The risk

of losing

YOUR SIGHT

Patients who develop retinal detachment and who do not go to the doctor immediately could have a loss of vision, even, totally. By Beatriz Flores A blow, very high myopia, and diabetes can cause retinal detachment, a condition that if left unattended can even cause vision loss. Diabetic patients who have presented with diabetic retinopathy, that is very advanced damage to the blood vessels, who have had bleeding inside the eye, cause bands of fibrosis to form inside the eye that can cause their detachment. This was said by the ophthalmologist José Guillermo Escudero Ferrer, who mentioned everything related to this condition. He explained that it is a disease in the eye that is characterized by the detachment of the retina, which is the nervous tissue with which we see, from the back of the eye”, he explained. He said that those who have graduations of very high myopia can suffer it because the retina in its insertion of the part of inside the eye becomes very thin and degenerate problems are created in the periphery, which favors its early detachment. By not presenting discomfort as pain, itching or itching, or any indicator that can be appreciated by the doctor, the only way to detect it is the changes in the vision presented by the patient and that external to the specialist when consulting. “Most patients begin to notice spots within the visual field, sometimes flashes of light and in slightly more severe cases, begins the decrease in vision or the perception of a veil in the visual field”, explained the member of the Ophthalmology Society. He said that in the case of children, the blows could also cause them this condition, although it is not so common. Having an advanced age does not favor its appearance, but it could occur in patients who have undergone cataract surgery. “When you feel that your vision changes, it is very important to go to the doctor, because if not treated as soon as possible, the disease may progress to losing vision”, said the doctor belonging to the

American Academy of Ophthalmology. If this happens, he added, there is a risk that the visual recovery is not good. “When the patient can not longer seen with the eye, it means that the retina has already affected the central area of vision, which is the one part that most interests us to be preserved; When the detachment is detected in time and it is small, treatment is given and we prevent the process from progressing, but when is not detected, sometimes not even with the treatment can the vision be saved”, he warned. Regarding the treatments, he said that they could be surgical interventions or interventions with a laser beam, which will depend on the type of detachment and its extension.

“This are treatments that require retinal application where it was and that we do with external eye treatments to close a little, stick the retina or sometimes inject into the eye what we call heavy liquids that are expanders to apply the retina”, detailed Escudero Ferrer, who belongs to the Mexican Council of Ophthalmology, added that, like any surgical procedure, there are risks, for example, that of developing an infection, although it is not very common. RECOMMENDATIONS

Escudero Ferrer issued a series of recommendations to maintain good visual health. He warned that the most important thing is to go to frequent monitoring with the

specialist and above all, do it before any symptoms. Patients who have suffered retinal detachment due to trauma should not perform activities that involve physical effort. He said that diabetic patients should have strict control of their disease to prevent the eye from bleeding inside. While the myopic patients will be a study by means of which they will check the periphery of the retina to see their conditions. “There are patients who when they start to see spots or a veil believe that they will be vanish on their own and time goes by and when they come there is a greater or total detachment and although the surgical procedure is done and the patient does not recover his vision”, the doctor said. The ophthalmologist is at the Don Bosco Medical Center, located on Colón Street and Ortiz Rubio, Colonia Del Prado. Telephones: 899-922-4928 and 899-922-3238.

A mystery of

genetics Diseases of which not much is known in this respect, which cause an impact or which are still being studied, are called rare diseases and occur in a very low percentage of the world population. By Beatriz Flores There are different diseases that cause great impact between people because they are little known or not much is known about them. These affect a very small portion of the world population and many are still in the stage of study and research. According to the Mexican Federation of Rare Diseases, are those that affect a small number of people compared to the general population, that is, when it affects five people out of every 10 thousand. In its page femexer.org it is mentioned that 6 to 7 thousand rare diseases have been discovered, but there are constantly new diseases being added to the medical literature. Most genetic diseases are considered rare, but not all rare diseases are caused by genetic defects, such as infectious, autoimmune and poisoning. Many are chronic and without a cure, which can manifest during birth, childhood or adulthood. For many of them, there is no effective treatment, but good care can contribute to improving the patient quality of life. Although many patients are not diagnosed, due to the lack of information of many doctors, scientists work constantly and exchange results to spread the achievements. GENETIC DISEASE

Epidermolysis bullosa is known as “butterfly skin” and can be mild or severe. Dermatologist Patricia Sánchez Rodríguez explained that the origin of the disease is genetic. “It is a genetic alteration that results in a malformation of some proteins that cause the tissue on the surface of the skin to remain attached to the continuous tissue”, she said. The skin is divided into epidermis and dermis, for the epidermis to always remains adhered to the dermis, certain proteins are required this will form a membrane that gives an adhesiveness. She added that this protein has to be produced not only for the skin but also for the internal surfaces where there is also a superficial tissue and all those superficial tissues are called epithelia. “The epidermis is an epithelium, but we have epithelium in the mucosa of the mouth, the digestive tract, everywhere and so that they remain fixed on the tissue that follows proteins are

10

required, those are the ones that do not form well genetically”, she said. She mentioned that when you do not have that firmness of union, the skin that is the most visible epithelium is very fragile and “sweeps” easily. When this happens there are blisters that fill with fluid, which can be water or blood. Patients who suffer from it should avoid performing actions that generate any trauma, but it is more complicated if they are children due to their Dermatóloga Patricia restless nature. Sánchez Rodríguez.

