intramuscularly (IM) are effective in 80% of cases.

Conclusion

Temporary hiccups may not be a cause for concern, but chronic hiccups could suggest a more serious underlying problem. These are some of the causes for chronic hiccups: • A brain tumor, certain metabolic diseases, and kidney failure can sometimes cause chronic hiccups.

• Diabetes and pneumonia patients may also experience frequent bouts of hiccups.

• An accident that involves injury to the brain can also cause chronic hiccups.

• The vagus nerve is responsible for the proper functioning of various muscles and so any damage to this nerve could result in chronic hiccups.

• Furthermore, since the vagus nerve runs through a portion of the ear, it is also possible that any irritation of the inner ear can cause hiccups.

• The vagus nerve is closely linked to various cardiac functions and so a malfunctioning pacemaker could cause hiccups in some individuals.

Chronic hiccups should never be ignored as they are generally associated with serious health conditions.

There is no need for unnecessary anxiety as there are also common conditions such as a sore throat or GERD that may be the root cause of the problem, but visit your doctor for a thorough checkup and an accurate diagnosis.

If the root cause of the problem cannot be identified, your doctor may request a chest X-ray and/or an ECG to rule out cardiac or chest problems.

Self-help strategies

Hiccups are harmless and usually resolve by themselves in a few minutes. Some hiccup ‘cures’ include: • Hold your breath • Take deep breaths • Breathe into a paper bag • Eat a bit of fresh ginger • Suck on a lemon • Have a hot water and honey drink • Eat a spoonful of sugar • Drink a glass of water slowly • Eat ice • Gargle • Sit down & lean forward over your knees • Ask someone to give you a fright.

• One of the simplest methods to get rid of persistent hiccups is to laugh loudly. Read your favorite comic strip or watch a funny video to laugh away your hiccups!

And the most famous treatment - having someone jump out and scare you when you’re not expecting it - helps some people wave goodbye to their hiccups. Boo!

How to Prevent Hiccups

There is no proven method for preventing hiccups.

However, if you experience hiccups frequently, you can try to reduce your exposure to known triggers.

The following may help reduce your susceptibility to hiccups: • Don’t overeat.

• Avoid carbonated beverages.

• Protect yourself from sudden temperature changes.

• Don’t drink alcohol.

• Remain calm, and try to avoid intense emotional or physical reactions.

My Struggle with Patience When people knew I was studying psychology they used to tell me that I had chosen a very apt subject. They said I was a very patient and calm individual. Patience is a valued virtue in this profession, more so when working children. With all the appreciation coming my way, I thought I was going to be a great psychologist. That was when I was still in college with no practical knowledge of what I was studying.

Once I joined work, I was shocked to realize that people were wrong about me. My temper would slip dangerously on so many occasions and used to get really worked up when a child refused to comply with instruction. There was one boy who took me on a ride the entire time I handled him.

“Farooq* please finish your work” I used to say, frequently reminding this boy who was cuteness and evil packed together. He was being introduced to writing activities and was given simple tasks like circling the stars, drawing standing lines, sleeping lines, joining the dots etc. Farooq had no preferred hand for writing and absolutely no intention to complete his work. He simply lacked interest. He would draw one huge circle covering the whole page of stars, and I remember thinking to myself where he would be if he used so much of his brains into doing something productive. I would erase the big circle and ask him to do his work properly, but by the time I checked on the other kids and got back to him he would have scribbled or colored on the page. And there were only so many ways of saying “Farooq, please finish your work”. He would not even respond and look at you vacantly. After five minutes of getting nowhere, ‘Please’ gets thrown out from my vocabulary.

“Farooq I am going to count to three. And you will pick up your pencil and start circling the stars.”

He would shake his head. If I picked up the pencil from the table and handed it to him he would refused to accept it.

Any sane person would have got his point. He just didn’t want to write. But no, I guess pride comes into play, how can he disobey me? Moreover, there was no choice. He had to do the activities being given to him, isn’t that why he came to therapy?

“Farooq I am not sending you home till you finish your work”, I would say in a final attempt and watch the threat fail miserably. He would get up, and lie on the floor or on the bench and start his tantrums, well if you could call it that.

He would not make any noise or cry, he would just lie down shake his head and refuse to do what is being told. By then other children have stopped their work and are having their share of fun watching Farooq and his antics.

Now this should not make me mad. But imagine this happening every day. There was no getting through to him.

Eventually someone would see my struggle with this boy and intervene asking him politely, very politely, to behave himself and complete his work. And of course he would comply. This is not how it was supposed to work. Farooq was my responsibility, and I had to learn to get through to him by myself. I couldn’t believe a 7-year-old was making me question my abilities. This continued for days and I used to dread that hour of therapy.

