I

n recent years, gastrointestinal endoscopy has presented an important technological development and has gone from being an effective diagnostic tool to become a tool with which we perform advanced minimally invasive treatments, with which we are replacing surgical procedures that require long stays. intra-hospital, as well as high consumption of beds per patient days, blood transfusions and prolonged medical treatments, managing to perform procedures by approach by mouth or anus, short hospital stay or ambulatory.

For several years now, at the Endoscopic Gastroenterology and Surgery Center, in Reynosa Tamaulipas, we performed endoscopic procedures with advanced technology equipment, such as electronic chromoendoscopy and magnification, with which we managed to identify and characterize incipient lesions, increasing the size of the images up to 130 times. We have the ability to diagnose pre malignant lesions such as dysplasias of low and high grade and incipient malignant lesions such as adenocarcinoma in situ in the esophagus, stomach and colon, among others.

The above opens the opportunity to perform what is now known as Endoscopy of the Third Space. This consists of performing surgical procedures by transoral or rectal endoscopic route, as the case may be, such as, for example, the resection of localized benign or malignant tumors, which encompass the full thickness of the digestive wall, intentionally perforating the wall to resect completely the lesion and subsequently close the defect with different endoscopic attachments such as hemoclips or endoscopic suture. In this way, we avoid open or laparoscopic surgery, especially in those patients with high surgical risk, elderly, with added pathologies, or those who prefer this procedure.

We have technological resources such as high definition endoscopes, with electronic chromoendoscopy and magnification, Argon Plasma, ERBE Vio300D and ERBE JET of last generation, which is an intelligent electro surgery system designed specifically for advanced endoscopy, we also perform all procedures with CO2 (Carbon dioxide) thus achieving safe and painless procedures.

Personally, I have more than 20 years of experience. The last 6 years, I have trained and developed in the diagnosis of premalignant or incipient lesions of the digestive system. I have received training from Japanese experts, attending international courses. I have been a student of Dr. Michel Kahaleh, international professor, researcher and creator of advanced techniques in endoscopy, Chief of Endoscopy at the Robert Wood Johnson Hospital, in New Jersey, with whom I have performed more than 100 endoscopic surgery procedures in the third space. This has allowed me to be a pioneer in Mexico, Latin America, South Texas and I am currently an international professor in the training of these innovative techniques of advanced therapeutic endoscopy.

During the last three years we have carried out a practical theoretical course of Advanced Therapeutic Endoscopy where we trained specialists in endoscopy of our country and Latin American countries in the development of these techniques turning Reynosa into an International Training Center for specialists in Endoscopy of the Third Space . We have replicated this course on two occasions in the Republic of Argentina, where I have been a professor and main organizer along with my team, with a total crowd, this theoretical and practical course being the best in its kind in that country.

At the Endoscopic Gastroenterology and Surgery Center, we have innovative technology and the highly trained and certified human resources to perform the world-class advanced therapeutic endoscopy demanded by society today.

n los últimos años, la endoscopia gastrointestinal ha presentado un importante desarrollo tecnológico y ha pasado de ser una herramienta de diagnóstico eficaz para convertirse en una herramienta con la que realizamos tratamientos avanzados de invasión mínima, con los que estamos desplazando procedimientos quirúrgicos que requieren largas estancias intra hospitalarias, así como altos consumos de camas días por paciente, transfusiones sanguíneas y tratamientos médicos prolongados, logrando realizar procedimientos mediante abordaje por la boca o por el ano, de corta estancia hospitalaria o ambulatorios.

Desde hace ya varios años, en el Centro de Cirugía y Gastroenterología Endoscópica, en Reynosa Tamaulipas, realizamos procedimientos endoscópicos con equipos de avanzada tecnología, por ejemplo la cromoendoscopia electrónica y magnificación, con la que logramos identificar y caracterizar lesiones incipientes, incrementando el tamaño de las imágenes hasta 130 veces. Tenemos la capacidad de realizar diagnósticos de lesiones pre malignas como son displasias de bajo y alto grado y lesiones malignas incipientes como adenocarcinoma in situ en esófago, estómago y colon, entre otras.

Lo anterior abre la oportunidad de realizar lo que ahora se conoce como Endoscopia del Tercer Espacio. Esta consiste en realizar procedimientos quirúrgicos por vía endoscópica transoral o rectal, según sea el caso, como son por ejemplo, la resección de tumores benignos o malignos localizados, que abarcan el espesor completo de la pared digestiva, perforando de manera intencionada la pared para resecar completamente la lesión y posteriormente cerrar el defecto con diferentes aditamentos endoscópicos como son hemoclips o la sutura endoscópica.

De esta manera, evitamos la cirugía abierta o laparoscópica, sobre todo en aquellos pacientes con alto riesgo quirúrgico, de edad avanzada, con patologías agregadas, o aquel que prefiere este procedimiento.

Contamos con recursos tecnológicos como son endoscopios de alta definición, con cromoendoscopia electrónica y magnificación, Argón Plasma, ERBE Vio300D y ERBE JET de última generación, que es un sistema de electro cirugía inteligente diseñado específicamente para endoscopia avanzada, además realizamos todos los procedimientos con CO2 (Dióxido de Carbono) logrando así, procedimientos seguros e indoloros.

Personalmente, cuento con más de 20 años de experiencia. Los últimos 6 años, me he capacitado y desarrollado en el diagnóstico de lesiones premalignas o incipientes del sistema digestivo. He recibido capacitación de expertos japoneses, asistiendo a cursos internacionales.

He sido alumno del Dr. Michel Kahaleh, profesor internacional, investigador y creador de técnicas avanzadas en endoscopia, jefe de Endoscopia del Hospital Robert Wood Johnson, en New Jersey, con quien he realizado más de 100 procedimientos de cirugía endoscópica del tercer espacio. Esto me ha permitido ser pionero en México, Latinoamérica, sur de Texas y actualmente soy profesor internacional en la capacitación de éstas técnicas innovadoras de endoscopia terapéutica avanzada.

