The enigma of the President’s Covid EL ENIGMA DEL COVID AL PRESIDENTE PANDEMIC

Although he said he had mild symptoms, President Andres Manuel Lopez Obrador announced via Twitter that he contracted Covid-19. On January 25, the memes were hard on him by reminding him of some of his facts, such as saying that having a religious picture or having a four-leaf clover would protect him, because “not lying, not stealing, not betraying helps to not have Covid”. Since the start of the pandemic in March of last year, Lopez Obrador refused to wear a mask, which the WHO itself has said is essential to avoid the spread of the coronavirus. El 25 de enero los memes fueron duros con él al recordarle algunos de sus hechos como decir que tener una estampita religiosa o tener un trébol de cuatro hojas lo iban a proteger, porque “no mentir, no robar, no traicionar ayuda a no tener Covid”. Desde el inicio de la pandemia en marzo del año pasado, López Obrador se negó a usar cubrebocas, que la misma OMS ha dicho que es fundamental para evitar el contagio del coronavirus. Durante casi una semana el jefe de la Nación, de 67 años y quien tuvo un infarto hace 7 años, no salió a dar declaraciones, luego de tener acostumbrada a la población a participar en ruedas de prensa diarias. Fue hasta el 4 de febrero que dijo “estoy bien de salud”, sin explicar cuál era su padecimiento. Lo cierto es que dijo que se realizó la prueba de antígeno y que el resultado fue negativo. Se supo que el presidente de la república estuvo siendo monitoreado permanente por un equipo de neumólogos, infectólogos e inmunólogos por si requería otro tipo de tratamiento. Cuando se enfermó Donald Trump, se reveló que tomó un coctel de medicamentos para su cura que incluía vitamina D, zinc, melatonina para proteger el corazón y mejorar el sueño, farmotidina para la acidez estomacal, además de tomar Regeneron, Remdesivir y dexametsona para tratar la inflamación, pero en el caso de AMLO, no se ha dado a conocer ninguna información. Señaló ser voluntario en tratamiento anti-Covid, pero no especificó en qué consistió. Fue parte de un estudio clínico que realizan investigadores del Instituto Nacional de Ciencias Médicas y Nutrición Salvador Zubirán. “Tuvo síntomas Los dos primeros días y fueron síntomas mínimos, febrícula temperatura mayor a 37 grados y no mayor a 38 grados, y ha tenido dolor de cabeza un par de días, el resto ha sido muy tranquilo”, expresó Hugo López-Gatell, subsecretario de Prevención y Promoción de la Salud. El médico pidió “respetar su privacidad” como principio de ética médica, pero lo pone como asintomático y en recuperación. “Desde luego por indicación del doctor Jorge Alcocer y el grupo de médicos que lo atiende ha permanecido en movilidad reducida, tanto por estar en cuidado de él mismo y por proteger a quienes le circundan, casi no interactuado con nadie aquí en el Palacio Nacional. Se ha mantenido en la sección que corresponde a su residencia oficial”, dijo. La opinión pública esperaba un “golpe de timón” del mandatario en la campaña contra el Covid-19, pues su ejemplo influye a millones de mexicanos. Liver cancer: learn how to avoid it CÁNCER DE HÍGADO: APRENDA A EVITARLO By | Por Redacción | Cd. de México.

Mexico ranks second in mortality from liver cancer worldwide with more than 5,000 deaths per year, so it is essential to standardize its care in the country’s public health systems, to strengthen from prevention and early identification, to access to specific treatments for all patients. In the framework of the International Month of the Fight against Liver Cancer, Mayra Galindo, general director of the Mexican Association for the Fight against Cancer AC, pointed out that this is a disease that has not been given priority and its attention continues to be a challenge for the public health system. It is essential to ensure a healthy liver through a healthy diet, moderate alcohol consumption, take care of overweight and obesity, prevent viral hepatitis by avoiding unprotected sexual contact. In addition to avoiding injection drug use, application of tattoos or piercings, blood transfusions with unsafe blood, and in the case of Hepatitis B through vaccination, as well as annual medical supervision, especially in people who have suffered from liver fatty tissue, fibrosis, cirrhosis, and hepatitis B or C, he emphasized.<

