President’s Perspective TECHNICIAN MEMBERS

IPhA President Cindy Russell, RPh April is finally over! It has been a really difficult and stressful month in my pharmacy. We lost several technicians this month. We had one get evicted and she went back to live with her parents. But, we also had two technicians who had been licensed for 2 years and needed to take the certification test in order to renew their licenses.

Both of them had failed the test several times by just a couple of questions.

It’s now May and those 2 technicians are gone. They either decided that pharmacy was not for them or they were too scared to try to take the test for that fourth and final time. So, I am left scrambling trying to find replacements and I’m having a difficult time.

You would think that with all the technician programs out there, it would not be so difficult to PAGE 4

find quality certified pharmacy technicians. Where are ALL of the pharmacy technicians who are looking for jobs? I get applications but I have had a couple no shows on interviews. I hired one who didn’t show up for their first day. Maybe I am just having a bad run of luck.

It has never been this difficult to find pharmacy technicians.

Where are all the technicians out there who want a long term job? You know who I mean.

The technician who works for years and who knows how to do anything you ask them to do? The one you can depend on to solve any insurance problem or technical problem. I am lucky that my store has a couple of those super techs. My current technicians are all certified and they are all good, hard working technicians.

They are all dependable and they certainly have had to be flexible this last month because we are so short handed. I am going to have to think of something special for them. I was thinking about buying them memberships to IPhA. We need more technicians in our organizations and we need more technicians represented.

In case you missed it, the House of Delegates voted to add technician ILLINOIS PHARMACIST (APR/MAY/JUN 2015 - VOL. 77-NO. 2)

representation in the House of Delegates and on the Board of Directors. Certified Technicians are vital to what we do. We probably don’t let them know as often as we should how much we appreciate them.

IPhA has pharmacist members in such varied practice sites. But we don’t have nearly as many pharmacy technician members.

We need our technicians in the organization. They work along side us to make sure the pharmacy runs smoothly. They are that extra pair of hands and eyes that help us provide pharmacy care to our patients. They are often the unappreciated part of our team.

So, I challenge each of you to invite your technicians to join IPhA. How about showing your appreciation for your technicians by giving them a membership? Do something special for your staff to show them just how much you appreciate them.

I have 3 applications waiting for me this morning. So, hopefully, my bad run of luck finding technicians is over. I am looking forward to May!

IPhA continues to move legislative initiatives that will advance the profession and level of patient care in Illinois. This legislative session, IPhA introduced five pieces of legislation that would advance and protect pharmacy including pharmacy technician continuing education, immunization care expansion, state-level provider recognition, and managed care reform.

Your IPhA works diligently with legislators, regulatory agencies, and other health care stakeholders to protect your practice and improve patient care.

Continuing Professional Development Your professional competency and honing your skill set is a continually effort that must be maintained throughout our careers. IPhA is with you providing relevant continuing pharmacy education programming and certificate training programs to advance your knowledge and skills. IPhA is proud to announce three new advanced training opportunities for our members. IPhA has established agreements to provide the APhA Pharmacy-Based Cardiovascular Disease Risk Management certificate training program and Pharmacy-Based Travel Health Services advanced training program and the NACDS Community Pharmacy-Based Point-of-Care Testing certificate program. This is in addition to the APhA Pharmacy-Based Immunization Delivery, The Pharmacist and Patient-Centered Diabetes Care, and the Delivering Medication Therapy Management Services certificate training programs that we currently offer annually.

The practice sections offer you the opportunity to engage with fellow peers to discuss new innovations and challenges to your practice settings. The transition from a student pharmacist to licensed practitioner can be challenging and the IPhA New Practitioner Network is with you to provide networking and resources to assist you establishing your career path.

Pharmacy is an every changing health care profession and the upcoming years will be paramount in the history of pharmacy.

Pharmacy is on the verge of obtaining provider status and taking a larger role in our patient’s care in all practice settings.

From the Editor It is the worst of times and the best of times free falling and the rats are deserting the ship, that is usually the best time to go “all in” in the market. When the sun is shining and the market is high, look out. (Very few of us have the discipline to actually do this.) This analogy applies to pharmacy.

is a frightening time to be in retail pharmacy.

Perhaps it is frightening in all professions, we are just intimately involved in pharmacy. All the old business models seem to be disintegrating before our eyes.

And perhaps we define the concept of “the middleman.”

This is nothing new. We have been predicting this for years, but it is still daunting to see it happen right in front of you while your very subsistence is dependent upon the old model. No one seems to value the product we are dispensing or the service we are providing. If we are, in fact, providing service. Sometimes—a lot of the time—I feel like a glorified “Coke” machine. The first two questions out of everyone’s mouth when dropping off prescriptions seem to be 1. When? and 2. How much? These are valid questions and not easily answered while not being the most important questions. And the short sightedness of a lot of patients when getting prescriptions is astounding. They feel no compunction about belittling and criticizing at the top of their lungs people who literally hold their lives in their hands. (I would be a little more circumvent in critiquing the ancestry of pharmacy personnel, but that’s just me.) It really does feel like the pharmacy profession (and the world) is becoming more chaotic and more unpredictable... which, I guess, defines chaotic.

