Tri-City Health Center’s (TCHC) quality care helps seniors achieve their healthy goals. “Seniors at the Fremont Senior Center, are doing just that,” said Sangeetha Sekhar, Care Coordinator for TCHC Chronic Care Team. “They are taking control of their health, by making life style changes that improve their health outcomes.” TCHC is committed to delivering excellent health services in a caring, nurturing, and respectful atmosphere. Their team is dedicated to improving the quality of life for every senior in the community. At the senior center, TCHC provides comprehensive services to seniors, including: • • • • •
Primary Care Services: Including Blood Pressure Screening, and Diabetes Screening Vision Screening: Comprehensive screenings for cataracts, glaucoma and eyeglasses Dental Screening: Provides a full range of services to achieve good oral health Podiatry Program: Provides foot care including, ankles and lower leg Patient Health Education: Encouraging healthy lifestyle choices