México ocupa el segundo lugar en mortalidad por cáncer de hígado a nivel mundial con más de 5 mil decesos anuales, por lo que es fundamental homologar su atención en los sistemas de salud pública del país, para fortalecer desde la prevención e identificación temprana, hasta el acceso a tratamientos específicos para todos los pacientes. En el marco del mes Internacional de la Lucha contra el Cáncer de Hígado, Mayra Galindo, directora general de la Asociación Mexicana de Lucha contra el Cáncer A.C., señaló que esta es una enfermedad que no se le ha dado prioridad y su atención sigue siendo un reto para el sistema de salud pública. De acuerdo con la Secretaría de Salud, se estima que en México cada año 8 mil personas son diagnosticadas con cáncer de hígado, el cual tiene un alto nivel de mortalidad de entre el 80 y 85 por ciento, y se prevé un incremento en el número de casos debido a que suele identificarse en etapas muy avanzadas, aseveró la doctora Margarita Dehesa, médica internista con subespecialidad en hepatología y miembro de la Fundación Mexicana para la Salud Hepática A.C. (Fundhepa). Destacó que el cáncer hepatocelular es la forma más común de cáncer de hígado en adultos y dado que es una enfermedad que avanza rápidamente, su supervivencia es de uno a tres años. Por ello, es muy importante identificar los factores de riesgo asociados como el hígado graso, la hepatitis B y C, los diferentes tipos de cirrosis, así como una dieta alta en lípidos y carbohidratos, enfermedades metabólicas hereditarias y hepatitis tóxicas por fármacos o hierbas. Es fundamental procurar un hígado saludable a través de una sana alimentación, consumo moderado de alcohol, cuidar el sobrepeso y obesidad, prevenir hepatitis virales evitando contacto sexual sin protección. Además de evitar consumo de drogas inyectables, aplicación de tatuajes o perforaciones, transfusiones sanguíneas con sangre no segura, y en el caso de la Hepatitis B a través de la vacunación, así como la supervisión médica anual, sobre todo en personas que hayan padecido hígado graso, fibrosis, cirrosis y hepatitis B o C, enfatizó.< Febrero 2021 | FRONTERA DOCTORS

Brain aneurysm: a neurological emergency LA ANEURISMA CEREBRAL: UNA URGENCIA NEUROLÓGICA By | Por Dr. Erick García | Matamoros, Tam.-

A cerebral aneurysm is an abnormal dilation in the wall of the blood vessels generating a sac with blood inside. Their walls are often very thin, and the pressure of the naturally flowing blood can break them and cause a hemorrhage commonly known as a “stroke.” This is called a subarachnoid hemorrhage and it is lifethreatening for the sufferer. A cerebral hemorrhage is therefore a neurological emergency. Sometimes, if it is due to an aneurysm, it is primarily worthy of surgical management. Brain aneurysms do not usually give any symptoms until they rupture spontaneously and cause bleeding. Headache is rarely associated with a healthy patient. Once the aneurysm ruptures, it tends to generate an extremely severe headache that sets in like lightning, due to its severity and suddenness, it is referred to in Anglo-Saxon medical literature as “thunderclap headache”. Aneurysms are usually unstable, that is, once they bleed there is a very significant risk of rebleeding, which unfortunately most of the time is fatal. Once the cerebral bleeding has occurred, it is strictly necessary to demonstrate the aneurysm, since this will support us for its correct. Once the vascular lesion has been identified, it should be operated as a priority. Surgical manipulation must be with strict delicacy to obtain good results. There are two methods to treat them, both are effective. First is an open surgery, craniotomy and clipping. In this method, the skull is opened and microsurgical manipulation and visualization of the aneurysm is performed, and then a vascular clip (made of titanium and cobalt alloy) is placed that isolates the aneurysm from the circulation, therefore the risk of bleeding is partially null. Another method is endovascular surgery: this method is usually very attractive since there is no incision in the head, therefore it does not leave any scar. In the city, the method of choice is vascular surgery because it has the necessary equipment (surgical microscope, micro dissectors, etc.) and a server has the experience for its correct treatment.<