But, I really believe that in chaos lies opportunity. In investing, the very best time to invest is when it appears the whole world is falling apart. When the stock market is

This may be the best time to consider opening your own pharmacy. I know this is counter intuitive given third party payment practices, but specialty pharmacies seem to be printing money. Compounding pharmacies seem to be fairly successful, with a few high profile, notable exceptions.

Consultant pharmacies also appear to be approaching realization. No inventory and smaller spaces mean lower rent and costs, which are important considerations. Medicare payment is tied to Star ratings which could be affected by pharmacist input, so insurances, hospitals and soon chain pharmacies will be extremely interested in increasing these ratings. Hospital pharmacists look like they are an accepted, integral part of the health care team. The world is screaming for credible information on vitamins and supplements which is an area pharmacists would appear to be uniquely qualified. Even homeopathy, although I have yet to hear a realistic mechanism of action of how homeopathic products would actually do something. (The placebo effect is not a valid explanation.)

We may look back to today as an exciting, progressive time for our profession. The turning point could be now for those brave enough to grasp it.

PS: All these prospects didn’t just happen. Pharmacy organizations such as the Illinois Pharmacists Association in coordination with other associations and academia brought the profession to this point. And they need your support to continue. Who else do we have?

Alan Granat Memorial Scholarship

?

This award was established as a memorial tribute to Alan Granat, who served as Executive Director of IPhA from 1979 until his death in 1989. The award will be presented annually by the IPhA Foundation to a pharmacist and/or pharmacy student who has exhibited a commitment to pharmacy and community, as evidenced by membership and participation in pharmacy organizations and community involvement.

To be eligible, pharmacists must be members of IPhA, Illinois residents, and licensed to practice in Illinois.

Eligible students must be entering their third year of a Pharm.D. program, be student members of IPhA, and have their permanent residency in Illinois. The recipient(s) of this award will be required to contribute an article to Illinois Pharmacist, IPhA’s journal.

Bowl of Hygeia Award This award honors a pharmacist who has compiled an outstanding record of community service, which apart from his/her specific identification as a pharmacist, reflects well on the profession. Also: Pharmacist licensed in Illinois; IPhA member; has not previously received the award; must not be currently serving or have served, within the past two years, as an officer of IPhA, other than an ex-officio capacity, or as member of its awards committee. Sponsored by the APhA Foundation and NASPA.

Distinguished Young Pharmacist/Edmond P. Barcus Award Originally sponsored by IPhA to honor the memory of IPhA President Edmond P. Barcus, who died in office, this award honors an up-and-comer pharmacist.

The recipient is an individual who shows tremendous commitment to the profession and his/her community, leadership potential, professional aspirations, and involvement in community service. Also: Member of IPhA in the year selected; entry degree in pharmacy received less than ten years ago; licensed to practice pharmacy in Illinois and must have practiced in community, institutional, managed care, or consulting pharmacy in the year selected; participated in national pharmacy associations, professional programs, and/or community service. Educator of the Year

The Cardinal Health GenerationRx Award is designed to recognize a pharmacist who has demonstrated excellence in community-based prescription drug abuse prevention. The award is intended to recognize outstanding efforts within the pharmacy community to raise awareness of this serious public health problem.

It is also intended to encourage educational prevention efforts aimed at patients, youth, and other members of the community.

This award includes a $500 donation to the charity of the recipient’s choice.

Honorary President

Legislative Day

What’s in it for th

On March 11, 2015 I was fortunate to be able to once again attend Legislative Day. I was joined by fellow pharmacists, student pharmacists and educators. The purpose of our visit was to be updated on Legislative issues and to share and update our Legislators as to issues that face pharmacy.

But what is in it for the typical staff pharmacist? Besides the fellowship and education, it is the role you play as ADVOCATE. Every day in your practice, you think nothing of advocating for your patient. You will go out of your way to advocate for your patient to make sure that they have the correct drug therapy.

But at Legislative Day, you take on the role of advocate for your profession. Millions and millions of dollars are spent on health care every year by our state. The decisions for how these dollars are allocated are done by Legislators that have NO EXPERIENCE in Health Care. Because of budget constraints, it is absolutely critical that

these dollars are allocated correctly. Your Legislator needs experts in pharmacy to be able to explain pharmacy because of the complexity of our profession (Alice in Wonderland comes to mind). By attending Legislative Day and connecting with your Legislator, YOU become the face of pharmacy and ultimately what you are hoping to accomplish is that before they place that vote that they take into account what you have told them.

