he times the world is going through are changing and evolving in many ways. Stopping or slowing down means falling behind in the rhythm of life that demands intensity in many ways. The publishing industry is no stranger to this evolution that marks the new trends of communication to learn about what is happening in the world, in our city and our immediate environment. Until a few years ago, we saw the news on TV and we waited for the next day’s newspaper to learn profusely and in detail about what happened. Today, social networks surf at high speed through space and information comes almost in real time to any tablet, screen or smartphone with a connection to the web. Of course, much of what circulates in networks is garbage, but like junk food, many consume it without noticing the immediate or subsequent consequences. Until a few years ago, the circulation of newspapers and magazines in the world reached millionaire figures or in other cases, hundreds of thousands of copies every day. Today, it is very different. Many publishing houses around the world have decided to close their printed editions and focus on the digital versions of their media as a way to temper the onslaught of an inflationary spiral that hits with the force of a storm. Large amounts of sugar and chemicals are part of the ingredients of industrialized juices that are sold as made with fruit, so they are not so healthy. By Beatriz Flores Thanks to misleading advertising and to believe that what is held as natural should always be so are that many people consume industrialized juice without knowing that it has everything, but fruit. Nutritionist Karen González Riestra, recommended to people, especially to parents, not to trust that the “fruit” juices that they sell in the supermarket or in the corner store are natural. “Many people see the juices and think they are healthy because they are peach juice and they think it is nutritious, but behind that, there are many things, it is not recommended for children, you have to stop putting them in the lunchbox because they are like sodas”, she explained. But she said they are not healthy for adults either. She mentioned that these drinks are made with almost the same amount of sugar than soft drinks. The WHO (World Health Organization) said in a bulletin issued in 2016, that sugary drinks favor the prevalence of obesity, type 2 diabetes, and tooth decay. “It’s not about prohibiting, it’s about teaching us to consume those foods sporadically, to eat healthier”, said González Riestra. The recommendation was to do it once every three weeks or once a month and avoid them as much as possible. The graduate in Nutrition and Food Sciences highlighted the importance of reading the labels of what is bought and consumed, comparing products and choosing the healthiest option.

She also said that if you do not read “sugar” among the ingredients, it may say high fructose corn syrup, honey sweetener, melase, fructose, brown sugar or maltose. She added that they contain additives that can be dyes, preservatives and high amounts of sodium. The official Dietetic Guide of the United States mentions that since the juice is extracted from the fruit, processed and concentrated, it loses all its properties and only the sugars remain. In addition, she mention that industrialized beverages, in this case, juices, are made from concentrated juices that are rehydrated.
Por Beatriz Flores Gracias a la publicidad engañosa y al creer que lo que se ostenta como natural siempre debe serlo es que muchas personas consumen jugos industrializados sin saber que tienen de todo, menos fruta. La nutrióloga Karen González Riestra, recomendó a las personas, sobre todo a los padres de familia, no confiar en que los jugos “de fruta” que venden en el súper o en la tiendita de la esquina son naturales. “Muchas personas ven los jugos y piensan que son saludables por ser jugo de durazno y creen que es nutritivo, pero detrás de eso hay muchas cosas, no es recomendado para los niños, hay que dejar de ponerlos en la lonchera porque son como refrescos”, explicó. Pero dijo, tampoco son saludables para los adultos. Mencionó que estas bebidas están elaboradas casi con la misma cantidad que los refrescos. La OMS (Organización Mundial de la Salud), dijo en un boletín emitido en el 2016, que las bebidas azucaradas favorecen la prevalencia de la obesidad, la diabetes tipo 2 y la caries dental. “No se trata de prohibir, se trata de enseñarnos a consumir esos alimentos esporádicamente, a comer de forma saludable”, dijo
González Riestra. La recomendación es hacerlo una vez cada tres semanas o una vez al mes y evitarlos en la medida de lo posible. La licenciada en Nutrición y Ciencias de los Alimentos destacó la importancia de leer las etiquetas de lo que se compra y consume, comparar productos y elegir la opción más saludable. “Entre más ingredientes traiga un producto, más dañino va a ser para la salud, lo mejor son los productos naturales, les recomiendo que opten por opciones naturales, lo mejor es tomar agua de fruta natural que sea de temporada para que no tengan que endulzarla”, expresó. Dijo también que si entre los ingredientes no se lee “azúcar”, puede decir jarabe de maíz de alta fructosa, miel endulzante, melasa, fructosa, azúcar morena o maltosa. Agregó que contienen aditivos que pueden ser colorantes, conservadores y altas cantidades de sodio. La Guía Dietética oficial de los Estados Unidos menciona que desde que el jugo es extraído de la fruta, procesado y concentrado, pierde todas sus propiedades y quedan solamente los azúcares. Además, menciona que las bebidas industrializadas, en este caso los jugos, se elaboran a partir de jugos concentrados que son rehidratados.

PULMONARY THROMBOEMBOLIC HYPERTENSION
What is it? Chronic Thromboembolic Pulmonary Hypertension (CTEPH) is one of the 5 types of Pulmonary Hypertension. CTEPH occurs when the obstruction of the pulmonary vessels is not eliminated due to the formation of a clot or several episodes of pulmonary embolism, which leads to an increase in pulmonary pressure, which over time can cause right heart failure. Although the exact prevalence is unknown, it is estimated that it affects almost 4 percent of people who survive an episode of pulmonary embolism.
HPLC WHAT DOES IT MEAN? Hypertension: is the medical term to refer to high blood pressure. Pulmonary: related to the lungs. Thromboembolic: refers to the blockage of a blood vessel that can initially arise from a blood clot that originates in the lower extremities. Chronic: long-term illness.
WHAT ARE THE CAUSES OF HPLC?
In most cases, CTEPH occurs in people who have a history of pulmonary embolism, that is, a blood clot in the lungs that originated in the lower extremities. Some of these clots do not dissolve completely, so they remain glued to the wall of the pulmonary artery. Over time, new tissue forms inside the clot, which becomes more fibrous and creates a blockage that restricts blood flow. WHICH ARE THE RISK FACTORS?
The people most likely to develop CTEPH are those who: - Had one or more episodes of acute pulmonary embolism. - Suffered from deep vein thrombosis or chronic ulcers in the leg. - Had an infection with the wires of a cardiac pacemaker. - Have a history of cancer. - Have a thrombophilia. - Suffered a chronic inflammatory disease. SYMPTOM:
One of the problems with CTEPH is that its symptoms are very general and can be attributed to more common heart or lung problems. Its most frequent symptoms are: - Shortness of breath (dyspnea).

- Swelling of the tissues of the lower limbs (Edema). - Tiredness. - Thoracic pain. - Fainting (Syncope). HOW IS HPTEC DIAGNOSED?
Although the exact prevalence is unknown, it is estimated that it affects between 8 and 40 people per million. Depending on the clinical history and symptoms, the doctor may suspect HPLC, but tests are needed to distinguish it from other forms of pulmonary hypertension. A definitive diagnosis of CTEPH should be made in the hospital, which has a team of doctors and radiologists who specialize in it.
TREATMENT
In patients with an average pulmonary artery pressure (mPAP)> 50mmHg, mortality at 3 years reaches 90 percent. Right ventricular heart failure is the most frequent cause of death. However, CTEPH is potentially curable, since the surgical procedure known as Pulmonary Endarterectomy (PEA) can be effective in those patients considered candidates for the procedure. The PEA is adequate for two of three patients with CTEPH and the quality and life expectancy of those who undergo this surgery are significantly improved, so it can be considered as a cure for many of them.
However, it is important to mention that between 20 and 40% of patients are not candidates to undergo this surgical intervention, and it depends on several factors, among which are: If the obstruction is very far inside the artery and it is impossible to reach it. The physical condition of the patient is also an important factor so that before the operation his cardiopulmonary capacity is evaluated. When the hemodynamic severity of the disease is not related to the degree of obstruction. In addition, up to 35 percent of operated patients have pulmonary hypertension recurrence after surgery. Currently the Ministry of Health, the FDA, in the United States and the EMA in the European Union, among other regulatory agencies, approved an innovative therapeutic alternative for patients with CTEPH if surgery is not an option for the patient or when the disease persists despite the completion of surgery, offering a significant improvement in exercise capacity and optimizing cardiac and pulmonary function, allowing the patient to improve their symptoms, and consequently recover their independence to perform daily activities. HIPERTENSIÓN PULMONAR TROMBOEMBÓLICA
¿Qué es eso? La Hipertensión Pulmonar Tromboembólica Crónica (HPTEC) es uno de los 5 tipos de Hipertensión Pulmonar. La HPTEC ocurre cuando no se elimina la obstrucción de los vasos pulmonares debido a la formación de un coágulo o varios episodios de embolia pulmonar, lo que conduce a un incremento en la presión pulmonar, que con el paso del tiempo, puede provocar insuficiencia cardiaca derecha. A pesar de que se desconoce la prevalencia exacta, se calcula que afecta a casi el 4 por ciento de las personas que sobreviven a un episodio de embolia pulmonar.
HPTEC ¿QUÉ SIGNIFICA? Hipertensión: es el término médico para referirse a la presión sanguínea elevada. Pulmonar: relacionado con los pulmones. Tromboembólica: se refiere al bloqueo de un vaso sanguíneo que inicialmente puede surgir por un coágulo de sangre que se origina en las extremidades inferiores. Crónica: enfermedad de larga duración.
¿CUÁLES SON LAS CAUSAS DE LA HPTEC?
En la mayoría de los casos, la HPTEC se presenta en personas que tienen antecedentes de embolia pulmonar, es decir, un coágulo de sangre en los pulmones que se originó en las extremidades inferiores. Algunos de éstos coágulos no se disuelven por completo, por lo que permanecen pegados a la pared de la arteria pulmonar. Con el paso del tiempo se forma tejido nuevo dentro del coágulo, el cual se vuelve más fibroso y crea un bloqueo que restringe el flujo sanguíneo. ¿CUÁLES SON LOS FACTORES DE RIESGO?
Las personas que tienen más probabilidad de desarrollar HPTEC son quienes: - Tuvieron uno o más episodios de embolia pulmonar aguda. - Padecieron trombosis venosa profunda o úlceras crónicas en la pierna. - Tuvieron una infección con los cables de un marcapasos cardiaco. - Cuentan con antecedentes de cáncer. - Tienen una trombofilia. - Padecieron una enfermedad inflamatoria crónica.

