Patients who develop retinal detachment and who do not go to the doctor immediately could have a loss of vision, even, totally.

By Beatriz Flores A blow, very high myopia, and diabetes can cause retinal detachment, a condition that if left unattended can even cause vision loss.

Diabetic patients who have presented with diabetic retinopathy, that is very advanced damage to the blood vessels, who have had bleeding inside the eye, cause bands of fibrosis to form inside the eye that can cause their detachment.

This was said by the ophthalmologist José Guillermo Escudero Ferrer, who mentioned everything related to this condition.

He explained that it is a disease in the eye that is characterized by the detachment of the retina, which is the nervous tissue with which we see, from the back of the eye”, he explained.

He said that those who have graduations of very high myopia can suffer it because the retina in its insertion of the part of inside the eye becomes very thin and degenerate problems are created in the periphery, which favors its early detachment.

By not presenting discomfort as pain, itching or itching, or any indicator that can be appreciated by the doctor, the only way to detect it is the changes in the vision presented by the patient and that external to the specialist when consulting.

“Most patients begin to notice spots within the visual field, sometimes flashes of light and in slightly more severe cases, begins the decrease in vision or the perception of a veil in the visual field”, explained the member of the Ophthalmology Society.

He said that in the case of children, the blows could also cause them this condition, although it is not so common.

RECOMMENDATIONS

Escudero Ferrer issued a series of recommendations to maintain good visual health.

He warned that the most important thing is to go to frequent monitoring with the

specialist and above all, do it before any symptoms.

Patients who have suffered retinal detachment due to trauma should not perform activities that involve physical effort.

He said that diabetic patients should have strict control of their disease to prevent the eye from bleeding inside.

While the myopic patients will be a study by means of which they will check the periphery of the retina to see their conditions.

“There are patients who when they start to see spots or a veil believe that they will be vanish on their own and time goes by and when they come there is a greater or total detachment and although the surgical procedure is done and the patient does not recover his vision”, the doctor said.

Los pacientes que presentan desprendimiento de retina y que no acuden al médico de inmediato, podrían tener gran pérdida de la visión, incluso, totalmente.

Por Beatriz Flores Un golpe, miopía muy elevada y la diabetes pueden ocasionar desprendimiento de retina, padecimiento que si no se atiende puede provocar, incluso, la pérdida de la visión.

Los pacientes diabéticos que han presentado una retinopatía diabética, es decir, daño muy avanzado a los vasos sanguíneos, que han tenido sangrados dentro del ojo, ocasiona que se creen bandas de fibrosis dentro del ojo que pueden ocasionar su desprendimiento.

Así lo dijo el oftalmólogo José Guillermo Escudero Ferrer, quien mencionó todo lo relacionado con este padecimiento.

Explicó que es una enfermedad en el ojo que se caracteriza por el despegamiento de la retina, que es el tejido nervioso con el que vemos, de la parte posterior del ojo”, explicó.

Dijo que quienes tienen graduaciones de miopía muy alta pueden sufrirla, debido a que la retina en su inserción de la parte de adentro del ojo se adelgaza mucho y se crean degeneraciones en la periferia, lo que favorece a su pronto desprendimiento.

Al no presentar molestias como dolor, picazón o comezón, ni algún indicador que pueda ser apreciado por el médico, la única forma de detectarlo son los cambios en la visión que presenta el paciente y que le externa al especialista al acudir a consulta.

“La mayor parte de los pacientes empieza a notar manchitas dentro del campo visual, a veces flashazos de luz y en casos un poco más severos, inicia la disminución de la visión o la percepción de un velo en el campo visual”, explicó el integrante de la Sociedad de Oftalmología.

Expresó que en el caso de los niños, los golpes también pudieran ocasionarles este padecimiento, aunque no es tan común.

El tener una edad avanzada no favorece su aparición, pero sí pudiera presentarse en pacientes que han sido operados de cataratas.

“Cuando sienta que su visión cambia, es muy importante acudir al médico, ya que de no atenderse lo antes posible la enfermedad

puede evolucionar hasta perder la visión”, comentó el doctor perteneciente a la Academia Americana de Oftalmología.

Si esto pasa, añadió, se corre el riesgo de que la recuperación visual no sea buena.

Escudero Ferrer, quien pertenece al Consejo Mexicano de Oftalmología, agregó que, como todo procedimiento quirúrgico existen riesgos, por ejemplo, el de desarrollar una infección, aunque no es muy común.

