WHAT ARE THE CAUSES OF PRESBYOPIA?
The transparent lens rests on the inside of the eye behind the iris and changes shape to focus the light on the retina so you can see. When a person is young, the lens is soft, flexible and can change shape easily. This allows you to focus on objects both near and far. After age 40, the lens becomes stiffer and can not easily change shape. This makes reading difficult, it becomes more difficult to perform tasks that require close observation. There is no way to stop or reverse the normal aging process that causes presbyopia. However, presbyopia can be corrected with glasses, contact lenses or surgery. If you do not correct presbyopia, you may feel discomfort caused by headaches and eye fatigue. WAYS TO CORRECT PRESBYOPIA
Use reading glasses: If presbyopia is the only problem in vision (you do not have nearsightedness, farsightedness or astigmatism), you may only need glasses. Reading glasses help correct vision problems by bending (refracting) the light before it enters the eye. If glasses are already used for other vision problems, the use of bifocal, trifocal or progressive lenses will now be needed. • Bifocals correct vision from near and far. A line that can be seen or not, divides the lens. The lower part of the lens refracts light for near vision. The upper part refracts the light so that you can see distant objects. • Trifocals have three zones on the lens to correct near vision, intermediate vision, and far vision. • Progressive lenses correct vision such as bifocals and trifocals. However, instead of having a line that divides each refractive area, the refraction changes gradually in the lens from top to bottom. Contact lenses: Some people prefer to wear contact lenses instead of glasses. There are two types of contact lenses that help treat presbyopia: • Monovision contacts. These contact lenses correct the vision of one eye to see far and the other to see up close. • Multifocal contacts. These lenses have different rings or zones with different graduations. With this design, both near and far vision are used at the same time. Refractive surgery: Some people decide to undergo surgery to get monovision. This can reduce the need to wear glasses to see near and far objects. With a laser, the ophthalmologist changes the shape of the cornea for clear vision from far in one eye and closely in the other. The ophthalmologist may recommend that you try monovision lenses before undergoing LASIK surgery. In this way, you will know if monovision is a convenient option for you. ¿Qué es la presbicia?
L
Shady celebrations
The effects of the december parties
Despite the atmosphere of celebration and joy that is experienced at Christmas and New Year, some people experience emotions such as sadness, melancholy and depression. By Beatriz Flores With the arrival of Christmas and the New Year the atmosphere changes, it becomes festive, full of multicolored lights, there are many parties, hugs and gifts, but is everything happiness? for most, but not for all. It is very common for some people to experience feelings of nostalgia, sadness or even depression. Psychologist Arely Martínez Jiménez, explained that they can be presented by three main factors, the family, the social and economic that practically go hand in hand. She said that there may be problems in the family or that some member is no longer present, also because of the pressure that society makes on how to live this season or forget the true essence of Christmas, which is to share the good times with loved ones instead of focusing on the material. Contrary to what one might think, not only adults go through these situations, children also do it. “The children can suffer due to the lack of a family member, or because they do not have the gifts they would like, there are some shortcomings, in this case it is not so serious, but remember that these are sad situations that affect them because of their age,” she said. This can be identified because people manifest sadness, crying or absence of the group gathered. WHO CAN SUFFER IT?
People who have lost a relative, who live family situations such as fights, arguments, or differences that perhaps for some may not be so serious, but the affected person does. “Also those who are insecure or who have some need of appearance, or some ‘masks’,” she said. The psychologist said that it can be considered that there is a higher incidence in women due to emotional and socially permissible situations, that is, although men also present them, they do not manifest them so easily.
4
“Remember that from boyhood to men they are repressed to be able to manifest an emotion,” she said. WHAT TO DO?
Martínez Jiménez pointed out that the ideal is to learn to overcome the depression situation and to understand that the season motivates us to share and live with those we love despite the absences or sorrows that we have. “We have to take into account that we can and should live with sadness, but also learn to be grateful for those who are with us and those we have, even with shortcomings, to approach people and try to integrate those who leave, but not invade them or get angry if they do not want to do it,” she said. She added that it is very important to give their space to people so that they can express their emotions, it was also emphatic to say that it is not right to ask them to repress their feelings. “Emotions, however unnecessary they may seem, must be manifested, so that in this way they can value those who are next to them and give thanks for that,” she said. The specialist in behavior modification, said that when the depression manifests itself, not only by the approximation of these dates, but also much sooner or later or there is no interest to try to overcome the situation, professional help should be sought. The most important thing, she said, is to recognize that there is a mental problem, to understand that more people suffer from the situation and to thank those who stay close to help. On the other hand, the support from family and friends towards who suffers a depression is very important to make them feel that they are not alone. Listening becomes very important for those who experience an emotional state that is not optimal, although for a person who does not deal with it, it is too simple or unimportant.
Finally, she commented that respect is fundamental for the proper development of human relations. “When we speak of respect, we mean or do not belittle the person’s emotions because we do not all express the same feelings, or with the same intensity,” she said.

