WE ARE HERE TO HELP THE CITY OF FREMONT’S HUMAN SERVICES DEPARTMENT is very proud to assist older adults in obtaining information and support. One popular program is called “Care Coordination”. This Aging and Family Services program is a collaborative process between an older adult, his/her family and our multi-disciplinary team to meet an individual’s health, financial, and social needs. The overall goal is to provide individualized support in the Tri-City area so people can continue to live safely and well at home. Experienced Human Services staff, often called Care Coordinators, will make home visits to complete a thorough assessment and care plan and then provide regular followup over the phone or in person. Some common examples of care coordination services are helping with medical insurance and figuring out benefits, finding transportation alternatives, assessing home safety and need for modification, obtaining healthy meals, accessing legal resources, and understanding recreational and social activity options. The Fremont Senior Center will be closed Monday, January 1st and Tuesday, January 2nd. In observance of Martin Luther King Jr Day the center is also closed Monday, January 15. January Birthday Party Celebration Friday January 12 1pm—2pm Join us as we celebrate January birthdays. Entertainment will be provided by piano/keyboardist Kelly Keys. Everyone is welcome, even if it is not your birthday! Free cake and ice-cream will be given to all attendees and all those celebrating a January birthday receive a FREE gift. Family Caregiver support program offers walk-in counseling hours on second and fourth Fridays of the month where you can talk with a counselor and find better ways of managing caregiving stresses and other community resources. No appointment needed. You can also take advantage of the Family Caregiver Education Workshop Series starting on Wednesday January 17th, 2017.

The shifts we are in need of are: Tuesday’s 12–3pm, Wednesday’s 8am–12pm or Friday’s 12–3pm. We are looking for kind, patient, focused volunteers who love to talk to others. The start time is as early 6.30am and ends usually by 9.30am–10am. Kitchen Volunteers Needed for Lunch Love customer service and making connections with adults 55+? If so, come help with delivering lunch to disabled seniors, communicating to our 55+ customers, decorating the dining room, cleaning dining room, assisting our disabled seniors to their vehicle/ride/bus stop, and other various jobs.