“Cuando ya no se ve con ese ojo, quiere decir que la retina ya afectó al área central de la visión, que es la que más nos interesa que se conserve; cuando el desprendimiento se detecta a tiempo y es pequeño se da el tratamiento y evitamos que avance el proceso,

El oftalmólogo Escudero Ferrer emitió una serie de recomendaciones para mantener una buena salud visual.

Advirtió que lo más importante es acudir a monitoreos frecuentes con el médico especialista y sobre todo, hacerlo ante cualquier sintomatología.

Expresó que los pacientes diabéticos deberán llevar un control estricto de su enfermedad para evitar que el ojo sangre por dentro.

Mientras que los pacientes miopes se harán un estudio por medio del cual les revisarán la periferia de la retina para ver sus condiciones.

“Hay pacientes que cuando empiezan a ver manchas o un velo creen que se les va a quitar y va pasando el tiempo y cuando vienen ya hay un desprendimiento mayor o total y aunque se haga el procedimiento quirúrgico ya el paciente no recupera su visión”, precisó el doctor.

genetics Diseases of which not much is known in this respect, which cause an impact or which are still being studied, are called rare diseases and occur in a very low percentage of the world population.

By Beatriz Flores There are different diseases that cause great impact between people because they are little known or not much is known about them.

These affect a very small portion of the world population and many are still in the stage of study and research.

According to the Mexican Federation of Rare Diseases, are those that affect a small number of people compared to the general population, that is, when it affects five people out of every 10 thousand.

In its page femexer.org it is mentioned that 6 to 7 thousand rare diseases have been discovered, but there are constantly new diseases being added to the medical literature.

Most genetic diseases are considered rare, but not all rare diseases are caused by genetic defects, such as infectious, autoimmune and poisoning.

Many are chronic and without a cure, which can manifest during birth, childhood or adulthood.

For many of them, there is no effective treatment, but good care can contribute to improving the patient quality of life.

Although many patients are not diagnosed, due to the lack of information of many doctors, scientists work constantly and exchange results to spread the achievements.

GENETIC DISEASE

Epidermolysis bullosa is known as “butterfly skin” and can be mild or severe.

Dermatologist Patricia Sánchez Rodríguez explained that the origin of the disease is genetic.

“It is a genetic alteration that results in a malformation of some proteins that cause the tissue on the surface of the skin to remain attached to the continuous tissue”, she said.

The skin is divided into epidermis and dermis, for the epidermis to always remains adhered to the dermis, certain proteins are required this will form a membrane that gives an adhesiveness.

She added that this protein has to be produced not only for the skin but also for the internal surfaces where there is also a superficial tissue and all those superficial tissues are called epithelia.

“The epidermis is an epithelium, but we have epithelium in the mucosa of the mouth, the digestive tract, everywhere and so that they remain fixed on the tissue that follows proteins are

required, those are the ones that do not form well genetically”, she said.

She mentioned that when you do not have that firmness of union, the skin that is the most visible epithelium is very fragile and “sweeps” easily.

When this happens there are blisters that fill with fluid, which can be water or blood.

Sánchez Rodríguez said that some patients may manage to live up to 18 or 20 years with a lot of care, for which, unfortunately, many economic resources are also required to pay for medical expenses.

The boy with butterfly skin The case of Brayan Alexis Padilla Baltazar is already known in Reynosa due to his strange bullous epidermolysis disease, known as “butterfly skin”.

The small four-year-old boy began to show symptoms of his illness three days after he was born when he got some sores which the doctor who saw him at that time said they were normal and were due to dust and heat.

But seeing that it did not improve, doctors from the General Hospital recommended taking him to Monterrey where there are specialists who could treat him better.

Blisters of water or blood erupt in his skin, which must be burst with sterilized needles to avoid infection.

“It is not a common disease, we had never seen this, they explained to us that it is something genetic that comes in the blood, it has no cure, only for God, science still does not find something, its treatment is ointment for itching, moisturize the skin and not wear so much”, said his mother.

His wounds may take a week or up to 15 days to heal.

“He had a very large sore, almost the size of my hand, I burst it and it’s been 15 days and it has not healed yet, but the heat does not help”, said Erika Guadalupe.

But the sores are not only external, also internal, which have severely damaged his esophagus causing vomiting of blood when they burst.

This makes feeding difficult, so

he must consume food supplements such as Ensure or Pediasure.