New procedure to lose weight,

surgery At the beginning of the year, one of the main purposes is to lose weight, either for aesthetics or health, and thanks to advances in technology, it is possible to achieve this through a new method in bariatry and with extraordinary results: the endoscopic gastric sleeve. By Pamela Escobar It is a minimally invasive procedure, which is done through the mouth. Using an endoscope, the size and shape of the stomach is modified without injury to the abdomen and without altering the functioning of the abdomen. The endoscopic gastric sleeve is performed in a few places in Mexico and Europe, but it has reached our city thanks to Dr. Juan Carlos Caramés Aranda, Surgeon General and Interventional Endoscopist of the Interventional Endoscopy Center of Reynosa.
AHEAD ON MEDICAL AND TECHNOLOGICAL PROCEDURES 4The Center for Gastroenterology and Interventional Endoscopy, in the Del Prado neighborhood in Reynosa, has the only dualchannel endoscope in Northern Mexico. With this high-tech endoscope, Gastric Sleeve Endoscopy is performed, a procedure that began only five years ago in the United States in two hospitals, one of them, the Weill Cornell Hospital, where Dr. Michel Kahaleh works, with whom he is associated. Dr. Juan Carlos Caramés to perform Endoscopic Gastroplasty procedures.
PROCEDURE AND RECOVERY
The doctor explained the advantages of this new procedure that began in the United States; by means of which modifications are made in the shape and size of the stomach, without making incisions in the skin of the abdomen; introducing an endoscope through the mouth with a sophisticated suture system for the modification that will reduce up to 80 percent the capacity of the stomach. “The duration of the procedure is about an hour and a half, depending also on the size of the stomach and its length, in addition to the number of sutures that have to be performed, because in each patient it is different,” explained Caramés Aranda. He added that the surgery is performed under a general anesthesia in an operating room, but with a very quick recovery, because in one of the most recent cases, after three hours of having performed the endoscopic gastric sleeve, a patient in his room was sitting playing video games with their children, just waiting the next day to go home. “As a precaution we leave the patient 24 hours under observation and hospitalized, although he is only waiting for the time to pass, as there are no wounds, no pain or discomfort and he must be fasting. It is until
8
the next day when it starts with a soft diet (water, tea and gelatin) and if the patient tolerates this well, he is discharged to go home and on the third day he can rejoin his daily tasks,” the doctor added. ADVANTAGES OF ENDOSCOPIC GASTRIC SLEEVE
Dr. Juan Carlos Caramés Aranda highlighted the advantages of endoscopic gastric sleeve over other procedures to lose weight since there are three techniques to perform bariatric surgery, one is the endoscopic gastric sleeve, the same surgical gastric sleeve that is performed laparoscopically and finally Gastric bypass by the same route. “The big difference between the endoscopic gastric sleeve (oral trans) and the other two procedures is that nothing is being alte-
red in the function, digestion and absorption of the digestive system, but only with the endoscopic gastric sleeve what we are doing is to decrease mechanically the size of the stomach,” he said. He said that this leads the patient to eat a small amount of food and feel satisfied quickly and lose weight. “We are not cutting or removing part of the stomach, or altering the mechanism of digestion or absorption; that’s a very big advantage of the endoscopic gastric sleeve,” reiterated Caramés Aranda. FOLLOW UP PROGRAM
Endoscopist Juan Carlos Caramés Aranda, detailed that the endoscopic gastric sleeve is part of a complete plan. It is a support program in which the patient is trained in his
eating habits, exercises, psychological support, to help the patient in case of requiring medications to calm the anxiety. “It is necessary that the patient remains in contact with us, at the beginning, we do the endoscopic procedure, the follow-up is constant and includes that many patients have significant obesity and that consequently they suffer high blood pressure, diabetes or other disorders; same that are the foundation to carry out the procedure and that will help to heal of these problems by decreasing weight in an important way,” detailed Caramés Aranda. Emphasized an important point in the plan, which is the use of body creams that help lose weight faster, in addition to others that serve to restore skin tone, so that the patient although lose a lot of weight, skin tone up