“Each scrap is going to make a blister, the same with eye problems, if the eyelid was cut too hard, the mucosa becomes fragile, they should not drink very hot liquids or foods that hurt them or at temperatures that damage their tissues because they form blisters in the digestive tract”, she said. The specialist said that the blisters will be treated like a burn, applying compresses and antibiotics in an ointment. “The healing process is not altered, but the healing ends up being a bigger problem for them because every time a blister is made and it heals, its tissues atrophy, it is so much the activity of blisters that when healed if they do not have the cares appropriate they are stuck their little fingers”, he explained. Sánchez Rodríguez said that some patients may manage to live up to 18 or 20 years with a lot of care, for which, unfortunately, many economic resources are also required to pay for medical expenses. In addition to the external problems visible to the naked eye,

Abril 2018 | FRONTERA DOCTORS HEALTH MAGAZINE

there are also blisters and bleeding in the digestive tract, which causes anemia, poor absorption of nutrients, malnutrition, and diarrhea. Some people tend to develop carcinomas in scars of long ago, so they will need to be monitored so that, if present, remove it. The disease has no cure, so the doctor re-

commended that if you have identified a case in the family of “butterfly skin” it would be worthwhile to consider avoiding a new pregnancy. “It is very personal, but if you are already pregnant and you have doubts about whether the child can have it, you can do genetic tests on the baby”, he said.

The boy with butterfly skin The case of Brayan Alexis Padilla Baltazar is already known in Reynosa due to his strange bullous epidermolysis disease, known as “butterfly skin”. The small four-year-old boy began to show symptoms of his illness three days after he was born when he got some sores which the doctor who saw him at that time said they were normal and were due to dust and heat. But seeing that it did not improve, doctors from the General Hospital recommended taking him to Monterrey where there are specialists who could treat him better. Blisters of water or blood erupt in his skin, which must be burst with sterilized needles to avoid infection. “It is not a common disease, we had never seen this, they explained to us that it is something genetic that comes in the blood, it has no cure, only for God, science still does not find something, its treatment is ointment for itching, moisturize the skin and not wear so much”, said his mother. His wounds may take a week or up to 15 days to heal. “He had a very large sore, almost the size of my hand, I burst it and it’s been 15 days and it has not healed yet, but the heat does not help”, said Erika Guadalupe. But the sores are not only external, also internal, which have severely damaged his esophagus causing vomiting of blood when they burst. This makes feeding difficult, so

he must consume food supplements such as Ensure or Pediasure. In addition of taking care and healing his wounds, his mother and grandmother give him therapies helped by a ball to keep his bones and joints in good condition. “It’s like a cocoon, your skin is like a cocoon, your hands should always be open, he has to wear gloves so he can keep them always open because they can be deformed”, she explained. Some time ago, Brayan was detected with a heart murmur, which has grown since so as soon as they have the financial resources they will do a medical study to verify its current status, because when the child is doing some physical activities he gets tire very quickly. The treatment of Brayan is based on several creams and ointments that are administered several times a day to calm the itching, burning, keep his skin hydrated, help in its regeneration and prevent infections. Another one helps prevent the clothing from sticking to his body, which causes more injuries. The cream Curefini, said her mother, is ordered online to Argentina, to get to Miami, from there it is sent to McAllen and a friend brings them to Reynosa because they do not cross into the U. S. A. Both agreed that it is the only place where it is available. “We thank the good-hearted people, thank you for all that you have helped us, but Brayan’s disease has no cure, he needs every day of his treatment, to continue fighting every day with us, we ask you to continue to support us and have us in your prayers”, said his grandmother María Esther Baltazar López.

Suicidal children, a problem today

When listening to cases of children who attempt against their lives, it is easy to think of the reasons that would lead them to do so and to believe that it was only to get the attention of adults, but it is not so, children also suffer.

By Beatriz Flores Child suicide refers to the fact that a child commits in which he voluntarily ends his life. According to a study carried out in the United States, suicide is the fourth cause of death among children between 10 and 14 years old, however, there is a record of the suicide of a 7-yearold child. This information is published on the website of Humanism, a non-governmental organization that works internationally through the sponsorship of children, committed to working in the fields of education, health, microfinance, human rights, the environment, and peace. Claudia Lizeth Olivares, a general psychologist, and teacher in Education said that suicide in children is increasing due to the social problems that exist, among which is violence, bullying, dysfunctional families, etc. There are personal and family risk factors that influence a child to threaten his integrity. Among the personal factors are anxiety, depression, not being so easy to socialize, bullying, among others. The family plays an important role in this situation, for example, it influences when a comfortable environment is not offered, there is family violence or there is no trust and adequate communication conditions. “The child does not have the confidence to ask for help or help and his only way out of suffering is suicide”, said the psychologist. However, the expert said that

Psicóloga Claudia Lizeth Olivares.