Durante los últimos tres años hemos realizado un curso teórico práctico de Endoscopia Terapéutica Avanzada donde capacitamos a especialistas en endoscopia de nuestro país y países de latinoamérica en el desarrollo de éstas técnicas convirtiendo a Reynosa en un Centro Internacional de Adiestramiento para especialistas en Endoscopia del Tercer Espacio. Este curso lo hemos replicado en dos ocasiones en la República de Argentina, donde he sido profesor y organizador principal junto con mi equipo, con un lleno total, siendo este curso teórico práctico el mejor en su tipo en ese país.

A maximum quality of life and pain relief are the goals of palliative medicine, which is offered to patients who suffer from a life-threatening disease.

Priscila Aquino Choy, specialist in Palliative and Pain Medicine, said that the most common disease in which it is indicated is cancer, but it is also focused on patients who have chronic kidney disease, dementia, Alzheimer’s, Parkinson’s, multiple sclerosis or cerebral paralysis.

“Its main objective is the quality of life of the patient and his family, giving him comfort measures, especially when the oncologists say that the treatment has been completed, because what follows afterwards, when there is no cure, is very difficult,” she said.

Palliative medicine, said Aquino Choy, has four fundamental aspects that are: physical, psychological, social and spiritual.

“Sometimes they only focus on the physical, on healing, but we see the whole environment of the patients to give them the best attention and that makes us more human,” she said.

The spiritual side is not approached based on a religion, but from the faith in a supreme being to achieve an inner peace and set goals or objectives in life.

“We talk with the patient to see if he is afraid, resentful or angry because having cancer is something very strong and we know that it is related to death and pain, that is why we are healing those wounds,” she explained.

The treatments for the physical area are individualized and depend on the type of cancer of each patient; to control pain they use opioids and the best known is morphine, although there are others as well.

The psychological aspect is extremely important during a disease of this type, as it is common to experience depression, anxiety and sleep disorders.

“It is necessary to take a separate psychological therapy, even sometimes you have to medicate, because it is difficult to handle the situation,” said the specialist.

Due to this type of diseases, the roles in the patient’s family change and this is where the social item must be supported.

“Sometimes the one who gets sick is the father and can no longer work or is the child and leaves school, the

dynamics are changing, work is done on how the individual moves in society and how much impact he had for his style of life,” she said.

THE FAMILY AND THE CAREGIVER

In palliative medicine, the health of the family and the patient’s caregiver are very important, so they pay special attention to their care.

“They are pillars, caregivers sometimes do not know anything about medicine and learn, sometimes the caregiver is a direct relative so it is a fundamental part,” she said.

She added that they also provide training to the caregiver, since it is a difficult task that not only entails physical, but also emotional, wear and tear, since he remains in the task 24 hours a day, seven days a week.

“We also talk to the family so that it is not one caregiver but several and play roles, we know that they are wearing out and we suggest nursing support or someone who can support taking care of them,” she said.

Sometimes his dedication is so great that he forgets his own health and often suffers from diseases such as diabetes, hypertension, anxiety, among others.

“He should not neglect to take care of his family member, he should take care of himself and that is why we give him support measures so that he can do it,” said the specialist.

Finally Dr. Priscila Aquino Choy said that despite the idea that we have about cancer, thanks to science and medicine have emerged various treatments to relieve pain and suffering of patients.

“We are here to help them deal with cancer and all the related symptoms,” she said.

La medicina paliativa ayuda a curar el dolor físico del paciente con cáncer, pero también aborda los aspectos psicológicos, sociales y espirituales.

Por Beatriz Flores Una máxima calidad de vida y alivio del dolor son los objetivos de la medicina paliativa, que se brinda a los pacientes que padecen una enfermedad que amenaza su vida.

Priscila Aquino Choy, especialista en Medicina Paliativa y del Dolor, dijo que la enfermedad más común en la que está indicada es el cáncer, pero también está enfocada en pacientes que tienen una enfermedad renal crónica, demencia, Alzheimer, Parkinson, esclerosis múltiple o parálisis cerebral.

“Su objetivo principal es la calidad de vida del paciente y de su familia, darle medidas de confort, sobre todo cuando los oncólogos dicen que ya se finalizó el tratamiento, porque lo que sigue después, cuando ya no hay cura, es un paso muy difícil”, expresó.

Síntomas como náuseas, vómito, estre-

ñimiento, diarrea, convulsiones o delirios, son solamente algunos en los que se le brinda acompañamiento al paciente.

Comentó que aunque la medicina paliativa está indicada durante la etapa terminal no debería de ser así, sino desde que el paciente es diagnosticado, ya que las noticias de este tipo son impactantes y marcan la vida.

Hablando de cáncer, si se supera, los tratamientos de medicina paliativa deberían continuar, ya que durante el período de remisión el paciente debe estar en supervisión y lo más recomendable serían citas cada tres o seis meses.

LOS NIÑOS

Con los niños debe realizarse un enfoque diferente en el que están incluidos sus padres.

Aquino Choy manifestó que, a veces por miedo, no expresan sus emociones o les

resulta más difícil interactuar.

“Por eso nos aliamos con los padres para que nos digan si detectan que tiene temor, ansiedad, dolor y cómo lo expresa”, sentenció.

SUS CUATRO ASPECTOS

La medicina paliativa, afirmó Aquino Choy, tiene cuatro aspectos fundamentales que son: físico, psicológico, social y espiritual.

“A veces solo se enfocan en lo físico, en el curar, pero nosotros vemos todo el entorno del paciente para darle la mejor atención y eso nos hace más humanos”, destacó.

El lado espiritual no se aborda basado en una religión, sino desde la fe en un ser supremo para lograr una paz interior y fijarse metas u objetivos en la vida. “Platicamos con el paciente vemos si tiene miedo, rencor o enojo porque tener cáncer es algo muy fuerte y sabemos que está relacionado con muerte y dolor, por eso vamos sanando esas heridas”, detalló.

