hat is the pancreas? The pancreas is an organ is located in the upper abdomen, behind the stomach, and it produces a fluid that contains enzymes that participate in the digestion and absorption of food. The fluid is transported to the small intestine through a duct called chole ducts. The pancreas also mediates the glucose leves through the production of insulin. What is pancreatitis? It is the sudden inflammation of the pancreas caused by the activation of digestion enzymes within itself before they are released to the intestine. In severe cases, the pancreas destroys itself in a process called necrosis which results in a generalized inflammatory reaction that may affect other vital organs. Pancreatitis more frequently affects men. What are the causes of acute pancreatitis? The two most frequent causes are stones in the biliary duct (cholelithiasis, 80%) and excessive alcohol consumption. Stones in the gallbladder may migrate to the biliary duct and obstruct the release of the pancreatic substances into the intestine. Alcohol has a direct toxic effect and may also result in chronic pancreatitis. Other causes may include some medications, significant increase in blood fat levels, increase in blood calcium, some surgeries, obstructions in the pancreatic duct, and physical trauma to the abdomen. In approximately 20% of the patients, a cause if not found. What are the symptoms? It is frequently manifested as a continuous and intense pain mainly located in the upper part of the abdomen. Pain may be accompanied by nausea and vomit. It can worsen in a matter of minutes after a meal, especially if the food high on fats. The pain can increase in severity or frequency over time. People with pancreatitis often look sick and present fever, nausea, vomit, sweating, and jaundice (yellow coloration of the skin and eyes). How is pancreatitis diagnosed? Blood tests typically reveal elevated pancreatic enzymes (amylase and lipase). However, a CAT scan is necessary to confirm the diagnosis. The CAT scan may provide a way to visualize the level of necrosis in the pancreas along with other abdominal complications. And abdominal ultrasound allows physicians to see the presence of stones in the gallbladder, but generally inflammation of the pancreas obstructs the view of the pancreas. An MRI may be an alternative to a CAT scan if there is an allergy to IV contrast, in case of kidney failure, and if the possibility of a stone in the bile duct
wants to be discarded. Endoscopic ultrasound is useful for the examination of the pancreas and detect biliary duct stones not seen through other examinations. What are the complication? Pancreatitis may reappear as a chronic pancreatitis. The probability of return depends on the original cause. Complications may include: kidney failure, syndromes of severe respiratory failure, accumulation of fluid in the abdomen, pseudocysts of the pancreas, or heart failure. What is the treatment? There is no specific treatment available for pancreatic inflammation. Patients must resort to abundant IV fluids, pain relief medications, and avoiding intake of food by mouth. Mild cases often need no further treatment and recovery generally happens in a matter of a week. Sever cases may require observa-
tion in the intensive care unit to be administered antibiotics in order to combat the infection caused by the pancreatic necrosis, as well as to establish a feeding tube. In cases where the necrosis becomes infected it is necessary to remove the dead fragment of the pancreas. This procedure used to be performed surgically but can now be done by means of endoscopy. An Endoscopic Retrograde Pancreatography (ERCP) can be required in order to remove the obstructing stones. Pseudocysts can also be treated. What is the prognosis? In the majority of the cases inflammation is mild. However, 1 of every 5 cases become severe. Mild cases have a good prognosis and result in complete recovery. Severe cases may be fatal 20% of the time; generally patients with extensive necrosis of the pancreas and failure of vital organs.

Deafness can be caused by several factors and be of different types so a specialist will perform the necessary studies to give the indicated treatment.
WHAT DID
HE SAY?! By Beatriz Flores
The word “deafness” refers to the loss of hearing, a condition that in medicine is known as hearing loss. “It is a decrease in auditory acuity and can be classified depending on the intensity or depth of that loss, it can be mild, moderate, severe or profound,” said the otolaryngologist, José Luis Salinas González. He also said that it is one of the most frequent causes of consultation, but fortunately, most of the problems are reversible. TYPES OF HEARING LOSS CONDUCTIVE AND NEUROSENSORIAL
He noted that conductive hearing loss occurs when the ability of the ear to carry sound to the inner ear is prevented or reduced and can occur through a cerumen plug, an ear infection, which is punctured, by exposure to noise or by the practice of some sport like hunting. “That acoustic trauma can be corrected if it is treated on time, if not we can not recover it, we have a week to do it,” the doctor explained. The solution will depend on the case, for example, if the patient has a cerumen plug,
4
the ear is cleaned; if there is an infection, the deafness will end when the infection is cured; If there is a perforation in the tympanic membrane it is corrected by repairing it. There are three small bones that connect to the eardrum to amplify sound waves and otosclerosis is a disease that occurs when one of those bones is unable to vibrate freely.
The doctor explained that it is corrected by replacing, by means of an implant, a prosthesis that performs its function. Sensorineural hearing loss occurs when the snail or nerve is damaged and is not reversible, but fortunately there are few cases of this type. There is a disease called sudden hearing
loss and it is the rapid loss of sensory type hearing. “For some strange reason the nerve or the snail stopped working, as if someone turned off the power or the current to a speaker and we have a week to correct the hearing,” he warned. He pointed out that the ear “is infarcting” then your family doctor my tell you that it is a little inflamed and prescribes an antibiotic in drops, if there is no improvement it is necessary to go with a specialist. “It is not known what the cause is, it can be a viral infection or an increase in blood pressure,” he said. Until recently, there was no way to correct this hearing loss, but now with new technologies and medical advances is a reality. “When the loss is not so profound, the patient can be instructed to use a hearing aid, which are only sound amplifiers,” he said. He said that the sound in the speech is at 30 decibels and the auxiliaries can amplify it to 60 or 90 decibels, there are some that can give up to 110 decibels of sound. COCHLEAR IMPLANT
When the hearing loss is very deep and the auxiliary is no longer effective, the indicated thing is to use a cochlear implant, which is