In addition of taking care and healing his wounds, his mother and grandmother give him therapies helped by a ball to keep his bones and joints in good condition.

GENÉtica Padecimientos de los que no se sabe mucho al respecto, que causan impacto o que aún se encuentran en estudio son las llamadas enfermedades raras y se presentan en muy bajo porcentaje entre la población mundial.

Existen diferentes enfermedades que causan gran impacto entre las personas debido a que son poco conocidas o no se sabe mucho al respecto.

Estas afectan a muy pocas personas y muchas aún se encuentran en etapa de estudio e investigación.

Según la Federación Mexicana de Enfermedades Raras, son aquellas que afectan a un pequeño número de personas comparado con la población general, es decir, cuando afecta a cinco personas de cada 10 mil.

En su página femexer.org se menciona que se han descubierto de 6 a 7 mil enfermedades raras, pero constantemente se van agregando más a la literatura médica.

La mayoría de las enfermedades genéticas son consideradas raras, pero no todas las enfermedades raras están causadas por defectos genéticos, por ejemplo las infecciosas, autoinmunes e intoxicaciones.

Muchas son crónicas y sin cura, que pueden manifestarse durante el nacimiento,

la infancia o adultez.

Para muchas de ellas no hay algún tratamiento efectivo, pero un buen cuidado puede contribuir a mejorar la calidad de vida del paciente.

Aunque muchos pacientes no son diagnosticados, debido al desconocimiento de muchos médicos, los científicos trabajan constantemente e intercambian resultados para difundir los logros alcanzados.

ENFERMEDAD GENÉTICA

La epidermólisis bullosa es conocida como “piel de mariposa” y puede ser leve o severa.

La dermatóloga Patricia Sánchez Rodríguez, explicó que el origen de la enfermedad es genético.

“Es una alteración genética que se traduce en una malformación de unas proteínas que hacen que el tejido de la superficie de la piel se quede adherido al tejido continuo”, mencionó.

La piel se divide en epidermis y dermis; para que la epidermis se mantenga siempre adherida a la dermis se requieren ciertas proteínas que forman una membrana que otorga como una adhesividad.

Agregó que esa proteína se tiene que producir, no solo para la piel sino para las superficies internas donde también hay un tejido superficial y a todos esos tejidos superficiales se les llama epitelios.

“La epidermis es un epitelio, pero tenemos epitelio en la mucosa de la boca, del tracto digestivo, en todos lados y para que se queden fijos sobre el tejido que sigue se requieren proteínas, esas son las que no se forman bien genéticamente”, dijo.

Mencionó que cuando no se tiene esa firmeza de unión, la piel que es el epitelio más visible es muy frágil y se “barre” con facilidad.

Cuando esto pasa surgen unas ampollas que se llenan de líquido, que puede ser agua o sangre.

Los pacientes que la padecen deben evitar realizar acciones que generen cualquier traumatismo, pero es más complicado si son niños debido a su naturaleza inquieta.

“Cada rasponcito les va a hacer una ampolla, igual con problemas oculares, si se tallaron muy fuerte el párpado la mucosa se fra-

giliza, no debe beber líquidos muy calientes o alimentos que les lastimen o a temperaturas que dañen sus tejidos porque se forman ampollas en el tracto digestivo”, expresó.

La especialista dijo que las ampollas se tratarán como una quemadura, aplicando compresas y antibióticos en ungüento.

“El proceso de cicatrización no está alterado, pero la cicatrización termina siendo un mayor problema para ellos porque cada vez que se hace una ampolla y cicatriza, se van atrofiando sus tejidos, es tanta la actividad de ampollas que al cicatrizar si no tienen los cuidados adecuados se les quedan pegados sus deditos”, detalló.

Sánchez Rodríguez dijo que algunos pacientes quizás logren vivir hasta 18 o 20 años con muchos cuidados, para lo cual, lamentablemente, también se requieren muchos recursos económicos para solventar los gastos médicos.

Además de los problemas externos perceptibles a simple vista, también se presentan ampollas y sangrados en el tubo digestivo, el cual provoca anemias, mala absorción de nutrientes, desnutrición y diarreas.

Algunas personas tienden a desarrollar carcinomas en cicatrices de hace tiempo, por lo que necesitarán vigilarse para que, en caso de presentarse, extirparlo.