Los tratamientos para el área física son individualizados y dependerá del tipo de cáncer de cada paciente; para poder controlar el dolor utilizan opioides y el más conocido es la morfina, aunque también existen otros.

El aspecto psicológico es sumamente importante durante una enfermedad de este tipo, pues es común experimentar depresión, ansiedad y trastornos del sueño.

“Es necesario llevar una terapia psicológica aparte, incluso a veces hay que medicar, porque es difícil manejar la situación”, expresó la especialista.

Debido a este tipo de enfermedades, los roles en la familia del paciente cambian y aquí es donde el rubro social debe ser apoyado.

“A veces el que se enferma es el padre y ya no puede trabajar o es el hijo y deja la escuela, la dinámica va cambiando, se trabaja en cómo se va moviendo el individuo en la sociedad y qué tanto impacto tuvo para su estilo de vida”, manifestó.

LA FAMILIA Y EL CUIDADOR

En la medicina paliativa la salud de la familia y el cuidador del paciente son muy importantes, por lo que dedican especial atención a su cuidado.

“Ellos son unos pilares, los cuidadores a veces no saben nada de medicina y aprenden, en ocasiones el cuidador es un familiar directo por lo que es una parte fundamental”, externó.

Agregó que también otorgan capacitación al cuidador, ya que es una tarea difícil que no solamente conlleva desgaste físico, también emocional, ya que permanece en la tarea las 24 horas, los siete días de la semana.

“También hablamos con la familia para que no sea un sólo cuidador sino varios y hagan roles, sabemos que se están desgastando y sugerimos el apoyo de enfermería o de alguien que pueda apoyar a cuidarlos”, apuntó.

olvida de su propia salud y muchas veces padece enfermedades como diabetes, hipertensión, ansiedad, entre otras.

“El no se debe de descuidar para cuidar a su familiar, debe de cuidarse a sí mismo y por eso le damos medidas de soporte para que lo haga”, comentó la especialista.

Finalmente la doctora Priscila Aquino Choy comentó que a pesar de la idea que se tiene sobre el cáncer, gracias a la cien-

cia y la medicina han surgido diversos tratamientos para aliviar el dolor y el sufrimiento de los pacientes.

“Estamos aquí para ayudarlos a afrontar el cáncer y todos los síntomas relacionados”, expresó.

But today it is possible to improve many aspects of the skin thanks to modern, sophisticated and effective treatments.

According to the experts behind each procedure there are many years of research, but most important if you want to improve the presentation of hair, eyelids, neck, nose, nails and remove spots, wrinkles and stretch marks, it is best to be in the right hands.

Pero hoy en día es posible mejorar muchos aspectos de la piel gracias a modernos, sofisticados y eficaces tratamientos. “Esto lo vemos mucho con la gente que quiere realizarse una aplicación de bótox o un relleno de ácido hialurónico en nariz, ojeras, labios, etcétera, ya llega con nosotros con muchas ideas.

“Normalmente han leído, revisado, visto videos, pero no son cosas oficiales como tal, sino que lo vieron en Instagram, en Facebook o YouTube. Tenemos que tomar consciencia que las fuentes de información deben ser confiables y las precisas”, subrayó este médico por la Universidad del Valle de México (UVM).

Dijo que hablando de los procedimien-

Además, el láser también ha demostrado ser muy efectivo para aclarar zonas oscuras de la piel.

“Es algo que en la población ahorita tocar ese tema ya no le causa la misma pena que antes. Se ha convertido en un motivo frecuente de consulta, el área genital oscurecida, el aclaramiento de esas zonas sí se puede lograr.

“Tenemos que valorar específicamente, porque existe la aplicación de sustancias para el área, que dan un buen resultado y también en la zona axilar”, detalló Martínez Zorrilla.

“Lo del cabello, es algo que antes se le caía a los hombres, y se resignaba, pero tiene solución. Existe la manera de prevenirse y evitarse. La sudoración excesiva la podemos controlar mucho y la papada también.

“Las ojeras antes no se conocían. Era un tema poco abordado, pero actualmente existen procedimientos estéticos no invasivos, ambulatorios con recuperación inmediata, donde podemos mejorar la apariencia de la coloración de la ojera y ese aspecto, hablando de personas jóvenes, que se desvelan y tienen unos moretones. Una sonrisa, admitió el entrevistado, puede ser una buena carta de presentación para estrechar mejores relaciones humanas, tanto familiares como laborales. Es por ello que la apariencia desempeña un rol importante en la sociedad.

“Nosotros trabajamos el área facial y es muy cierto que la primera impresión es importante, tu rostro, cómo luces. Si duermes mal se refleja en tu cara, si comes de más se refleja en tu cara, si te asoleas lo mismo.., entonces es la carta de presentación de la persona. Esto habla mucho de la importancia y la autoestima, de cómo te reflejas tú ante los demás”, comentó.

Dr. Hannia Matt Hernandez said that globally, the most common allergy is rhinitis in 85 percent, followed by asthma, in 45 percent of the people with allergies.

“We all are born with the predisposition to be allergic to something, if the mother has rhinitis there is a 50 percent chance that the baby has an allergic disease and not only rhinitis can also be asthma or conjunctivitis; if both parents the chances increases up to 90 percent and if none is, we have a 10 percent chance of developing one and this will depend on their condition of life,” she said.

It is also believed that overprotective parents influence in that children do not develop defenses and antibodies against diseases.

PRICK TESTS

La pediatra alergóloga e inmunóloga clínica, Hannia Matt Hernández, dijo que siempre ha sido un tema polémico y de mucha controversia, pero que ya existen alternativas para que pueda darse la convivencia entre dueños alérgicos y sus mascotas.

“Todas las asociaciones médicas te van a decir que si eres alérgico a una mascota lo ideal es retirarlo de tu am-

biente, pero ya hay otras opciones porque sabemos que hay un vínculo muy estrecho que y es difícil sentimentalmente deshacerte del animalito y esto es gracias a las vacunas antialérgicas”, mencionó.