a high-tech hearing device that is placed inside the ear and transforms the acoustic signals into electrical ones to stimulate the auditory nerve. “This is beneficial for patients that have a hearing loss so they can hear again or so that children who have never heard can develop a language,” he said. Children from four or six years who have not developed language are candidates for this surgery, however the process to achieve it continues. “You will have to start a therapy or a learning acceleration process when you already hear noise,” he said. In adults it is different, if you have never heard anything and did not develop language when the implant is placed you will not have the capacity to recognize and memorize the words. In adults only applies when the hearing is lost, that is, he had previously had. There are some congenital disorders that include deafness and congenital complications that prevent some people from being candidates for a cochlear implant. The otorhinolaryngologist said that the implant that is for life is an expensive device and its price can range between 20 and 25 thousand dollars, without counting the cost of surgery. AUXILIARY HEARING
It is the most known and common device for deafness, as easy to use as a pair of lenses, according to the doctor. “Socially it can impact, there are people who do not want them to realize they have a hearing aid, they think it makes them look vulnerable or old, but nowadays there are some that nobody notices,” he said. He said that the best known are “of the curveta type,” which goes behind the ear, but there is also another one that “is inside the shell,” located in the auricular pavilion and one “placed inside the canal where nobody sees it.” STUDIES AND ASSESSMENTS
Salinas González commented that when people go to a consultation they perform a study called tonal audiometry, on which hearing is assessed in both ears.
“It measures from 0 to 120 decibels, at a frequency of 500 to 8000 hertz, and based on that we can catalog if a patient has a mild or moderate hearing loss, we are classifying in depth and we can say in what frequency is damaged,” he said. In hospitals, a control study called otoacoustic emissions is performed to detect hearing problems in newborns. “The most common study is audiometry, a device that emits sounds at different intensities is used and it is called tonal audiometry, patients are taken to an acoustic booth where a sound is emitted by a hearing aid and the signal is indicated at what frequency what intensity of sound listens,” sentenced. Then a graph is made where it will be shown if the patient has a normal hearing or with some alteration. RECOMMENDATIONS
The otolaryngologist José Luis Salinas González said that there are some recommendations that can be carried out to avoid hearing loss. “In our geographical area there are many maquila companies and they have a security department where the use of hearing protection is required to avoid exposure to a certain intensity of noise during the day,” he said. People who practice hunting can develop “hunter’s deafness” if they do not use the right hearing aids to protect their hearing. Perforations for introducing some object in the ear, blows in the ears or a fracture in the base of the skull can also be reasons to lose the hearing. He stressed that when a patient only has one ear, they should tell the doctor before starting any treatment, as some may represent an important risk to their hearing health. “There are medications such as aminoglycosides in antibiotics that should be used with reserve, such as amikacin gentamicin, sometimes doctors value the risk because it is the only substance that will cure the patient, put on a scale using the medication with the indicated dose and the patient is explained what the risks are,” he revealed.
¡¿QUÉ
DIJOOOO?!

Get to know
your baby
before birth
You should not wait until your baby is born to know how he or she looks like, now it is possible to appreciate its features with great definition and detail thanks to 4D ultrasound. By Beatriz Flores Ultrasound in 4D is a technique for imaging that has great growth in obstetrics by allowing to see with great detail and greater definition the face, the features of the spine and feet of the baby, to mention examples. Dr. Álvaro Gómez Peña, obstetrician gynecologist at the Lancet Clinic, spoke about this procedure and what it offers. “It has some advantages over the traditional form of 2D, it gives you an idea about the space the baby has in the uterus, it shows you the contour lines of the face for certain malformations and allows us to detect a defect, which facilitates the explanation to parents,” he explained. The specialist in Fetal Medicine said that it is best to do it between week 25 and 30, because it is at that time that the baby has a good amount of amniotic fluid and can look good. In addition, the further the pregnancy progresses the space is reduced and it is difficult to appreciate it. “The baby moves a lot and has enough fat to be able to see his features well because if we do it at week 15 or 16 sometimes they are not well defined because they do not have enough fat and the bones stand out a lot,” he said.