and do not become flaccid or hang He added that at the end of the year of follow-up this patient changed his mentality, learned to eat, has a different lifestyle and continues to lose weight, as well as controlling the ailments that could lead him to perform the procedure to improve health, achieving thus the primordial objective, which is to have a better quality of life. “When removing the gastric fundus; we eliminated where substances are secreted that will give the brain the information of feeling of appetite or of fullness, of when it is already satisfied; and even to say at the end of the food that they were rich,” he said. While the second, during surgery, the shape, function and process of digestion and absorption of the stomach and small intestine are modified; because in the first sixty centimeters of the small intestine after the stomach is where more than 60 percent of the total absorption of the digestive system takes place. “ It is call Gastric bypass because that segment is skipped from where the greatest absorption takes place and then only a portion of the stomach is left, a segment of intestine is brought up to join it with that small segment of stomach that remains and the whole absorption system,” he said. For this reason, the doctor emphasized that the patient is denatured, has demineralization, decreases calcium, the patient looks aged and tired, his hair falls off; all these alterations will never be seen with the endoscopic gastric sleeve because the digestion and absorption system is respected; only the form of the procedure is modified, this being the biggest advantage. THE PROFESSIONALS WHO PERFORM IT
The endoscopic gastric sleeve procedure should be carried out in a hospital, in an operating room where surgeries are performed, since that is what it is, but it is done through an endoscope, without a single incision in the abdomen. It requires a very sophisticated dual-channel endoscope and a new suture system called Apollo System. “In addition to the fact that it is necessary for those who perform it to have an arduous training and understand the physiology of the stomach to achieve weight loss, without altering the function,” said Juan Carlos Caramés Aranda. THE REDO
The specialist in interventional endoscopy Juan Carlos Caramés Aranda commented that currently there is a large percentage of people who underwent a surgical gastric sleeve and after a while have gained weight again, even more than before their surgery. He added that currently the problem can be reversed with the endoscopic procedure of “Redo”, which consists of suturing the inside of the stomach and reducing its size again.

ADVANTAGES AND BENEFITS
4Procedure to lose weight accelerated. 4No incisions, not even superficial, so there are no scars. 4With a duration of about an hour to an hour and a half. 4Does not cut the stomach, so it does not affect the digestive process and absorption of nutrients. 4Immediate recovery. 4The postoperative follow-up avoids sagging and hanging the skin, avoiding large surgeries in the skin due to excess.
“It is done through an endoscopy, which represents a great advantage because if it is done by open or laparoscopic surgery, you will find fibrosis, adhesions or scarring around the stomach due to the previous procedure,”said the doctor. He also said that there are patients who have undergone gastric bypass surgery, who ingest more food than necessary, causing the union of the stomach with the intestine to become larger and gain weight again. “And then with the Endoscopic suture with the Apollo system, we can reduce again the size of that anastomosis (the union of the intestine and stomach) to close it to the original size that is around one and a half centimeters or two maximum and then the patient will lose weight again,” said the interviewee.
copy in the United States, while in Mexico we are only doing it at the Center for Gastroenterology and Interventional Endoscopy in the Del Prado neighborhood, due to the ease we have to be able to obtain the endoscopic suture,” explained Caramés Aranda. He also mentioned that it is not just about having the double channel endoscope and the suture system; it requires a very arduous and important training to be able to learn to manage the system and understand the physiology of the stomach, the pros and the benefits of these procedures; understand the alterations that are made with the other procedures and undergo training of this endoscopic technique to be able to offer it to patients. SUCCESSFUL PROCEDURES
He mentioned that among the patients who have undergone this procedure in Mexico and the United States, there are already six patients with excellent success, there have been no complications, everyone is satisfied; the one that has lost more weight has more than a year since the procedure, has lost almost 60 kilos. “That patient is very happy and even his wife says she does not know him as he is now, because his personality has changed as well as his sense of humor. Now he looks and acts differently,” he added.
PREPARATION OF THE PATIENT
The Interventional Endoscopist commented that the patient does not require any special preparation, should only eat something light, but no rigorous effort or that requires modifying habits or dieting. “However, the patient should be studied well before the procedure, since a previous endoscopy should be performed to identify the digestive symptoms, as the great majority have ulcers or gastritis, an infection caused by a bacterium called Helicobacter pylori, which must be treated. be taken care of before carrying out the Endoscopic Gastric Sleeve,” specified. PIONEERS IN THE PROCEDURE
The endoscopic gastric sleeve procedure is about five years old and began in the United States, in two hospitals, one of them was the Well Cornell Hospital where Dr. Michele Kahaleh works, a pioneer and friend of Juan Carlos Caramés Aranda who in partnership perform said procedure in Reynosa. “Dr. Michele Kahaleh was the first to perform a Gastric Sleeve procedure by endos-