CUALES SÍ Y CUALES NO

La especialista dijo que cualquier animal que tenga pelo, tiene alto potencial alérgico, siendo los perros y los gatos los que más lo provocan, aunque también puede ser un cuyo o un caballo, entre otros.

Sin embargo, también se puede sufrir por las aves.

Los animales que menos alergénicos se consideran son los que no tienen pelo, por ejemplo, los peces, reptiles,

tortugas o iguanas.

El gato es considerado el animal más alérgico y la sustancia que predomina en él es la proteína Fel D1, presente en su saliva.

Muchas personas atribuyen su alergia al pelo del gato, pero lo que sucede es que al “bañarse” o acicalarse con su lengua, moja su pelaje con la saliva que es en realidad la que la ocasiona.

La especialista mencionó que, hay gatos considerados hipoalergénicos, pues aunque contienen dicha sustancia es una cantidad menor como lo son los siameses y los egipcios, sobre todo si son hembras y están esterilizadas.

Según la doctora, es fácil descubrir si la alergia es provocada por la mascota, pues al estar en contacto con ella se experimentará ojos llorosos, escurrimiento nasal, estornudos, comezón, irritación, entre otros síntomas.

Sin embargo, no es necesario vivir con animales para que pase, basta con estar en contacto con quien si los tiene.

“A veces no saben que son alérgicos pero están en contacto con personas que les queda el pelo de su mascota en la ropa y al saludarla empiezan a estornudar y a sentirse mal”, recomendó.

Comentó que hay algunas publicaciones en las que se menciona que si una familia tiene mascotas desde antes que nazcan sus hijos evitará que desarrollen alergias porque estarían en contacto desde muy pequeños con las sustancias.

RINITIS Y ASMA LAS MÁS COMUNES

La doctora Hannia Matt Hernández expresó que a nivel mundial, lo que más se padece es la rinitis alérgica en un 85 por ciento, seguido de asma, en un 45 por ciento.

“Se nace con la predisposición a ser alérgico, si la mamá tiene rinitis hay un 50 por ciento de probabilidades de que el bebé tenga una enfermedad alérgica y no solo esa también puede ser asma o conjuntivitis; si lo son ambos padres se incrementa hasta en un 90 por ciento y si ninguno lo es tenemos 10 por ciento de posibili-

PRUEBAS DE PRICK

La forma ideal para saber si este es su caso es mediante las pruebas realizadas por un alergólogo certificado, específicamente con las pruebas de Prick.

Por medio de una lanceta que puede ser de plástico o metal se hace una pequeña punción y se coloca una gota de la sustancia sobre la piel del paciente.

“Luego de 15 o 20 minutos se producirá una roncha que se tiene que comparar con un control positivo y un control negativo para saber si es alérgico”, explicó.

Matt Hernández manifestó que anteriormente la sustancia debía inyectarse bajo la piel, pero gracias a los avances médicos la colocación por medio de lancetas

“Es una mínima molestia, si eres alérgico vas a sentir un poco de comezón, a veces es más el estrés que tienen, yo prefiero hacerlo en los antebrazos porque me gusta que el paciente vea lo que estoy haciendo pero hay quienes prefieren la espalda”, apuntó.

VACUNA Y RECOMENDACIONES

“La vacuna dependerá del grado de alergia que tenga el paciente, los protocolos de inmunoterapia son prolongados, vamos a reacostumbrar a nuestro sistema inmunológico a tolerar sustancias que no tendrían porque hacernos daño”, declaró la pediatra.

Señaló que son dos a tres años de tratamiento, en los que se aplica la vacuna mensualmente y puede ser tomada o sublingual, o inyectada o subcutánea.

“La vacuna sublingual es un poquito más tediosa porque se aplica más veces, inicialmente es cinco días a la semana y luego tres cuando se llega a una fase de mantenimiento, y la vacuna inyectada son dos piquetes a la semana y después sólo uno, hasta completar el tratamiento”, precisó.

Además de la vacuna deben seguirse algunas sugerencias proporcionadas por la alergóloga para facilitar la convivencia con su mascota.

No debe dormir en la misma habitación y preferentemente estar afuera de la casa, bañarla cada quince días y proporcionarle los cuidados y control necesarios.

“Al paciente vamos a darle tratamiento sintomático, si tiene moco, nariz tapada y estornudos en lo que hace efecto la inmunoterapia específica, es decir, la vacuna de alergia específica para esa mascota”, expresó.

La finalidad de la vacuna es controlar los síntomas y que el paciente tenga tolerancia inmunológica, es decir, que cuando se exponga a esta mascota el niño o el adulto pueda tolerarla.

La inmunoterapia se recomienda a partir de los dos años de edad, si hay niños que tienen enfermedades muy severas menores de dos años como por ejemplo asma contra sustancias que no puede controlar se sugiere aplicarla desde el año y medio.

“Hay que ser muy observadores con nuestros hijos, ver que es lo que hacen o hacían una hora o 15 minutos antes de presentar los síntomas, si estaban jugando con la mascota, estrujando su pelo o abrazándolo, ya que al hacerlo se libera un polvito que contiene más de 15 sustancias diferentes que pueden provocar la alergia”, sentenció.

Mature women can conceive thanks to

reproduction

However, there are techniques and methods to become mothers over 40 years of age.

Dr. Felipe Arturo Morales Martínez, director of the Ceumer University Center for Reproductive Medicine at the Autonomous University of Nuevo León, where 200 to 300 in vitro fertilizations are currently carried out each year, specified some points on the subject of assisted reproduction. “It is estimated by statistics from the World Health Organization (WHO) that 10 percent of the world’s population will consult at some time for infertility. PRECISE DIAGNOSIS

Dr. Felipe Arturo Morales, mentioned that the couple who comes to the consultation for infertility has to be valued in an integral way, since 40 percent of the time you will find a cause in the male, while the other 40 percent It is in women, and 20 percent are a combination of factors.

“You should never give a treatment without having a diagnosis because then we’re going to make a mistake.