Doctor Álvaro Gómez Peña.
ONE FOR EACH STAGE
Depending on the weeks of pregnancy that the patient has, it will be the ultrasound that must be done, according to the doctor.
The genetic ultrasound or screening of the first trimester is focused on the detection of genetic abnormalities, mainly discarding Down Syndrome, Edwards Syndrome or Turner Syndrome and is
performed between week 11 to 14. “The risk of preeclampsia is also assessed so that the patient’s pressure is not increased, the risk of premature birth and anatomy, to see that the baby has a normal formation,” he said. From week 20 to 25 it is recommended to perform anatomical ultrasound, also called structural or morphological, in which the organs are checked in detail, from the head to the feet. “You can see the brain, the heart, the little mouth, the fingers are counted, everything is checked in detail to see that there is no malformation, the bones are not calcified and they let you see the internal organs well and certify that everything is fine formed,” he said. He asserted that if it is close to the end of the pregnancy and the patient has not undergone ultrasound or if it has been very superficial, it is advisable to do so. “As the pregnancy progresses, less is perceived, but there are some malformations that can be seen throughout the pregnancy, even at week 38 or 39, they have to be looked for,” he said. It is also important because it assesses safety issues such as the amount of amniotic fluid, the position of the baby, its size and how is the placenta, since that is very important for the time of delivery or cesarean section.

TWIN AND MULTIPLE PREGNANCY
These types of pregnancies are considered high risk, since they are different from those of a single type, so surveillance is more frequent and reviews should be carried out more frequently. “It is very important to say that not all twins are the same, there are twins who have two placentas and those who have only one for both and it is totally different what we see and what we look for in these babies,” he warned. He commented that usually it is possible to identify well and review in detail each twin, so from the first trimester, in week 14 or 15, the doctor who takes control should ensure in writing what kind of twings are to follow up properly. A SWEET OR CHOCOLATE BEFORE ULTRASOUND
Dr. Álvaro Gómez Peña said that for a genetic or anatomic ultrasound it is desired that the baby move a lot to have different angles and be able to review it well. The baby is stimulated with glucose or sugar so we recommend that you eat a chocolate 15 or 20 minutes before the study and it is important not to cream that day so you can see better the resolution of the image “, asked the doctor. He said that some patients have doubts about whether they should drink water before ultrasound, stating that it is only necessary when performing a pelvic ultrasound and there is no pregnancy. “It is required to have a full bladder
to see well the uterus and the ovaries but if there is pregnancy it is not necessary, in fact it is better to have an empty bladder so that the water does not interfere with us,” he explained. COMPLETE AND QUALITY ATTENTION
The gynecologist Álvaro Gómez Peña said that the 4D ultrasound allows to know the baby in depth and detect any abnormality or problem in time, however, so that their benefits can be used should be done by a specialist. “There are many people in the field of medicine who perform ultrasounds, such as radiologists, general practitioners, ultrasonographers, gynecologists or perinatologists, what matters is that they are qualified, certified, have experience and are really dedicated to prenatal diagnosis,” clarified . The Lancet Clinic offers gynecology and obstetrics specialties completely and openly. “We can attend low-risk, high-risk prenatal care, perform genetic, anatomical ultrasound in 4D and carry the pregnancy to term, either by delivery or cesarean,” said the doctor. This thanks to the professional team of the clinic, among which are eight gynecologists available to patients who require them. “The cost of ultrasound in 4D is 600 pesos, it is quite economical compared to other places,” he said. The medical center is open 24 hours, however, for an ultrasound you must obtain an appointment. For this you can call (899) 309 64 13 and (899) 924 15 13, or go directly to Avenida Zertuche, number 1020, Fraccionamiento Los Virreyes.
Conozca
FRONTERA DOCTORS HEALTH MAGAZINE | Marzo 2019