SÍNTOMAS:
Uno de los problemas de la HPTEC es que sus síntomas son muy generales y pueden atribuirse a problemas cardiacos o pulmonares más comunes. Sus síntomas más frecuentes son: - Falta de aire (Disnea). - Hinchazón de los tejidos de los miembros inferiores (Edema). - Cansancio. - Dolor torácico. - Desmayo (Síncope). ¿CÓMO SE DIAGNOSTICA LA HPTEC?
A pesar de que se desconoce la prevalencia exacta, se calcula que afecta a entre 8 y 40 personas por cada millón. En función de la historia clínica y los síntomas, el médico puede sospechar HPTEC, pero es necesario realizar pruebas para distinguirla de otras formas de hipertensión pulmonar. Un diagnóstico definitivo de HPTEC se debe realizar en el hospital, que tiene un
equipo de médicos y radiólogos que se especializan en ello.
TRATAMIENTO
En pacientes con una presión media de la arteria pulmonar (PAPm) > 50mmHg, la mortalidad a los 3 años alcanza el 90 por ciento. La insuficiencia cardiaca ventricular derecha es la causa más frecuente de muerte. Sin embargo, la HPTEC es potencialmente curable, ya que la intervención quirúrgica conocida como Endartetectomía Pulmonar (PEA por sus siglas en inglés) puede resultar eficaz en aquellos pacientes considerados candidatos para el procedimiento. La PEA es adecuada para dos de tres pacientes con HPTEC y la calidad y esperanza de vida de quienes se someten a esta cirugía mejoran significativamente, por lo que puede considerarse como una cura para muchas de ellas. No obstante, es importante mencionar que entre el 20 y 40% de los pacientes no son candidatos para someterse a esta intervención quirúrgica, y depende de varios factores entre los que destacan:
Si la obstrucción se encuentra muy lejana dentro de la arteria y es imposible llegar a esta. El estado físico del paciente también es un factor importante, por lo que antes de la operación se evalúa su capacidad cardiopulmonar. Cuando la severidad hemodinámica de la enfermedad no guarda relación con el grado de obstrucción. Aunado a ello, hasta 35 por ciento de los pacientes operados presentan recurrencia de hipertensión pulmonar después de la cirugía. Actualmente la Secretaría de Salud, la FDA, en los Estados Unidos y la EMA en la Unión Europea, entre otras agencias regulatorias, aprobaron una innovadora alternativa terapéutica para los pacientes con HPTEC si la cirugía no es una opción para el paciente o cuando la enfermedad persiste a pesar de la realización de la cirugía, ofreciendo una mejoría significativa en la capacidad de ejercicio y optimizando la función cardiaca y pulmonar, permitiendo al paciente mejorar sus síntomas, y en consecuencia recuperar su independencia para realizar actividades cotidianas. HEMOPHILIA You must learn
to live with it This is a hereditary condition that causes hemorrhages and affects only men. Its treatment is multidisciplinary and although there is no cure, it can be controlled.
By José Manuel Meza According to Águeda Elizabeth Magallán Hernández, hemophilia is a disease linked to the “X” chromosome and the patients who suffer from it are male, but those who transmit it are women. The doctor from the Autonomous University of Tamaulipas (UAT) and hematologist from the National Autonomous University of Mexico (UNAM), said that this is a recessive condition, which means that a woman can have a sick or healthy child, depending on the inheritance of the gene. “It is a disease that can affect 1 to 2 percent for every 10 thousand males born in the entire world population, however, hemophilia is intentionally sought by family history or is detected by a finding, because there is also the type acqui red, or that

which is spontaneous without family background, although these latter cases are rarer”, she said. SOME MANIFESTATIONS
In the study of pathological disorders, hemophilia can be determined with several signs, which can appear early. “We see it in our patient at birth: when the umbilical cord is cut, abnormal bleeding occurs, which is not expected by the pediatrician and is when studies begins. “Or depending also at very early ages when they start walking or crawling they begin to have inflammation in their joints and there are traces of bleeding”, she said. Then the diagnosis must be made through laboratory tests that are very simple and give physicians a certainty. “You must make a good clinical record of the patient, a physical examination. Also make a basic laboratory study such as a biometrics, a time of bleeding, a clotting time, among others, that will give us certain characteristics, which determine whether the person have a hereditary hemorrhagic problem”, specified Magallán Hernández. She stated that when doctors observe in the analyzes of the platelets (which are the cells that participate in coagulation) are normal in quantity and see that the bleeding time, which is what it takes to activate that platelet is also correct, then they analyze other factors. “We also review the period of coagulation or prothrombin protein to see if they are normal, but if the partially activated thromboplastin has prolonged or lengthened, that tells us that the patient has a deficiency in the synthesis of coagulation factors. “We value 13 factors and as doctors, we know that the most frequent ones in which there is a deficiency are the eight and nine, that being deficient, can lead us to the definitive diagnosis of hemophilia A or B”, described Magallán Hernández.
IT CAN BE CONTROLLED
However, the hematologist said that the disease is not fatal if it is well diagnosed and treated correctly. To do this, it must be given a multidisciplinary follow-up. “That is why we must educate the family and the patient so that they can make a life within normalcy. We need to avoid having permanent injuries at the level of joints, which is where most suffer from bleeding. This is due to a deficiency in the production of the synthesis of the proteins that when there is a trauma or the rupture of a capillary for them represents a hemorrhage. “And the bleeding will be by pressure or by blow. Children at the time of crawling their joints are the ones that sustain it and it is a direct trauma”, she said. In addition, when the patient is already diagnosed there are three levels of activity to address hemophilia: “It will depend on the production that is being made in the organism, for which we qualify it as mild, moderate and severe. In general terms the disease causes a deficiency in the formation of the clot, which is what will stop the bleeding of that injury, either by an organ or a part where it is closed”, she illustrated. If the hemorrhage is internal, said Maga-
llán Hernández, it can affect the gastrointestinal and urinary tract; to the abdomen, to the muscles, the joints, the skull, while when it occurs in open areas, it bleeds at the level of the nose or the gums. “This means that patients with hemophilia may bleed at different levels and depending on the factor we are going to catalog them or not in danger and the latter would have to be the one that is being administered continuously that factor or only at the moment who needs it”, she said. The specialist added that the life expectancy for a person with hemophilia can be long, receiving the correct attention. “It usually reaches adulthood and patients can make their normal life. Within our participation as doctors, it is sought that they have no physical limitations. “If there is no history in the family that makes us think about the presence of these diseases, our medical colleagues, who are the ones who can have the most contact with children, such as pediatricians and gynecologists make us participate in what is the diagnosis to base it on factors”, she said. So hemophilia is a disease present in the male group to which we must pay attention since it affects an average of 200 people per 10 thousand, 20 per thousand and two per 100.