Un aneurisma cerebral es una dilatación anormal en la pared de los vasos sanguinos generando un saco con sangre en su interior. Sus paredes suelen ser muy delgadas y la presión de la sangre que naturalmente está fluyendo puede romperlos y producir una hemorragia conocida comúnmente como “derrame cerebral”. A esto se denomina una hemorragia subaracnoidea y pone en peligro la vida de quien la padece. Una hemorragia cerebral es por la tanto una urgencia neurológica. En ocasiones, si es debida a un aneurisma es meritorio prioritariamente de manejo quirúrgico. Los aneurismas cerebrales no suelen dar síntoma alguno hasta que se rompe espontáneamente y genera el sangrado. Raramente se asocian a cefalea en un paciente aparentemente sano. Una vez que se rompe el aneurisma suele generar dolor de cabeza sumamente severo que se instaura como de rayo, por la severidad y lo súbito se le denomina en literatura médica anglosajona como “thunderclap headache”. Los aneurismas suelen ser inestables, es decir, una vez que sangran existe riesgo muy importante de re sangrado que la mayoría de las veces es fatal desafortunadamente. Una vez ocurrido el sangrado cerebral es estrictamente necesario demostrar el aneurisma, ya que ello nos apoyará para su correcto tratamiento. Se realiza un estudio de neuroimagen vascular llamado angiografía cerebral, en que se pinta el árbol vascular cerebral y podemos observar la dilatación aneurismática. Una vez identificada la lesión vascular se debe de operar prioritariamente. La manipulación quirúrgica debe ser con estricta delicadeza para obtener buenos resultados. Existen dos métodos para tratarlos, ambos son efectivos. Primero es una cirugía abierta, craneotomia y clipaje. En este método se abre el cráneo y se realiza manipulación y visualización microquirgica del aneurisma y posteriormente se coloca un clip vascular (hechos de aleación de titanio y cobalto) que aísla al aneurisma de la circulación por lo tanto el riesgo de sagrado es parciamente nulo. Otro método es la cirugía endovascular: este método suele ser muy atractivo puesto que no hay incisión en la cabeza por lo tanto no deja cicatriz alguna. En la ciudad el método de elección es la cirugía vascular debido a que se cuenta con el equipo necesario (microscopio quirúrgico, micro disectores, etc.) y un servidor tiene la experiencia para su correcto tratamiento.< Febrero 2021 | FRONTERA DOCTORS

Systemic Inflammatory Syndrome (MIS-C) is a condition that involves several signs and symptoms, has been shown to affect children more, and has lately been linked to Covid-19. The MIS-C consists of the inflammation of several organs or tissues and that are of vital importance such as the heart, kidneys, digestive system, lungs, brain, skin, and blood vessels. The signs and symptoms depend on the inflamed organs. It appears to appear weeks after suffering from Covid-19 and rarely during infection. With the coronavirus, the body enters an inflammatory state so therefore MIS-C can develop either in children or in adults MIS-A. Some of the symptoms of this syndrome in children are fever lasting several days, chapped and dry red lips, red eyes, skin rashes, swelling in the hands and feet. Of the most important is thrombosis. Despite being a serious disease, statistically, it does not affect all children with Covid-19 and those who have suffered manage to recover by treating it on time. In terms of prevention, sanitary measures such as hand washing, healthy distance, and the use of face masks should be followed.<