WHAT TO DO?

when children attempt against their lives are not trying to ended it, because being small are not aware that they will die, they only think of escaping from the situation that afflicts them. RED ALERTS

There are some behaviors that can alert parents and may be indicators that your child may become suicidal, such as sleep disorders, loss of appetite, neglecting their image, isolation, lack of interest in playing, lack of motivation, low grades, aggressiveness and negative thoughts. “The nightmares can be recurrent and if they are suffering from bullying they may not want to go to school, they can invent things or make excuses not to do

many activities that were previously part of their routine”, said the psychologist. Events such as the death of a relative, or the arrival of adolescence can cause great stress in children and border them to present emotions difficult to understand. Depression is one of the main causes of suicide, according to Claudia Olivares, which could be due to several causes, including sexual abuse. When several of these symptoms are present, the risk increases, so Olivares mentioned that parents should be very aware of their children. “You have to investigate to find out what is happening to them, go to school, get information with the teachers, with the principal, with

the friends, even with mothers of other kids and try to approach the child”, she added. Good communication is the key to preventing a situation that in the future could be regretted and in case children do not want to talk with parents, they should use their ingenuity to achieve it. Because they are children, you can get through in the form of a game, asking them questions about how was their day, who are their friends ... that the child feels that you care and see that there is interest in him”, she said. She regretted that due to the new lifestyle in which father and mother work, there is less time for family coexistence and encourage the exchange of information among all its members.

If the child unfortunately already tried to end his life, parents should act quickly to prevent him from trying again. “When there was already a suicide attempt, psychological therapy is recommended, the specialist will provide psychotherapy and there are some cases in which pharmacological treatment is recommended”, she added. Recommended to change habits and customs for the welfare of the child and give them the tools for their proper development. “That in the house there is a pleasant atmosphere, without shouting or behavior that leads to depression, if they have problems of school performance, seek support for their regularization”, she said. But recommended considering the personality of each child, because not all have the ability to express themselves, so you have to give them and offer different communication options.

FRONTERA DOCTORS HEALTH MAGAZINE | Abril 2018

Between

chickenpox and 'shingles'

there is only one step By Pamela Escobar After chickenpox, remains a remnant commonly called “shingles”, which is when the virus is permanently in the ganglia and each time the defenses in the patient’s body fall, it is presented again Varicella zoster is a disease that children and adults suffer from, caused by a virus of an alpha type, similar to herpes simplex, for this reason it is sometimes confuse with it, but with its evolution, that is, in the pustule phases (the bulges in the skin with pus) is easy to recognize, said doctor Eulogio Cano García. “Chickenpox herpes is found first in the respiratory tract, and with that, it is contagious, so sometimes children get infected because they are already carriers and the contact of the contamination by saliva is sufficient for its dissemination”, he added. It can be easily confused with measles, herpes simplex, with the bite of an animal or simply with a dermatological infection. CONTAGION

The doctor explained that it takes two to three days to start with the symptomatology of the eruptions, since its phases are: the macula, stain; the papule, eruptive tumor of the skin and pustule, bulging with pus; to finally give way to the crust, although in its stage of vesicles (blisters) is when they tend to burst and contaminate. “There are two types of infection, one in the scab phase, after direct contact and the other when the herpes is in the respiratory tract; in the latter case, coughing, sneezing or mucus is enough to contaminate. The virus is very volatile, we easily inoculate it through the respiratory tract, but with treatment like Acyclovir, it is easy for it to die”, explained the graduate from the Universisad Autonoma de Nuevo Leon. The specialist commented that the affectation of the patient is not only in the skin but also in the liver, at the encephalic level and the spinal cord; as well as the respiratory tract, which is why the latter is considered a source of contamination.

Dr. Eulogio Cano García.

He added that when inhaled it goes into the blood and is distributed throughout the body and that is why it sometimes causes neurological or digestive symptoms, presenting abdominal pain, vomiting, and fever. “There is a herpes varicella zoster infection that occurs in two phases, either in childhood or in adulthood but it accumulates at the level of the dorsal ganglia commonly known as ‘shingles’, and remains permanently in the body and even when healthy, rashes may appear on the skin, on the ribs”, the doctor warned. He also stated that the people who had it, normally had chickenpox in their childhood or adolescence and when there is a decrease in defenses due to the flu, the virus starts to invade the nerves. “It presents the blisters, pain in the ribs, nerves of the face; in some cases, it can happen even if they are moderate cases; while in severe cases it appears in the eyelid, the conjunctiva of the eyes and even in the genitals”, explained the member of the Reynosa surgeon college. He commented that in moderate cases the risks of not attending to chickenpox are minimal, even without treatment, a total recovery will be reached; but, if it is a severe version, the problem will be permanent in the organism.

