Chairman Speaks Dear friends, Hope the New Year 2017 has ushered in lot of activities to all!!!

Number of interesting activities and developments have taken place at the Institute. The Honourable Chief Minister Shri Siddaramaiah appreciated the Mobile Speech and Hearing Clinic (Accessible Bus) on 14th January 2017.

Prof. Lirong Lilly Cheng, President of the World Body of Speech and Hearing Association and Chair of the Education Committee for the International Association of Logopedics and Phoniatrics and current President of IALP visited the Institute and nominated the Institute to become the first affiliate society of IALP from India.

During this period, we also have the pride of Dr. Madhuri Gore, our principal to have sworn in as the President of the Indian Speech and Hearing Association. Prof. B.S. Premalatha, HOD, Speech Language Studies, delivered Ratna Oration at the 49th Conference of the ISHA held in Kolkata in January 2017.

The activities of the Institute has been recognized by National Program for Prevention and Control of Deafness (NPPCD) on World Hearing Day Celebration organized by Govt. of Karnataka on 3rd March 2017.

The Advisory Board of the Institute met under the Chairmanship of Shri D.H. Shankaramurthy, Chairman, Legislative Council, Govt. of Karnataka has recommended a number of developmental actions.

I am proud to inform that our Institute has been recognised by UGC, Ministry of HRD, Govt. of India under Section 2 (f ) & 12 (B) of the UGC Act, 1956 as un-aided/self financed college permanently affiliated to Bangalore University.

Events and Celebrations As part of Makara Sankranthi celebrations, on the 12th of January, 2017, mothers in the Mothers’ Training Centre prepared Pongal and Ellubella, and the children with hearing impairment were given an explanation on the significance of Sankranthi, the change in direction of the sun and rituals associated with the festival.

On the 13th of January 2017, Sankranthi was celebrated in Sunaad School. The students were given an explanation on the importance of the harvest festival. Ellubella was prepared and distributed to all.

Awards and Accolades

Dr. Madhuri Gore, Principal, was sworn in as President of Indian Speech and Hearing Association (ISHA) at the 49th ISHA Conference at Kolkata in January, 2017. Dr. B.S. Premalatha, Head of Department of Speech and Language Studies, delivered the highly prestigious Dr. Ratna Oration and was awarded the Dr. Ratna Oration award 2016.

by NPPCD of Government of Karnataka, on the 3rd of March, 2017. In this function, Karna, the mannequin which was developed to assess the listening loudness of personal music players was demonstrated to the Hon’ble Health Minister, Mr. Ramesh Kumar, and that was appreciated by the Hon’ble Minister.

AUDISHCON 2017

The annual national workshop, AUDISHCON, was conducted by the Department of Hearing Studies on the 9th, 10th and 11th of March, 2017 on “Hearing Instruments: an update”. The inaugural function was presided by Mr. Sridhar Pabbisetty, CEO of Namma Bengaluru. The keynote address was delivered by Prof. Ramesh Oza, a doyen in the field of hearing aids. Other experts who shared their knowledge about the practical aspects of hearing aid fitting were Dr. P. Manjula from AIISH, Mysore, Prof. Dattatreya, from NITTE, Mangalore, Dr. Crystal Variava from Sonova group, Mumbai, Dr. Sivaprasad Reddy, from

Amplifon, Mr. Arivudai Nambi from KMC Mangalore, Dr. Hariprakash from SOAHS, Manipal, Prof. R. Rangasayee, Dr. SRCISH, Mr. Avinash Grubb, Sonova group and Mr. Nagarjun, Widex India Pvt. Ltd. Audiologists from five hearing aid companies enlightened the delegates about their latest innovations. Two panel discussions, one on the ‘Perspective of end-users of Hearing Aid Technology’, and the other about ‘Dispensing of Hearing Instrument and Entrepreneurship’, were well appreciated by all delegates. Mr. Vishnu from C-DAC spoke about the digital programmable hearing aids manufactured in India, as part of the Make-In-India theme. This workshop recorded an all-time high of about 260 registrations.

Mobile Clinic and Outreach Unit

On the 14th of January, 2017, Dept. of Health and Family Welfare, Govt. of Karnataka, organized an event held at Vidhana Soudha. Hon’ blé Chief Minister Sri Siddaramaiah distributed Hearing Aids, Artificial limbs and Laptops to people with hearing impairment. Hon’ble Minister for Health and Family Welfare graced the occasion. Hon’ble Chief Minister visited and acquainted himself of the facilities and functioning of the Mobile Speech and Hearing Clinic of Dr. SRCISH.