"And the diagnoses can be: problems of ovulation, problems in the tubes and added problems such as endrocrinológicos such as diabetes, hypertension or problems in the thyroid.

He noted that in the CEUMER University Center for Reproductive Medicine of the Autonomous University of Nuevo Leon, between 300 and 400 in vitros per year are carried out.

“We have more than a thousand babies born and we are the only university center in the north of the country that has all the certifications, including ISO 9001;

we have many patients from the Texas Valley because the UANL offers these very accessible treatments, almost a fifth or a quarter of what it costs in the United States but with the same quality standards,” said the doctor.

Regarding the age range in which these procedures can be performed, he commented that CEUMER has an ethics committee that set an age limit of 50 years, but even so, with the treatment of egg donation they can get pregnant.

“With current techniques we can even make them menopausal moms, because the pregnancy rates with an egg donation program are between 60 and 65 percent success, that is, 10 pregnancies are 6 or 7 per attempt.

“But it is also very important to consider that more than the age, that the patient’s health is optimal to be able to get pregnant, since there are 50-year-old women who exercise and have a good diet, while there are others of 40 who suffer diabetes or hypertension, which are risk factors,” he said.

Mujeres maduras pueden concebir gracias a la

El ritmo de la vida moderna provoca que la maternidad no sea la prioridad número uno de las mujeres, y cuando al fin deciden buscar el embarazo, se dan cuenta que no pueden concebir. Sin embargo, existen técnicas y métodos para que se conviertan en madres con más de 40 años de edad.

El doctor Felipe Arturo Morales Martínez, director del Centro Universitario de Medicina Reproductiva Ceumer de la Universidad Autónoma de Nuevo León, donde actualmente se realizan entre 200 y 300 fertilizaciones in vitro al año, precisó algunos puntos sobre el tema de la reproducción asistida. “Se calcula por estadísticas de la Organización Mundial de la Salud (OMS) que el 10 por ciento de la población mundial va a consultar en algún momento por infertilidad. Esto ha ido aumentando con el tiempo, porque el mundo ha cambiado socialmente en cuanto al tema de tener hijos.

“Hasta hace una generación o dos generaciones, las señoras se casaban más jóvenes y por eso tenían problemas con la infertilidad, pues el valor para poder predecir el éxito en un tratamiento siempre

va a ser la edad de la mujer”, indicó el ginecólogo con subespecialidad en Biología de la Reproducción.

En el pasado, las mujeres no estudiaban ni trabajaban, pero ahora lo más

común es que las mujeres terminen la universidad y ejerzan profesionalmente, un plan perfecto que en ese momento no incluye tener familia.

“Muchas mujeres se realizan profesionalmente pero han tenido que posponer la maternidad. Y eso ha hecho que ahora tengamos mucho trabajo, porque ahora quieren tener el primer hijo a los treinta y tantos años cuando anteriormente empezaban a los 20 o 25 años.

“Llegan a mi consultorio mujeres directoras de empresa, con maestría, doctorado de 45 años, recién casadas que quieren embarazarse”, comentó Morales Martínez.

Destacó que todo está relacionado con el tema de la reserva ovárica y es un tema que mucha gente desconoce y es importante que la población femenina lo considere.

“Es decir, las mujeres nacen con cierta cantidad de óvulos y esos óvulos se forman cuando están en la etapa fetal en el vientre de su madre y no vuelven a formar o crear uno nuevo.

“En la pubertad empiezan a gastarlos y se calcula que en cada ciclo se gastan de 200 a 300 para que la paciente ovule solamente uno y eso hace que se vayan agotando.

Si una paciente de 40 años de edad ovuló un ciclo, ese ovulo tenía 40 años y la ha acompañado cuando ha tenido fiebre, infecciones, altas temperaturas, contaminación, radiación, etcétera”, refirió el especialista.

Explicó que los mejores óvulos son los que se liberan en edades tempranas; por esa razón, es difícil que las pacientes jóvenes tengan problemas de infertilidad y es muy poco probable que tengan abortos espontáneos.

50 años, pero aún así, con el tratamiento de donación de óvulos pueden conseguir embarazarse.

“Con las técnicas actuales incluso podemos hacer que sean mamás con menopausia, pues las tasas de embarazo con un programa de donación de óvulos rondan entre el 60 y 65 por ciento de éxito, es decir de 10 se nos embarazan 6 o 7 por intento.

“Pero también es muy importante considerar que más que la edad, que la salud de la paciente sea óptima para poder embarazarse, ya que hay mujeres de 50 años que hacen ejercicio y llevan un buen régimen alimenticio, mientras que hay otras de 40 que padecen de diabetes o hipertensión, que son factores de riesgo”, subrayó.

DIAGNÓSTICO PRECISO

El doctor Felipe Arturo Morales, mencionó que la pareja que llega a la consulta por infertilidad tiene que valorarse de manera integral, ya que el 40 por ciento de las veces se va a encontrar una causa en el varón, mientras que el otro 40 por ciento es en las mujeres, y un 20 por ciento de factores combinados.

“Nunca se debe dar un tratamiento sin tener un diagnóstico porque luego nos vamos a equivocar. Y los diagnósticos pueden ser: problemas de ovulación, problemas en las trompas y problemas añadidos como endrocrinológicos como pueden ser diabetes, hipertensión o problemas en la tiroides.

“En cuanto al varón, se le realiza un estudio de esperma. Los varones producimos espermatozoides hasta que somos ancianos; un hombre anciano puede ser papá, es probable que no tenga una erección, pero podemos obtener espermatozoides del testículo de algún conducto y hacerlo papá por medio de la reproducción asistida con una fertilización

Una vez que se haya obtenido el diagnóstico, dijo que el tratamiento puede ser tan sencillo como indicarle unas pastillas para inducir la ovulación; efectuar una microcirugía para reparar trompas; o en el último de los casos, una técnica de reproducción asistida como la fertilización in vitro.

