MULLICA HILL 5 EAST ACUTE

Learning Boards During Executive Patient Safety Rounds in Mullica Hill, Pat Nolan, chief operating officer, used the 4 C’s of the Rounding to Influence HRO Leadership Tool and met with the 5 East Acute team to discuss safety, experience and engagement. Heather Smith, nurse manager, and the team attended rounds around the corner from the department Learning Board – where they reviewed recent items that have been placed on the board. Kathy Wolfrom, unit clerk, shared that she uses her HRO badge tag as an easy reference when she needs to use her HRO Tools, and that any concerns she and the team have are placed immediately on the department’s Learning Board for review. Smith shared, “We are working really hard on quietness scores on 5 East Acute. Staff identified that the front doors slam loudly, and that slam could be heard in multiple nearby rooms. The staff had the suggestion to add magnets to the front doors to see if that would help.”

Kay Fogg, nurse manager of Elmer 2 East, and her team added their 2020 Engagement and Safety Survey Action Plan items to the department Learning Board. This included the following three items: job stress, job security, and patient and employee safety. These items are reviewed and discussed monthly at staff and Unit Based Practice Council meetings and the Learning Board is updated during huddles. “To support job stress, staff shared they wanted to reinstate Quiet Hour. Quiet Hour allows staff time to chart in a less stressful atmosphere and provides patients time to rest or nap undisturbed,” said Fogg. This item made it to the Solved (green) section of the Learning Board with the go-live date of July 15, 2021. Quiet Hour is from 2:30 p.m. - 3:30 p.m. daily. Prior to Quiet Hour each day, aides do purposeful rounds with 4 P’s (pain, potty, position, and possessions). Following rounds, the lights are dimmed on the unit and patients are encouraged to rest or nap. Signs that explain “Quiet Hour” are placed in the patient rooms on bedside tables and a poster is placed on the windows entering the unit that announces Quiet Hour times. Education was provided to the staff and aides

This item was placed on the Learning Board in the New (red) section and continuously reviewed during the department’s Daily Huddle until the item landed in the Solved (green) section of the Learning Board. “The magnets placed on the doors has had one of the greatest impacts on our patient’s experience – the staff feel that it has made a huge difference,” said Smith.

during Department Huddles. In addition to a less stressful atmosphere while charting, the staff have access to the Relaxation Room where they can enjoy a massage chair, sound machine, and aroma therapy.

ELMER 2 EAST

The second Action Plan item added to the Solved (green) section of the Learning Board was Job Security. Sharon Slavic, administrative director, attended the 2 East Staff Meeting to answer questions and concerns related to job security. She shared information on the new residency program and the many improvements at the Elmer campus. Following her presentation, the Elmer Board in the breakroom was updated. The third Action Plan item added to the Solved section of the Learning Board was around Safety. “Engaging the Transporters with department action planning is extremely important” said Fogg. Transporters requested that mirrors be installed in the hallway to help prevent accidents. At the request of nursing staff, windows were installed in patient doors allowing them to see patients in isolation – ultimately improving patient safety. The windows also make patient feel less isolated as they are able to see into the hallway.

Nicole Sparks, R.N. and safety coach of Elmer 2 East shared, “Daily huddles provide open communication between staff and leadership that allow us to continuously improve safety. Significant issues brought up in the Daily Huddle are placed on the Daily Safety Briefing by our manager. The utilization of our Safety Tools and Learning Board keep us focused and moving forward to resolve all issues brought to leadership by the staff.” Pictured left to right: Mayra Montes Bautista, CNA; Walter Soro, R.N.; Craig Williams, R.N.; Diane Curry, R.N.; Nicole Sparks, R.N.; Dorca Cruz. SafetyStory Patient Experience and WOW Experience This month’s Safety Story was shared by a patient who visited Inspira Vineland’s Radiology department and focuses on the patient experience and creating a “Wow” Experience. “I wanted to contact you regarding my visit today to obtain an x-ray. Everyone from the valet parking attendant to the X-Ray Technician were extremely kind and attentive. I have severe back issues and can only get around on my rollator. The valet attendant removed it from the trunk of my car for me. Upon entering the lobby, the young woman at the front desk saw me and noticed my very tiny wife. She immediately obtained a wheelchair for me, so my wife didn’t have to push me in the rollator. She obtained visitor passes for us and wheeled me to the registration area, stayed with me throughout the process and assisted with signing in. I was called for my x-ray within 15 minutes and was thoughtfully and professionally taken care of. Compliments and a huge thank you to all the staff! They made what could have been an arduous process into a very pleasurable experience.”

Safety Tool of the Month: Clarifying Questions

When to Ask Clarifying Questions:

When in High Risk Situations When information is incomplete When information is ambigious

WHY: To reduce the probability of making a wrong assumption. Asking clarifying questions reduces the risk by 2.5 times! HOW: Phrase your clarifying questions in a positive way and in a manner that will get an answer that improves your understanding of the information

Inspira Medical Centers Receive ‘Primary Stroke Center’ Certification All three of Inspira’s medical centers, Inspira Medical Center Mullica Hill, Elmer, and Vineland, have received certification by DNV GL Healthcare as Primary Stroke Centers (PSC). Accreditation as a Primary Stroke Center ensures necessary staffing, infrastructure and programs to stabilize and treat emergent stroke patients. This accreditation serves as a testament to Inspira’s dedication to patient safety and clinical excellence. The PSC accreditation affirms that Inspira’s hospitals can provide treatment for a broad range of stroke conditions and some acute therapies, as well as admit patients to specialized units for inpatient care. “At Inspira, our care teams are focused on clinical excellence, and we are honored to receive this recognition from DNV GL, a recognized authority on clinical excellence,” said Amy Mansue, president and CEO. “If you or a loved one needs stroke care, you can be confident that our hospitals will provide timely evidence-based care. We are grateful to every physician and staff member at Inspira who provides this critical care at a moment’s notice, and we celebrate their efforts in achieving Primary Stroke Center accreditation.” Inspira’s advanced capabilities and protocols for rapid response, diagnostic evaluation and treatment of stroke can reduce the risk of permanent disability. As part of its community education efforts, Inspira’s stroke teams urge anyone who might be experiencing stroke symptoms to call 911 immediately. Early diagnosis and treatment of stroke significantly reduce the possibility of permanent disability. In most cases, clot-busting drugs can only be administered within a few hours of the onset of stroke symptoms. Inspira underwent a robust certification process with DNV GL to earn the Primary Stroke Center designation. The DNV GL conducted thorough observations, interviews, and document reviews during its certification audit to assess potential patient outcomes and processes, care and service quality, patient care environments, and more.