Este es un padecimiento hereditario que provoca hemorragias y afecta solamente a hombres. Su tratamiento es multidisciplinario y aunque no existe una cura, puede controlarse. Por José Manuel Meza De acuerdo con Águeda Elizabeth Magallán Hernández la hemofilia es una enfermedad ligada al cromosoma “X” y los pacientes que la padecen son del sexo masculino, pero quienes la trasmiten, son las mujeres. La doctora por la Universidad Autónoma de Tamaulipas (UAT) y hematóloga por la Universidad Nacional Autónoma de México (UNAM), señaló que éste es un padecimiento recesivo, lo cual quiere decir que una mujer puede tener un hijo enfermo o sano, dependiendo de la herencia del gen. “Es una enfermedad que puede afectar del 1 al 2 por ciento por cada 10 mil varones nacidos en toda la población mundial, sin embargo, la hemofilia es buscada intencionadamente por los antecedentes familiares o se detecta por un hallazgo, pues también existe la de tipo adquirido, o la que es espontánea sin antecedentes familiares, aunque estos últimos casos son más raros”, indicó.
el tiempo de sangrado, que es lo que tarda en activarse esa plaqueta también está correcto, analizan otros factores. “Revisamos también el periodo de coagulación o de la proteína protrombina viendo que está normal, pero el de tromboplastina parcialmente activado se ha prolongado o alargado, eso nos habla de que el paciente tiene una deficiencia en las síntesis de los factores de coagulación. “Nosotros valoramos 13 factores y como médicos sabemos que los más frecuentes en los que hay deficiencia son los ocho y nueve, que al ser deficientes, nos pueden llevar al diagnóstico definitivo de una hemofilia A o una B”, describió Magallán Hernández.
ALGUNAS MANIFESTACIONES
En el estudio de los trastornos patológicos la hemofilia se puede determinar con varios signos, los cuales pueden aparecer de manera temprana. “Lo vemos en nuestro paciente en el momento del nacimiento: cuando se hace el corte del cordón umbilical se presenta un sangrado anómalo, que no es esperado por el pediatra y es cuando comienza la batería de estudios. “O dependiendo también en edades muy tempranas al momento de empezar a caminar o gatear comienzan a tener inflamación en sus articulaciones y hay rastros de sangrado”, mencionó. Entonces se debe de llegar al diagnóstico por medio de pruebas de laboratorio que son muy sencillas y le dan a los médicos una certeza. “Se debe realizar un buen historial clínico del paciente, una exploración física. En términos generales la enfermedad provoca una deficiencia en la formación del coágulo, que es el que va a parar el sangrado de esa lesión, ya sea por un órgano o una parte donde se encuentra cerrada” ilustró. Si la hemorragia es interior, dijo Magallán Hernández, puede afectar a los aparatos gastrointestinal y urinario; al abdomen, a los músculos, las articulaciones, el cráneo, mientras que cuando se da en zonas abiertas, sangra a nivel de la nariz o por las encías. “Esto quiere decir que los pacientes con hemofilia pueden tener sangrado de distinto nivel y según del factor lo vamos a catalogar o no en peligro y éste último tendría que ser al que hay que estarle administrando de forma continua ese factor o solamente en el momento en el que lo necesita”, expresó. La especialista agregó que la expectativa de vida para una persona con hemofilia puede ser larga, recibiendo la atención correcta. “Generalmente se llega a la edad adulta y los pacientes pueden hacer su vida normal. Dentro de nuestra participación como médicos se busca que no tengan limitaciones físicas. “Si no existen antecedentes en la familia que nos haga pensar en la presencia de estas enfermedades, nuestros compañeros médicos, que son los que pueden tener mayor contacto con los niños, como los pediatras y ginecólogos nos hacen participar dentro de lo que es el diagnóstico para fundamentarlo con bases”, expresó. De modo que la hemofilia es una enfermedad presente en el grupo varonil a la que hay que poner atención, pues afecta a un promedio de 200 personas por cada 10 mil, 20 por cada mil y dos por cada 100.

Mayor Maki Ortiz’s Goverment increases Scholarship budget to more than 45 MDP. The 2018 Municipal Scholarship Program was redesigned by Mayor Maki Esther Ortiz Dominguez and the municipal cabinet, providing a record amount of over 45 million pesos, which the Mayor announced on Thursday, February 1, at the opening event for registration of applicants of the preschool-grade that lasted until Friday 2, and made a symbolic delivery of two vouchers for students of all academic levels and career completion. The Secretary of Social Development, Eduardo Bladinieres Cámara, received the attendees, who emphasized that “Dr. Maki Esther Ortiz Dominguez has presented the Honorable Cabildo de Reynosa the largest Scholarship Program of recent times and the honorable council sharing the vision that an educated town is a town with a better future has responded in solidarity by approving this beautiful project that finally is a Project for Everyone “. El Programa de Becas Municipales 2018 fue rediseñado por la alcaldesa Maki Esther Ortiz Domínguez y el gabinete municipal, aportando una cifra récord superior a los 45 millones de pesos, que la presidente municipal dio a conocer el jueves 1 de febrero en el evento de apertura para inscripción de solicitantes del grado preescolar que se prolongó al viernes 2 como únicos días, e hizo entrega simbólica de dos vales para estudiantes de todos los niveles académicos y de finalización de carrera. Recibió a los asistentes el secretario de Desarrollo Social, Eduardo Bladinieres Cámara, quien resaltó que, “la doctora Maki Esther Ortiz Domínguez, ha presentado al Honorable Cabildo de Reynosa el Programa de Becas más grande de los últimos tiempos y el cabildo, compartiendo la visión de que un pueblo educado es un pueblo con mejor futuro, ha respondido solidario aprobando este hermoso proyecto que finalmente es un Proyecto de Todos”. ‘Gallery of Mexican Castes’ Starting on February 19, the San Antonio Museum of Art, Texas, will exhibit at the temporary exhibition “San Antonio 1718: Art from Viceregal Mexico. A Tricentennial Exhibition” consisting of 19 pieces selected by the Museum of Mexican History of Monterrey, including the series of caste painting by the Novo-Spanish artist José de Páez. The 15 pieces of the artist are an important part of the “Gallery of Mexican Castes” of the Museum of Mexican History. These oil paintings also called “American curiosities” or “traditionalist paintings” illustrate the racial variety and show the cultural richness, occupations, and trades of the time; fashions, textiles, hobbies, readings, flora, fauna, private housing space, public space and aspects of the family coexistence of New Spain. San Antonio 1718: Arte del México Virreinal will be exhibited from February 17 to May 13, 2018 in San Antonio Texas, and tells the history of the first century of the city through more than a hundred landscapes, portraits, narrative paintings, sculptures and devotional and decorative objects, many of them never before exhibited in the United States. In general, the works evoke the lineage and authority of peninsular Spain, while revealing the lives and times of the first inhabitants of San Antonio. San Antonio 1718: Arte del México Virreinal se exhibirá del 17 de febrero al 13 de mayo de 2018 en San Antonio Texas, y narra la historia del primer siglo de la ciudad a través de más de un centenar de paisajes, retratos, pinturas narrativas, esculturas y objetos devocionales y decorativos, muchos de ellos nunca antes exhibidos en Estados Unidos. En general, las obras evocan el linaje y la autoridad de la España peninsular, a la vez que revelan las vidas y los tiempos de los primeros habitantes de San Antonio. Celebrando las profundas raíces hispánicas de la ciudad y los lazos culturales con México, la exposición muestra obras de los artistas más talentosos de la Nueva España del siglo XVIII, como Cristóbal de Villalpando (1649-1714), Miguel Cabrera (1695-1768) y José de Páez (1720-1790), así como piezas de talentosos artistas vernáculos desconocidos. La esclerosis es un padecimiento progresivo del sistema nervioso que provoca múltiples lesiones en las neuronas. Se manifiesta con parálisis en las extremidades y la pérdida de sensibilidad. Por José Manuel Meza La esclerosis está clasificada por la Organización Mundial de la Salud (OMS) dentro de la categoría de enfermedades no curables. Aunque no se le presta mucha atención, es importante considerar las cifras, ya que de acuerdo con estadísticas en México la padece entre el 2 y el 5 por ciento de la población. “Es una enfermedad inmunológica, la cual tiene un origen desconocido que predomina en mujeres (dos por uno), gente blanca, caucásicos, y lo que afecta fundamentalmente es la relación que hay entre los músculos y los nervios. Hay una debilidad progresiva discapacitante”, expresó Ramiro Zenón González García. El médico general por la Universidad Nacional Autónoma de México (UNAM), con especialidad en Neurocirugía en el Centro Médico Nacional de Occidente del Instituto Mexicano del Seguro Social (IMSS), detalló que se están haciendo muchas investigaciones sobre la esclerosis múltiple, aunque no se le ha encontrado solución. “Es una enfermedad desafortunadamente no curable, solamente tratable. Hay medicamentos que se han descubierto. Eso les permite a los pacientes hacer una vida más suficiente, pero aún así hay limitantes”, detalló. TAMBIÉN AFECTA A LATINOS
Aunque originalmente se le había asociado a personas anglosajonas, siendo el mayor grupo étnico afectado, otras razas como la latina también padecen en un porcentaje importante la esclerosis múltiple. “No estamos excluidos de que tengamos la afección. Lo que se ha estudiado es que la gente blanca tiene mayor afectación inmunológica que la gente que tiene mejor condición física de piel oscura. Son patrones”, agregó el doctor González García. Diversas investigaciones vinculan al desarrollo de la esclerosis con infecciones virales, parasitosis, tabaquismo, deficiencia de vitamina D, toxinas ambientales, dietéticas y gota. De acuerdo con la ciencia médica, se destruye la vaina que aisla a las fibras nerviosas causando una conducción deficiente de las órdenes cerebrales, ocasionando una descoordinación motora irreversible. “Hay varios tipos de esclerosis, pero generalmente los pacientes tienen una sobrevida mediana. Puede vivir de los 55 a los 75. Varía. “Entre los principales síntomas se encuentran la debilidad, la afectación sensitiva de brazos y piernas; las contracturas musculares, básicamente. También hay una afectación visual. Trastornos en los nervios ópticos”, dijo el neurólogo.
‘TRISTE ENFERMEDAD’
El especialista lamentó que existan personas que sufran esclerosis múltiple, ya que las personas no pueden sostener ni siquiera un vaso con agua, porque no tienen fuerza. “Se ve afectada la fuerza muscular, la sensibilidad de brazos y piernas. Ésta se pierde y la persona no puede peinarse, moverse, los lleva a postrarse en una silla de ruedas o una cama. “Se les receta interferón. Hay contriciones, medicamentos desinflamatorios. Es un tratamiento paliativo. La esclerosis múltiple tiene fases agudas, graves, crónicas, pero deseamos que siempre el paciente llegue en una etapa temprana para que sea tratado y tenga una mejor calidad de vida”, señaló. Es muy importante acudir con el neurólogo de forma oportuna, aunque es una enfermedad en la que intervienen otras especialidades, ya que también puede aparecer en edades tempranas. “Es una enfermedad que requiere un tratamiento multidisciplinario, pero fundamentalmente primero tienen que acudir con el neurólogo, porque está conectado con la cuestión cerebral.