In the few days I worked with Farooq I realized that he wasn’t all that bad. Of course he wasn’t interested in doing his work but making the task interesting was my duty as a therapist. I just had to be more sweet, polite and encouraging. I had to understand his pace and wait for him to finish instead of hurrying him. I had to patiently say “Very good Farooq”, however annoying it may be, after every star he circled, or every line he drew. I had to reward him appropriately after he completed it the whole task. The theory behind reinforcement began to make sense. Please jumped back into my vocabulary again and brought along Yay and Hi-Fi and Super with it.

It is true that they say children are the best managers in the world. And it is also true that they are also excellent teachers.

I lose my patience many times even now. But the ride Farooq took me on has taught me to watch out for things that my impatience has blinded me to. I do not know if patience is my virtue, but I believe that like any other skills I need time and practice to master. *child’s name has been changed.

Screening Camp at Kollegal

Staff and students participated in a public screening camp for speech, language and hearing difficulties on 12th January, 2014 organized at Kollegal by Mattam Seshadri Shetty Subbalakshmamma Charitable Trust, Bangalore in discussions with CA. D.L. Suresh Babu, Secretary, Bangalore Speech and Hearing Trust. Ms. Prema Kumari, coordinated the camp. A total of 62 persons with range of complaints such as difficulty hearing, middle ear infection, occluded wax, voice problems and stammering were screened at the camp. Nine persons with hearing loss were provided with free hearing aids sponsored by Lions Club of Bangalore East after due evaluation. Individuals were counselled regarding the need for detailed evaluations and provided with referral notes to Dr. S. R. Chandrasekhar Institute of Speech and Hearing Bangalore, All India Institute of Speech and Hearing, Mysore and ENT doctors near Kollegal.

Sign Language Cell This quarter, the Sign Language Cell moved into its new larger location within the main building of the institute. Several activities kept the cell busy this quarter. A total of 12 Individuals enrolled for the ‘A’ level interpreters sign language and functional sign language courses.

During the last week of January, ‘S.G. Associates’ a private advertising agency approached us with a request to interpret their script on Provident Fund into sign language. We arranged for two hearing sign language users who practiced the rewritten script for a few days and then signed for video graphing. The rewriting of the script was necessary to simplify the spoken English word and phrases to facilitate comprehension by viewers with hearing impairment.

For example, Original script – I want to withdraw from the amount standing to my credit for the marriage of my daughter.

Signed text – My PF money saved, I want/take for my daughter’s marriage.

Staff of the Institute, both technical and administrative, underwent a training program on functional sign language for communication in the month of February. A total of 16 staff enrolled for the program conducted in two batches in the afternoon comprising of eight staff in each batch. The staff enjoyed the program and now we can see them interacting with each other using signs to keep practicing what they learnt. The duration of the training was 24 hours spread over 21 days.

A three day workshop was conducted for parents of children with Hearing Impairment from 5th to 7th March. The programme was well received by the parents. The newspaper (Times of India) report and feedback from parents on the training stand testimony to benefit of the workshop for the parents. Parents of children studying in two special schools for children with hearing impairment participated in the program. Parents saw the two sign language instructors Mr. Dibyendu and Mrs. Prema Lohia communicating and interacting with the group without difficulty.Mr. Dibyendu was using only Indian Sign Language and Ms. Prema used speech and commons signs. Parents were able to appreciate the importance of modes of communication other than speech such as sign language, body language and facial expressions which are widely used by persons with deafness all over the world.

We conducted a review programmeon 8th March for persons who had completed the ‘A’ level’ interpreters course and wished to pursue the advanced sign language courses. The entire syllabus of ‘A’ level, which we had earlier covered over 60 to 75 hours was reviewed in 5 hours. Participants were amused and thrilled to take part in the programme as it brought back the memory of their school days.

The counselling sessions & job placement services for persons with hearing impairment were carried out, whenever we were approached for the same. Police and court cases, involving persons with hearing impairment and intellectual impairment were also taken up.

In a lighter vein, we share with you below, a story narrated by Mr. Dibyendu using only Indian sign language.

A Lion under a tree basking. A dog from a distance admiring the beauty of the lion, comes slowly near him and says ‘I love you’, I like to marry you. The lion was furious and about to pounce over the dog, she ran through the branches of the trees. The lion chased her till there was v shaped branch, where it got caught in-between. The clever dog found a garland of flowers nearby, brought it and garlanded him. Ah! The desire of the she dog fulfilled.

Alumni Interaction with Students

We were proud to host three of our Alumni, Mr. Nagarjun, Ms. Ria and Ms. Sowmya Nayak for an interaction with students of 3rd year BSLPA and internship on 26th January, 2014. Mr. Nagarjun is Clinical Audiologist, Widex India Pvt., Ms. Ria is working with The Com DEALL Trust, Head-Office, Bangalore and Ms. Sowmya Nayak is working at the St. Johns Hospital, Bangalore. An informal discussion over tea and snacks, gave the students an opportunity to understand the work scenarios awaiting them soon after graduations. The alumni encouraged students to make the most of extensive clinical exposure being provided at the institute as it will surely help them when they step out to work independently after graduation. The alumni also gave some tips on searching for jobs and the application process.