Mobile Clinic and Outreach Unit

In order to better reach our services to the public, free speech and hearing camps were held at various sites in and around Bengaluru. Those who were found to be suffering from speech/hearing disorders were referred back to the Institute for further detailed evaluations and intervention.

In addition, an outreach branch of the Institute functions at Seshadripuram, where speech therapy services are provided.

Sunaad School for the Hearing Impaired On 27th of January, 2017, the students of Sunaad School participated in the interhouse “Cooking without fire� competition. The dishes prepared by the students were very delicious. A total of eight dishes were prepared and were high in visual appeal as well. The interhouse sports was also held on 27th of February, 2017. The students all participated with gusto.

Tiny tots of pre-school were taken for a field trip to Cubbon Park on the 16th of February, 2017. The students all had fun, in addition to learning.

Sign Language Unit In the month of January the unit conducted a training program for deaf employees at Lifestyle Adarsh Opus at Austin town. It is very encouraging that this organization employs deaf individuals at front end as sales persons.

They had impeccable etiquette and style of functioning.

The training was aimed at conveying to them their prospects of growth and development in the organization.

In this quarter, there were nine admissions for Level ‘A’ sign language and two admissions for Functional sign language. Fifteen students pursuing Diploma in special education were trained in sign language. One of the students learning Level ‘A’ sign language volunteered to teach handicrafts to the two deaf individuals who learn functional English in the unit.

The pieces made by them were so attractive that the first batch of 25 products was sold immediately.

The sign language unit acted on the program ‘Arivina Sinchana’ as directed by the Government of Karnataka. This program directs all schools for the deaf to conduct sign language training for the deaf students and their parents. It was an opportunity for us to spread the light of Indian sign language.

The children of Mothers’ Training Centre along with their mothers visited Lalbagh for the flower show on the 24th of January, 2017. The children were highly excited seeing the model of Gol Gumbaz made entirely out of rose petals. In addition, there were displays of fruits and vegetables. This enabled them to practically experience what was taught in their classes.

On the 30th of January, 2017, Martyr’s Day was observed at the Centre. The children were spoken to about the greatness of Mohandas Karamchand Gandhi and his contributions to today’s independent India.

Chandramana Ugadi, New Year’s Day was celebrated on the 28th of March, 2017. Pooja was performed and sweets like Obbattu were prepared, and distributed to all.

International Women’s Day

International Women’s Day was celebrated on the 7th of March, 2017. The theme was ‘Be Bold for Change’. On that day, both men and women in the institute wore blue and purple, to show their solidarity for women and their safety. A motivational short film, ‘Swashakthi’ was screened. Eminent women who contributed immensely to society and science were remembered.

International Day of Forests Environment committee celebrated the “International Day of Forests” on March 21st. An hour long program was organized in which Dr. Harish Bhat, a renowned ecologist and research scientist from Indian Institute of Science, Bangalore, interacted with the audience.

Eye Testing

Sri Eye care, HBR layout conducted a free eye - screening for the staff and students of the institute on the 28th of February, 2017, as part of their Sight Saving Outreach program. A total of 114 individuals were screened.

Donations

We welcome donations in cash or kind. Donations to the Institute are exempt U/s 80-G of the Income Tax Act. The cheque or demand draft may kindly be drawn in favour of “Bangalore Speech and Hearing Trust” and mailed to The Director, Dr. S. R. The New Year 2017, was ushered in with joy and excitement. On the eve of New Year, students celebrated New Year by singing songs and dancing and a fashion show was put up. Various games were organized and students participated enthusiastically.

Ethnic day was observed on 14th February 2017 and students and staff turned up in ethnic wear to celebrate the day. Students put up cultural programs which consisted of dances, songs and a fashion show to depict the ethnic diversity in India. At the end of the program, the title of Mr. Ethnic was awarded to Mr. Vinay, a 1st M.Sc. student (dressed in traditional Kodagu attire) and Ms. Ethnic to Ms. Kiruthika, a 2nd M.Sc. student (dressed in traditional Tamil Nadu attire). The winners and runners up of Rangoli competition (conducted on the previous day) were the Diplomas and 2nd M.Sc. students, respectively.