“Son cuestiones inmunológicas, con efectos que no tienen una razón específica, pero sí se afecta y limita la vida. A common error of the parents of children with strabismus is to think that the problem will be solved on it’s own when “the nerves of the eyes mature” but if not taken care of in time can develop damage in the vision. By Beatriz Flores It is considered strabismus, any abnormal deviation of the eyes, both in children and adults and occurs most of the time, by innervation, that is, the action that the nervous system exerts on the organs of the body through the transmission of nervous impulses. “The nerves that move the eyes of children or adults must be coordinated, they must move at the same time and when there is an alteration of this movement, an eye moves more or does not move, there is that deviation we call that strabismus”, explained José Guillermo Escudero Ferrer, general ophthalmologist. The doctor who belongs to the Mexican Society of Ophthalmology and the Mexican Council of Ophthalmology mentioned that it can be presented at any stage of life, although it occurs most commonly in children. He said that if parents notice that their child has deviated eyes should go immediately to the pediatrician, who will review and channel their attention to the ophthalmologist to avoid vision problems. “The most important thing in the case of children is that in the first four or five years of life the vision is developed and if one of the eyes is deviated, that eye does not develop an adequate vision and that could cause a decrease in the visual perception of that eye”, he explained. There are little children, he added, born with strabismus and when they are tiny, the treatment is usually surgical and should be operated at an early age to correct this deviation and to develop a vision. He said that adult patients realize their problem because they have double vision, while in children the eye only deviates and that happens because one of the nerves stopped working well. No es normal, no sana solo
Un error común de los padres de niños con estrabismo es pensar que el problema se solucionará sólo cuando “maduren los nervios” pero si no se atiende a tiempo se pueden desarrollar daños en la visión. Por Beatriz Flores Es considerado estrabismo, cualquier desviación anormal de los ojos, tanto en niños como en adultos y se presenta la mayoría de las veces, por la inervación, es decir, la acción que el sistema nervioso ejerce sobre los órganos del cuerpo mediante la transmisión de impulsos nerviosos. “Los nervios que mueven los ojos de los niños o de los adultos deben de ser coordinados, se deben de mover al mismo tiempo y cuando existe una alteración de este movimiento, que un ojo se mueva más o que no se mueva, existe esa desviación y a eso le llamamos estrabismo”, explicó José Guillermo Escudero Ferrer, oftalmólogo general. El médico que pertenece a la Sociedad Mexicana de Oftalmología y al Consejo Mexicano de Oftalmología, mencionó que puede presentarse en cualquier etapa de la vida, aunque lo más común es que se presente en los niños. Dijo que si los padres notan que su niño tiene desviados los ojos deben acudir inmediatamente al pediatra, quien lo revisará y canalizará su atención al oftalmólogo para evitar problemas en la visión. “Lo más importante en el caso de los niños es que en los primeros cuatro o cinco años de vida se desarrolla la visión y si uno de los ojos está desviado hace que ese ojo no desarrolle una visión adecuada y eso pudiera causar una disminución en la percepción visual de ese ojo”, explicó. Hay pequeños, añadió, que nacen con el estrabismo y cuando son pequeñitos generalmente el tratamiento es quirúrgico y deberá operarse a temprana edad para corregir esa desviación y que desarrolle una buena visión. Otros tipos de estrabismo llamado acomodativo y generalmente se presentan después de los 2 años de edad. “Son niños que nacen con sus ojos normales y que los papás alrededor de los 2 años notan que uno de los ojos empieza a desviarse; hay que revisar al niño y la mayor parte de esos estrabismos se corrigen con lentes, les ponemos sus lentes y el ojo vuelve a su posición normal y tiene que seguir usándolos durante toda la niñez y cuando llegan a la adolescencia o a la adultez el ojo se mantiene, pero la necesidad de los lentes va a ser de por