Audiometry: measurement of hearing/measuring sensitivity of hearing Hearing Aid (HA) prescription and Fitting: relates to trial and selection of a hearing aid suitable to the individuals nature and degree of hearing loss and comfort level.

Custom Ear Mould Making: devices, which couple the hearing aids to the ear and are made to fit in the individuals ear.

Hearing Aid Repair and Service: any problems in the working of the hearing aid can be repaired or worn out parts replaced.

A hearing aid can also be periodically checked to see if it is functioning properly.

Auditory Verbal Therapy: is a specialized type of therapy designed to teach a child to use the hearing provided by a hearing aid or a cochlear implant for understanding speech and learning to talk.

Post cochlear Implant rehabilitation: Rehabilitation for children post cochlear implantation is an accepted part of the implant process. A young child with a cochlear implant and her family require a comprehensive habilitation program to help her utilize the auditory signal and to naturally integrate the various components of communication including listening, speech, language, reading and thinking.

Mapping for Cochlear Implant: refers to the setting of the electrical stimulation limits necessary for the cochlear implant user to “hear” soft and comfortably loud sound. Since the Cochlear Implant can only process sounds in a very narrow range (6-15 dB), it must be able to compress the natural range of sounds (normal speech is usually between 40-60 dB) into this small band.

Noise Measurement: measuring the noise levels in industries and public areas and ascertain that it is within permissible limits and not a hazard to the hearing sensivities of the people in the environment.

Department of Speech Science

Misarticulation: is a speech defect in which an individual substitute, omit, add or distort the speech sounds as result make the speech unintelligible.

Stuttering: Stuttering is a speech disorder in which sounds, syllables, or words are repeated or prolonged, disrupting the normal flow of speech. These speech disruptions may be accompanied by struggling behaviors, such as rapid eye blinks or tremors of the lips. Stuttering can make it difficult to communicate with other people, which often affects a person’s quality of life.

Cleft lip and Palate: refers to wherein a child is born with a split or separation in the lips or roof of the mouth.

Cerebral Palsy (CP): is a term used to describe a non progressive group of disorders affecting body movement and muscle coordination due to an insult to or anomaly of the developing brain.

Autism: is a developmental disorder characterized by difficulties in social interaction, communication and by restricted and repetitive behavior and manifests within the first two years of life.

Aphasia: is the impairment of language function which results from strokes and other injuries to the language areas of the brain.

Dysarthria: is an acquired speech problem caused by paralysis, weakness, or inability to coordinate the muscles of the mouth.

Learning Disability: as aneurological, condition that prevents a person from learning or severely impairs the learning process in one or more academic areas, despite his having average or higher intelligence.

Laryngectomy: is a procedure that involves removing part or all of the voice box usually as a result of a cancerous tumour in the larynx resulting in loss of voice and requiring to learn new methods of speaking.

Dysphagia: is a medical condition that causes a person to have problems swallowing.

Psychology Unit

IQ Assessment: intelligent quotient (IQ) or IQ tests are intended to measure aptitude and intellectual capacities and provide and estimate of a individuals mental abilities.

Intelligence Tests: are tests of general intellectual abilities.

They measure not only a single trait but also a range of abilities including memory, verbal ability, reasoning skills and numerical ability.

Personality test: aims to describe aspects of a person’s character that remain stable throughout that person’s lifetime, the individual’s character pattern of behavior, thoughts, and feelings.

Behavior modification: is a treatment approach, based on the principles of operant conditioning, that replaces undesirable behaviors with more desirable ones through positive or negative reinforcement.

Psycho-educational Assessment: evaluates an individuals psychological and academic issues and spells out the needs for special education placement and services required for effective remediation of the child.

Voice clinic: provides services in the diagnosis and management of disorders that affect the voice.

Endostroboscopy: It is a quick effective procedure to study the structure and functions of the vocal folds.

ENT valuation: Routine evaluation of ear, nose and throat are available from an ENT consultant.

School Screening: Service offered to schools for identification of speech, hearing and learning problems in children.

Rural screening Camps: Service offered to any health centre, institution, organisation or industry or companies to identify people’s (adults and children) abilities in speech and hearing.

Sunaad School for Hearing Impaired: Provides primary and middle school education in Kannada and English medium for hearing impaired children.

Sign Language Training: Equips individuals with basic skills necessary to communicate with individuals with hearing impairment and become professional interpreters for them through short term and diploma level recognized courses in Indian Sign language.

National Open School (NIOS): Institute provides coaching and training for individuals appearing for examinations conducted

by NIOS. Mothers Training Centre: provides early and intensive intervention for young children with hearing impairment and provides training for their parents.