Combination of drugs could reduce risks of
cardiovascular problems By Irma Idalia Cerda Monterrey, N. L. The risk of a heart attack and stroke can be reduced by up to 24 percent thanks to the combination of the drugs rivaroxaban and acetylsalicylic acid that were approved in Mexico by Cofepris. This was announced by medical specialists from the Bayer company to the media on March 5 in Mexico City, who explained that the new therapeutic scheme is based on phase III of the COMPASS study. Dr. Alejandra Meaney, clinical cardiologist certified by the Mexican Board of Cardiology and Medical Affairs Operation Manager of the Pharmaceuticals Division of Bayer of Mexico, said that the rate of mortality from cardiovascular diseases is increasing, since every four seconds there is a heart attack and every five seconds a cerebral vascular event. “The first cause of death worldwide, are coronary artery disease or myocardial infarction and this represents great costs for institutions and is even more prevalent than cancer and this is true for all continents, including the Americas.” She warned that this situation can generate an economic impact in the future because it is expected that it will be more difficult to face the expense of these diseases than diabetes mellitus. “According to 2012 data, heart diseases are the leading cause of death in Mexican men and women and that has not changed. “Of the heart diseases two-thirds are explained: by heart attack, followed by diabetes mellitus, but here we must consider that the diabetic patient dies almost usually by myocardial infarction, that is to say that it is basically the same,” Meaney mentioned. Cardiovascular diseases are a group of disorders of the heart and blood vessels,
14
among which are Coronary Artery Disease (CAD) and Peripheral Arterial Disease (PAD), which is caused by atherosclerosis, characterized by the accumulation of plaque (deposits of fat) inside the arteries that cause them to narrow and harden. For his part, Javier Antezana Castro, clinical and interventional cardiologist, attached to the Coronary Unit of the Cardiology Hospital Century XXI of the IMSS, explained how Coronary Arterial Disea-
se occurs. “When we talk about CAD, we usually move to a heart attack, to angina; It happens that the coronary arteries that supply the cardiac muscle suffer a reduction in their caliber due to the presence of plaque atherosclerosis, a plaque that is formed from high levels of cholesterol. “We know that when we have high cholesterol, especially a high-density lipoprotein fraction, these travel in the blood penetrate the inner part of the arteries and form a fat plate causing atherosclerosis,” said the specialist. He added that the risk factors for developing atherosclerosis are diabetes, high cholesterol, hypertension, sedentary lifestyle, overweight and smoking. Regarding Peripheral Arterial Disease, the angiologist and vascular surgeon, Gerardo Peón Peralta, said that every seven minutes, a Mexican loses a leg due to this condition. “The EAP happens in the arteries of
the legs and this means that the amount of blood that reaches the feet decreases and the oxygenation of the tissues in the lower extremity to levels that begin to manifest first in more nonspecific symptoms. “But this way, as the tissue dies in some places of the heart when it happens in the coronary arteries, or with a stroke in the brain when it passes into the nervous system, the same happens in the legs there is the danger of losing the leg,” he said. He stressed that according to INEGI data it is estimated that by 2030, 58 percent of all deaths of Mexicans are a result of complications from the obstruction of the arteries that carry oxygen to any of the tissues. Doctors agreed that a healthy lifestyle, exercise, balanced diet, not smoking, staying relaxed and in the right weight, along with pharmacological treatment, cardiovascular diseases can be treated successfully.

It arrives! but not suddenly
Andropause represents a change in the lifestyle of men, similar to what happens to women with menopause, however, unlike them, it occurs slowly and gradually. By Beatriz Flores Much is said about menopause, that stage in a woman’s life where great physiological and psychological changes are experienced, but what happens with men? Does the same thing happen to them? The answer is “yes”, those changes begin after 40 years and are called andropause. The specialist in Urology, Rogelio Gómez Loredo, mentioned that it consists of the reduction of hormones such as testosterone, which occurs in the testicles, which will cause a decline in certain functions. “Testosterone is considered to be the hormone of youth, so by decreasing its levels, some significant changes are generated in the patient, which are also accompanied by age,” he explained. It also has a great participation in mental, physical and biochemical functions, in the modification of cholesterol and hemoglobin levels. It is important to emphasize that it is not a change that occurs immediately, but that
evolution develops gradually, slowly and progressively. Those who are in this stage have a decrease in the pace of their activities, for example, they become more calm, sedentary, thinking slows down and loses mental acuity. There are also some physical changes such as loss of muscle tone, sagging skin, hair loss, and erection problems. ACCEPT IT AND SEEK MEDICAL HELP
The doctor indicated that the first step is to recognize that there is a problem and that health is at stake because in this way a health professional can indicate the appropriate treatment. Most of the time the biggest conflict they face is due to erection problems because they go from “it’s not going to happen to me”, or “I do not have that”, to blaming their partner for the situation. “They start to dress with very youthful clothes, to have a preference for sports cars and to relate with younger girls, they thing that

its their partner fault, that she does not exited him anymore,” said the doctor. The difficult thing is to make the decision because upon arriving at the office they realize that it was more their fear and the taboo of going to the doctor. “Daring to recognize that they are failing is the most difficult, the rest is easy, the support of the couple is very important, it is more than 50 percent, of course, as long as they want it,” he said. TREATMENT
When presenting the first problems of erectile dysfunction, you should go as soon as possible with the doctor to assign the appropriate treatment and prevent it from persisting and advancing the problem. The doctor said that the patient will undergo hormonal tests to verify their testosterone levels and will be assigned the indicated hormonal treatment, in addition to treating the dysfunction. Also, multivitamins will be prescribed and
in case the patient presents problems to sleep, which is not very common according to the doctor, will prescribe the indicated for it. “You should change your sexual activity; the heart and arteries no longer pump blood as before, the erection is slower and the technique has to be changed, they should take more time to play before penetration and that will help the erection to be stronger,” he said. He also said that “the older you have to add testosterone that also has its cycle, you do not have to give it permanently because it can cause problems, if you are a very old person, you will use viagra every time you have sex”. The urologist said that each patient is different, so each treatment is personalized, however, was emphatic to say that the ideal is to go with a specialist and not with a pharmacist, since you need to create a medical record to manage what be more beneficial Dr. Rogelio Gómez Loredo is at the San Francisco Medical Center, located at Pedro J. ¡Sí llega! pero no de golpe La andropausia representa un cambio en el estilo de vida de los hombres, similar a lo que les ocurre a las mujeres con la menopausia, sin embargo, a diferencia de ellas, se presenta de forma lenta y gradual.