VULNERABLE GROUPS

Chickenpox is a virus, We don’t need to be low on defenses to contaminate the blood, however, the fact that it is so can make the disease more aggressive, explained the doctor. “The most vulnerable groups are patients who have tumors, or who suffer from the human immunodeficiency virus (HIV), cancer patients, as well as those who are being treated with radiation therapies”, said Cano García. It takes about 10 days for the chickenpox virus to run its curse. Unless you have HIV, these types of patients are quarantined, as well as all those immunosuppressed groups (low defenses). VACCINE

Currently, chickenpox can be prevented by means of a compound vaccine, which has prevention against measles, mumps, blond and chickenpox, diseases that mainly affect during childhood and that in adulthood cause severe problems. “To be immune to this disease, we apply a vaccine that is administered in two doses, 28 days apart to reinforce the first. When the vaccine is applied and the gallbladder forms on the skin, if someone touches it and bursts it can infect the person because it has direct contact with the virus”, the doctor warned.

At 4 years, vaccinated children are free from the possibility of getting chickenpox; since at that age is when they apply the last booster of the vaccine and with it is protected to not suffer it. And therefore do not suffer from shingles. “To identify it, they begin to manifest rashes on the scalp, then face, chest, and then extends to the back and extremities, the eruptions throughout the body are in their different phases at the same time”, explained the member of the college of surgeons of Mexico. He explained that this condition in adults never occurs moderately, so he recommended going immediately to the doctor, as it can cause death. “In the case of patients who have severe type chickenpox and who do not receive treatment have a 7 percent mortality, therefore, the worst consequence happens with the contamination of rashes by bacteria on the skin, which causes a severe infection and that can cause death”, said the doctor. DURING PREGNANCY

On the other hand, the surgeon warned about the risks of suffering during pregnancy, as he said can be very dangerous and more for the product; affecting it at the encephalic, ocular, extremities, cardiac level and causing important congenital affections. “That is why when the product is less than three months or 20 weeks it is recommended to perform a therapeutic abortion because the child will be born with many genetic alterations”, said the interviewee. If the mother refuses, the specialist added that it is a fact that the product at birth presents some alteration; either from mental problems, cardiac, small arms or without hands. “If you have more than three months of pregnancy you can not prescribe an antiviral because that type of medication passes the placental barrier and can affect the baby”, he said. To the doctor Eulogio Cano García, he can be consulted in Pedro J. Méndez street number 100 in the San Francisco Medical Center, office number 4 in the Del Prado neighborhood or call for appointments at 922-4707

STAFF / DOCTORS Some men are affected by the growth of their breasts, which generates grief, discomfort, and low self-esteem. This is referred to as gynecomastia and refers to the abnormal growth of the gland or breast tissue in man. Its volume is also due to excess fat and skin. It can affect one or both breasts and occur in 50 percent of the male population. The most prone to this pathology are males between 18 and 50 years old with overweight and a sedentary life. Dr. Guillermo Castaño Ruiz, a plastic surgeon, explained that although this condition can be hereditary, it can also develop due to the consumption of various medications or drugs such as marijuana. But the main cause is due to changes in estrogen levels, hormones responsible for female characteristics, such as breast growth. Castaño Ruiz said that there are different types of gynecomastia, which can be glandular or mixed, the latter with glandular resection and fat suction. He mentioned that all healthy men, who are emotionally stable and of any age, in

Reynosa celebrates its

269

anniversary

Mayor Maki Ortiz led the festivities By Editorial After 269 years of its foundation, where Captain Carlos Cantú met with the 279 settlers who accompanied him, on March 14 Mayor Maki Esther Ortiz Domínguez paid tribute to Don José de Escandón and the colonizers of this southern bank of the Rio Grande. Inhabitant of Reynosa Díaz, Mrs. Graciela Campos Turrubiates, received Mayor Maki Ortiz and the official delegation. “We thank you for coming to remember that here are the roots, the effort and the work of the inhabitants of those years”, she said. Later Historian Martín Salinas Rivera gave a brief chronicle of the city. The mayor said that remembering the story is to remember that Reynosa is a great city that has always received migrants. “A group of men and women, knowing or not knowing, 269 years ago had the idea of settling here and I do not know if maybe they already dreamed of having Reynosa, as the most important city of the State of Tamaulipas”, she said. She said that if they imagined it that way, the responsibility is greater for everyone.