Señaló que en el Centro Universitario de Medicina Reproductiva Ceumer de la Universidad Autónoma de Nuevo León, se realizan entre 300 y 400 in vitros al año.

“Tenemos más de mil bebés nacidos y

somos el único centro universitario en el norte del país que tiene todas las certificaciones, incluyendo el ISO 9001; tenemos muchas pacientes del Valle de Texas porque la UANL ofrece estos tratamientos muy accesibles, casi una quinta o una cuarta parte de lo que cuesta en Estados Unidos pero con los mismos estándares de calidad ”, afirmó el médico.

En relación al margen de edad en que se pueden realizar estos procedimientos, comentó que el Ceumer cuenta con un comité de ética que fijó una edad límite a

Más de 105 millones 194 mil 385 pesos serán invertidos por el Estado y municipio en la rehabilitación de la red de agua potable y drenaje, a fin de mejorar el servicio en distintas colonias de esta ciudad.

El gerente general de la Comapa, Jesús Ma. Moreno “Chuma”, reveló la millonaria inversión a realizarse durante el presente año en diversos proyectos ya aprobados, gracias al trabajo coordinado del gobernador del Estado Francisco García Cabeza De Vaca y la alcaldesa Maki Ortiz .

“El gobierno municipal que encabeza la

alcaldesa Maki Esther Ortiz Domínguez realizó la gestión de recursos federales por más

Cuadrillas de la Coordinación General de Servicios Públicos Primarios efectuaron labores de limpieza en el parque de la Bocatoma y trabajan actualmente en la reparación y repintado de asadores, juegos infantiles y la rehabilitación de postes, cable

y lámparas del alumbrado público.

Las palapas, que son instalaciones complementarias importantes para el esparcimiento de las familias reynosenses y de los visitantes, también son objeto de limpieza y reparación, al igual que los baños sanitarios, a fin de que pronto estén listas y que ofrezcan seguridad para todos.

Además del trabajo que las direcciones de Limpieza y Recolección están efectuando dentro del recreativo La Playita, la Dirección de Vialidad y semaforización lleva a cabo el repintado de las líneas de tráfico, y la reparación o sustitución de señalamientos en la carretera ribereña.

Por Pamela Escobar De acuerdo con la ginecóloga Teresa Tijerina Ortega, las mujeres más propensas a padecerlo son: las que se ubican en los extremos de la edad; es decir, pacientes muy jóvenes de 15 años de edad, así como las que están por encima de los 36 o 38 años.

“Este tipo de grupo etario son las más propensas a presentar complicaciones; además de cumplir con otros factores de riesgo, como antecedentes de diabetes u obesidad; o que en gestaciones anteriores sufrieron abortos, hemorragias o preeclampsia”, detalló el médico.

Así mismo añadió que la hemorragia obstétrica es la principal causa de muerte a nivel mundial en la gestación y explicó que el embarazo de alto riesgo se define como cualquier situación que pueda compro-

meter la salud de la madre y del producto.

“Cualquier situación que ponga en riesgo a alguno de los dos, o a ambos, puede complicar el embarazo, que es la de un nacimiento exitoso; es decir, un bebito en buenas condiciones de salud y vida al igual que de la madre para poder brindarle los cuidados necesarios”, expresó la entrevistada.

DETECCIÓN OPORTUNA

Tijerina Ortega precisó que en el área obstétrica para poder detectar a tiempo una complicación durante el embarazo, lo más importantes es llevar a cabo un control prenatal.

“Es un programa de salud secuenciado, que consiste en consultas médicas de la embarazada con cierta periodicidad, al inicio, son mensualmente, pero a medida

que avanza la gestación pueden aparecer complicaciones, por lo tanto, las citas van a ser más regulares”, refirió la doctora.

Relató que durante el control prenatal se valora el estado de la mamá, desde el principio de la gestación, las condiciones en las que se encuentra, e incluso hay mujeres que antes del embarazo asisten a una consulta de preconcepción.

“En éstas valoramos el estado materno, el fetal, detectamos qué factores de riesgo tiene la paciente que pudieran complicar el embarazo, se verifica la salud de la mamá, se solicitan exámenes de laboratorio y ultrasonido del producto, además se monitorea el peso de la futura mamá, con la intención de evitar que se presenten complicaciones”, manifestó.

LOS DAÑOS Y COMPLICACIONES

La especialista Teresa Tijerina Ortega advirtió que la hemorragia obstétrica es la principal complicación a nivel mundial durante la gestación, además de otras más graves como lo son placenta previa, acretismo placentario, éstas últimas, muchas veces de la mano y que a su vez, incrementan el número de cesarías.

“Si mi paciente es hipertensa, tengo que medicarla desde un principio; o si comienza a subir de peso y durante los controles notamos que la presión empieza a subir, muy probablemente se pueda complicar; y la preeclampsia, que desgraciadamente puede afectar el riñón, el hígado, convulsionar a la madre e incluso muerte cerebral de la paciente”, alertó.

Señaló que debido a las hemorragias, uno de los síntomas que se presentan durante las embarazos de alto riesgo, lo más grave es que puede provocar la muerte, o bien que la paciente termine con una histerectomía.

“Al presentar con placenta previa, acretismo placentario, que es la mala colocación de la placenta y que genera una hemorragia, a veces, existe la necesidad de extraer el útero”, recordó.

Mencionó que una hemorragia puede provocar que los bebés no lleguen al término de la gestación y al ser prematuros requieren el área de terapia intensiva neonatal; mientras que a causa de la preeclampsia en el embarazo se comprometen los órganos como el riñón e hígado de la madre y del neonato.

Tijerina Ortega comunicó que lo más grave de padecer un embarazo de alto riesgo es que puede provocar la muerte; las hemorragias obstétricas están muy relacionadas con el deceso de la paciente y en el caso de los bebés casi siempre suelen ser prematuros.

“Al darse el nacimiento antes de tiempo, los bebés suelen pasar tiempo en el área de terapia intensiva neonatal, pero es muy difícil poner un pequeño tan prematuro, pues difiere mucho su progreso dependiendo de

las semanas de gestación en el vientre; definitivamente los embarazos de alto riesgo pueden comprometer la vida, tanto del bebé como de la madre”, recalcó.