‘I’m from here
and I'm not leaving' ”:
Juan Villarreal
In the second tribute that Reynosa gives to accordionist Juan Villarreal, he assured that he adopted this city to stay and live here, and here his musical career took off. By Alejandra Arellano Victor Briones photos Musicians, compadres and friends accompanied Don Juan Villarreal in the second tribute that the city of Reynosa gives him, for his more than 50 years of life on stage. Born in the ejido Veracruz y Progreso, a couple of kilometers from Nuevo Progreso, at 17 years of age he heads to Reynosa, where he is received with open arms, and which has since become his adopted city. When asked his origin, without hesitation he answers: De Reynosa! Even in Europe when it has been presented, it presumes that “from here I am meritito, from the cradle of the northern accordion.” As part of the celebration of the 270th anniversary of Reynosa, on March 14 in the main square, starting at 4:30 p.m., the event began with the participation of those who are more than their companions are their friends: Lalo Mora, Eliseo Robles , Los Barones de Apodaca, Los Archis del Control, Los Nuevos Relampagos, La Gente del Viejo Paulino and Los Duendes del Control, among other groups and soloists; all of them with the accordion, like the instrument that gave the note to the show. In the homage to Los Cachorros by Juan Villarreal, the vocalist surprised the audience with the presentation of “Los Hermanitos Villarreal.” “I did not know they were taking accordion and bass lessons with Valeria, and then I’m going to take them on stage to play here,” he said. In addition, he donated an accordion to the Reynosense Institute for Culture and the Arts in the interest of continuing the tradition among the new generations of young people. After 7:00 p.m., the mayor of the city, Maki Ortiz de Domínguez, expressed that “we are from here or we have come here, it is a pride to have characters like Don Juan, in him we have an example, the most valuable pledge to recognize us like Reynosenses, this is his house, his land, he is respected and valued ... He is the best accordionist in the world.” Before municipal officials, the delegate of Nuevo Progreso, Maribel Hernández, and an audience that has followed in the footsteps of don Juan Villarreal since its inception, passed the concert with the

“I am very happy with this second tribute that I am doing in Reynosa, it feels very nice, because I started it here and I’m to staying here,” he said. The most representative songs in his career were, among others, those that integrated his repertoire: “Knowing that it was me,” “I’m going to cut my veins,” “Baltazar ballasted them.” Before his presentation, don Juan Villarreal walked like any other passerby, dressed in a plaid shirt and jeans. People took pictures with him, and he agreed with a big smile. “They are 53 years of many satisfac-
tions, 312 recorded songs and many more that have not been recorded ... also movies,” he recalled. With a certain nostalgia, he recalled his youth when he decided to settle in Reynosa. “From here I am, since then I presume where I am, because Juan Villarreal’s Cubs have traveled all over the world,” he emphasized. With the simplicity that characterizes him, he answered that all his songs are his compositions, and he loves them in the same way because they have given him a lot of money, thank God, besides receiving countless applauses and expressions of affection. “It’s like when you have a good children, you all want them in the same way,
Singer-songwriter Don Juan Villarreal reported that they are scheduled to tour the United States and are recording an LP, (a term he later corrected), “or a CD, as it is called now.” They also have plans in the cinema but due to the current situation that Reynosa is going through, he considered it inappropriate to continue with the recording of a film of which he is the author of the plot and the synopsis. He summarized that it is the story of a very famous commander of Zacatecas, Zenón Torres Velez, in the presidential term of President Manuel Ávila Camacho. “But due to the insecurity, it was interrupted, as actors of the likes of Fernando Sáenz, Leonel González, and Lina Santos participate, just imagine if anything happens!” He exclaimed. Among his new musical productions, the run of “El mango” hopes that it will be a success.