“We have to work from our trench to continue honoring that great idea of settling in this region with valuable people; As I always say, the best people in Mexico”, said Mayor Maki Ortiz, accompanied by Vice Admiral Rubén Ceballos Guevara, special command of the Border Zone of the Tamaulipas Coordination Group and the cavalry colonel Ariel Gómez Romero, commander of the 19th Regiment of Motorized Cavalry. CUASTECOMATES

An inclusive beach

It is located in the municipality of Cihuatlán, Jalisco, is the first town and beach inclusive of Mexico for people with some physical disability because it has facilities, streets, and services adapted to the world standards in the matter, so that this type of people and adults Seniors can enjoy this tourist destination without “obstacles”. Anchored within the tourist corridor Costa Alegre and more specifically in Bahía de Navidad, Cuastecomates is a place with beaches of calm waves, where you can perform water sports such as sailing, swimming, diving, and snorkeling. Precisely the calm waves and the shallow depth of its waters along 150 meters walking “offshore” without exceeding the knees of an adult person, is what has allowed the area to be ideal for the project. In addition to being a magnificent place for conventional tourists, for some years, the federal and state tourism secretariats have launched actions to convert Cuastecomates into a beach completely accessible for people with some type of disability or difficulty in mobility because of their age. There are only nine inclusive beaches in the world. According to figures from the State Population Council, in Mexico, there are 6 million people with some type of disability. This site is adapted with signage, ac-

cess, roads, street and amphibious furniture, as well as being a cardioprotective and self-sustainable beach in medical and emergency care. These are the actions in which we have worked to comply with international standards for people with disabilities, some of them have already been implemented and others are in process:

Adapt the infrastructure and urban furniture to allow universal accessibility to people with some type of disability. A special floor was installed that allows you to get closer to the beach. It was equipped with amphibious equipment that can work both on land and at sea and that is intended to facilitate access to the beach and its environment for

people with disabilities. The roads have sidewalks without slopes, there are also special parking spaces. Areas for guide dogs. In recent years the flow of tourists has increased as an inclusive destination, receiving up to 1,200 people a day. The hotel offer is 110 rooms, enough for travelers who come to the place.

FRONTERA DOCTORS HEALTH MAGAZINE | Abril 2018

Gastroparesis

G

astroparesis (or delay in emptying the contents of the stomach into the small intestine) occurs when the stomach takes a longer time to empty its contents. It is caused by a gastric motility problem (the movement of the stomach muscles is affected). For many people, it has been a problem for many years, but treatment can relieve symptoms and prevent complications. In gastroparesis, the nerves or gastric muscles are damaged so that the motility becomes slower or stops completely. As a result, food does not move properly to get out of the stomach. This delay in emptying can cause some or all of these symptoms: • Nausea and vomiting • Feeling full very soon after eating • Acidity or burning in the stomach • Abdominal pain • Abdominal distention or swelling • Weightloss • Malnutrition The causes of gastroparesis are diverse, such as: • Severe gastric diseases such as ulcers, lumps, narrowing of the pylorus • Diabetes • Gastric or intestinal surgery • Some diseases such as systemic scleroderma, Parkinson’s, among others The diagnosis of gastroparesis is made through a complete clinical history

and complementary studies such as Superior Digestive Endoscopy, which allows us to see the changes directly, as well as perform biopsies of the affected tissue and plan the most appropriate treatment. The main objective of the treatment is to help control your problem. This may consist of one or more of the following: • Changes in feeding. Make adequate feeding schedules, avoid prolonged fasting. More snacks of less quantity each one. Avoid fats, excess fibers and carbohydrates. In some patients, it will be necessary to place percutaneous feeding tubes that avoid passage through the stomach and reach the small intestine directly. • Medicines. In some cases, medications that accelerate gastric emptying will be necessary. • Therapeutic Endoscopy. With the use of therapeutic or interventional endoscopy, surgeries are avoided, since transoral endoscopy is possible to perform procedures where the stomach outlet is opened and the food can continue its proper passage to the intestine. This is done on an outpatient basis in most cases. It is important that the patient must continue to control gastroparesis, since everything is aimed at helping the gastric emptying.

Gastroparesia

L

Junk food destroys your

intestine Correctly called Irritable Intestine Syndrome is a condition often confused with gastritis, esophagitis and other disorders of the digestive system. An expert talks about it. By José Manuel Meza José Joaquín Villegas Villalobos is a doctor from the Universidad Autonoma de Mexico (UNAM). He is has 40 years as a specialist. He trained in Tokyo, Japan, is a gastroenterologist expert in digestive and therapeutic endoscopy by the University of Showa in Yokohama. Throughout his career he has seen countless cases related to problems of the digestive system. One of the most frequent is one that has to do with inflammation of the large intestine, popularly known as colitis. “To all the patients who come and complain about their stomach, I have to ask them to point a finger at exactly where it hurts. If it is from the bellybutton to the top it is not colitis, but gastritis or some stomach condition, such as a gastric ulcer, duodenal ulcer and even a problem of esophagitis (reflux), which they called the disease of the century because it was very frequent due to overweightenfermedad diarreitica”, he mentioned. Villegas Villalobos indicated that when patients complain of colitis they usually point below the umbilicus and that has to do with the large intestine since usually the thin intestine presents very few pathologies. “In fact, the correct thing is Irritable Bowel Syndrome, although people call it colitis, chronic colitis, nervous colitis and spastic colitis. When we speak of acute colitis it is usually translated by diarrheic disease, where the stools are diminished in consistency. It can be associated with fever, headache, body painenfermedad diarreitica”, he warned. COLITIS AFFECTS MORE WOMEN

