Leadership Announcements Cecilia Hilerio Appointed Assistant Vice President, Health Information Management As a High Reliability Organization, patient safety is at the core of what we do every day, and that includes keeping our patients’ health information safe. To support this effort and its role in providing quality patient care, we have welcomed Cecilia Hilerio as assistant vice president, Health Information Management and Clinical Documentation Improvement.

In this position, Cecilia will focus on three main areas of responsibility: Health Information Management (HIM) for Inspira Health, HIM for Inspira Medical Group and the Clinical Documentation Improvement (CDI) program.

In her HIM work, she will lead the management of material, financial, and human resources, as well as oversee the HIM outsourcing contracts and professional coding. Additionally, she will develop and implement an operational plan that aligns with our network’s overall strategic plan, while also working collaboratively with leaders of our hospitals and physician practices to support our financial

goals. Cecilia will also provide CDI leadership, implementing new technologies and promoting consistent, standardized operations and documentation across the network. She will report to our chief financial officer, Tom Baldosaro.

Cecilia comes to Inspira from Thomas Jefferson University Health System, where she most recently served as enterprise senior director for Coding, CDI and HIM operations. In this role, she led and directed all enterprise teams across 11 hospitals with a focus on EMR initiatives, including implementation, enterprise transformation and training for employees. She has a depth of experience in new technology implementation and extensive knowledge in revenue cycle operations, and is also certified in Lean Six Sigma.

Congratulations to our Patient Safety Awareness Week Giveaway Winner!

In honor of Patient Safety Awareness Week in March, Inspira hosted several opportunities for employees to enter a giveaway via the Employee Facebook Group.

From highlighting their favorite or most used HRO tool, to shouting out colleagues who always put safety first, dozens of employees participated in the giveaway.

Kristen Carione, manager, Education, Inspira Medical Center Vineland, was randomly selected as the winner and received a $50 Visa gift card.

Speak Up for Safety Great Catch of the Month Paul Lanza, D.O., Inspira Urgent Care Laurel Springs Paul Lanza, D.O., physician at Inspira Urgent Care Laurel Springs, treated a patient who presented with complaints of right-handed, middle finger pain lasting approximately two weeks.

The patient reported hearing clicking when bending her finger. An X-ray of the area was negative for a fracture and the patient was diagnosed with trigger finger. She was advised to follow up with a hand surgeon.

At the completion of the visit, the patient asked for a work note due to having difficulty writing as she is right-handed.

When Dr. Lanza asked about weakness during her examination, the patient denied any weakness, but a pronator drift was noted. The patient stated the drift was due to an ulnar nerve injury seven years ago. The patient stated she had always had difficulty writing since the accident, but it has gotten worse over the last two weeks. The patient further admitted to having occasional numbness in her right hand and wrist. She was adamant that it was nothing new, stating

it had been going on for seven years. The patient was discharged to follow up with a hand surgeon.

This didn’t sit well with Dr. Lanza. Applying a Questioning Attitude, Dr. Lanza Validated and Verified the patient’s history. Given the patient’s symptoms and history of diabetes and hypertension, Dr. Lanza contacted the patient and advised her to go to the Emergency Department for further evaluation and a CAT scan of the head. The patient agreed.

In a follow-up call to the patient’s husband, Dr. Lanza was informed that the patient was diagnosed with a CVA (cerebral vascular accident) and was going to be admitted for further treatment.

This Great Catch highlights why we must lead with safety first in all we do, apply a questioning attitude when something doesn’t feel right, and use our HRO tools such as Validate and Verify. By using these tools, Dr. Lanza put the patient first. He compared his clinical assessment to her history and followed his intuition to get a more thorough work-up for the patient. Had Dr. Lanza not acted as he did and the CVA been left untreated, it could have potentially caused severe harm, or death to this patient.

Each year, Philadelphia Magazine compiles its Top Doctors list for the region. Doctors on the list must be nominated by their peers as the very best in their field. Once the physicians are nominated, the publication’s healthcare research partner, Castle Connolly Medical Ltd., vets and screens the nominees for their educational and professional experience. This is a tremendous honor for our physicians to be recognized among leading experts throughout the southeast Pennsylvania, South Jersey and Delaware region.

Quality is the foundation of patient care and is a responsibility for all of us at Inspira. Every doctor that has been recognized should be proud of their impact and influence in elevating the quality and clinical excellence at Inspira through their own work.

Medical Outpatient, Vineland

A recent stickie note that landed in the Solved (green) section of the Medical Outpatient Learning Board was titled Blood Bank Peer Check and Timer Process.