Problems with
thethisknees? will interest you
Chondromalacia patellae is one of the most common conditions in the elderly, overweight patients, high-performance athletes and increasingly affects young people. The remedy in the voice of a specialist. By José Manuel Meza Reynosa Tam. In 2005, when the Argentinean star Gabriel Omar Batistuta retired prematurely from the soccer fields, he had just turn 36 years old (in which many are still active). His heart asked him to keep playing, but his knees were ruined. In his last season he only played three games and did not score any goals. A complete frustration for someone who had been a scoring champion in the Argentine Super League, in Italian Serie A and in the Copa América (twice) with his national team. Examples like that are too many. Not long ago his compatriot, Diego Armando Maradona, could not walk because of problems in the knees, but the reality is that this is a condition quite common not only in athletes, it afflicts people of all kinds. According to Armando Jesús Cantú André, orthopedic surgeon and traumatologist graduate from the Universidad Autonoma de Nuevo Leon (UANL), it is important to know the causes that originate it, as well as the treatments, because many people live with knee pain without know that the remedy can be simpler than you imagine. “One of the most frequent pathologies is chondromalacia, which is a wear of the cartilage. It’s where the kneecap scrapes and ends. It represents a problem not only for the elderly, but also for young people, football players, runners,” he specified. Some of the most common symptoms are pain and crepitus when knees buckle when flexed, up and down stairs, or when squatting. “The lower part of the knee is called the femur and the upper part is the patella, so this problem is also called patellofemoral syndrome,” he said. WHY DOES IT HAPPEN?
The explanation that Dr. Cantú André gives to the suffering of the knees is overwhelming. “Everything is due to wear and tear”, although the causes, he said, are multifactorial. “One of them is age. Other: overweight, excessive exercise, lack of exercise, feeding, falls, misalignment; those who have their knees
30
as charrito outward or inverse, or for inside as the one of ‘Quico’, the character of “Chavo del Ocho”. “Then the cartilage is scraped, which must be exactly aligned with the patella, but for some reasons is on the side,” he explained. The specialist said that because of this issue there are people who feel that they scrape bone with bone. “They feel a touch, pain in the first steps. Or when they sit for a long time when standing they experience discomfort in their knees, as well as those when they get out of bed. “They suffer discomfort when walking on steps, mainly when lowering them, because with each step they re-stretch and it hurts. “Those would be one of the first signs, so when you see and feel that way you have to rule out a chondromalacia. It’s the first thing we need to be thinking about,” Cantú André said. The doctor acknowledged that inflammation can also be a sign that is released by the shock between the bones. “More fluid is formed, the knees get big, red and this is known as synovitis, which is the inflammation of the synovial membranes,” he warned. He said that sometimes it is easy to realize who suffer from this condition, because instead of walking forward do not bend the knees and make a wobble at the sides to be able to move; However, not all of them present the same signals and some do not even know it, that is why it is important to seek help. WHAT HAPPENS IF YOU DO NOT DO A CHECK UP?
The interviewee pointed out that pain and inflammation will continue in a patient who does not receive medical attention in his knees, until it causes a disabling or wandering state. “The worst thing that can happen with this is pain and a lack of mobility. This will cause the knee to swell, to load more weight elsewhere. “First to make an excellent treatment we have to make a better diagnosis. Currently there are chondroprotectors. They are substances that cause the cartilage to come out again, in quotes, because the main challenge of the orthopedist is to avoid the loss of this tissue,” he said.
Jesús Cantú André.
Cantú André highlighted that to date a large number of studies have been developed to reach this achievement of medicine: replace the original cartilage with an artificial one. “These chondroprotectors are taken orally causing a tissue to come out as similar. “There are different formulas, depending on the state of wear of each patient such as medications with glucosamine, chondroitin sulfate, collagen, vitamin C, magnesium sulfate; substances that make them improve and the main thing to correct the damage that is had,” he said. The orthopedic surgeon said that not all people are candidates for a prosthesis. This is determined according to the type of damage that exists in the knees, as well as the invasive methods that must be practiced. “We are talking about how these surgeries can be small, like arthroscopy, which involves making a small wound of one centimeter in the knee, introducing a television camera, a blade, like a lawn mower and with that we aspire and clean everything the cartilage. “On the other hand, if there is a lot of wear, you can even change a piece of bone, place an implant instead of that cartilage,” he illustrated. WHERE TO GO
A very important point to consider is that in the border of Tamaulipas there are trained specialists to carry out the rehabilitation of the knees. Because the doctors are certified, they also receive foreign patients from the Rio Grande Valley. “The patellar chondromalacia should be treated by the orthopedic surgeon. He is in
charge of handling all these cartilage and injury problems. “As for the recommendations, what is gelatin, collagen, of course it benefits. I tell my patients, if it does not help it does not hurt and if it feels good to take it, definitely, “he described. The orthopedic surgeon guessed that in addition to the elderly, it is very common to receive patients who practice sports. “Those who have certain disciplines come with chondromalacia problems. I have here in my practice patients of 10, 11, 12, 13 years of age who are professional players, selected state that every day train and have injured knees and very young. “The girls from high school and the boys who are running at six in the morning. They do 10 miles on the morning and at noon another 10 miles, they obviously get hurt,” he said. MODERATE EXERCISE
The specialist remarked that the physical activities in which there has to be a use of the knees need to be graduated. “I just came from a congress of chondroprotectors and in the end one of the best surgeons there was saying: - sport is bad, exercise is good-. “Because everything in excess is going to damage the knees, that’s why the life of the players is very short, because they finish the knees, they finish the cartilage,” he said. However, when someone undergoes a knee restoration treatment, they can return to normal physical activities. “But it’s very important: all surgeries are worth 50 percent and the other 50 is rehabilitation. If the recommendations are followed, it improves life completely, because the simple fact of not living in pain changes everything,” said Cantú André. He asked this orthopedist to pay attention to knee discomforts, because a person who loses mobility, he acknowledged, tends to remain sedentary, triggering other conditions such as obesity, overweight, diabetes and high blood pressure. So if you want to have a better quality of life you should pay more attention to the knees and visit the specialist.