When the patient is older than 40 years old and is female, irritable bowel syndrome occupies a predominant number of daily consultations, according to this gastroenterologist. “Until recently it is associated with a pathogenic infection, or when the patient

has received a series of treatments, if it is a woman for cervicovaginal or cervico-urinary infections. “What happens is that the bacterial flora is altered and yes, the antibiotic kills the pathogenic cells but also sweeps away the bacteria that are normally in the intestine. The normal bacterial flora and changes occurenfermedad diarreitica”, he added. And he also said that the origin of the Irritable Bowel Syndrome has to do with the food issue, junk food like hamburgers, pizzas, French fries and soft drinks. “But from the moment it is a functional disorder it is also related to emotional stability. If she is a very apprehensive patient, she worries, because that influences her so that she feels worseenfermedad diarreitica”, she said. PAY ATTENTION

However, for the specialist, there are other aspects that should not go unnoticed, since he warned that in the last 30 centimeters of the rectum is where 50 percent of the cancerous tumors of the whole intestine settle. “According to the American Association of Gastrointestinal Endoscopy, of which I am an international member, has established that from 40 years, everyone, men, and women have a 7 percent chance of developing colon cancer. “As the main factor for this disease is age, then it is known for sure that every 10 years that 7 percent is going to double, so at 50 years all individuals have a 14 percent probability and at 60 until 28 percent, therefore from age 40 should have a revision of the large intestine, but at 50, if they have not done so, an endoscopy is a priorityenfermedad diarreitica”, he advised. Villegas Villalobos explained that the advantage is that through this procedure other pathologies can be detected that do not have to do with Irritable Bowel Syndrome, such as diverticula, which are defects in the wall of the large intestine. “If the person is constipated and eats wrong foods or soft drinks, or pizzas, or legumes with beans, lentils or cabbage, these balloons will spread, inflate and cause discomfort. “Typically the diverticula are on the left side of the colon, below, above the groin, it is the most common site, but they can be located throughout the intestine. Here, also on the left side, is the most frequent site for gas pressure to accumulate before evacuating or evacuationsenfermedad diarreiticaenfermedad diarreitica”, he explained. SYMPTOMS AND SOMETHING ELSE

The interviewee emphasized that when there is chronic colitis, as people know it, it can be divided into three types. “One is when the patient who is consti-

pated and does not go to the bathroom for more than three, four or five days. The other is diarrhea and is a patient with nervous colitis and goes four or five times a day. While there are types that include the two and alternate”, he enumerated. However, once the colonoscopy is done, and it is ruled out that there are other pathologies. If the patient is 40, 50 or more, the most delicate thing is to make sure that there is no malignant disease. “The curse of cancer is that when it starts you do not feel anything. There are no discomforts, but in my specialty, in the gastro, there are two warning signs. When you lose weight inexplicably, it is not because you are on a diet, on an exercise regimen or depressed. It’s not even because he or she is in love. “When there is no result that justifies a loss of weight, it is a sign of alarm. And the other very important sign is when the patient bleeds, which can be red, fresh; little dark, like red wine; but when it is black like pavement and has a very foul odor, that means that the blood comes from the stomach and has already traveled seven meters of small intestine, before being dislodged. “These are signs that there may be an advanced disease. For that reason, in a preventive way, it is recommended to do a colonoscopy, as women do mam-

mography every two years. The study is enough to do it every five years, but it is convenient after 40 and mandatory after 50”, said Villegas Villalobos. The average cost of this study is 8 thousand pesos, which is just under $ 400. “Images are taken and if we see any lesion, ulcer or small swelling that seems suspicious, biopsies are taken. Those small lesions are called polyps, which can be one or two centimeters. They are usually benign, but when it is the size of a cherry or olive, it is known that from six or 12 years that polyp will be transformed into cancer. Cancer does not appear overnight and it is very important to take into account that hemorrhoids never turn into cancer because there are many patients who get confused”, he warned. And in relation to colitis, this specialist said that 30 to fifty percent of the population suffer from it. “The main cause are functional disorders, poor diet, and anxiety. Emotional instability, stress, that has to do, the lack of sufficient liquid intake in your diet; the absence of exercise, sedentary lifestyle. “I as a specialist would do very badly in prescribing the patient without first knowing what he has. First, you have to know that it is not something dangerous or that needs more attention, at least, “he said.