vida”, mencionó. En el caso de los adultos, dijo, quienes presentan estas desviaciones regularmente son los pacientes diabéticos, ya que tiene alteraciones de los nervios y algunos dejan de funcionar adecuadamente y empiezan a desviarse. “Los diabéticos deben controlar su diabetes, porque no se han controlado en forma adecuada, a veces se dan complejos de vitamina B12 o B6, hay pacientes que en dos o tres semanas se corrigen y en caso de que sean estrabismos que persistan se puede pensar en algún tratamiento quirúrgico”, precisó.
Pero los pacientes que tienen traumatismos o algún problema cerebral como tumores o cualquier alteración del cerebro también pueden sufrir un estrabismo. Expresó que los pacientes adultos se dan cuenta de su problema porque presentan visión doble, mientras que en los niños el ojo sólo se desvía y eso pasa porque alguno de los nervios dejó de funcionar bien. El médico, perteneciente a la Academia Americana de Oftalmología dijo que dependiendo del tipo de estrabismo y de si es en niños o adultos será el procedimiento
quirúrgico que se realice. “Para los niños se utiliza anestesia general, pero para saber si se requiere una cirugía depende de la severidad de la desviación, sobre todo cuando son estrabismos congénitos o sea estrabismos importantes que aparecieron desde el nacimiento, entonces sí se tienen que tratar. Hemos operado a pacientes desde los 4 meses o 6 meses de edad”, precisó. One of the dangers that a newborn have is unexpected death, but there is an alarm monitor, which under medical prescription, can alert the parents if the baby stops breathing. By Pamela Escobar According to pediatrician José Gabriel Rosado Triay, the causes of this death, also known as crib death, are still unknown, but he said that it can occur during the first year of life, being during the second and fourth month of birth where is more commonly observed. “It is an unexpected death in small patients and although the causes are unknown, there are a few characteristics that have in common, for example, that are completely healthy, without any other condition”, the doctor warned. Uno de los peligros que corre un recién nacido es el fallecimiento por muerte inesperada, pero existe un monitor alarma, que bajo prescripción médica, puede alertar a los padres si el bebé deja de respirar. Por Pamela Escobar De acuerdo con el pediatra José Gabriel Rosado Triay, las causas de dicho deceso, también conocido como muerte de cuna, aún se desconocen, pero dijo que se puede presentar durante el primer año de vida, siendo durante el segundo y cuarto mes de nacido donde más se observa. “Es una muerte inesperada en pequeños pacientes y aunque las causas se desconocen, hay características, como por ejemplo, que son completamente sanos, sin ningún otro padecimiento”, alertó el médico. Ante la también conocida como muerte súbita del lactante, el pediatra agregó que es uno de los grandes enigmas que existen en la medicina y sobre todo en el área pediátrica. “No hay una explicación de qué es lo que provoca esta situación en la que el menor deja de respirar por completo y fallece por esta circunstancia. “No sabemos exactamente cuál es la causa. Se han invocado una serie de agentes que pueden hacer que estén más en riesgo, pero sin ser absolutos todavía”, explicó el
especialista en niños. Rosado Triay detalló algunos factores a tomar en cuenta: son fumar en casa, que sean bebés prematuros, exceso de ropa en el recién nacido en especifico el rostro, o bien, que el pequeño duerma en cama con los padres. Sin embargo, tampoco son alguna certeza. “En casos específicos en niños que nacieron antes de tiempo y hayan dejado de respirar durante su estancia en terapia intensiva o en los cuneros de cuidados intermedios o si sufrieron apnea del sueño; es decir, una afección en la que se suspende la función de respirar, se debe prestar más atención”, advirtió el egresado de la Universidad Nacional Autónoma de México. Agregó que en dichos casos se aconseja un monitor de alarmas, que únicamente es previsto para hogares bajo prescripción médica, ya que no está consensado para la población en general. “Este aparato detecta las pulsaciones que tiene el niño en una forma continua, el oxígeno que está aprovechando y cuando baja de ciertos límites, suenan las alarmas. Los que son para uso de casa tienen un precio en el mercado de hasta 100 dólares”, expre-

só el miembro del Colegio de Pediatría de Tamaulipas. El pediatra comentó que de acuerdo con estadísticas en los Estados Unidos, sucede aproximadamente en uno de cada 2 mil recién nacidos vivos y existe el riesgo hasta el primer año de vida del menor. “Para ayudar a disminuir esta cifras se tienen que estar observando a los bebitos, checando que la respiración se logre sin dificultad, que no haya obstrucción de las fosas nasales por moco, que su coloración esté sonrosada, que tengan una temperatura normal”, agregó el colegiado en pediatría de Reynosa. Por otro lado, el especialista comentó que si se observa que el niño deja de respirar, hay que hacer una estimulación externa por medio de masaje a nivel de las costillas y respiración de boca a boca para ayudar a recuperar la oxigenación del bebé. “De preferencia, se debe tener un adiestramiento correcto para dichos ejercicios, ya que la capacidad pulmonar del recién nacido no tiene nada que ver con la de un niño mayor o de un adulto, en cuanto a la respiración debemos lograr el levantamiento de
la parrilla costal, es decir, de las costillas o tórax, ya que es indicativo de que se está ejecutando de la manera correcta”, advirtió. El médico dijo que el haber tenido un hijo fallecido por muerte súbita, no exime de que los siguientes no puedan sufrir de lo mismo; recalcando que podría ser otro de los causales de la muerte de cuna. Sin embargo, aconsejó proveer al recién nacido de una calidad médica desde el embarazo, que la futura mamá no tenga sobrepeso, vacunas al corriente, control adecuado prenatal, es decir, que el nacimiento sea en las mejores condiciones posibles. “Una vez que nazca el bebé, tener el control de niño sano con el pediatra, las medidas básicas para tratar de disminuir los problemas y consultar al medico; no necesariamente guiarse por las medidas que vienen en Internet, porque a veces hay opiniones de gente que no es experta en el tema y genera mucha confusión”, comentó. High levels of cholesterol in the blood is not always negative because it is also essential for the body to perform various metabolic functions; however, it can put cardiovascular health at risk if it exceeds normal limits. By Pamela Escobar High levels of cholesterol in the blood are not always bad since there are two different types: known as “good”, DLH (for its acronym in English) which are high-density lipoproteins and LDL, low-density lipoproteins, called also as “bad”, explained Cardiologist Nelson Eloy Ochoa Becerra. The doctor explained that good cholesterol is a type of fat essential for the organism, which has many metabolic functions and serves as a structure that they contain in the membranes of cells. “For example, cholesterol is necessary for hormones to form and for proper function of the body. It is necessary that it exists as long as it is within normal limits, because when it goes be-

yond it, some problems may appear”, said the vice president of the Internal Medicine College of Reynosa. According to the American Heart Association, people who suffer from high cholesterol levels, especially “bad” cholesterol, are more likely to stick to the arteries and clog them in the long term. “High cholesterol increases the risk, in the long term, of suffering heart attacks; remembering that myocardial affectations are the number one cause of death worldwide, so it is very important to have cholesterol within normal ranges”, added Ochoa Becerra. The specialist explained that the clogging of the arteries specifically of the heart are the most worrisome since it makes it difficult for the blood to pass
through and that initially causes an angina of the chest. And finally, if an entire artery is covered, a heart attack occurs, because it stops passing blood and nutrients to the arteries of the heart. “It is currently recommended to perform a lipid profile. No siempre el nivel elevado de colesterol en la sangre es negativo pues también es fundamental para que el organismo realice varias funciones metabólicas; sin embargo, puede poner en riesgo la salud cardiovascular si supera los límites normales. Por Pamela Escobar Los altos niveles de colesterol en la sangre no siempre son malos ya que existen dos diferentes tipos: el conocido como “bueno”, DLH (por sus siglas en inglés) que son lipoproteínas de alta densidad y el LDL, lipoproteínas de baja densidad, denominado también como “malo”, explicó el cardiólogo Nelson Eloy Ochoa Becerra. El médico detalló que el colesterol bueno es un tipo de grasa fundamental para el organismo, que tiene muchas funciones metabólicas y sirve como estructura que contienen en las membranas de las células. “Por ejemplo, el colesterol es necesario para que se formen las hormonas y para la función adecuada del organismo. Es necesario que exista siempre y cuando esté dentro de los límites normales, ya que cuando los rebasa pueden aparecer algunos problemas”, advirtió el vicepresidente del Colegio de Medicina Interna de Reynosa. De acuerdo con la Asociación Americana del Corazón las personas que sufren de elevados niveles de colesterol, sobre todo del “malo” son más propensos a que esa grasa se pegue en las arterias y las obstruya a largo plazo. “El colesterol alto aumenta el riesgo, a largo plazo, de sufrir infartos; recordando que las afectaciones al miocardio son la causa de muerte numero uno a nivel mundial, por ello es muy importante tener el colesterol dentro de rangos normales”, añadió Ochoa Becerra. El especialista explicó que el taponamiento de las arterias específicamente del corazón, son las que más preocupan, ya que
dificulta el paso de la sangre y que de manera inicial provoca una anguina de pecho. Es muy común que algunos pacientes se practiquen análisis y vienen con una cifra que es la del colesterol total, pero a nosotros como especialistas nos sirve más el perfil, porque se desglosa el LDL y el HDL”, expresó el cardiólogo. RECOMENDACIONES
Ochoa Becerra recomendó realizar una dieta saludable, privilegiar el consumo de vegetales, frutas y granos, así como disminuir el consumo de grasas saturadas, aunado a realizar ejercicio físico, que ayuda considerablemente a la reducción de los niveles de colesterol. “Alimentos altos en grasa, los procesados, mantequilla, huevo, mayonesa, lácteos y sus derivados como el queso, son algunos de los que contienen más grasas saturadas, así como las papas fritas y la comida rápida”, mencionó el médico. El cardiólogo expresó que se debe visitar preferentemente a un especialista para que maneje mejor la elevación que se pueda presentar en los rangos del colesterol. “No a todos los pacientes se les trata igual, depende del riesgo global que tenga cada uno, es decir, se requiere de un nivel diferente. Si el enfermo además tiene algún otro padecimiento como diabetes, hipertensión o alguna enfermedad coronaria”, advirtió el entrevistado. Detalló además que existe un factor hereditario donde hay