¿Problemas en las
LAS RODILLAS? esto le va a
interesar
A DÓNDE ACUDIR

A woman
with vocation and service
Who has been for almost two decades the administrator of one of the most important international bridges between Mexico and the United States says goodbye to her job by showing her gratitude and treasuring great life experiences. By José Manuel Meza Progreso, Texas The year 2000 not only marked the beginning of an era, but one of the most important periods of personal and professional improvement for a woman triumphing in the United States. Julie Ann Guerra Ramírez came to give a more human face to the international
crossing of Progreso, Texas, which links the border of Nuevo Progreso, Tamaulipas, Mexico. The challenge was to take down the old infrastructure and build a modern vehicular and pedestrian crossing as a secure platform for international-private-commercial transportation between two nations and this was: In October 2001, after a year of rigo-
NUEVO PROGRESO - Magazine | March 2019
rous studies, tests and multiple meetings every week, the new bridge began operating with an intelligent woman at the helm. For several years Julie Ann, originally from San Benito, had worked as a banker for the private sector and faced an ambitious and difficult task, to consolidate one of the most promising cross port in the country. Why in this area of the United States and Mexico ?, Because this region of South
Texas became a meeting point for winter tourism, which migrates every year from the north of the continent, looking for friendlier areas to pass the low temperatures of the north. And on the Mexican side, because it’s right there, in Nuevo Progreso, where a prestigious Medical Tourism cluster was installed. Intelligently developers and chambers of commerce saw an opportunity to
37
bring vacationers the services they need such as opticians, dentists, pharmacies, crafts, gastronomy and liquors, at a competitive price. Capitalizing on these opportunities, we looked at the need to have a more efficient international crossing, which would give fluidity to an increasing number of people interested in tourism on the other side of the border. A WOMAN COMPLIED
It was there that Julie Ann played a very important role. She was chosen for her warmth and vocation to service and for her tenacity to attend to problems that may arise. Not long after she began her work that year 2001, an event occurred that for her and her companions represented a litmus test: the terrorist attack on the World Trade Center in New York was a severe blow for the economy of the border region and for the mood of the Americans. Fear seized millions of people. The country was under attack. Changed the ways of tourism, toughened security controls at all entrances and exits of the nation and collapsed the number of visitors who used to use the International Bridge of Progreso, customs were in check. Julie Ann and her work team faced this new reality, but never lowered their arms. The crossing continued to operate, opening daily, 24 hours a day, seven days a week, 365 days a year. There was a lot of uncertainty about the future and the safety of the citizens. Even though very few had the courage to go on vacation, this woman worked to manage difficult times. Far from distancing relations abroad, Julie Ann shook them, organizing events and humanitarian donations. Gradually, confidence returned. The people understood that life had to go on. She recalls that since the international bridge has been in Progreso, the only time it closed was on the day the assassination of President John F. Kennedy happen. From then on, it has always been open to provide a courteous and effective service. Even when the new port was rebuilt in 2001 there was a way for pedestrians and cars to cross. The bridge that was before was torn down completely, but there was a part that stand so as not to interrupt the circulation. In fact, in the pedestrian area, today is not only one of the international ports that seasonally becomes one of the busiest in the world, but also crosses a large number of vehicles and cargo transport. That is why the effort of 19 years at the head of this bridge has been worthwhile and in the region there are many people who recognize the work Julie Ann has done since being in charge of this company. “I came here bringing some ideas of