Outdoor Sports for students were held on 17th February 2017 at ISKCON Temple grounds, Kacharakanahalli, and students competed in skipping, athletics, cricket, football, shot put and tug of war. The 3rd B.Sc. students emerged the overall winners followed by the 2nd B.Sc. students and then the 2nd MSc students.

Sports for the staff were conducted in the month of February 2017. The winner of Table Tennis was Mr. Shobin and runner up was Prof. Rangasayee, Mr. Suman and Mr. George won the Badminton Doubles and Mr. Shobin and Mr. Suman won in Carrom Doubles.

Ethnic Day celebration

The most awaited event, Talents Day – “IMPULSE” was organized on 17th March 2017. It was a day filled with entertainment, games, dances, songs and many more fun events. The day started off with the off-stage events such as Pencil Sketching, Face Painting, ‘Cooking without fire’, ‘Best out of waste’ and Mehendi competition. The theme dance/act, and the newly introduced ‘Mannequin Challenge’ saw a lot of creativity from the students. The other on-stage events were group dance, group song, duet dance, comedy dance/act and fashion show. The photography competition was themed ‘Silhouette’, movie making competition, ‘Savdhaan India’ and Fashion show, ‘Zodiac’ and these events saw incredibly talented presentations from the participants. These competitions brought out hidden talents of many students. For the very first time, the staff walked the ramp, showing their fashionable side and set the stage on fire.

The overall winners were students of 1st MSc, ‘Vikings’, 1st runners up were students of 1st B.Sc., ‘Invictums’ and 2nd runners up were students of 2nd B.Sc., ‘Fierce Immortals’. The title of ‘Talent of ISH’ was bagged by Mr. Nitin Damam, a student of 1st year M.Sc. Audiology.

In most places in India, there may or may not be audiologists and speech language pathologists, educators, let alone ENT consultants with or without training/experience in dealing with cochlear implant recipients. Issue 5: Device related A lot of the issues that are device related are due to improper care of the processor. Parents have a poor understanding of what is covered by the warranty. Warranty is not the same for processor and all accessories- cables, batteries have shorter warranties. Sometimes it is possible to extend the warranty for the processor. A speech processor may 22

cost about Rs. 3 lakhs on an average and has a warranty of 3 years. So if not taken care of properly, it lasts for three years only. Thus, costs can be Rs.100,000.00 annually !!

It is important to know that the loss of the processor is not covered by insurance. Nor does warranty cover improper use such as not keeping processor clean leading to damage caused by sweat.

Spares are important. A child is without hearing when there is a malfunction and when parents don’t keep a set of spares. Spares are not always available over the counter, in many centres. Some issues are linked to improper use and handling delicate spares roughly.

With respect to the coil, parents have a tendency to tighten the coil so that it will not fall off . This can lead to breakdown of skin and explantation at the worst.

Issue 5: Problems that may occur at later age The child with a cochlear implant is as susceptible to other infections as a hearing child. As was mentioned earlier, they may have a learning disability which is detected only after they reach the reading stage in school. Some develop other problems like middle ear infections with complications, seizures or head injuries inspite of all care. To minimize these it is better to always consult the implant surgeon. The problem of reimplantation though rare also looms large.

Issue 6: Professional services Due to paucity of trained professionals, most clinicians in the field of cochlear implants are overworked and their services get stretched. This may lead to untrained persons providing services and their related problems.

Issue 7- Poor communication in team It is important to choose a CI team that is strong. All teams do not have good interaction between surgeon and all other members- so selection may become a problem as also mapping and habilitation. Professionals may not agree on philosophy of rehabilitation, causing problems.

Issue 8: Rehabilitation Philosophy The approach chosen for re/habilitation- is client related.

The first choice is to emphasize audition so that child begins to use the fastest mode of communication. Especially in young children, greatest progress is made when audition is emphasized and natural conversation used by family members. In case progress is limited, and there are no other issues related to the map, the therapist refers the child to another specialist such as OT/PT/psychologist to find out if additional support is required.

In rare cases, other modes may be necessary. In all cases a team approach is used. Language is another issue. Parents may select one language, but grandparents speak in another.

Parents choose English assuming it is easier, or there are greater opportunities. If so they must ensure fluency and use it as their first language of communication.

Most children with hearing loss can be considered as normal in every way except with respect to hearing.

But, and it is important to realize that many have other issues. One such is sensory integration which may lead to other disorders. Some need behavioural management.

Cochlear implant as a treatment has made us understand that hearing loss in children is a complex problem and must be managed accordingly.