una participación genética que participa activamente en algunos pacientes y es denominado hipercolesterolemia familiar y en esos casos, quienes lo sufren, tienen más dificultad para controlar los niveles de colesterol. “Ese sector de la población debe realizarse análisis de manera más temprana, análisis para medir el colesterol y qué tipo de colesterol es. Al igual que existen otros factores de riesgo como obesidad, antecedente familiar de alguien con infarto, diabetes, hipertensión o si superan los 40 años de edad”, agregó el doctor. Agregó que los pacientes con colesterol alto por el cargo genético, les resulta más complicado disminuirlos con dieta y ejercicio. Se debe de apoyar con un tratamiento farmacológico alterno. “A veces hay una participación del metabolismo de las grasas y aún haciendo dieta y prácticas deportivas es difícil mantener el colesterol en rangos adecuados”, explicó Ochoa Becerra. El doctor añadió que la mayoría de las personas que sufren de colesterol alto es por tener una mala alimentación, por el consumo de más comida procesada que contiene grasas trans (ácidos grasos insaturados). “Las grasas trans no sólo hacen que el colesterol malo suba, sino que el bueno baje... con ello ocurre un desbalance y aumenta la tendencia a la arterioesclerosis, es decir, que se endurezcan las arterias por pegarse el colesterol LDL en ellas” advirtió el miembro de la Sociedad de Internos y Becarios del Instituto Nacional de Cardiología Ignacio Chávez. The reaches of therapeutic colonoscopy
W
e have heard or we have become familiar with the term colonoscopy, which is equivalent to conducting a study with an endoscope, which is a flexible probe, at the tip has a lens (camera) that allows clear and real images of the interior of the lower digestive tract (colon and rectum). This revision procedure is called diagnostic colonoscopy, since through direct observation and taking biopsies (small sample of tissue) it is possible to obtain accurate information about the condition. What is important to highlight is that digestive endoscopy is not limited to giving precise diagnoses, but in case of presenting any pathology such as the ones that we will comment next, it is possible to achieve that a patient can do without hospital stays, transfusions or surgeries. Removal of polyps or tumors. When abnormal growths are located inside the digestive tract, it is possible to remove these tumors in the same endoscopic procedure, in a simple way, and without pain. Los alcances de la colonoscopia terapéutica
H
emos escuchado o nos hemos familiarizado con el término de colonoscopia, que equivale a realizar un estudio con un endoscopio, que es una sonda flexible, en la punta posee una lente (cámara) que permite obtener imágenes claras y reales del interior del tubo digestivo bajo (colon y recto). A este procedimiento de revisión se le denomina colonoscopia diagnóstica, ya que a través de la observación directa y tomar biopsias (pequeñas muestra de tejido) se logra obtener información exacta del padecimiento. Lo que es importante hacer resaltar es que la endoscopia digestiva no se limita solo a dar diagnósticos precisos, sino en caso de presentar alguna patología como las que a continuación vamos a comentar, es posible lograr que un paciente pueda prescindir de internamientos, transfusiones o cirugías. Retiro de pólipos o tumoraciones Cuando se localizan crecimientos anormales en el interior del tubo digestivo, es posible retirar esas tumoraciones en el mismo procedimiento endoscópico, de una manera sencilla y sin dolor. Los pólipos en el colon, a diferencia de pólipos del estómago, en todo momento deben de ser retirados al realizarse el estudio, ya que estos en un corto período de tiempo pueden degenerar en cáncer colorectal. El paciente, al final del procedimiento, puede reincorporarse inmediatamente a sus actividades normales. DETENER SANGRADOS
En algunas ocasiones, los pacientes se encuentran afectados tan seriamente, que empiezan a sangrar ya sea de forma activa (sangrado rojo, abundante) o un sangrado más lento (sangrado oscuro-negro) en la evacuación. Esto manifiesta un sangrando en el tubo digestivo, de acuerdo a las características es su origen. A través de un procedimiento endoscópico, es posible localizar rápidamente el sitio del sangrado, el cual puede ser causado por úlceras en diferentes sitios, alguna malformación de
venas, entre otros. En ese mismo procedimiento se pueden aplicar diferentes métodos para detener la causa de la hemorragia. Al lograr esto, el paciente logra estabilizar su condición y se disminuyen o evitan las trasfusiones de sangre así como los días de estancia hospitalaria y la posibilidad de necesitar una cirugía, logrando que el paciente regrese muy pronto a sus actividades normales. DILATACIONES
Las estrecheces o estenosis del tubo digestivo bajo se manifiesta con dificultad para evacuar, y distensión importante. Sus causas son variadas, desde una infección importante hasta por la presencia de una tumoración, como también por cirugías previas, aplicación de métodos de radio o quimioterapia. Todo lo anterior ocasiona una estrechez en algún sitio y éstas pueden ser muy variadas, longitudinales, concéntricas, etc. Lo importante es que este tipo de estenosis es posible dilatarlas o abrirlas a través de un procedimiento sencillo endoscópico y la utilización de diferentes tipos de instrumentos dilatadores, evitando cirugías y estancias hospitalarias. Al final del procedimiento es posible que el paciente regrese a sus actividades diarias. COLOCACIÓN DE PRÓTESIS
Cuando existe alguna tumoración en algún nivel del tubo digestivo bajo, es posible colocar una prótesis que permita mantener abierto o funcional el tubo digestivo. Por lo anterior resumimos que la endoscopia es una útil herramienta en la medicina actual, lo que se traduce en resolución pronta, evitar hospitalizaciones o corta estancia en el mismo, disminuir trasfusiones, evitar cirugías, y lo más importante, disminuir los costos en el tratamiento de una enfermedad. Almost 30 percent of physicians believe that patients do not follow their recommendations to address their health problems, and 55 percent indicated that there is resistance from patients to adequately follow their treatments. This was revealed by a study conducted by the Doctoralia platform, in which some patterns of behavior between health specialists and patients were analyzed. The results also showed that 42 percent of doctors believe that patients often do not follow their treatments properly due to laziness, and 37 percent said that they simply forget their doctor specific indications to fight their own ailments. The study also offers an overview of new patients. In this regard, 62.7 percent of specialists said, trust in the doctor is essential for patients to follow the recommendations and come again if they have another health problem. Other factors considered crucial by health professionals to attract patients are kindness and empathy with patients (58.3 percent), offering solutions and

alternatives (43 percent) and ensuring that the patient feels accompanied during the process (39.7 percent). “The good treatment and permanent availability with patients increase not only the probability that this patient decides to return for another consultation”, said the specialist in Nephrology, Nestor Jesús Santiago Hernández, one of the winners of the 2017 Doctoralia Awards for his high reputation on said digital platform.

lidad y empatía con los pacientes (58,3 por ciento), ofrecer soluciones y alternativas (43 por ciento) y lograr que el paciente se sienta acompañado durante el proceso (39.7 por ciento). “El buen trato y disponibilidad permanente con los pacientes eleva no sólo la probabilidad de que este paciente decida regresar por otra consulta”, afirmó el especialista en Nefrología, Nestor Jesús Santiago Hernández, uno de los ganadores de los Doctoralia Awards 2017 por su elevada reputación en dicha plataforma digital. COMPORTAMIENTO EN OTROS PAÍSES
El estudio arroja resultados en cuatro naciones clave para el mercado de la e-salud: México, Brasil, Polonia y España. En cuanto al seguimiento de las indicaciones de los especialistas, México es el país en el que más profesionales de la salud piensan que los pacientes no siguen sus indicaciones. En Brasil casi un 17 por ciento de los doctores piensa que sus pacientes no lo hacen, y, a su vez, Polonia y España marcan una tendencia similar con 16 y 21.7 por ciento, respectivamente.

Desde que era pequeñita, la directora de orquesta mexicana vivió rodeada de música. “Mis papás nos llevaban a conciertos de todo tipo, ópera, sinfónica, música popular, en las fiestas siempre había alguien tocando el piano o la guitarra, y para mí siempre fue una aspiración muy grande y un sueño que yo pudiese tener una vida en la música”, aseguró. en los lineamientos, plasmados por las instituciones civiles y los ciudadanos en el PMD 2016-2018, fortalecer a la Dirección de Tránsito y Vialidad con elementos mejor preparados y por ello este viernes 26 de enero los agentes y oficiales recibieron el Curso “Seminario de Manejo a la Defensiva”, que los actualiza en el modo de actuación ante una detención en vía pública. El director de Tránsito y Vialidad de la Secretaría de Seguridad Pública, Tránsito y Vialidad, Juan Humberto Reyna Hernández, dio a conocer que el curso promovido por la doctora Maki Ortiz para la corporación lo impartieron los agentes de la Policía Federal Saúl Rivera, Ismael Ledezma y Nora Bermúdez y “es para que ellos en las calles sepan conducir cuando hay la detención de algún vehículo por faltas administrativas, que esa es la cuestión de Tránsito; es algo que ya saben, es un recordatorio”.

en implantes Por medio del ‘All on Five’ se colocan prótesis individuales de mayor duración y eficacia. Esto ya es posible en clínicas de Nuevo Progreso, Tamaulipas.