Because looking good is also a way of fighting, the Red de Emergencias Reynosa, A.C., collects hair and now produces oncological wigs for people with limited resources. because once the braids were collected they had to be sent to Ciudad Victoria, to the State DIF and from there to Puebla to an association that made the wigs. “The wigs were made and channeled to people who needed them, but they practically did not stay here, they went to other places,” said Ramírez. For that reason, the founder of Red de Emergencias Reynosa attended training for the development of oncological wigs and that, in this way, the people of Reynosa who needed them could have access to them. In October, due to the International Day of the Fight Against Breast Cancer the campaign was resumed and photographs were taken to the donors with a thank you to share it in social networks, which generated a good response in society and prompted to join. “In October we gathered eight braids and at the beginning of November Leticia started making the wigs here, so it was decided to implement the campaign permanently,” she said.
By Beatriz Flores Because facing cancer is not only fighting the disease, but also having good emotional health and strong self-esteem to win the battle, Red de Emergencias Reynosa, A.C., launched the Permanent Hair Donation Campaign. Its purpose is to collect hair for the realization of oncological wigs for women who have lost it during cancer treatment. FROM REYNOSA FOR REYNOSA
Leticia Larios is the founder and president of the Reynosa Emergency Network, A.C. and who along with other volunteers implemented this campaign. It was in May of last year when it began after a person with limited resources with cancer came to ask for help and the Oncology Center of Tamaulipas channeled it to Ciudad Victoria to be attended. “The lady did not have insurance, nor Seguro Popular, hence the idea of doing a campaign for hair collection, because in addition, the association seeks to help vulnerable people,” said volunteer Karina Ramírez. Her entry into the association was four years ago, after finding a publication and since then she supports the toy collection campaign that takes place in December and in the school supplies that takes place in June and July. The president of the association, Leticia Larios said that what is intended is “create the culture of hair donation because we know how important it is to support people who are going through this stage and especia-

The Miravalle Center, the Universidad Interamericana del Norte and Bella Imagen Salón, served as collection centers during the campaign, in the last business, people could come to donate their braid and the haircut was free. Until Bella Imagen Salon, located in Balcones de Alcalá and owned by Karina Ramírez, they arrive on average three people a week to donate their braids and later it is picked up by Larios to make the wigs. People wishing to donate their hair should wear it clean and preferably not dyed, although Karina Ramírez said that she does not necessarily have to be a virgin and that if she is dyed she is not very blond. It must also be healthy, resistant, not too brittle or dry. The braid should measure at least 25 centimeters, however, they are also accepted up to 20 centimeters because the short Enero 2019 | FRONTERA DOCTORS HEALTH MAGAZINE

hair is used to make fringes and beanies. “With 25 centimeters you make a wig with shoulder length, there are very thin braids and other thicker ones, but on average to do one they occupy between five and six”, she detailed. They are made with a special Lycra type fabric and materials that could put patients’ health at risk are not used. Leticia Larios handcrafts them, as she does not yet have the necessary machinery. They should be given some care for their preservation, for example, washing the hair twice a week, but do not wet the part of the cap and untangle it with a wide bristle brush. If treated properly the oncological wig can last up to a year or even longer. “We have had more affluence of girls and adolescents, but it does not matter how old they are to donate their hair,” Karina Ramírez clarified. However, those who do not meet the requirements to donate their hair can al-

so support the campaign by donning scarves, caps or hats. Those interested in donating their braid can take it in a Ziploc bag to the Interamerican University of the North, to the Miravalle Community Center or to Bella Imagen Salón, where they also use Mondays from 4:00 pm to 7:00 pm to cut their hair completely free. “Anyone who is going through some oncological treatment and needs a wig or winter hats we have available and if we do not have their measure they are taken and placed on the waiting list while it is being prepared,” Larios said. The Reynosa Emergency Network has been working for about three years with approximately ten volunteers in order to provide support to vulnerable people who suffer from different situations. “We promote the love for our most needy neighbors with volunteers who volunteer their time and support in a disinterested way,” said the founder.

