Marisol vanished during the Cuban Revolution, her fate unknown and lost to time. Now, haunted by atrocities long-forgotten, her foul-mouthed spirit visits her nephew, Ramon, in modern-day New Jersey. Her hope: That her presence will prompt her descendants to unearth their painful family history.

RECOMMENDED READING

The House on Mango Street is the remarkable story of Esperanza Cordero, a young Latina girl growing up in Chicago, inventing for herself who and what she will become. Told in a series of vignettes-sometimes heartbreaking, sometimes deeply joyous-Sandra Cisneros' masterpiece is a classic story of childhood and selfdiscovery. Few other books in our time have touched so many readers.

Ramon launches a haphazard investigation into the story of his ancestor, unaware of the forces driving him on his search. Along the way, he falls in love, discovers a new sense of his own identity, faces a run-in with a murderous gangster, and learns of each "lost saint" who helped Marisol during her imprisonment under Batista's reign.

Outside of work, Vanché is family-oriented. She especially loves bowling, and her family bowls with her. She explains, “My son was in a bowling league when he was younger. So we all went and got bowling balls and matching shoes. It all started from there, and we’ve been bowling ever since.”

Vanché received a gift package that included a pizza party with her team, a day off with pay, a Team Hurley All-Star fleece jacket, a one-month membership to the YMCA, a $25 Sodexo gift card, a $25 gift card from Sovita Credit Union, a $25 gift card to the Hurley Lobby Shop and a framed certificate. Congratulations, Vanché!

Being named Employee of the Month was a huge surprise. Her supervisor told her she needed two minutes of her time. Vanché went to see what was needed and found Melany Gavulic and a large crowd in the cafeteria. As she says, “Normally I’m the one nominating people, so the situation really got reversed on me this time.”

VANCHÉ GOLDEN

As the opening supervisor, Vanché makes sure that there is sufficient staff to cover all of the department’s responsibilities. That includes scheduling and assigning roles, while also completing payroll. During the day she does whatever is needed to keep things running smoothly, like counting down cash to help the cashiers stay prepared and covering call-off shifts. There are a lot of moving parts. Vanché explains, “The toughest part of my job is making sure there are enough staff members on duty to get everything done, both in regards to patient care and the needs of the cafeteria. But the best part is just getting to interact with so many coworkers and customers every day. Even problem solving when there is a customer complaint. I like to work towards end results that make the customer happy.”

Vanché Golden, Hurley’s Employee of the Month for August, started here back in 1998 as a tray passer in the Nutritional Services Department. After being born in Florida, her family moved here when she was still a young child. When she first started at Hurley, she intended on eventually transitioning to work in the medical field, but she found she didn’t have the stomach for that work. She has stayed in the department since, working just about every job, including the department call center, cooking, and cashiering. Today she holds the position of food services supervisor, and still has her hand in a lot of the day to day operations.

6 • HURLEY MEDICAL CENTER

Vanché has been a solid member of the department for many years.  Her capability to juggle so many things, from payroll to the schedule, to filling open spots and handling the ever changing landscape of the department on a daily basis makes her such a valuable asset to us.  Vanché has also been instrumental in handling both patient and retail customer concerns as well as troubleshooting equipment problems so things run smoothly and has a great source of past knowledge to share with the members of the team.  It is a pleasure to work with her.

Winning the DAISY Award was a big surprise. Ashley was observing in a patient’s room when her manager came and asked her to come to the nursing station. Her coworkers were waiting there to tell her that she won. The PICU is a demanding environment, with a variety of responsibilities. Besides the normal duties that nurses have, like administering medications to patients, Ashley also puts a lot of energy into helping the patients through the experience of being admitted to the

daisy award recipient

Ashley grew up in Goodrich and got her bachelor’s in biology from Oakland University. After finishing that degree, she didn’t really know what she wanted to do. She worked as an EMT for a few years. Seeing trauma nurses work as she delivered patients to the ER helped her decide to pursue that career. While she earned her RN degree at University of Michigan-Flint, she completed a clinical rotation in the PICU. As she says, “I loved it. It was the only position I applied for. Luckily, I got the job.” She’s been there for six years now.

hospital. PICU patients range in age from newborns to twenty, so on any given day she may be playing with kids with toys and games, comforting them with cuddling, or rocking babies and toddlers. Older kids might need to talk. Parents need to be comforted too, as it is always difficult for them to see their child sick and in the hospital. Ashley has to be there to help them understand everything that’s happening. It’s a tough environment, but worth it, as she explains, I like working with that wide age range, going from working with a baby to a teenager the next day. With that variety, you never know what you’re walking into. And the best part is seeing kids get better. They may come in really sick, but by the time they leave they’re back to their baseline and running around. Playing around like normal.”

Looking back, she says her favorite memories at Hurley all involve her fellow nurses. Ashley says, “I’ve become so close with so many nurses, and the good and bad moments really tie us together.”

When not at Hurley, she spends a lot of time with her family. She has a husband, Charlie, and two sons, Reed and Owen. Her sons are three and one years old, respectively. Ashley says, “I depend on them to help me, and my parents help us tremendously with babysitting. We love to travel and take family trips. We’re going to Germany next week. We get to see a German wedding. I’m really excited.”