La enfermedad no tiene cura, por lo que la doctora recomendó que si se tiene identificado algún caso en la familia de “piel de mariposa” valdría la pena el considerar evitar un nuevo embarazo.

“Es algo muy personal, pero si ya se está embarazada y se tiene la duda sobre si el niño pueda tenerlo, le pueden hacer pruebas genéticas al bebé”, advirtió.

El niño con piel de mariposa El caso de Brayan Alexis Padilla Baltazar, es ya conocido en Reynosa debido a su extraña enfermedad epidermólisis bullosa, conocida como “piel de mariposa”.

El pequeño niño de cuatro años empezó a presentar síntomas de su enfermedad a los tres días de nacido, cuando le salieron unas llagas las cuales el médico que lo vio en aquel entonces dijo eran normales y se debían al polvo y el calor.

Pero al ver que no mejoraba, doctores del Hospital General recomendaron llevarlo a Monterrey donde hay especialistas que podrían atenderlo mejor.

En su piel brotan ampollas de agua o sangre, las cuales le deben reventar con agujas esterilizadas para evitar una infección.

“No es una enfermedad común, nosotros nunca habíamos visto esto, nos explicaron que es algo genético que viene en la sangre, no tiene cura, solamente para Dios, la ciencia todavía no encuentra algo, su tratamiento es de pomadas para la comezón, para hidratarle la piel y que no se le desgaste tanto”, explicó su mamá.

Sus heridas tardan en sanar desde una semana hasta 15 días.

“Tenía una llaga muy grande, casi del tamaño de mi mano, se la reventé y ya lleva 15 días y aún no ha cicatrizado, pero es que el calor no le ayuda”, comentó Erika Guadalupe.

Pero las llagas no son solamente externas, también internas, las cuales han dañado severamente su esófago provocándole vómitos de sangre cuando se revientan.

Esto dificulta su alimentación, por lo que debe consumir suplementos alimenticios como Ensure o Pediasure.

Además de cuidarle y curarle sus heridas, su mamá y su abuela le dan terapias

ayudadas por una pelota para mantener sus huesos y articulaciones en buen estado.

“Es como si fuera un capullo, la piel se te va haciendo como un capullo, las manos debes tenerlas siempre abiertas, hay que meterle guantecitos para que las tenga siempre abiertas porque se le pueden ir deformando”, explicó.

Hace algún tiempo, le detectaron a Brayan un soplo en el corazón, el cual ha crecido desde entonces por lo que en breve, en cuanto tengan los recursos económicos le harán un estudio médico para verificar su estado actual, pues cuando el niño realiza alguna actividad física se cansa rápidamente.

Suicidal children, a problem today

When listening to cases of children who attempt against their lives, it is easy to think of the reasons that would lead them to do so and to believe that it was only to get the attention of adults, but it is not so, children also suffer.

By Beatriz Flores Child suicide refers to the fact that a child commits in which he voluntarily ends his life.

According to a study carried out in the United States, suicide is the fourth cause of death among children between 10 and 14 years old, however, there is a record of the suicide of a 7-yearold child.

This information is published on the website of Humanism, a non-governmental organization that works internationally through the sponsorship of children, committed to working in the fields of education, health, microfinance, human rights, the environment, and peace.

Claudia Lizeth Olivares, a general psychologist, and teacher in Education said that suicide in children is increasing due to the social problems that exist, among which is violence, bullying, dysfunctional families, etc.

There are personal and family risk factors that influence a child to threaten his integrity.

Among the personal factors are anxiety, depression, not being so easy to socialize, bullying, among others.

The family plays an important role in this situation, for example, it influences when a comfortable environment is not offered, there is family violence or there is no trust and adequate communication conditions.

“The child does not have the confidence to ask for help or help and his only way out of suffering is suicide”, said the psychologist.

Depression is one of the main causes of suicide, according to Claudia Olivares, which could be due to several causes, including sexual abuse.

When several of these symptoms are present, the risk increases, so Olivares mentioned that parents should be very aware of their children.

“You have to investigate to find out what is happening to them, go to school, get information with the teachers, with the principal, with

the friends, even with mothers of other kids and try to approach the child”, she added.

Good communication is the key to preventing a situation that in the future could be regretted and in case children do not want to talk with parents, they should use their ingenuity to achieve it.

Al escuchar sobre casos de niños que atentan contra su vida, es fácil pensar en las razones que lo llevarían a hacerlo y creer que solamente fue para llamar la atención de los adultos, pero no es así, los niños también sufren.