Speech & Language Impairment Variation Over a 5 year Period The study was conducted in the department of speech and language studies, at Dr. S. R. Chandrasekhar Institute of Speech and Hearing (Dr. SRCISH), Bangalore. The study was taken to observe how the trend of speech and language disorders have changed over a period of five year, that is between 2007 & 2008 and 2014 & 2015 at the institute. The survey helps in collecting information about the type of speech and language disorder which are more prevalent in the city of Bangalore. Case files from the two time periods of 2007 & 2008 and 2014 & 2015 were reviewed to collect information on details pertaining to age, gender, urban/rural, types of speech and language disorders.

The total number of cases that were seen in the department of speech and language studies for assessment, in the two time periods was 6278. Out of these, 3068 files were seen from the years 2007 & 2008 and 3210 case files were seen in the time period of 2014 & 2015.

Speech and language acquisition and development in boys seem different especially because of different ways of upbringings and interaction with their environment. As we can see speech and language disorders were seen to be more prevalent in males (70%) than in females (29%).

The reason for this much of difference between males and females in the Indian scenario could be attributed to the fact that males receive more attention and so are better attended for deviation in health status as compared to females.

It was observed clearly from the above figure that the number of cases seen in the department showed a peak or rise in the months of April- July. This could be because of closure of the schools during these months, as they are considered the peak summer months in India. April and May see a good inflow of cases of all ages, as they are the holidays and clients have time to get checked as well as time to come regularly for therapy. Also in the figure there is a rise in the number of cases in the month of December except in the year 2014 where we can see a fall in the number of cases. This seasonal variation of cases could help us better understand how to manage the influx of a large no. of cases during some months and also improve upon any aspect that requires our attention.

The highest no. of cases was seen in the age range of 2-15 years which was around 4581 cases out of 6278 (that is 72.9%), followed by 1016 cases seen in the age range of 15-60 years (that is 16.9%) which were seen for speech and language disorders in all the four years taken. In the age range of individuals greater than 60 years of age, there was a lower percentage of just 1.5% (that is 98 cases) cases seen.

It was observed that hearing impairment was the highest (27%) as compared to other language disorders. The higher prevalence of hearing disorders when compared to other language disorders could be due to the fact that hearing impairment often coexists with various signs and symptoms, owing to which medical help is often sought as against the other language disorders which are symptomless. The prevalence of receptive and expressive language disorder of unknown etiology was seen to be the second highest (at 20%).

The prevalence of speech and language disorders with respect to locality (urban/rural), it is seen that irrespective of the time period the percentage of cases from the urban locality was higher (87%) as compared to the rural locality (with 13%).

As far as the different types of disorders were considered, language disorders were seen to be the highest with 4605 cases (73.35%) more as compared to speech disorders, which were seen in all four years. The percentage of language disorders irrespective of the type seems to have increased over a period of time from 864 (that is 18.7%) to1284 (that is 27.84%).

From the figure, it is seen that between the two time periods the highest number of speech disordered cases was seen in the time period of 2007& 2008 as compared to the time period of 2014 & 2015. Maximum number of cases of speech disorders 505 (30.1%) were seen in the year 2008 as compared to 286 (17%) which was seen in the year 2015. Among the different types of speech disordered individuals, fluency disorders were seen to be the highest followed by voice disorders and articulation disorders in all the four years.

Perception of Environmental Sounds by People with Hearing Impairment

of a gentle breeze sounds pleasurable to a normal hearing individual, to the hearing impaired person, it is noise.

This is because his or her hearing device is designed to treat it as noise.

‘Environmental sounds’ by definition refers to all the sounds around us other than speech and music. When we look around, the world is filled with a plethora of sounds. Each of these sounds is independent of the other, varying in loudness, pitch and duration.

Speech therapy is not complete with just language development. It is complete only when the child learns to converse independently with the right amount of vocabulary, precise articulation, fluent, smooth flow of words without improper pausing, and use of an appropriate intonation pattern to suit the occasion.

Department of Hearing Studies

Audiometry: measurement of hearing/measuring sensitivity of hearing Hearing Aid (HA) prescription and Fitting: relates to trial and selection of a hearing aid suitable to the individuals nature and degree of hearing loss and comfort level.

Custom Ear Mould Making: devices, which couple the hearing aids to the ear and are made to fit in the individuals’ ear.

Hearing Aid Repair and Service: any problem in the working of the hearing aid can be repaired or worn out parts can be replaced.

A hearing aid can also be periodically checked to see if it is functioning properly.