This month’s DAISY Award Honoree is Ashley Sedlarik, RN. The DAISY Award celebrates the andcompassionextraordinaryanurseshowstoherpatientstheirfamilies.

The community applauded our service and continuously thanked Hurley for being the hospital of choice at the Gus Macker. We had a variety of nurses volunteering from Michigan United and of course our amazing residents / physicians who worked with us to care for our community.

Most of all we had fun, built relationships with each other and demonstrated our passion to care our community.

We were professional and showed the community what Hurley was really about, educated on underlying medical conditions and referred a few to follow up with their primary physician. We bandaged, iced, wrapped, comforted, hydrated, and made sure everybody felt welcomed and cared for with smiles, Gatorades, fruit snacks, oranges, etc.

Hurley was asked to provide first aid, health information, and cooling tent services for the event which brought out players and spectators of all ages.

This weekend at Gus Macker was incredible. We cared for our community and those visiting with clinical excellence and service to people. We worked together as a team and passionately demonstrated quality care to every patient that visited the medical tent and patients we had run to care for on the court pulling our red wagon.

Micah is spending the free time she has this summer with family and friends that she does not get to see during the school year. She is the oldest of four siblings, and she especially enjoys playing basketball with her younger sister. They also have a two year old black lab named Koda that loves to play.

Because of everything on her schedule, being named Volunteer of the Month came as a surprise to Micah. As she says, “I’m really happy. I’m only here for the summer, and only for the last month this summer. It feels good that I was making some sort of difference in people’s lives while I was here. I want to say thank you to Hurley for selecting me as Volunteer of the Month. I feel very honored, very happy.”

especially parents of little kids. And there are those here for emergency surgeries; the families don't necessarily know why their patient is here, and they can’t necessarily know all the details.”

Volunteering at Hurley is a small part of a very busy schedule for Micah. She’s also working for a group called STEMnetics this summer, where she teaches STEM subjects to kids in Flint elementary schools. This fall she has to head back to the University of Pennsylvania, where she is a premed student. Even with everything else on her plate, she still recommends volunteering at Hurley to others, “because I love the people that I work with, and it’s a very friendly environment. If you’re not sure how to help your community, Hurley is a great place to do that.”

In that position, Micah acts as a liaison between the patients’ families and doctors. To start each day, she gets the schedule from the nurses’ station, sets up the lounge, and makes coffee. Throughout the day she does whatever needs to be done to help patients and their families have the best experience possible. That includes greeting everyone, finding out who they are here for, handing out coffee, adjusting the TV, walking families to recovery or patient rooms, and communicating updates from the doctors. She explains, “I love being there for the families. I do my best to make sure everyone is getting what they need, since it's not easy for them to be here,

Here’s what you may not know: Pneumonia will, by default, code to a “simple pneumonia” unless a suspected causative organism or mechanism of action is specified in the physician documentation. For example, community-acquired pneumonia, healthcare associated pneumonia, pneumococcal pneumonia, and viral pneumonia all code to “simple pneumonia”. Alternatively, aspiration pneumonia, Staphylococcal pneumonia, most Gram-negative pneumonias, many fungal pneumonias, and pneumocystis pneumonia in patients without HIV disease all code to “respiratory infections and Additionallyinflammation.”,thesputum

This material is confidential and proprietary property of FairCode Associates, LLC Revised November 2021 30 • HURLEY MEDICAL CENTER physician connection

Here’s what you already know: All pneumonias can be thought of as having a causative organism and/or mechanism of action e.g., aspiration.

What Caused Your Patient’s Pneumonia?

Furthermore, if you document only “aspiration” without “pneumonia” or “pneumonitis”, then the coder will not capture a pneumonia at all and may code a foreign body in the respiratory tree!

Let’s look at a clinically similar scenario and possible Your 56-yo patient is homeless, found in a gutter, clutching a bottle of Jim Beam, and presents with pneumonia and RML infiltrates. You admit the fellow to treat aggressively.

In the first scenario, you documented "pneumonia" in the progress notes. Conversely in the second scenario, you documented “pneumonia: likely due to aspiration” in your progress notes and discharge summary As a result, your patient was approved for a longer hospital stay to address his more complicated pneumonia under your expert care.

Medical Documentation Tip o’ the Month ~or~

The moral: Always indicate the organism (if known or presumed) and/or whether you suspect the mechanism of action was a cause of the pneumonia.

Why the difference?

DMSRG Description Average Medicare Compensation Average Length of Stay 195 Simple pneumonia & pleurisy w/o CC/MCC $4,021.88 2.5 days 179 Respiratory infections & inflammations w/o CC/MCC $5,271.70 3.0 days

coding outcomes:

culture results on the lab report will not suffice unless you connect the results to the patient’s pneumonia in your notes or discharge summary. Therefore, unless you document the causative agent, the case will code to simple pneumonia. While, it may be clear to you that an obtunded nursing home patient with a right middle lobe infiltrate has aspiration pneumonia, the coder is not permitted to make the same logical leap and code aspiration pneumonia unless you document that you think this is the cause.

The Federal Government has enough regulations to handcuff any coder and significantly impair his or her ability to demonstrate how sick your patients are.