El suicidio infantil se refiere al hecho que comete un niño en el que consigue acabar con su vida de forma voluntaria.

Según un estudio realizado en Estados Unidos, el suicidio es la cuarta causa de mortalidad entre niños de 10 y 14 años, sin embargo, se tiene registro del suicidio de un pequeño de 7 años.

Esta información se encuentra publicada en el sitio web de Humanium, una organización no gubernamental que trabaja a nivel internacional mediante el apadrinamiento de niños, comprometida a trabajar en los rubros de educación, salud, microfinanzas, derechos humanos, medio ambiente y paz.

Claudia Lizeth Olivares, psicóloga general y maestra en Educación, dijo que el suicidio en niños está aumentando debido a la problemática social que se vive, entre la que se encuentran la violencia, el bullying, familias disfuncionales, etc.

Existen factores de riesgo personales y familiares que influyen para que un niño pueda atentar contra su integridad.

Entre los factores personales se encuentran la ansiedad, depresión, el no tener tanta facilidad para socializar, el bullying, entre otros.

La familia juega un papel importante en esta situación, por ejemplo, influye cuando no se ofrece un ambiente confortable, existe violencia intrafamiliar o no existe la confianza y las condiciones comunicativas adecuadas.

“El niño no tiene la confianza de pedirles ayuda o auxilio y su única salida para dejar de sufrir es el suicidio”, expresó la psicóloga.

Sin embargo, la experta dijo que cuando los niños atentan en contra de su vida no están terminando en acabar con ella, pues al ser pequeños no tienen conciencia de que morirán, pues solo piensan en escapar de la situación que los aqueja.

FOCOS ROJOS

Existen algunos comportamientos que pueden alertar a los padres de familia y que pueden ser indicadores de que su hijo puede convertirse en un suicida, tales como, trastornos del sueño, pérdida de apetito, descuidar su imagen, aislamiento, falta

de interés en jugar, falta de motivación, bajas calificaciones, agresividad y pensamientos negativos.

“Las pesadillas pueden ser recurrentes y si están padeciendo bullying quizás no quieran ir a la escuela, pueden inventar cosas o poner pretextos para no hacer muchas actividades que antes eran parte de su rutina”, expresó la psicóloga.

Acontecimientos como la muerte de un familiar, o la llegada de la adolescencia pueden provocar gran estrés en los niños y orillarlos a presentar emociones di-

fíciles de entender.

La depresión es una de las principales causas de suicidio, según Claudia Olivares, la cual pude deberse a varias causas, inclusive a un abuso sexual.

Cuando varios de estos síntomas se presentan el riesgo aumenta, por lo que Olivares mencionó que, los padres de familia deben estar muy al pendientes de sus hijos.

“Hay que investigar para saber qué les esta pasando, ir a la escuela, informarse con los maestros, con los directivos, con los compañeros, incluso con madres de familia y tratar de acercarse al niño”, agregó. ¡La buena comunicación es la clave para prevenir una situación que en el futuro pudiera lamentarse y en caso de que los niños no quieran hablar con los padres, deberán hacer uso de su ingenio para lograrlo.

“Como son niños puede ser a través del juego, haciéndoles preguntas sobre cómo les fue durante el día, quiénes son sus amiguitos... que el niño se sienta atendido y vea que hay interés en él”, dijo.

Lamentó que debido al estilo de vida en el que padre y madre trabajan, hay poco tiempo para la convivencia familiar y fomentar el intercambio de información entre todos sus miembros.

¿QUÉ HACER?

Si el niño lamentablemente, ya intentó acabar con su vida, los padres deberán actuar rápido para evitar que vuelva a intentarlo.

“Cuándo ya hubo un intento suicida se recomienda terapia psicológica, el especialista otorgará psicoterapia y hay algunos casos en los que se recomienda tratamiento farmacológico”, añadió.

Recomendó cambiar hábitos y costumbres para el bienestar del niño y darle las herramientas para su buen desarrollo.

“Que en la casa haya un ambiente ameno, sin gritos ni conductas que lo orillen a la depresión, si tienen problemas de rendimiento escolar, buscarles apoyo para su regularización”, mencionó.

Pero recomendó considerar la personalidad de cada niño, pus no todos tienen facilidad para expresarse, por lo que hay que motivarles y ofrecerle diferentes opciones de comunicación.

It can be easily confused with measles, herpes simplex, with the bite of an animal or simply with a dermatological infection.