Quienes carecen de dientes, colmillos o incluso de muelas optan por utilizar piezas postizas y en algunos casos dejar los espacios vacíos, sin embargo, esto último puede resultar contraproducente, ya que la encía tiende a desgastarse y de ese modo es más difícil reparar los problemas bucales. Existen numerosos procedimientos para mejorar la dentadura, pero no todos son igual de efectivos. De acuerdo con Aurora Ruiz Herrera, miembro de la Asociación Mexicana de Implantología Bucal (AMIB), una nueva técnica está generando beneficios a esta área de la salud y se trata del “All on Five”. “Se aplica principalmente en personas desdentadas, en las que este problema ocurre con mayor frecuencia. Es una prótesis que se coloca sobre cinco implantes en la zona anterior para poner una prótesis soportada. “La pieza queda atornillada a la dentadura y es más cómoda para los pacientes porque no es algo que se van a andar quitando o poniendo. Para la conservación de estas dentaduras el paciente debe de acudir al dentista. Nosotros nos encargamos de desatornillarlas para hacerles sus limpiezas y conservar la vida de los implantes”, explicó. TECNOLOGÍA DE PUNTA
Egresada como médico cirujano dentista por la Universidad Valle del Bravo, con especialidad en Prostodoncia Avanzada por el Instituto Profesional Odontológico Contemporáneo (IPOC) y diplomados en Implantología y Trastornos Temporomandibulares, Ruiz Herrera comentó que con los años han avanzado las técnicas para la implantación de piezas dentales. “Ahorita en la actualidad tenemos un

gran apoyo que es el ‘Cone Beam’, porque nos permite saber con mayor exactitud donde vamos a colocar los implantes por medio de una guía quirúrgica. “Se trata de un examen en 3D en el que vemos la dimensión del hueso, donde pasan los nervios, para darnos una idea y hacer una buena planeación del caso”, mencionó. Para ello, es recomendable que antes de acudir al dentista los pacientes se hagan análisis clínicos para verificar cómo están sus niveles de sangre y su tiempo de coagulación, previo a cualquier cirugía de rango mayor como lo son los implantes. “Son exámenes de sangre muy económicos que se realizan en cualquier laboratorio”, agregó. La doctora, originaria de Nuevo Progreso, Tamaulipas donde tiene su consultorio, expresó que generalmente los pacientes piensan que entre más implantes
es mayor el dolor, pero eso depende de cada organismo. “El ‘All on Five’ es lo más cómodo que se recomienda y ahorita en la actualidad uno como prostodoncista busca la mayor satisfacción para los pacientes, quienes requieren algo que ya no sea tan removible, fijo para que puedan tener un mayor control en su alimentación y digerir mejor la comida”, manifestó. CONFIANZA Y BIENESTAR
Ruiz Herrera destacó que atiende muchos pacientes de Estados Unidos y Canadá, lo cual refleja la confianza que le tienen a Nuevo Progreso. “Aparte de que los tratamientos son más económicos aquí en México. Una dentadura como ésta en la Unión Americana tiene un costo elevado, pero aquí en Nuevo Progreso habemos muy buenos especialistas en implantología, muy preparados.
“Estamos yendo a congresos dos veces al año y siempre nos actualizamos para brindarle un mejor servicio a los pacientes. Queremos dar más a conocer este trabajo de que el tratamiento funciona, pero hay que saberlo realizar”, abundó. Para que se preparen los visitantes, muchos de los cuales son Winter Texans, sugirió tomar un día para la consulta y los estudios. “Porque se necesita la revisión, realizar los exámenes de laboratorio y el 3D, para que nosotros tengamos el tiempo adecuado de planeación en la colocación de los implantes. “Posteriormente verificamos el tiempo que estará aquí el paciente. En ese periodo nosotros ya checamos si es candidato a la cirugía, que dura aproximadamente una hora a hora y media, y llevan unos puntos de sutura que en dos semanas se deben retirar”, explicó. Afirmó que si el paciente es candidato y no dispone de mucho tiempo, se opta por hacer una carga inmediata, para que al siguiente día se lleve su prótesis, o de lo contrario se programa para cuatro meses. “Cuando hacemos carga inmediata con cinco implantes sí tardamos más de tiempo, porque hay que tomar impresiones y colocar aditamentos especiales”, refirió. La dentista comentó que éste es un trabajo especial y dedicado por la realización de cosas tan minuciosas. “Todo lleva sus detalles. Hay que checar muy bien el sistema de oclusión para que los implantes tengan éxito, es decir, la mordida del paciente. Ese es el éxito de cualquier trabajo que se haga en la boca. “Y es muy satisfactorio cuando los pacientes se sienten muy bien y se muestran contentos, y es cuando uno dice que vale la pena cualquier sacrificio, desvelada, esfuerzo que realizamos, para que éstos queden satisfechos”, expresó.

By Mario Alberto Palacios With high prevalence in women over 40 years, although it is common in children, patients with hypothyroidism and hyperthyroidism can, through appropriate medical treatments and timely detection, have a high quality of life, considers Dr. María Elena Romero Ibargüengoitia. Internist at the General Hospital of Mexico and Endocrinologist at the National Institute of Medical Sciences and Nutrition Salvador Zubirán specialized in Clinical and Diabetes Research at the Joslin Clinic (Harvard Medical School), talks with DOCTORS about the way in which they can detect these conditions, and how to treat them clinically. Located in the neck, a little above the clavicle, the thyroid is a butterfly-shaped gland whose function is in addition to producing hormones called thyroid, regulate metabolism and the speed with which the heart beats, the brain and intestines function, and practically all the movements of the internal organs, the specialist explains. When the production of thyroid hormones is lower the patient has a disease called hypothyroidism, and conversely, when the segregation is excessive is called hyperthyroidism. “There are many causes in our countries that derive these diseases, mainly due to the lack of iodine deficiency. The good news is that there are more iodine supplements in the diet through salt, cereals, and dairy products, and although it is more frequent, the prevalence is decreasing”, adds Romero Ibargüengoitia. With a Master’s degree in Medical Sciences, and winner along with her research team Glaxco Funsalud National Research Award, María Elena Romero received a special mention by Harvard Medical School in relation to diabetes research projects in vulnerable population and a Prize for the Mayo Clinic for Conducting Research in the area related to in the Metabolic field.

Among the symptoms of hypothyroidism are depression, reduced heart rate, irregular heartbeat or arrhythmia. This could cause a bradycardia that in extreme cases can end with a cardiac arrest, in addition to high blood pressure and low cholesterol. Also highlights, fatigue and drowsiness, muscle and joint pain, weight gain regardless of the type of diet that is carried out, and in the precise case of women constipation or hard stools, abnormal monthly periods and fertility problems, hair loss and weak nails, swollen face, hands, and feet.
Con alta prevalencia en mujeres mayores de 40 años, aunque es frecuente la afectación en menores, los pacientes de hipotiroidismo e hipertiroidismo pueden, mediante los tratamientos médicos adecuados y la detección oportuna, tener una alta calidad de vida, considera la doctora María Elena Romero Ibargüengoitia. Médico Internista en el Hospital General de México y endocrinólogo por el Instituto Nacional de Ciencias Médicas y Nutrición Salvador Zubirán, con especialidad en Clínica y de Investigación en Diabetes realizado en la Clínica Joslin (Harvard Medical School), conversa con DOCTORS sobre la forma en que se pueden detectar estos padecimientos, y cómo tratarlos clínicamente. Ubicada en el cuello, un poco arriba de la clavícula, la tiroides es una glándula con forma de mariposa cuya función es además de producir hormonas llamadas tiroideas, regular el metabolismo y la velocidad con la que late el corazón, funciona el cerebro y los intestinos, y prácticamente todos los movimientos de los órganos internos, detalla la especialista. Cuando la producción de las hormonas tiroideas es menor el paciente presenta una enfermedad denominada hipotiroidismo, y al contrario, cuando la segregación es excesiva se llama hipertiroidismo. “Hay muchas causas en nuestros países que derivan de estas enfermedades, principalmente por la falta de yodo. La buena noticia es que ya hay más suple-
mentos de yodo en la dieta a través de la sal, de los cereales, los lácteos, y aunque es más frecuente la prevalencia va disminuyendo”, añade Romero Ibargüengoitia. Con una Maestría en Ciencias Médicas, y ganadora junto con su equipo de investigación del Premio Nacional de Investigación Glaxco Funsalud, María Elena Romero recibió una mención especial por la escuela de Medicina de Harvard en relación a proyectos de investigación en diabetes en población vulnerable y un Premio por la Clínica Mayo por realización de investigación en área relacionada al campo de la Metabólica. “Y la otra causa de estos padecimientos son las enfermedades autoinmunes, una de ellas es el síndrome de Hashimoto y otra causal es si alguien mediante cirugía le extirparon la tiroides, o por presencia de cáncer le radiaron el cuello; o bien, que el paciente recibió yodo en altas dosis”. Entre los síntomas del hipotiroidismo están la depresión, reducción del ritmo cardiaco, latido irregular o arritmia. Esto podría provocar una bradicardia que en casos extremos puede acabar con un paro cardiaco, además de alta presión arterial baja y colesterol. También destaca, fatiga y somnolencia, dolor muscular y articular, aumento de peso independientemente del tipo de dieta que se lleve a cabo, y en el caso preciso de las mujeres, estreñimiento o heces duras, periodos menstruales anormales y problemas de fertilidad, pérdida del cabello y uñas débiles, rostro, manos y pies hinchados.