December A cycle begins, as well as new purposes and one that almost never lacks in the list is to lose weight but, according to a specialist, the excesses of the December dates are the bad precedent with which some started the 2019 in trouble of obesity and, even, with diabetes.
years old, and the question that a pancreatitis can trigger diabetes, because of the pressure on the pancreas that causes its functioning to be affected,” he warned.
By José Manuel Meza The care of the organism in the feeding is an aspect that is neglected especially in the last months of the year and 2018 was not the exception. The colorful atmosphere that surrounds the holiday season, the posadas and the arrival of the New Year have implicit celebrations and also exquisite meals. The cakes, desserts, tamales, turkeys, pastas, punch and other cravings dominated this season, and even with reheated and Dia de Reyes (days of kings). However, not everything was happiness and mainly for those who have health problems related to food or for those who ate excessively and now want to take care of their figure. “Everyone has in mind that December comes and you can not diet, you will consume everything, because at last it party time ... So many are the ones who think they should let themselves go and eat until they are satisfied. “And if we analyze it well in that month there are two main dates, which are the equivalent of other months where we have dinners like that, but the detail lies in that they also grab the posadas as a pretext to unleash everything they have not done in the rest of the year and that’s where the problems come from,” said Jorge de Jesús Garza Flores. The graduate in Nutrition and Food Sciences from the Autonomous University of Tamaulipas (UAT), educator in Diabetes from La Salle University and Master in Public Health from the Universidad Mexico Americana del Norte (UMAN), said it is precisely at this ti-

me when people end up in hospital emergency rooms, because they have many high-pressure problems and diabetes. “These days they eat too much and their glucose, cholesterol and triglyceride levels are altered. Then they come with hypertensive crises, very high glucose levels that lead to diabetic ketoacidosis. “Prancreatitis is something very common and often spoil the celebration to others, because they must move emergency at midnight or very early to the hospital,” he said. END UP WITH DIABETES
Therefore, the specialist since before the holidays began stressed to his patients that they needed to be very careful, because too much food or excessive intake of alcohol can control the body and cause medical complications and even lead to diabetes. “In general, it must be understood that someone with one of these chronic diseases must take the recommendations to take ca-
re of the amount of sodium, sugars and fats. These three things everyone should attend to, regardless of whether or not one of the aforementioned ailments is present,” he insisted. Garza Flores argued that during the festive season there were people who were already in the levels as prediabetic, but as a consequence of food abuse they started 2019 as new patients with diabetes or hypertension. “There are people who can be sick and do not feel sick, who can suffer from high blood pressure, cholesterol, triglycerides and sugar and do not realize it. “And there are those who are in the limit and those days consumed large amounts of sugars, fats or salt and they shot the disease, becoming visible with the symptoms and from that moment it is triggered. “There are people who I have seen that come, patients who come to intensive therapy because of a very large dinner with giblets, pozoles and things like that, that end with pancreatitis and young people, 20, 22
The interviewee argued that some of the foods that contain a lot of fat are menudo, meats and ribs too, as well as pastas with creams, butters and dressings. “There it is advisable to go to the hospital to check the pancreatic enzymes and check if they are elevated. There are conditions in which we do not reach such serious situations, but that do require a hospitalization, such as alcoholic congestion and gastritis, esophagitis, colitis, reflux and other very annoying symptoms that can cause severe pain due to the consumption of fatty, irritating or too many condiments. “It is necessary to reduce inflammation of the digestive system, relieve pain. It does not matter that people do not have a chronic degenerative disease and avoid eating as much as they can,” he said. About the beer the nutritionist pointed out that it contains sugars and alcohol, with many calories that affect the pancreas, as well as the liver. “We must be careful with the quantities. The recommendations in general are very low for what the population is used to, which are two servings of alcohol for men and one for women. “But at a party they consume larger quantities individually and there are those who, understanding that, think not to drink beer, but

some other distilled beverage such as whiskey and tequila, because it has fewer calories. “However, although distillates do not carry as much sugar, the concentration of alcohol is much higher. The volume in a beer can be 3 to 5 percent, while in other drinks is 40 percent, and often what they do is combine them with sugary drinks such as soft drinks, juices and is a bad combination,” he illustrated. KNOCKING DOWN MYTHS
On the contrary, Garza Flores said that there are those who mistakenly, in their intention not to gain so much weight on the December dates fasted throughout the day. “But it must be the opposite: do not stopped eating to wait for dinner. “The recommendation was that in the course of the day should take a regular diet and eat a small snack to avoid consuming large quantities. “We refer to a portion of fruit, sugar-free yogurt, some gelatin or salad with lemon, cucumber and jicama to try to reduce the feeling of hunger and when dinner comes we behave and eat only the necessary portions,” he reiterated. The dish, he said, should never be over-filled, but contain well-distributed foods. “Half must have vegetables. The other one needs to be divided into two parts: a quarter of carbohydrates (rice, spaghetti, mashed potatoes) and the other quarter of the plate of products of animal origin (turkey or other meat that will be consumed). “That way, what we do is reduce the amount of fats and carbohydrates and fill the volume of the stomach with vegetables that are low in sugars and fats with the rest is satisfied without incurring excess calories. Always that proportion is the most appropriate where we go, even if we are partying or not,” said the nutritionist. WHATS NEXT?
The 2019 arrived and with him, according