El Síndrome Inflamatorio Sistémico (MIS-C) es una afección que involucra a varios signos y síntomas, se ha demostrado que afecta más a los niños y últimamente se ha relacionado con el Covid-19. El MIS-C consiste en la inflamación de varios órganos o tejidos y que son de vital importancia como: corazón, riñones, sistema digestivo, pulmones, cerebro, piel y vasos sanguíneos. Los signos y síntomas dependen de los órganos inflamados. Al parecer aparece semanas después de padecer Covid-19 y rara vez durante la infección. Con el coronavirus el cuerpo entra en un estado inflamatorio entonces por lo tanto se puede desarrollar el MIS-C ya sea en niños o en adultos MIS-A. Algunos de los síntomas de este síndrome en niños son: fiebre de varios días de duración, labios rojos agrietados y secos, ojos rojos, erupciones en la piel, hinchazón en manos y pie. De los más importantes es la trombosis. A pesar de ser una enfermedad grave, estadísticamente no afecta a todos los niños con Covid-19 y quienes lo han padecido logran recuperarse tratándose a tiempo. En cuestión de prevención se deben de seguir las medidas sanitarias como lavado de manos, la sana distancia y el uso de cubrebocas.<

Durante el año 2020 se contabilizó un total de 152 casos confirmados y 42 fallecimientos, en el 2019, se detectaron 135 pacientes y más de 50 fallecidos. “El 2020 concluyó con mil 050 pacientes que reciben tratamientos retro vitales en las instalaciones del Centro Ambulatorio para Prevención y Atención en SIDA e Infecciones de Transmisión Sexual”, enfatizó Díaz García. Miguel Ángel Trejo Aguileta, médico tratante del Capasits en Reynosa, mencionó que el año pasado concluyeron con la estadística señalada. “Siguen las mujeres infectándose con el virus del VIH-SIDA, las amas de casa y jóvenes, que se contagian porque no tienen una pareja habitual o un sinfín de novios, al momento de relacionarse sexualmente, no cuentan con las medidas preventivas”. Para diagnosticar el sida, en el centro médico se aplican 15 pruebas diarias, donde los primeros 15 días de enero del 2021, hubo un saldo de 3 resultados positivos del padecimiento y en edad reproductiva: dos mujeres y un varón.<

The truth is that the correct use of the mask reduces the risk of contagion of Covid-19, so it is important to know how to use it correctly. The World Health Organization (WHO) points out that the Severe Acute Respiratory Syndrome virus (SARS-CoV-2) is transmissible by droplets of saliva and smaller particles that are expelled when breathing, coughing, sneezing, talking, and even sing. Dr. Jose Luis Pinacho Velazquez, an emergency pediatrician, explained how to make good use of the mask to reduce the risk of contagion from the coronavirus. “When putting on the mask, the following hygienic order should be followed: wash your hands with soap and water before putting it on”, said the former president of the Mexican Association of Pediatrics. El doctor José Luis Pinacho Velázquez, pediatra urgenciólogo, explicó cómo hacer un buen uso del cubrebocas para disminuir el riesgo de contagio del coronavirus. “Al ponerse el cubrebocas se debe seguir el siguiente orden higiénico: lávate las manos con agua y jabón antes de colocarlo”, dijo el expresidente de la Asociación Mexicana de Pediatría. Primero hay que extraerlo de su empaque o bolsa plástica y revisar cuál es el lado correcto de su colocación. Después hay que cubrir completamente nariz y boca y ajustarlo al puente nasal para minimizar cualquier espacio entre el rostro y la mascarilla. Uno de los puntos importantes, mencionó el médico, es que si el cubrebocas está húmedo hay que desecharlo y cambiarlo por uno nuevo. “No es para colocarlo debajo de la nariz, en la barbilla o el cuello, mucho menos de diadema”, dijo. Si es de tela, se debe guardar en una bolsa de plástico, para después lavarlo muy bien con jabón y agua caliente (al menos 60 grados centígrados) de acuerdo a las recomendaciones del doctor Pinacho Velázquez. También es necesario cumplir con el resto de las medidas sanitarias de prevención: Lavado frecuente de las manos con agua y jabón o gel antibacterial, mantener distancia física de al menos un metro y medio, evitar tocarse la cara, ojos y nariz. Además de productos de la canasta básica, artículos de limpieza y aparatos funcionales como sillas de ruedas, andadores, muletas y/o bastones. En representación del Sistema DIF Reynosa, el presidente del Patronato, la directora, María del Refugio Anzaldúa y personal del DIF, visitan a las familias con el objetivo de llevarles hasta sus hogares diversos beneficios y apoyos. El Sistema DIF Reynosa, está sumando esfuerzos con el Ayuntamiento y Protección Civil y Bomberos (PCyB), para atender a las personas que se encuentran albergadas por las bajas temperaturas en las instalaciones del Polideportivo. Por ello se activó el albergue en el Polideportivo; personal de las diferentes áreas del DIF Reynosa han llevado en forma permanente almuerzo, comida y cena. El personal del área de salud realiza revisiones médicas y, en aquellos casos que se requiere, se les proporciona el medicamento. También se les beneficia con la entrega de cobijas y pañales, entre otros apoyos a los albergados. Personal del Sistema DIF Reynosa permanece atento a las necesidades de las familias que se encuentran en el albergue, así como las que habitan en sectores vulnerables de la ciudad.<