what I already knew, how to run the operations, of running a business. I was mixing it here with everything ... and it went well,” she said. WITH HUMAN PROFILE
The Nuevo Progreso International Border is one of the three private bridges that exist on this border, but Julie Ann not only took charge of administering it charging tolls, but she always did a humanitarian and social work, close to the Mexican counterparts. “I feel very proud to be part of this work. When I arrived here 19 years ago I was approaching with Nuevo Progreso little by little, and for me, they are my second family, because they have seen me well, I have seen them well. “We have always been involved in other causes and associations, such as the Mabel Foundation, having the privilege of participating in supporting people in need,” she said. This bridge has not only allowed to build a solid trade in the area, but it has given happiness to many people. To the “Winter Texans” who enjoy their leisure seasons and those who use it to visit their loved ones or bring them gifts. The reasons can be many and for each there is a good reason. In addition, Julie Ann has shown leadership and vision to provide good service to the transportation system. There are companies that prefer to cross their products here or return their empty containers, because logistics is better than going to Reynosa or Nuevo Laredo. “We have worked with the different go-
vernment agencies, also providing the facilities to the transport companies. I believe that my greatest satisfaction in all these years has been being able to attend to the people as friendly as possible, that was always my priority. “And this is what I think the users want to receive, a warm welcome. It’s something I’ve told my workers, when I have meetings, a friendly smile, welcome, that means a lot to the people,” she said. SOMETHING TO BE PROUD OF
This is a privileged point of the nation, because not every city has an international bridge. Many cities have applied unsuccessfully and Progreso has it. “Mr. Sam R. Sparks had a vision of golden eyes. He was very helpful with people. May he rest in peace, I followed his goal as if he were here,” said the first director of the new crossroads, who has done a great team effort, keeping the bridge working all the time as an achievement. “We are seeing a lot of people flow. Obviously with the tragedy of 9-11 people got scared, they did not want to come, they did not know what was happening. Then the passport rules came in, which was very expensive. “Since then the flow has dropped quite a bit, but in the last five years we have seen very good capacity and we have many people crossing, both those from here and those who come from Mexico. A month ago on a saturday more than six thousand pedestrians passed,” she said. Julie Ann also acknowledged that
throughout her experience as director of the international crossing have been able to introduce the necessary improvements for users, in signs, ramps for disabled people and the pedestrian shadow. While on the family and cultural side, this infrastructure stands out as a point of stroll and union in which the bonds of hospitality are tightened and important events in the lives of people have been consummated. “Many simply want to see the river, stop in the middle, take pictures. We have had weddings at the bridge. Mexico’s boyfriend or girlfriend comes and they get married right there in the middle. The experiences that have been given here are quite nice and beautiful,” she said. That is why for Julie Ann there is a link that will always tide her to the international bridge of Nuevo Progreso. The time has come to assume other roles, dedicate more time to her family and culminate this professional career with golden letters, on a very important date as it is on March 21, which celebrates Tourist Day. “I hope to see you here. I’m going for personal things and I want to be with my husband (Mr. Roberto Ramírez) at a very important moment, but I do not say goodbye, but see you soon, God bless you! I will always appreciate all your attention”, said Julie Ann Guerra Ramírez, a beloved woman on the border of Progreso, Texas and Progreso, Tamaulipas, and known by merchants and tourists from various cities in the United States, Canada and Mexico for having contributed to the development and welfare of the region.

For the best smile,
Nuevo Progreso
This town on the Mexican border has numerous offices equipped with modern infrastructure and certified specialists for the care and restoration of dental pieces. Not in vain is it known as “the Medical Tourism Capital.” By José Manuel Meza Nuevo Progreso, Tam. Who has the means does not think twice: it doesn’t matter if you are now in Melbourne, Australia, the next day you take a commercial flight to do business in the United States and take advantage of the visit to the American Continent to go to a small corner of the world that has fame for its excellence in oral health care. Near the metropolitan area of McAllen, Texas, crossing into Mexican territory is Nuevo Progreso, Tamaulipas, a town that is not known for having beautiful beaches, much less spectacular casinos, but every year it is visited by thousands and thousands. of people looking to show off the best teeth and not spend so much money. That already makes it different, because it concentrates the best specialists in the field. For this reason, Juan Ricardo Badillo, patient coordinator for Dr. René Rosas, at the Artistry Dental Center, extends a cordial invitation to learn about Nuevo Progreso. “Of course I would invite them, to come, because we have the best prices on dental matters, plus there are many other services that can be found here, pharmacies, opticians. “Each one has his specialty, the orthodontist, the periodontist, rehabilitation doctors, implantologists, depending on the need of each patient here we can attend to them,” he said. CHANGES LIFE
This assistant, who has about 15 years of service to the community in Spanish and English, said that most of the people who come to Nuevo Progreso to solve a dental problem are “Winter Texans”.

He noted that some enter the office with the parts of his mouth very damaged and after receiving the treatment his mood is completely different. “We see it on a daily basis when they (the tourists) come. Almost everyone leaves happy with the work we do for them. They go away happy, because when they do not have good teeth or a good smile their self-esteem goes down a lot, and being restored means a lot, because their happiness returns,” said Juan Ricardo. The interviewee remarked a very important aspect: that is that the most effective marketing strategies can be taken, but the best publicity ends up being what patients do by telling others, like their relatives and friends. He said, speaks of the good work that is done in this town on the Mexican bor-
der with patients, many of whom return year after year. PATIENTS FROM AUSTRALIA
Juan Ricardo mentioned that one of the benefits of the globalized world is to connect the patients of the world with Nuevo Progreso, and that is what the medical tourism cluster has a lot to do with it. Doctors often point to patients who for years have come from so far to care for themselves. “In fact, the farther we have had a patient is from Australia, which arrives in Houston and from there he comes here. We have also received several patients from Alaska and from Canada and the United States. Thank God many people visit us in our humble town,” he added. In addition to inviting tourists in ge-
neral, as well as the “Winter Texans” who still remain in the region, the representative of Artistry Dental Center wanted to highlight the equipment available in the diagnosis and treatment of teeth. “We have the laboratory, which is high-tech computing. We have scanners, the printing models, they are put on the screen and we give the best perfection to all the crowns and zirconias; lithium and e.max, and all the materials with which we work here. “This gives our patients a better reassurance that they are in good hands; no matter where they come from, they will be treated in the best way,” he said. Thus, very close or far away, Nuevo Progreso is consolidated as one of the most important dental care destinations.