RECOMENDACIONES

Añadió que primordialmente se requiere llevar un control prenatal estricto y si se tiene alguno de los antecedentes mencionados, e independientemente de ello, llevar atender sus citas con el ginecólogo y apegarse a las indicaciones del médico.

“A veces puede ser un poco difícil apegarse a un tratamiento en el caso de las mamás es importante seguir las medidas necesarias que se deben emplear como el reposo,

uso de medicamentos, etcétera”, mencionó.

Enfatizó que las mujeres acostumbradas a trabajar, necesitan descansar en el embarazo, por ello la importancia de apegarse a las indicaciones del médico y al control prenatal; lo anterior, para conocer cómo se va desarrollando el embarazo y el desenlace que pudiera tener.

Algo importante, dijo, es que es un trabajo en conjunto, entre la paciente y el médico, para poder llevar a cabo un embarazo a buen término y entregar un bebito sano y una mamá que se pueda hacer cargo de él.

“Todos los embarazos tienen el riesgo y me enfoco mucho en la hemorragia, porque es a lo que nos enfrentamos día a día los ginecólogos, todas las embarazadas en el momento de la gestación o el parto se pueden complicar”, concluyó.

Finalmente comentó que al estar embarazada, el funcionamiento del organismo cambia completamente, pero para bien, precisamente por si se presentan este tipo de situaciones se equilibre, pero puede presentar complicaciones difíciles de superar.

La presidenta del sistema DIF Tampico, Aida Feres de Nader acompañada de su esposo, el presidente municipal Chucho Nader encabezaron el espectacular festejo, el cual tuvo dos funciones, a fin de que todos los niños de la ciudad, tuvieran la oportunidad de disfrutar su día.

Feres de Nader al dar su mensaje expresó, “a nosotros nos hace feliz ver esas caras de niños que estén bien contentos; es un gusto estar aquí disfrutando esta fiesta con todos ustedes. A nosotros como DIF, nos llena de gusto y nos fortalece ver familias completas en esta fiesta”.

Manifestó que para el sistema DIF Tampico, la familia es primordial; “pues es la

base de la sociedad y este festejo del día del niño, fue preparado con mucho cariño para todos ustedes, porque son nuestro mayor motivo para trabajar con entrega y entusiasmo”.

Durante el evento el grupo músico-vocal “Colorín colorado” ofreció una divertida presentación a través de su equipo de bailarines, cantantes, animadoras y botargas que pusieron a cantar y a bailar a todos los asistentes. El espectáculo, cautivo a chicos y grandes que jugaron, rieron, y bailaron.

El presidente municipal, Chucho Nader, dijo que este tipo de actividades refrendan el compromiso de su administración con la niñez y las familias de Tampico; “somos un gobierno cercano a la gente que toma en cuenta la participación de los niñas y niños; por que son ellos el motor que nos alienta a seguir trabajando en favor del progreso de Tampico y del bienestar de los ciudadanos”.

En ambas funciones celebradas en el auditorio municipal, el entusiasmo de los niños fue el marco perfecto para hacer de este domingo una fecha inolvidable.

Comprometidos con los estilos de vida saludables y el fomento de valores, el Sistema DIF Reynosa que encabeza Carlos Peña Garza, lleva el programa de Jóvenes a escuelas de la ciudad, a fin de concientizar sobre diversos temas.

En dos años y medio se han brindado acciones preventivas y de atención a 39 mil jóvenes de educación básica, media superior y superior.

Carlos Peña Garza, presidente del patronato del DIF Reynosa, encabezó la conferencia para jóvenes sobre sexualidad y valores en la Escuela Secundaria Número 1 “José de Escandón”, donde se ofreció una herramienta para la toma de decisiones responsables de los estudiantes.

“Seguiremos trabajando arduamente en coordinación con el sector educativo

y padres de familia, procurando para los estudiantes una adecuada orientación y un sano desarrollo, échenle muchas ganas, la actitud positiva los saca adelante, pero la preparación los lleva al éxito”, dijo.

Los estudiantes pudieron participar en diferentes actividades y escuchar una plática de valores.

“Me da mucho gusto estar aquí, en esta escuela, también soy de la Escandón y con mucho orgullo”, expresó el presidente del patronato del DIF municipal.

La directora del campus, Edginza Lariza Cantú, agradeció al DIF Reynosa por apoyar este tipo de acciones que contribuyen al desarrollo saludable de los estudiantes.

Para los visitantes que a lo largo de la temporada vacacional, se animaron a cruzar la frontera mexicana, confiando en los productos y servicios que ofrece la villa de Nuevo Progreso, el pasado 21 de marzo las autoridades llevaron a cabo –por cuadragésima tercera vez– un evento de agradecimiento.

Además de celebrar el tradicional abrazo con sus pares americanos, el gobierno de Río Bravo (al que pertenece el poblado de Nuevo Progreso) nombró a los personajes del año como parte de la fiesta organizada para los turistas.

Gilberto Sáenz fue distinguido con el reconocimiento de Señor Turista 2019, mientras que la Señorita Turista 2019 fue Denisse García.

En tanto que Jerry y Sue Machetta fueron elegidos como el Rey y la Reina del Invierno 2019” en una muy colorida ceremonia.

UN PUEBLO HOSPITALARIO

Desde muy temprano comenzó el aforo de visitantes a la avenida principal de Nuevo Progreso, la cual está conectada con el puente internacional, dentro de los cuales hubo también público hispano del Valle de Texas que acudió al concierto del grupo Los

Ya es una costumbre que los comercios coloquen estantes en la calle que es cerrada para la comodidad de los peatones.

Fue un día de fiesta, de alegría, de colorido, de bailables, de antojitos mexicanos, pero también de invitación para que los ‘winter texans’ conozcan todo lo que puede encontrarse en este rincón de la frontera.