Los Winter Texans siguen llegando a Nuevo Progreso, Tamaulipas, un poblado mexicano situado muy cerca del área de Mission, Texas, donde los visitantes residen durante el invierno. Algunas de sus actividades favoritas es cruzar con los vecinos del sur a comprar diversos productos como artesanías, medicinas y de paso atenderse cualquier asunto médico menor como la dentadura, cortarse el cabello, acudir a una sesión de spa y otros servicios muy baratos a comparación de Canadá y Estados Unidos, por el tipo de cambio de su moneda. Gracias a esa sinergia, el sector empresarial y médico de Nuevo Progreso ha florecido, lo cual permite atender de mejor manera a los visitantes. Muchos de los especialistas que tienen un consultorio cuentan con certificación y los Winter Texans se van contentos. Además, hay quienes pasan a los restaurantes de comida internacional que tiene este poblado mexicano, mientras que otros prefieren disfrutar de los antojos típicos gastronó-
micos que se venden por un par de dólares. De acuerdo con Ramiro, un vendedor de calzado guanajuatense, los turistas son muy asiduos a comprarse botas o llevarlas de regalo. La gran parte son anglosajones, aunque también se observan en las banquetas de Nuevo Progreso otros grupos étnicos que del mismo modo acuden a disfrutar de la gran variedad de cosas que aquí pueden encontrarse. En un kilómetro por la calle principal, la misma que conecta con el puente internacional, los visitantes encuentran de todo. Desde licores, sombreros, joyería de plata, adornos residenciales y artesanías talladas en madera, hasta los ortodoncistas, los oftalmólogos y traumatólogos, que les hablan en su idioma. El abanico de productos y servicios es variado, y con poco dinero, tanto el canadiense como el estadounidense, puede pasar un buen rato y llevar souvenirs del lado mexicano, que es precisamente en estos meses cuando atraviesa su mejor momento. Por ello, Nuevo Progreso, es y seguirá siendo un rincón pintoresco donde los americanos la pasan bien y se sienten mejor.

From the Isla de Tiburón, in front of the Coast of Hermosillo, Sonora, they bring the sculptures and handmade craft made from palo fierro to Nuevo Progreso and what is better, they are practically made for a lifetime. By José Manuel Meza Nuevo Progreso, Tam. Leonel Godínez Coatzin is a merchant who practically has been in his family’s business since he was born. It is his way of sustenance, but beyond that, he feels comfortable doing what he likes: selling handicrafts. The figures of palo fierro (ironwood), a solid wood, very heavy (hence its name), which can last for decades without being damaged, are worked by hand and are in great demand among vacationers who visit Nuevo Progreso. This wood is native to the Island of Tiburón, off the coast of Hermosillo, Sonora and only in that place of the country exists. In its elaboration, said Leonel, five artisans participate: “Some carve the pieces, others polish and shine them. Everyone has a job”, he specified. Leonel is the last link in the sale of these products, as he is responsible for polishing and putting them on sale. While his father and uncles are responsible for acquiring the wood and manufac-

Desde la Isla de Tiburón, frente a las Costas de Hermosillo, Sonora, traen las esculturas y artesanías de palo fierro a Nuevo Progreso y lo que es mejor, están prácticamente hechas para toda la vida. Por José Manuel Meza Nuevo Progreso, Tam. Leonel Godínez Coatzin es un comerciante que prácticamente toda su vida, desde que nació, ha estado en el negocio de su familia. Es su modo de sustento, pero más allá de eso, se siente cómodo haciendo lo que le gusta: vender artesanías. Las figuras de palo fierro, una madera maciza, muy pesada (de ahí su nombre), que puede durar decenas y decenas de años sin dañarse, son trabajadas a mano y tienen una gran demanda entre los vacacionistas que visitan Nuevo Progreso. Esta madera es originaria de la Isla de Tiburón, frente a las costas de Hermosillo, en Sonora y sólo en ese sitio del país existe. En su elaboración, dijo Leonel, participan cinco artesanos: “Unos tallan las piezas, otros las pulen y sacan brillo. Cada quien tiene un trabajo”, especificó. Leonel es el último eslabón en la venta de estos productos, pues se encarga de pulirlos y ponerlos a la venta. En tanto que su padre y tíos se encargan de adquirir la madera y fabricarlos en el es-
tado de Sonora. Después se los mandan hasta Tamaulipas, en la frontera con Texas, para que los Winter Texans y los visitantes en general puedan adquirirlos. DESDE 5 DÓLARES
El tiempo de fabricación varía dependiendo el tipo de talla que se esté realizando, pero en promedio, gracias a la práctica la figura de un caballo, por ejemplo, con todo y acabado puede quedar en un par de días. “Aquí vendemos búhos, caballos de mar, gallos, águilas, pelícanos, rinocerontes, osos, venados, alces, lobos, patos, delfines; dominós, alhajeros, carretas y hasta balones de futbol, de todo un poco”, comentó Leonel. El comerciante afirmó que en general los turistas compran más los dominós, los búhos y las águilas, aunque existen otros que también hacen pedidos especiales. Los precios de las artesanías de palo fierro van desde los 5 hasta los 100 dólares. “Lo más importante es que este arte es hecho a mano y aparte nuestros productos vienen desde muy lejos, a la mano de los Winter Texans”, expresó.

Estela is from Argentina but has been in the United States for many years. She is retired and decided to vacation in the south of the country to avoid the cold days that are currently experienced in the state of Iowa. For several years she has done the same, so she knows the region very well and usually comes to the Mexican town of Nuevo Progreso, to spend a pleasant time and buy some products. In Texas, she met several friends like Rose and David. The three of them cross the border together. Estela es de Argentina, pero lleva ya muchos años radicando en la Unión Americana. Es retirada y decidió vacacionar en el sur del país para evitar los días fríos que en estos momentos se experimentan en el estado de Iowa. Desde hace varios años hace lo mismo, por lo que conoce muy bien la región y suele venir al poblado mexicano de Nuevo Progreso, para pasar un rato agradable y comprar algunos productos. En Texas conoció varios amigos como Rose y David. Los tres se hacen compañía y cruzan la frontera juntos. Estela habla español, así que para ellos es más sencillo tener una amistad bilingüe con quien ir de compras. “Lo mejor que hay aquí son las ventas y las margaritas. Por supuesto que sí puedo recomendar que los turistas vengan a Nuevo Progreso. Ya tengo varias veces viniendo y me siento muy bien”, comentó.
Rose, quien es originaria de Wisconsin, dijo sentirse contenta, porque ha encontrado aquí muchos estadounidenses paseando por las calles, que si bien no se compara a la modernidad a la que están acostumbrados, tiene muchos productos y servicios de muy buena calidad. Dijo que aquí los medicamentos son más baratos que en su país. También observó que sus compatriotas visitan mucho los consultorios dentales y las ópticas. Eso le ha hecho sentirse con mayor confianza. En tanto que David, quien viene de Delaware, expresó su satisfacción de poder disfrutar unos días de vacaciones en México. En Nuevo Progreso comió y tomó algunas bebidas. Su dinero aquí vale mucho y eso, confesó, es más agradable. Además mencionó que ha podido sentir un cálido ambiente en este sitio y también consideró volver pronto. Los tres son trabajadores retirados de Estados Unidos que pasan días amenos conociendo otras culturas y realizando algunas compras.