to Garza Flores, the purposes that were supposed to have been made since last year or the year before must be fulfilled. “Weight and chronic degenerative diseases do not know about dates. What to do ? what we did not do. It’s very simple: try to do five hours of activity a week. Eat properly, increase vegetables and low-fat animal products (protein) and moderate beans, rice, tortilla, bread, pasta (carbohydrates). “If we do a constant physical activity and a good distribution of food I do not have to worry about anything, the problem is that we are very used to taking many sugary drinks and then combinations of fast food with many calories and sugary drinks are amounts of energy very large that not spend them are transformed into fat and we began to accumulate weight,” he said. Therefore, the Master’s degree in Public Health said that it is important to understand that the fact that someone feels at optimal state does not mean that they are well and that diseases can appear from one moment to another, so risks need to be reduced. “A good diet and physical activity is always the basis of any treatment. When we do not know the consequences of our actions, we do them, but once we have the information and we understand that what we are eating can affect our health it is easier for us to become aware and try to carry out the recommendations. “It is very valid to use the glucometer, especially if we have diabetes or manage high glucose levels. Must be known to use in the right times and will give us very valuable information to know how much we can or do not consume carbohydrates,” he recommended. In the end, Garza Flores praised that if glucose levels are already high, it is an indication that you should not allow yourself to eat too much, as well as those who want to start 2019 looking after your health, because otherwise, the 2020 more for unfulfilled purposes than for fulfilling.
Enero 2019 | FRONTERA DOCTORS HEALTH MAGAZINE
FRONTERA DOCTORS HEALTH MAGAZINE | Enero 2019

NEPHROLITHIASIS Bad habits
couses it
Stones in the kidneys are a condition that affects overweight children, with bad eating habits, by consuming junk or processed foods, with large amounts of salt, soft drinks or low water consumption. By Pamela Escobar Properly kidney stones or kidney stones by definition as such, does not exist, but this pathology is due to the association of crystals, minerals and/or components that are acquired in the diet, certain metabolic or digestive effects, which give rise to the formation of what is medically called nephrolithiasis or nephrocalcinosis, explained Lemuel Obed Meza Suárez. The pediatric nephrologist added that although this condition does not occur only in adults, but also in minors, where the symptoms vary depending on the age of the patients. He commented that in general, older children and adolescents produce the typical picture of nephritic colic and its diagnosis is simpler: pain in the lower back that radiates to the pelvis and genitals, difficulty urinating, bleeding from the urine, nausea, vomiting. “In the case of pediatric patients present abdominal pain and in the back or flanks associated with vomiting; in some cases, dysuria appears, that is, burning during urination or the sensation of constantly going to urinate, are subtle manifestations, sometimes without pain or other apparent manifestations,” he explained. He added that there are factors that influence the development of pathology in children, such as low fluid intake, who are out in

the open or who exercise or exercise constant physical activity. “These are the patients who in the future are at risk of developing kidney stones, in addition to other partners such as genetics, that is, with a family history with the same disease,” he said. THE MALES ARE A TARGET
Regarding the sex in which it occurs with greater frequency, it was noticed that it has been more predisposed in males, mainly because they initiate their sexual activity at an early age, associated with the infectious. He explained that lately, the rate of this type of pathologies has increased, in comparison with the 90s, due to the dietary changes of the minor patients, as well as certain physical changes. “There is also overweight in children, poor eating habits caused by eating junk or processed foods, with a large amount of salt or consumption of soft drinks, which increases the rate of kidney stones or kidney nephrolithiasis in the same way to those who consume very little liquidS,” he said. THE INFECTION
After mentioning that the etiology of the condition is caused by environmental, dietary, infectious, metabolic or genetic factors, he warned that nephrocalcinosis develops by infection, primarily, by acidifying the urine. “If the pH changes, that is, increases the acidity in the urine, it causes a different concentration in the liquid waste and causes infection, although bad hygienic habits can also be Enero 2019 | FRONTERA DOCTORS HEALTH MAGAZINE