Nuevo Progreso se ha destacado por tener un importante cluster de turismo médico dental y ser uno de los sitios más emblemáticos

para la atención del cuidado bucal. Existen muchas clínicas, consultorios, laboratorios, ópticas, farmacias, salones de belleza y spa´s que participaron en este importante evento.

El Día del Turista es considerada la fecha más emblemática de la temporada vacacional y sin ser excepción los asistentes pudieron disfrutar de un amistoso día con la gente de la frontera mexicana.

presencia de la escuela preparatoria federal Ricardo Flores Magón del vecino municipio de Matamoros, que se encargó de amenizar el ambiente, al igual que estudiantes de planteles locales.

Hubo música, aperitivos gratuitos para todos los visitantes, dulces y se entregaron regalos. Es por esto y por mucho más que el 21 de marzo está en el gusto de los extranjeros y de la comunidad de Nuevo Progreso que públicamente les dio las gracias y los espera pronto.

Quiroga Garza specified that the consumption of tobacco and drugs can compromise the health of the teeth, so it is necessary to create awareness and avoid them.

“A veces nos tocan personas que no saben en realidad el problema que les aqueja. Les duele mucho una muela pero desconocen la verdadera causa, por eso necesitamos hacer una exploración intraoral para verificar su boca y les decimos lo que tienen y ocupan.

“Después, cuando el paciente se somete al tratamiento y va notando el cambio, es muy satisfactorio ver que se va muy contento. Nos ha tocado gente que viene con problemas muy graves en su boca y al mirarse con su dentadura nueva, con sus coronas hasta casi lloran de felicidad.

“Y eso es algo muy gratificante para un dentista, porque sabes que al paciente le está gustando tu trabajo”, confesó.

El médico Quiroga Garza reconoció que la autoestima de una persona está estrechamente ligada al estado de su dentadura.

“A veces nos llegan pacientes muy tímidos o incluso renuentes, porque muchas veces se atendieron en otras partes, tuvieron una mala experiencia y se encuentran asustados.

“Nosotros siempre buscamos lo mejor para ellos y cuando les hacemos el trabajo van sintiendo la confianza. Se animan y eso es muy bonito”, agradeció el entrevistado.

PROBLEMAS COMUNES

Cuando los dientes y las muelas no están bien alineados se pueden llegar a presentar dificultades para masticar la comida.

“Al tener malposición o cuando se hizo una extracción sin poner nada en el mismo lugar los dientes empiezan a moverse, y no sólo tienen

problemas al comer, sino que pueden aparecer otros inconvenientes a nivel de la articulación, como rechinidos, dolor y desgaste.

“La dentadura se desacomoda, se les ve chueca y eso es todo lo que tratamos de arreglar aquí; se les informa lo que tienen y se les da una idea de lo que se necesitan hacer, un plan de tratamiento acorde para cada uno”, explica.

El entrevistado recalca en la importancia de prevenir, porque asegura que ese es justamente el método más barato.

“Si se tiene una cultura del cepillado y de hacerse las limpiezas se puede mantener toda la vida sin muchos problemas, pero a veces la gente por diferentes factores viene con sus piezas deterioradas.

“Y ahí es donde nosotros tenemos que educar al paciente, enseñarle porqué es importante que tenga en buen estado su boca y darle las facilidades para que pueda mantener los cuidados. Hay quienes sufren otros problemitas físicos como Alzheimer o Parkinson. Bueno, pues aquí les enseñamos técnicas y les damos instrumentos para que se les facilite su limpieza bucal”, comentó.

ADVERTENCIA POR CIGARRO Y ENERVANTES

Quiroga Garza especificó que el consumo de tabaco y las drogas pueden comprometer la salud de las piezas dentales, por eso es necesario crear consciencia y evitarlos.

“Eso provoca muchos inconvenientes, desde manchas hasta enfermedades más graves co-

mo periodontitis. El alcohol causa algunos problemas, pero no tanto como el cigarro y las drogas. También afectan el café, el vino tinto y diversos medicamentos. El betabel mancha mucho los dientes.

“Lo mejor desde niños es consumir verduras como la zanahoria, que le ayuda a hacer un barrido natural de los dientes. Masticas y te limpia también, por eso es muy importante”, agregó.

En relación a los fármacos el dentista refirió que algunos afectan directamente los dientes de las embarazadas y sus criaturas, como las tetraciclinas, que se asocian a las manchas. Hay otros que provocan resequedad en la boca y eso les afecta mucho con caries y otras enfermedades”, argumentó.

Sobre los cuidados recalcó evitar el uso de estupefacientes, ya que en los consultorios pueden verse personas jóvenes con sus piezas dentales destruidas.

“Los dientes y las encías sufren de una caries rampante y la droga provoca una abrasión química en el hueso que se empieza a desgastar de una forma acelerada, más los rechinidos que tensan los músculos cuando están drogados, haciendo movimientos que también los dañan.

“Hay muchos pacientes que nos llegan de esa forma, muy dañados de sus dientes y muchas veces no se pueden rescatar, tenemos que hacer extracciones.

“Estos pacientes también vienen con una autoestima muy baja y cuando nosotros les rehabilitamos la boca y se ven bien hacen un cambio ellos mismos en su vida. Se sienten más sa-

nos y seguros”, compara.

Otras ocasiones dijo el especialista, que las personas tienen problemas que no son tan graves como parecen, pero que no acuden al dentista porque piensan que es caro o les va a doler.

Aduce que hay veces que solamente se trata de restaurar una muela o un pequeño detalle donde falta aseo y así mejorar la apariencia de la sonrisa, retirando de paso el fastidioso problema del mal aliento.

“Generalmente esto se da por falta de higiene, porque a lo mejor traen una enfermedad periodontal o del estómago, pero casi siempre es por la boca. Los fumadores tienen una tendencia a tener halitosis, provocando mal aliento.

“A veces son las caries también muy profundas, porque se mete alimento donde está la caries y causa mal aliento, pero eso tiene solución”, subrayó.