Medical Outpatient Staff are required to document in Cerner all blood products that are given to patients within fifteen minutes of administering. Shanda Richer, nurse manager at the The Frank and Edith Scarpa Regional Cancer Pavilion at Inspira Medical Center Vineland, identified an increase in the number of blood bank audit forms, and it was discovered that vital sign documentation was not entered in time in Cerner. This was brought to the Medical Outpatient Daily Huddle where staff identified barriers in documenting in Cerner. Following the department huddle, staff placed this on the Learning Board.

Shanda shared,

“Staff implemented a process change to cross monitor each other - using our HRO Tool Peer Check to ensure accurate documentation. Staff trialed Peer Checking each other at the fifteenminute mark of vital sign documentation. Staff discovered even though they were present in the patient room, their entries for vital signs were still, at times, out of the fifteen-minute window. The

process was modified to incorporate timers on the IV pumps and on the MAK carts – providing the staff an alert one minute before vital signs needed to be entered. This one-minute window and audial alert act as a countdown to documentation and allowed staff to Peer Check each other and document at the fifteen-minute mark. When the timer goes off, staff pay attention to detail, utilize STAR and peer check each other to ensure the vital signs are completed and documented at the correct time interval. I’m proud of our team! We worked together using our HRO tools to improve our process and ensure the safe care of the patients we serve.”

The best way to maintain shared situational awareness is to work together like when we cross monitor. Cross monitoring starts with a scan and ends with a call-out. ‘Hey, Christina —is this your phone?’

Security, Mullica Hill The Security team recently identified the need to increase and maintain situational awareness across the network. The following Safety Message was immediately reported by Security on all five Daily Safety Briefings, presented by Max Caez, supervisor and safety coach, Mullica Hill Security, during the April Townhall Meeting and was included in Leading Reliably to be reviewed at department Daily Huddles: "Situational awareness is maintaining an accurate mental model of the space around you. We do this in our work with patients, in our work in and around patient care spaces and in our personal lives. Think of keeping track of children at the park or pets in the yard.

With visitors back in the hospitals, we have to resume our good habits of maintaining situational awareness of both the visitors and our personal items left lying around in the open. Medication left on the counter; sharps left in a patient room; these are patient and workforce safety risks. A cell phone left on the counter or a purse unattended on your chair —these are also risks.

The best way to maintain situational awareness is to scan and use a questioning attitude. Scanning means

The scan plus cross monitor multiplies our error probabilities. If I were to leave my phone unattended one out of a thousand times, I would need a new phone every three months. If my team cross monitors me, my one of one thousand (1 / 1,000) is multiplied by your one thousand (1 / 1,000) to become a probability of one in one million (1 / 1,000,000). Now, with cross checking, I would only need to buy a replacement phone once every 200 years.

Paying attention with STAR (stop, think, act and review) helps in preventing possessions left unattended.

But situational awareness, the scan plus the cross monitoring, makes us high reliability.”

Max shared,

“Even though we must always maintain situational awareness and it is never truly “Solved”, I added a “Situational Awareness” stickie note on our department Learning Board in the Solved (green) section. We view the Solved section of the Learning Board as our trophy case, but as an HRO we are continuously improving, and this stickie note is here as a visual for our team - highlighting that an employee spoke up for safety and asked for a network Safety Message to go out highlighting the importance of situational awareness. This stickie note on our Learning Board is now available for our staff to move back to the Working (yellow) section if/when they see a Safety Message is needed for situational awareness.”

Inspira Recognizes Volunteers with Drive-Through Celebration Inspira Health has multiple programs which utilize the services of volunteers. The donation of their time and talents to our patients and their families, caregivers, our staff and community, is very much appreciated. We salute our volunteers and thank them for their commitment to our organization’s mission, vision and values throughout the year.

To recognize our dedicated and caring volunteers, drive-through celebrations were hosted in April at each of our medical centers in Vineland, Elmer and Mullica Hill. Over 175 volunteers attended these events to interact with senior leadership, receive a thank you gift and milestone award, and a boxed lunch. To view photos from the celebrations, click here.

Inspira Hosts Senior Class Drive-Through Event On Friday, April 15, a drive-through event was hosted for over 75 Inspira Senior Class members at LIFE Vineland.

This engaging opportunity was a way to reconnect with members as their last in-person event was hosted prior to the pandemic. Members were provided with educational materials and interaction with various service lines along with a boxed lunch upon departure.