High-risk

pregnancy Ectopic pregnancy is a condition considered as one of the leading causes of maternal death in the world. By Pamela Escobar Medical statistics warn about the high numbers of maternal deaths in the world due to ectopic pregnancy, a condition that, according to gynecologist Ulises Guerrero Cuevas, is a pregnancy outside the womb. “This type of pregnancy can be located in different parts outside the womb, from the neck of the same to the ovaries, abdomen or tubes. The latter, the most frequent, just in the wider region of the uterine tube, because of space and cavity there is”, explained the doctor. He also said that there are no symptoms, as such, since the patients do not present any that could alert on the fact that the pregnancy is gestating outside the womb. Since, all the symptoms that appear are those of a normal pregnancy, starting from the pregnancy test comes out positive “The only detail that changes is the amount of beta fraction, named for the hormone that causes pregnancy. We have to see inside the matrix a bag and measure up to a thousand units of this hormone, but if we do not see that bag and the patient has more than a thousand units, we should suspect that it is an ectopic pregnancy”, explained. the doctor. He warned that this type of pregnancy does not come to term due to the lack of viability, in addition to all the risks faced by the mother, and explained that there are two types: ectopic pregnancy and broken ectopic pregnancy. “The so-called broken is the most dangerous, it threatens the life of the pregnant woman and is considered one of

36

the leading causes of maternal death in Reynosa, if the patient does not detect it early through prenatal monitoring, the pregnancy continues to grow in a location that does not fulfill the characteristics of it, for that reason it breaks and it bleeds in abundance and it can take the life of the patient”, warned the gynecologist. He explained on the other hand that although there are no specific premonitory symptoms, the method to detect it is a vaginal ultrasound; In addition, the patient may suffer a hemorrhage after having a broken ectopic pregnancy and remaining in critical condition. “In some cases when the mother does not realize that she suffers from such a pregnancy, she may have a sudden pain at the level of the abdomen, which in turn can lead to fainting, to a shock effect, all caused by the loss of blood after the hemorrhage”, explained the specialist. RISK FACTOR’S

Those who have a higher risk of suffering from this condition, are those who have had a previous ectopic pregnancy, smoking, vaginal infections that become pelvic, that is, a vaginal infection not well attended and that goes up to the womb, or patients who were recanalized of tubes after a salpingoclasm. In the case of the latter, the most dangerous. Guerrero Cuevas also mentioned that it is a condition that can not be prevented, just avoid the mentioned risk factors. In the case of vaginal infections, detect if these infections are recurrent and go to the gynecologist to treat them in time and thus prevent them from becoming pelvic. In case of smoking, lower

tobacco consumption. “There is no age group that can be said to be more susceptible to suffer it. The risk factor is widespread for any woman seeking pregnancy since age is not a distinguishing feature in cases of ectopic pregnancies”, said Guerrero Cuevas.

CONSEQUENCES

4Pedro J. Méndez number 100 high floor, office 16.

The consequences are mainly for the mother, said the doctor because the pregnancy does not come to term, because of the place where it develops and the ectopic pregnancy is also one of the main causes of death for the mother due to the loss of blood. “The pregnant woman can also lose the ovary if the pregnancy is located in that part of the reproductive area of the woman, or the uterine tube, one of the most serious if it is not detected in time and after that, it breaks and bleeds”, explained the interviewee. He added that also those who get pregnant by artificial insemination or by the use of fertility techniques can suffer from this problem. TREATMENTS

The ideal treatment for ectopic pregnancy is now laparoscopically, explained the doctor, and is done by means of three very small incisions of minimal invasion and section the pregnancy location. “It is currently the ideal method because we cause less damage to female reproductive structures such as the ovaries, uterus, and tubes. Sure, as long as it’s not broken. Otherwise, the treatment to follow is through exploratory laparotomy”, explained the doctor. He added that laparotomy is to open

the lower belly of the patient and locate the bleeding focus, since in those cases the patient’s life is at risk because it is unstable due to the loss of blood. The intervention is performed through an incision the size of a cesarean section. “You can also resort to treatment that is used by oncologists, which is methotrexate, a type of chemotherapy, as long as the fetus or embryo does not have a heartbeat, and it is a clinical treatment where the patient is monitored for the pregnancy”, said the gynecologist.

Time

bomb

Hernias of various types are a common condition that should be addressed and treated by a specialist ... before it becomes a severe problem and requires emergency surgery.

By Fernanda Alvarado Monterrey, N.L. Hernias are a common affliction in people of all ages and the scarce information that we have about them are the causes that the people that sufferers from them put in risk their health when not attending with a specialist. In Mexico, more than 10 percent of the population has some type of a hernia and they appear more frequently in people between 30 and 60 years old. Dr. Jorge Ballí Martínez explained that the gastroenterologist is the specialist indicated to treat this condition. In an interview, the specialist said that hernias without treatment are like “time bombs” that usually get complicated and require emergency surgery. 4What are hernias? The hernias are a protrusion of the content of the abdomen that comes out of its place, it can be said that it is a small ball, medium or large, that springs from a weakness of the abdominal wall. 4What types of hernias are there? There are four types, the most common is an umbilical hernia that goes around the bellybutton. The second is an inguinal hernia, which is commonly seen in men, is located in the groin area and is very difficult for patients to detect at the beginning. They are caused by the weakness of the inguinoscrotal wall. In this type of hernias, fat content is produced and later the intestines go to the front, near the testicle or the scrotum of the patient. The third type of a hernia is that of Spiegel, they are not very common and appear at the junction of the lateral edge of the rectus abdominis muscle.