Auditory Verbal Therapy: is a specialized type of therapy designed to teach a child to use the hearing provided by a hearing aid or a cochlear implant for understanding speech and learning to talk.

Post Cochlear Implant rehabilitation: rehabilitation for children post cochlear implantation is an accepted part of the implant process. A young child with a cochlear implant and her family require a comprehensive habilitation program to help her utilize the auditory signal and to naturally integrate the various components of communication including listening, speech, language, reading and thinking.

Mapping for Cochlear Implant: refers to the setting of the electrical stimulation limits necessary for the cochlear implant user to “hear” soft and comfortably loud sounds. Since the Cochlear Implant can only process sounds in a very narrow range (6-15 dB), it must be able to compress the natural range of sounds (normal speech is usually between 40-60 dB) into this small band.

Noise Measurement: measuring the noise levels in industries and public areas and ascertain that it is within permissible limits and not a hazard to the hearing sensitivities of the people in the environment.

Department of Speech Language Sciences

Misarticulation: is a speech defect in which an individual substitute, omit, add or distort the speech sounds as result make the speech unintelligible.

Stuttering: stuttering is a speech disorder in which sounds, syllables, or words are repeated or prolonged, disrupting the normal flow of speech. These speech disruptions may be accompanied by struggling behaviors, such as rapid eye blinks or tremors of the lips. Stuttering can make it difficult to communicate with other people, which often affects a person’s quality of life.

Cleft lip and Palate: refers to a condition wherein a child is born with a split or torn lip(s) with a hole in the roof of the mouth.

Cerebral Palsy (CP): is a term used to describe a non progressive group of disorders affecting body movement and muscle coordination due to anomaly of the developing brain.

Autism: is a developmental disorder characterized by difficulties in social interaction, communication and by restricted and repetitive behavior and manifests within the first two years of life.

Aphasia: is the impairment of language function which results from strokes and other injuries to the language areas of the brain.

Dysarthria: is an acquired speech problem caused by paralysis, weakness, or inability to coordinate the muscles of the mouth.

Learning Disability: as aneurological condition that prevents a person from learning or severely impairs the learning process in one or more academic areas, despite his having average or higher intelligence.

Laryngectomy: is a procedure that involves removing part or all of the voice box usually as a result of a cancerous tumour in the larynx resulting in loss of voice and requiring to learn new methods of speaking.

Dysphagia: is a medical condition that causes a person to have problems in swallowing.

Psychology Unit

Developmental assessments are conducted on children to identify children with developmental delays (DQ/SQ) IQ Assessment: Intelligent quotient (IQ) or IQ tests are intended to measure aptitude and intellectual capacities and provide an estimate of a individuals mental abilities.

Intelligence Tests: are tests of general intellectual abilities. They measure not only a single trait but also a range of abilities including memory, verbal ability, reasoning skills and numerical ability.

Personality test: aims to describe aspects of a person’s character that remain stable throughout that person’s lifetime, the individual’s character pattern of behavior, thoughts, and feelings.

Behavior modification: is a treatment approach, based on the principles of operant conditioning, that replaces undesirable behaviors with more desirable ones through positive or negative reinforcement.

Psycho-educational Assessment or Learning Disibility Assessment: evaluates an individuals psychological and academic issues and spells out the needs for special education placement and services required for effective remediation of the child. School for children with Autism and Learning Disability, slow learners.

Voice clinic: provides services in the diagnosis and management of disorders that affect the voice.

Endostroboscopy: It is a quick effective procedure to study the structure and functions of the vocal folds.

ENT valuation: Routine evaluation of ear, nose and throat are available from an ENT consultant.

School Screening: Service offered to schools for identification of speech, hearing and learning problems in children.

Rural screening Camps: Service offered to any health centre, institution, organisation or industry or companies to identify people’s (adults and children) abilities in speech and hearing.

Sunaad School for Hearing Impaired: Provides primary and middle school education in Kannada and English medium for hearing impaired children.

Sign Language Training: Equips individuals with basic skills necessary to communicate with individuals with hearing impairment and become professional interpreters for them through short term and diploma level recognized courses in Indian Sign language.

National Open School (NIOS): Institute provides coaching and training for individuals appearing for examinations conducted by NIOS, for out of school children with special needs.

Mothers Training Centre: provides early and intensive intervention for young children with hearing impairment and provides training for their parents.

Placement Cell: for adult deaf and graduating rehabilitation professionals.