Emotional anxiety also generates stress pictures during a dental visit and alters the patient’s behavior and even leads to the cancellation of appointments. Fear can affect the well-being of the person, especially due to the deterioration caused in oral health, it brings difficulties in chewing, recurrent infections, pain, and digestive problems. Sleep disturbances can also occur, due to discomfort and episodes of pain. By avoiding going to the dentist, they lengthen the care process to have good oral health and can cause tooth loss.<

Con frecuencia se ha demostrado que el miedo a la odontología es un aspecto que contribuye al descuido dental. No asistir al dentista regularmente puede provocar la evolución a un problema bucal simple, que requerirá después un tratamiento especializado. Este tipo de emoción afecta la calidad de vida e impacta el bienestar psicológico, fisiológico y social del paciente. La ansiedad emocional genera también cuadros de estrés durante una consulta odontológica y altera el comportamiento del paciente e incluso lleva a la cancelación de citas. El miedo puede afectar el bienestar de la persona, en especial por el deterioro causado en la salud oral, trae dificultades en la masticación, infecciones recurrentes, dolores y problemas digestivos. También se pueden producir alteraciones del sueño, por el malestar y episodios de dolor. Al evitar acudir al odontólogo se causa alargar el proceso de atención para tener una buena salud bucal y puede causar pérdidas dentales.<

La alcaldesa maki Ortiz Domíguez recorrió las colonias de mayor vulnerabilidad al frente del Operativo Carrusel, para entregar cobijas, despensas, cubrebocas, gel antibacterial, bebidas calientes y pan, a las familias, a quienes también ofreció el albergue temporal instalado en el Polideportivo Reynosa. “Feliz Día del Amor y la Amistad, así es como queremos festejarlo, ayudando a todos nuestros semejantes, no los vamos a dejar solos”, dijo la presidenta municipal en su reunión con los habitantes del sector de las vías del ferrocarril, para luego continuar con el Operativo. “Estamos buscando gente que se encuentre en situación vulnerable para ofrecerles ir al albergue, o acepten cobijas, alimentos calientes, atención médica, despensas y con lo que podamos ayudar; estamos aquí porque no los vamos a dejar solos”, recalcó, a nombre del gobierno municipal. El Operativo, que dio inicio el pasado fin de semana, busca proteger a las personas que no tienen hogar y a quienes su vivienda no les da suficiente seguridad para cubrirse de las bajas temperaturas en la región. La caravana municipal que busca resguardar la salud y la vida de quienes menos tienen, estuvo integrada por funcionarios del Ayuntamiento, miembros del Cabildo, elementos de Tránsito y Protección Civil, médicos del DIF y de la Dirección de Salud y Grupos Vulnerables. “Cualquier enfermo por favor reporten de inmediato al DIF, en el cubrebocas está el número telefónico; si se enferma alguien nos hablan; no están solos”, dijo la alcaldesa a las familias en Praderas de Oriente, el 14 de febrero. Protección Civil recibe reportes y peticiones de auxilio por medio de los teléfonos 899-955-0010, 899-955-0011 y por el número de emergencias 911.<