48

Dr. Jorge Ballí Martínez.

The last type is Post-incisional hernias, they are a protrusion that comes out after a surgical intervention. In the scar of the operation can leave a hernia that is very dangerous for the person who has it. 4How can someone get a hernia? There are many reasons why they can be produced but we have to go back to the synthesis of collagen because it is what gives us strength in the abdominal wall. Inheritance is one of the main reasons for

this condition. In many of the cases, the patient’s family has, like him, a deficiency in the synthesis of collagen. Physical activity that requires a lot of effort, can contribute to these hernias, that are caused by the increase in intra-abdominal pressure. 4Who is more likely to suffer from hernias? As previously mentioned, inheritance is an important factor for the appearance of hernias, but the most prone are: athletes, work-

ers (who perform constant heavy physical effort), pregnant women due to the pressure exerted by the product during pregnancy and last, the people who have been operated on. It is necessary to include those who suffer from constipation problems because at the time of evacuating they push a lot and that effort goes directly to the abdomen that can end up developing hernias. Also, those who have problems urinating, either because the prostate is swollen or because they have had problems with their bladder, may develop an umbilical or an inguinal hernia depending on where their weakness is. Others are those who suffer from a chronic cough because they make a lot of effort in the stomach every time they cough. Smokers often do so and inadvertently develop the problem. 4Is it true that babies who are allowed to cry for a long time, can herniate? It’s a total myth, there is no scientific article that proves that. If there are babies that can get to have hernias but for other reasons, like heredity or because they were born prematurely. 4What are the indicators or warning signs that a hernia is growing? When a hernia starts, some people show a cramp or pain in the inguinal region, by the navel or near the testicle in the case of men. Another symptom is to feel a ball that is growing in the body and that is totally diminished when we are lying down. 4Can the growth of a hernia be stop wearing sashes? Yes, you can stop the growth, but they will not heal or close a hernia, the problem is that the sashes are annoying and uncomfortable and hardly anyone can use it every day throughout the day.

April 2018 | NUEVO PROGRESO - Magazine

4How large can a hernia grow?

It can have a giant size, so much that the intestine and the complete colon fit in a hernia. This type is very difficult to deal with. We must prevent them from reaching that size and attack them in time. I had a patient that I successfully operated on, that had an inguinal hernia, that covered his genitals due to its large size. 4How can we prevent a hernia? Sometimes you can prevent it, if we put on a belt to perform physical activities of great effort, by not smoking, if we are not constipated. This way we can prevent them from appearing on our body, although this does not guarantee anything. 4Can a person live normally with a hernia? There are people who know that they have a hernia and prefer not to attend to it, they do their daily activities and of course they can live for many years, however, it is like having a time bomb inside the body because the intestine is stuck in that hernia, it is prone to get strangle and become a problem. Sooner or later the patient would need and emergency surgery which is a more risky operation than having it attended in time. There are people who may lose part of their intestines or suffer from peritonitis due to not being treated. 4What is the indicated treatment for hernias? For each type of a hernia, what must be done is to repair the wall, a mesh is placed to restore that abdominal wall, the hole of a hernia must be covered. Years ago an open surgery with suture was performed, but the recurrence rate of this operation was up to 48%, the patients returned, because the would appear again. Currently, we enter with mini paroscopy (mini surgery), it is done with small instruments no larger than 5 millimeters and then we put a mesh accepted by the body, made of a plastic material that reinforces that wall and covers the space. 4What is the recurrence rate of patients who are operated on?

Jorge Ballí Martínez 4Basic and Advanced Laparoscopic Surgeon General. 4Surgical Gastroenterology. 4Belongs to the Mexican Association of General Surgery. 4Graduated from Tecnológico de Monterrey. 4Medical office: A and B office, 15th floor of Ave Medical Center in Monterrey, Nuevo León. 4Telephones: 52 (81) 83336666, 52 (81) 8333-6699 and 52 (1) 899-212-7067.

In my case, the recurrence rate is 0.2 percent to 0.5 percent. The use of new materials dramatically decreases the incidence of returning patients. 4What care should be taken after surgery? Place a band so that the protrusion is taking its normal form in the body. 4Home remedies such as ointments and creams, cure hernias? No, patients who believe that home remedies will help to prevent the growth of a hernia are losing their time, what they should do is attend to a specialist and perform paroscopy to end the problem. 4What is the recovery time, after the operation of a hernia? The recovery is very fast, compared to open surgery there is much less pain and that same day the patient walks and eats. That same day they can go home to rest or they can stay in the hospital one night if they wish. Those who work in the office can return to work on the third or fourth day. Patients who perform heavy work, I recommend more time to rest at home, from 15 days to 30 days of recovery.
