Throughthisprocess,individualswith specialneedsanddisabilitieswillbe assignedlifepartnersknownas Navigatorstohelpnavigatethrough theobstaclesandbarriersofobtaining servicesinthecommunity.
HowTheMACNavigatorsSupportYou:
AllMACMemberswillbeassigneda Navigatorwhowillcoordinatealltheir needs.
TheNavigatorensuresinformation sharedbytheMACMemberwillbe documentedtoreducethenumberof timesanindividualhastotelltheir storytoareferralorganization. Navigatorscompletea comprehensiveassessmenttoassist inidentifyinggapsinservices. TheNavigatorwillfollowuponall referralstoensuretheMACMember hasbeenconnectedandcontacted bythereferralorganizationandcan supportinanyfollowup.