Next year, I would like to see YOU attend Legislative Day and begin the process of becoming your profession’s and your association’s ADVOCATE for Pharmacy in Illinois.

Pharmacists’ Patient Care Process:

Consistency is Critical

By Anne Burns, BSPharm and Sarah Durr Recognizing the need for a consistent process in the delivery of patient care across the profession, the Joint Commission of Pharmacy Practitioners (JCPP)1 recently released the Pharmacists’ Patient Care Process. The process is applicable to any practice setting where pharmacists provide patient care and for any patient care service provided by pharmacists.

This article describes the development of the Pharmacists’ Patient Care Process, what the process is, why it’s important, and initial implementation steps.

to emerge where health care professionals are paid for achieving desired outcomes for their patients (e.g., patients’ blood pressure at goal) instead of by the number of patient visits they complete.

To measure the outcomes of pharmacists’ services in a meaningful way, a consistent process of care must be used to deliver the services. That way an “apples to apples” comparison can be made for the collective value that pharmacists provide within the health care system.

The Foundation JCPP’s vision statement for the pharmacy profession and strategic plan for reaching this vision were revised in July 2013 as part of a strategic planning retreat that included thought leaders from inside and outside the profession. The newly adopted vision, “Patients achieve optimal health and medication outcomes with pharmacists as essential and accountable providers within patient-centered, teambased health care,” reflects the need for pharmacists to be patient-centered and accountable for patient outcomes while working collaboratively with other members of the health care team.

Key drivers to achieving the JCPP vision identified by pharmacy thought leaders include: 1) a widely-adopted and consistently delivered pharmacist patient care process, 2) quality measures to measure the value of pharmacists’ services, 3) robust health information technology to support patient care, and 4) payment for pharmacists’ services.

The need for pharmacists to use a consistent approach to patient care delivery has always been important. However, with the increasing movement to outcomes-based payment models in the health care system, this need is becoming more urgent. Payment models are starting PAGE 22

pharmacy. The patient care process is articulated in a manner aligned with the patient care processes of other health care professionals while at the same time detailing the unique medication-related aspects of pharmacists’ training. The development process included organizational comment periods and testing with clinicians to create the document approved by JCPP on May 29, 2014.

What is the Pharmacists’ Patient Care Process?

The Pharmacists’ Patient Care Process uses a patient-centered approach that depends first and foremost on the pharmacist having an established relationship with the patient. This relationship supports engagement and effective communication with the patient, family members, and caregivers throughout the process.

The process also involves the pharmacist working with prescribers and other practitioners to optimize patient health and medication outcomes (see sidebar).

The follow-up step signals that patient care process is repeated with each and every patient encounter, and the frequency of follow-up depends on the acuity of the patient and the nature of their care. The level of intensity for each step will vary with the service provided, but the process should not vary.

The process is intended to be used in all patient care settings, and while one pharmacist might be responsible for all the steps in some settings, in other settings, there may be more than one pharmacist involved at different stages of the process.

Pharmacist Involvement

The Pharmacists’ Patient Care Process was developed by a group of national pharmacy organizations working under the direction of JCPP. The foundation for the process is embedded within the pharmaceutical care model developed by Hepler and Strand in the 1990s and was developed by examining a number of key source documents on pharmaceutical care and medication therapy management.

These key documents were cataloged and compared to create a patient care process consistent with best practice models in ILLINOIS PHARMACIST (APR/MAY/JUN 2015 - VOL. 77-NO. 2)

National pharmacy associations are currently working to facilitate implementation of the process across the profession, including developing case examples for different practice settings. The Accreditation Council for Pharmacy Education (ACPE) has incorporated the Pharmacists’ Patient Care Process into the Draft Standards 2016 revision for the Doctor of Pharmacy curriculum. These Standards will be considered for final approval at the January 2015 ACPE Board meeting and will be distributed profession-wide shortly thereafter. With the ACPE Standards revision, schools and colleges of pharmacy will be working to incor-

Pharmacists’ Patient Care Process Using principles of evidence-based practice, pharmacists: A. Collect The pharmacist assures the collection of necessary subjective and objective information about the patient in order to understand the relevant medical/ medication history and clinical status of the patient. Information may be gathered and verified from multiple sources, including existing patient records, the patient, and other health care professionals. This process includes collecting: • A current medication list and medication use history for prescription and nonprescription medications, herbal products, and other dietary supplements.

• Relevant health data that may include medical history, health and wellness information, biometric test results, and physical assessment findings.

• Patient lifestyle habits, preferences and beliefs, health and functional goals, and socioeconomic factors that affect access to medications and other aspects of care.

