The City of Fremont’s Human Services Department (HSD) mission is to support a vibrant community through the creation and maintenance of services that empower individuals, strengthen families, encourage self-sufficiency, enhance neighborhoods, and foster a high quality of life for all residents in Southern Alameda County. Aging and Family Services (AFS), one of the HSD Divisions, provides an array of services that support older adults and their family caregivers. AFS enacts the Department’s mission through our Senior Support Services and two Age Well Centers. The Centers offer classes and activities, along with volunteer opportunities, that help those 55+ years stay healthy and active. Senior Support Services employs a team of caring professionals from diverse backgrounds in social work, counseling, nursing, gerontology, psychology, and public health who can help you develop an individualized plan that includes coordinating resources, educational opportunities, and emotional support.