acquired,” he said. He indicated that the existence of a risk factor that is associated with future diseases since the nephrocalcinosis can be treated at an early age, the disease may evolve in the obstruction or calculus in the kidney, or the organ may end up with a chronic kidney disease. “That is the main risk in this type of patients; what we try to avoid is that the kidney is injured and the patient ends up on dialysis or hemodialysis and a poor quality of life,” he said. DIAGNOSIS AND TREATMENT
In order to detect this condition, the symptomatology must be perfectly identified, according to Meza Suárez, it begins with affectations in the daily life of the minors. “There are patients who only arrive with alteration in the color of the urine, this can cause not only in the patient a certain fear of being bleeding from the waste, so risky that this may represent, but also the family member takes it to medical attention,” he commented. He explained that the change in coloration in the urine scares the patient about something very serious that affects health and can be confused with a tumor or something else. However, this may be the reason why this type of patient is studied in detail because a bleeding in the urine is not normal and he said that it does not mean that the patient is going to bleed, but to establish the origin, why. We mainly start with a urine test, to detect certain particles called solutes or densities that are kept in this liquid and these give us crystal formations and crystallize,” he explained. He added that in case of doubt, taking into account that lithiasis and urine infection can occur in the same child, one as a consequence of the other, antibiotic should be added to the treatment. “Another study that can be done for a good diagnosis is an abdominal ultrasound. In some cases it may be necessary to perform other imaging tests, such as Uro-TAC or magnetic resonance imaging,” he explained. He said that in case of recurrent lithiasis, or if there is a family history, it is recommended that the child is evaluated by a specialist in childhood nephrology. Since it produces intense pain and can affect the general state and hydration in children, hospitalization is usually required to administer intravenous and anti-inflammatory fluids and/or analgesics. Small stones tend to pass spontaneously, with urination. The larger ones usually need treatment with lithotripsy (a technique that favors the fragmentation of the stone) or finally a surgery. He added that after identifying the etiology produced by environmental, dietary, infec-

tious, metabolic or genetic factors, the strategic plan is defined, beginning with the inculcation of water consumption by patients and families. “The little ones who suffer from this disease oscillate between 5 and 10 years and as these patients reach their adolescence the pathology increases,” he said. He also stated that if the patient is less than 10 years old, it is obvious that his parents need to take care of him, therefore, the importance of instilling a good hydration, reducing the consumption of certain foods that increase minerals. “Therefore we try to maintain a relative restriction in this type of food and the respective hydration, of at least two liters of water per day, to try to maintain the greater fluidity of the urinary volume,” he said. NUMBERS IN THE WORLD
The doctor commented that in Mexico, unfortunately, there is no incidence rate for this type of pathology, while in the United States there has been an increase of twice as many in the patients. “Before the nineties, it was 7 percent of the population and currently stands at 14, figures that are handled by the U.S.A. ... It is important to know that in recent decades there has been a progressive increase in the number of people that suffer from it, as well as an advance in the age of appearance on patients, in such a way that in recent years his incidence in childhood has multiplied fivefold,” he explained. COMMON CAUSES
Obed Meza Suarez said that the causes are multiple, in some cases, there are genetic alterations that produce an alteration of the metabolism that may favor its appearance. For that reason it listed some as idiopathic hypercalciuria, which is the metabolic anomaly of genetic origin; hypocitraturia, that is, the elimination of citric acid; hyperoxaluria, hereditary metabolic disorder; hyperuricosuria, is the increased elimination of
uric acid and cystinuria, caused by an excess of cystine in the urine; all of them, cases in which there is usually a family history of people with kidney stones. The doctor explained that in other cases may be favored by the existence of urinary infections or malformations of the kidneys or the urinary tract. However, the increase mentioned above seems to be due to changes in dietary habits that have occurred in recent years. “On the one hand, the excess of salt, calcium (in dairy products), oxalate (present in chocolate and cola drinks) and proteins of animal origin favor the formation of kidney stones. On the other hand, the decrease in the consumption of ‘protective’ nutrients against their production, such as fruits, vegetables, legumes and whole grains,” he said. PATH TO DEATH
He warned about the importance of preventing this type of suffering since worldwide chronic kidney diseases reach the figure of
800 million patients with these diseases, of which 200 million are minors; so he reiterated it is essential to prevent it and give it the importance that is due. “And it can be deadly; because there are complications in the future; in the case of chronic kidney disease it leads to difficulties at the blood level; they are patients who live with severe anemia or can develop cardiovascular diseases on a large scale or due to their same renal tone develop bone defects, there are many complications at the chronic renal level,” he said. Given the above, recommended to educate the children with a good and healthy homemade and balanced diet, also teach them to drink enough water and hydrate well; more if they live in a hot place or where too many liquids are lost. “Ideally, hydration with water should not be replace with soda. In addition to physical activity to prevent weight gain, since it has also been seen in relation to this type of pathology,” he said.