B. Assess The pharmacist assesses the information collected and analyzes the clinical effects of the patient’s therapy in the context of the patient’s overall health goals in order to identify and prioritize problems and achieve optimal care. This process includes assessing: • Each medication for appropriateness, effectiveness, safety, and patient adherence.

• Health and functional status, risk factors, health data, cultural factors, health literacy, and access to medications or other aspects of care.

• Immunization status and the need for preventive care and other health care services, where appropriate.

C. Plan The pharmacist develops an individualized patient-centered care plan in collaboration with other health care professionals and the patient or caregiver that is evidence-based and cost effective. This process includes establishing a care plan that: • Addresses medication-related problems and optimizes medication therapy.

• Sets goals of therapy for achieving clinical outcomes in the context of the patient’s overall health care goals and access to care.

• Engages the patient through education, empowerment, and self-management • Supports care continuity, including follow-up and transitions of care as appropriate.

D. Implement The pharmacist implements the care plan in collaboration with other health care professionals and the patient or caregiver. During the process of implementing the care plan, the pharmacist: • Addresses medication- and health-related problems and engages in preventive care strategies, including vaccine administration.

• Initiates, modifies, discontinues, or administers medication therapy as authorized.

• Provides education and self-management training to the patient or caregiver.

• Contributes to coordination of care, including the referral or transition of the patient to another health care professional.

• Schedules follow-up care as needed to achieve goals of therapy.

E. Follow-up: Monitor and Evaluate The pharmacist monitors and evaluates the effectiveness of the care plan and modifies the plan in collaboration with other health care professionals and the patient or caregiver as needed. This process includes the continuous monitoring and evaluation of: • Medication appropriateness, effectiveness, and safety and patient adherence through available health data, biometric test results, and patient feedback.

• Clinical endpoints that contribute to the patient’s overall health.

• Outcomes of care including progress toward or the achievement of goals of therapy.

ACPE-accredited continuing education providers will also be encouraged to incorporate the process into continuing education programming for pharmacists.

You can play an important role in the adoption of this process. Start by reviewing the process (link:here) and thinking about how closely the patient care process used in your practice mirrors this process.

Talk to other pharmacists in your practice to make them aware of the process and to discuss how the practice as a whole can adopt a consistent process of care. Consider how pharmacy technicians can be incorporated into aspects of the process such as data collection to improve the efficiency of the process. Share the process with other health care practitioners you work with to make them aware of it.

Practice using the process and reflect on how effectively the process facilitated patient care.

Share your experiences with other pharmacists in your practice, your colleagues, and at professional meetings. Incorporate the process into the teaching materials for students you precept on IPPEs, APPEs, and internships. Make sure that students have the opportunity to practice using the process in their patient care encounters.

Students will be asking about the process so if you are a preceptor, it’s important to be prepared. Finally, watch for additional resources to assist you in the implementation and assessment of the pharmacists’ patient care process.

Pharmacists are often called upon to describe the value they bring to the health care system. A unified, consistent patient care process is critical to measuring pharmacists’ value and advancing the profession as a whole in the evolving health system.

CCP Students Help to Care in Guatemala

By Kailey Murphy, Pharm.D. Candidate Class of 2015 Midwestern University has a deep commitment to preparing its students to meet patient healthcare needs with compassion.

Because of this commitment, the University partners with DOCARE International, a non-profit organization that provides students in various healthcare programs an opportunity to work together to deliver essential care to the impoverished and isolated populations of Guatemala.

Chicago College of Pharmacy (CCP) has supported this mission for many years by annually seeking monetary and medication donations from its community. The College also helps staff the mission with faculty members and students, providing them with the opportunity to travel with DOCARE for two weeks to Antigua, San Andrés, Tecpán and several smaller, more remote areas in Guatemala. This year, I was fortunate enough to participate in this international mission, along with three other pharmacy students.

Once in Guatemala, we practiced patient-centered care in interdisciplinary healthcare teams to help determine appropriate treatments for a variety of disease states. We also provided medication and lifestyle education to local Guatemalans.

We worked with caregivers in other specialties, including podiatry, dermatology,

general medicine, dental medicine, pediatric medicine, and women’s health, to bring proper medical care and medicine to those that needed our help most.

On this trip, I had the pleasure of working alongside Alan Schalscha, D.O., the Medical Mission Director for DOCARE International. He offers his vision for the mission: “It is my hope that DOCARE will continue to bring practitioners and students together, to work as a team for an ultimate good. United, we will continue to work within many communities’ cultures and when possible, help those populations improve their health and well-being.”

Like many of the other students there, it was my first time traveling to a third world country – living and breathing the culture. Not everything about this mission was beautiful or comfortable, and it was at times even sad. It was, however, a journey well worth taking. Having the privilege of experiencing Guatemala in this way and to be welcomed into the lives and hearts of the people was remarkable.