CONTAGION

The doctor explained that it takes two to three days to start with the symptomatology of the eruptions, since its phases are: the macula, stain; the papule, eruptive tumor of the skin and pustule, bulging with pus; to finally give way to the crust, although in its stage of vesicles (blisters) is when they tend to burst and contaminate.

“There are two types of infection, one in the scab phase, after direct contact and the other when the herpes is in the respiratory tract; in the latter case, coughing, sneezing or mucus is enough to contaminate. The virus is very volatile, we easily inoculate it through the respiratory tract, but with treatment like Acyclovir, it is easy for it to die”, explained the graduate from the Universisad Autonoma de Nuevo Leon.

The specialist commented that the affectation of the patient is not only in the skin but also in the liver, at the encephalic level and the spinal cord; as well as the respiratory tract, which is why the latter is considered a source of contamination.

He added that when inhaled it goes into the blood and is distributed throughout the body and that is why it sometimes causes neurological or digestive symptoms, presenting abdominal pain, vomiting, and fever.

“There is a herpes varicella zoster infection that occurs in two phases, either in childhood or in adulthood but it accumulates at the level of the dorsal ganglia commonly known as ‘shingles’, and remains permanently in the body and even when healthy, rashes may appear on the skin, on the ribs”, the doctor warned.

He also stated that the people who had it, normally had chickenpox in their childhood or adolescence and when there is a decrease in defenses due to the flu, the virus starts to invade the nerves.

“It presents the blisters, pain in the ribs, nerves of the face; in some cases, it can happen even if they are moderate cases; while in severe cases it appears in the eyelid, the conjunctiva of the eyes and even in the genitals”, explained the member of the Reynosa surgeon college.

He commented that in moderate cases the risks of not attending to chickenpox are minimal, even without treatment, a total recovery will be reached; but, if it is a severe version, the problem will be permanent in the organism.

VULNERABLE GROUPS

Chickenpox is a virus, We don’t need to be low on defenses to contaminate the blood, however, the fact that it is so can make the disease more aggressive, explained the doctor.

“The most vulnerable groups are patients who have tumors, or who suffer from the human immunodeficiency virus (HIV), cancer patients, as well as those who are being treated with radiation therapies”, said Cano García.

It takes about 10 days for the chickenpox virus to run its curse. Unless you have HIV, these types of patients are quarantined, as well as all those immunosuppressed groups (low defenses).

VACCINE

Currently, chickenpox can be prevented by means of a compound vaccine, which has prevention against measles, mumps, blond and chickenpox, diseases that mainly affect during childhood and that in adulthood cause severe problems.

“To be immune to this disease, we apply a vaccine that is administered in two doses, 28 days apart to reinforce the first. When the vaccine is applied and the gallbladder forms on the skin, if someone touches it and bursts it can infect the person because it has direct contact with the virus”, the doctor warned.

At 4 years, vaccinated children are free from the possibility of getting chickenpox; since at that age is when they apply the last booster of the vaccine and with it is protected to not suffer it. And therefore do not suffer from shingles.

“To identify it, they begin to manifest rashes on the scalp, then face, chest, and then extends to the back and extremities, the eruptions throughout the body are in their different phases at the same time”, explained the member of the college of surgeons of Mexico.

He explained that this condition in adults never occurs moderately, so he recommended going immediately to the doctor, as it can cause death.

“In the case of patients who have severe type chickenpox and who do not receive treatment have a 7 percent mortality, therefore, the worst consequence happens with the contamination of rashes by bacteria on the skin, which causes a severe infection and that can cause death”, said the doctor.

DURING PREGNANCY

On the other hand, the surgeon warned about the risks of suffering during pregnancy, as he said can be very dangerous and more for the product; affecting it at the encephalic, ocular, extremities, cardiac level and causing important congenital affections.

“That is why when the product is less than three months or 20 weeks it is recommended to perform a therapeutic abortion because the child will be born with many genetic alterations”, said the interviewee.

If the mother refuses, the specialist added that it is a fact that the product at birth presents some alteration; either from mental problems, cardiac, small arms or without hands.

“If you have more than three months of pregnancy you can not prescribe an antiviral because that type of medication passes the placental barrier and can affect the baby”, he said.

Entre la

varicela y la “culebrilla”

De la varicela queda un remanente denominado comúnmente “culebrilla”, que es cuando el virus queda de manera permanente en los ganglios y cada vez que bajan las defensas en el organismo del paciente, se vuelve a presentar.