causes candidiasis, which is another fungus,” he said.
By Beatriz Flores A bright white smile is one of the great dreams for many people, so every day they resort to different treatments and actions to get their teeth clear, but not all are recommended. Going with a medical specialist is always the best option and dentist René Rosas, specialist in Restorative Dentistry and Implantology said that tooth whitening is the one that can achieve it. Teeth whitening is a treatment to achieve several shades of the original color of the teeth and although there are some techniques that can be done at home, it is ideal to do it with a specialist at his office. First a cleaning is done on the teeth, like a polishing so that the substance that is applied can penetrate correctly. Later a resin is placed in the gums to isolate them and thus protect them from suffering a burn. “We use a technique with a special light lamp that is placed for 30 or 40 minutes to activate the substance and perform the whitening, at home you can apply a substance in guards that are placed on the teeth, like sleeves but the effect it is smaller and long term,” he explained. It will be for two weeks that the patient must perform this procedure at home, while the professional technique can be done every six months as a maintenance. In the market there are different toothpastes that offer different teeth tones, but their effects are few, because they have a minimum concentration of whitening.

“A tooth whitening can cause some sensitivity because, so to speak, the pore of the tooth is opened a bit to extract the color you have and be able to whiten it, but using pasta at home increases it,” he said. DO NOT RISK YOUR TEETH
In case the patient has very decalcified teeth or has very exposed necks, that is, the area of the roots, it is not advisable to perform whitening. After undergoing some treatment to show whiter teeth, food or drinks with dyes should be avoided. “We ask you not to consume red wine, tomato or carrot puree, red sauces, cola
or coffee,” the specialist recommended. Similarly, if you have never cleared your teeth, consuming these foods gradually darkens them. The specialist stressed that for some time there has been a popular belief that with some home remedies white teeth can be achieved and although slight changes can be made, the teeth can suffer some damage. The most common is the use of bicarbonate and hydrogen peroxide, but both cause great damage to the denture. “By brushing the teeth with bicarbonate, a friction is made that makes the tooth porous, wears it and produces a type of fungus”; while oxygenated water
It is common to see on television, social networks or print media models or actors wearing beautiful smiles with resplendent whites that inspire ordinary people to want to look the same. However, it is not always possible to achieve it, because although there are multiple techniques, it also depends on the genetics of the person. “Everyone wants white teeth and you do what the patient wants to be happy, what he does not know is that there is a limit,” he specified. This means that there are people whose teeth have never been white, not because they are stained, but by their very nature and although they can be clarified several shades will not reach a pure white. “We have to use porcelain or resin veneers to reach a total target because whitening is never going to be achieved,” he said. He said that in states such as Chihuahua and Durango, where the concentration of fluoride in the water is very high, people have very stained teeth. Finally, the dentist stressed that the ideal is that people who are going to undergo tooth whitening are adults, since the nerve is further from the enamel and there are fewer problems of sensitivity.

GREAT FLAVOR AND CHEAP
With the eight or nine dollars that a hamburger costs in the United States in Nuevo Progreso, the ‘Winter Texans’ can buy a meal with the quality of a restaurant. “Apart the service is first class and, for example, a fish or chicken grilled here is very cheap and of very good quality, and even the taquitos. “This in gastronomy and talking about the medical issue the fellow doctors, dentists and chemists are also offering very high quality services, as they have attended important conferences to incorporate the advances of the new medicine. “We are at the forefront of nanotechnology and fellow dentists are very knowledgeable. “Also, Nuevo Progreso is characterized by something very important, that we handle ambulatory medicine: they come and in 24 hours we solve the problem, but if it is a matter of high specialty we channel the patients to Reynosa or Rio Bravo,” she said. In fact, García de González raised the distinction made by the National Health Coun-

The ‘Winter Texans’ should not travel to San Antonio, Dallas or Houston to have a better diagnosis of their oral health, since in the clinic of Dr. Mustre have a device that makes tomography in third dimension, which do not have nor in the Texas Valley. Of those equipment, there are only two in Mexico. So far has the fame of this town reached on the border that doctors receive patients not only from Canada and the United States, but also from Asia and Europe. Therefore, Nuevo Progreso received the year 2019 with a lot of work thanks to the vacationers who, in addition to having fun, come to take care of a health problem. “We appreciate the preference that the ‘Winter Texans’ have for us and we hope to continue contributing to their expectations for us as doctors and as a tourist town. “I take this moment to also thank the Verbo Libre Editores Group, which publishes the Hora Cero newspapers and the Nuevo Progreso Winter Magazine, the facilities that it has given us in the promotion that it offers us, including, in San Antonio, in Dallas and in Houston, many thanks to its general director, Mr. Heriberto Deándar Robinson, for his support and welcome all visitors in this tourist season that ends in April,” said Dr. García de González.
