Technique There are many decisions to make when considering breast implant surgery. The type of look you desire, the type of implant to use, its size and shape as well as the location of any surgical incisions, pocket locations and placements. It is important that these choices are determined on a patient by patient basis and Dr Kumar can guide you and facilitate this so that your choice results in exactly the appearance you are looking for. Breast enlargement or breast implant surgery is continually evolving and it is important that your surgeon is aware not only of the latest techniques but also of the latest trends that contribute to giving you as beautiful a result as possible.
Incision Location A small incision is required to allow creation of the pocket where the implant is then placed. The incisions are made in sites that aim to minimise scar visibility. There are three potential incision sites for the insertion of breast implants: Under or in the breast fold – inframammary fold Around the nipple – periareolar Through the armpit – transaxillary Dr Kumar exclusively uses the inframammary fold incision to produce safe, reproducible and beautiful results

Implant Placement Under the pectoralis muscle (Submuscular Technique)

In this technique, the implant is covered with the entire pectoralis major muscle for maximum coverage. Under the breast tissue (Subglandular Technique) The implant is placed in a pocket created under the existing breast tissue. The implant therefore lies over the muscle and doesn’t interfere with it. Partly under the muscle / Partly under the breast (Dual-Plane Technique) This technique combines the benefits of both the subglandular and submuscular techniques without their shortcomings. Some of the implant in key areas is covered by muscle whereas other areas are covered by fascia/breast tissue.
Implant Fill Modern implants are only filled with either saline or silicone cohesive gel. In the Australian market, almost 99% of breast implants used are silicone gel filled. Modern ‘silicone’ implants are filled with a cohesive silicone gel. Most manufacturers supply implants in both a soft and firm gel. The new generation silicone cohesive gels do not leak the way that older silicone gels did. All silicone gel implants used today hold their shape to some extent. However, the gel itself does vary on how firm it is to the touch and its shape-holding qualities. In general, the firmer a gel is the better shape-holding characteristics it has, whereas the softer it is the less shape-holding capacity it has.
Implant Shape The two very broad categories of implant shape are round and anatomical (teardrop). Many women who are considering breast augmentation surgery already have some sort of preference in mind when it comes to these shapes. However, the way these shapes are used, and in particular their placement, will affect the way they look in any individual. The amount and quality of pre-existing breast tissue is also very important in the look that these two implant types can achieve.
Anatomical Implants These are shaped like a breast, so they will help shape an abnormal breast and tend to look natural in most circumstances. The shape advantage is less noticeable when compared to a round implant if placed under the muscle, especially if the patient is standing upright.
Round Implants These are the most common shape of implants in Australia. Due to their shape, they provide better ‘cleavage’ than an anatomical shaped implant. They are a good choice for women who wish to increase the volume in breasts that already have a reasonable shape, or patients wishing for more cleavage (upper pole projection or roundness).
Vectra 3D Imaging Vectra 3D simulation technology is a cutting-edge system that will take a series of photographs from several angles, and then combine them to form a photo-accurate three dimensional model of your breast and body. Dr Kumar then uses its advanced software to make adjustments on your 3D model in order to demonstrate the actual effect different implants and procedures may have. Keller Funnel Implant Delivery System The Keller Funnel implant delivery system is a new way of placing breast implants inside the body. Before its advent, standard practice was to place implants by hand. This limited the level of accuracy with which they could be inserted, and restricted the ability of surgeons to put implants in their ideal location. Rather than pushing implants into place, it’s possible to now naturally slide them in.