La varicela zóster es una enfermedad que padecen niños y adultos, causado por un virus llamado tipo alfa, similar al herpes simple, por ello a veces se confunde, pero con su evolución, es decir, en las fases de pústula (los abultamientos en la piel con pus) en fácil reconocerlo, dijo el médico Eulogio Cano García.

“El herpes de la varicela se encuentra primero en las vías respiratorias, ya con eso es contaminante, por ello a veces los niños se contagian, porque ya son portadores y el contacto de la contaminación por saliva es suficiente para su diseminación”, agregó el cirujano general.

Se puede confundir con sarampión, con herpes simple, con la picadura de algún animal o simplemente con una infección dermatológica.

CONTAGIO

El doctor explicó que pasan de dos a tres días para que se empiece con la sintomatología de las erupciones, ya que sus fases son: la mácula, mancha; la pápula, tumor eruptivo de la piel y pústula, abultamiento con pus; para finalmente dar paso a la costra, aunque en su etapa de vesículas (ampollas) es cuando tienden a reventar y a contaminar.

“Hay dos tipos de contagio una en la fase de costra, tras el contacto directo y la otra cuando el herpes se encuentra en las vías respiratorias; en este ultimo caso, con la tos, estornudos u mucosidad es suficiente para contaminar. El virus es muy volátil, fácilmente lo inoculamos por las vías respiratorias, pero con tratamiento como el Aciclovir, es fácil que muera”, detalló el egresado de la Universidad Autónoma de Nuevo León.

El especialista comentó que la afectación del paciente no sólo es en la piel, sino también del hígado, a nivel encefálico y de la medula espinal; así como de las vías respiratorias, por eso ésta ultima es considerada una fuente de contaminación.

Añadió que cando se inhala se va a la sangre y se distribuye por todo el cuerpo y por

eso a veces causa sintomatología neurológica, o digestiva, presentando dolor abdominal, vómito y fiebre.

“Hay una infección del herpes varicela zóster que se presenta en dos fases, ya sea en la infancia o en la adultez pero éste se acumula a nivel de los ganglios dorsales conocido comúnmente como ‘culebrilla’, y se queda de manera permanente en el organismo y aun estando sano, pueden aparecer erupciones en la piel, en las costillas”, advirtió el médico.

Precisó además que las personas que lo presentan tuvieron varicela en la infancia o adolescencia y cuando se presenta una baja de defensas por gripa, empieza el virus a invadir los nervios.

“Se presentan los ámpulas, dolor en las costillas, nervios de la cara; en algunas ocasiones puede pasar aunque sean casos moderados; mientras que en los casos severos aparece en el párpado, la conjuntiva de los ojos y hasta en los genitales”, detalló el miembro del Colegio de Cirujanos de Reynosa.

Comentó que en casos moderados los riesgos de no atender la varicela son mínimos, aun sin tratamiento, se alcanzará una recuperación total; pero, si se trata de la versión severa, el problema será permanente en el organismo.

GRUPOS VULNERABLES

La varicela es un virus, no necesita estar bajo

de defensas para que contamine la sangre, sin embargo, el hecho de que así sea, puede favorecer que sea más agresiva la enfermedad, explicó el doctor.

“Los grupos más vulnerables son los pacientes que tienen tumores, o que sufren del virus de inmunodeficiencia humana (VIH), enfermos de cáncer, así como a quienes están siendo tratados con radioterapias”, advirtió Cano García.

Son alrededor de 10 días los que se padece la varicela; a menos que se tenga VIH, ese tipo de pacientes se ponen en cuarentena, así como todos aquellos grupos inmunosuprimidos (bajas defensas).

VACUNA

Actualmente la varicela se puede prevenir por medio de una vacuna compuesta, que cuenta con prevención contra sarampión, paperas, rubeola y varicela, enfermedades que principalmente afectan durante la niñez y que en la adultez causan severos problemas.

“Para ser inmune ante esta enfermedad, se aplica una vacuna que se administra en dos dosis, con 28 días de diferencia para reforzar a la primera. Cuando se aplica la vacuna y se forma la vesícula en la piel, si alguien la toca y revienta puede contagiar a la persona pues tuvo contacto directo con el virus”, advirtió el doctor.

A los 4 años, los menores vacunados están libres de la posibilidad de contraer varicela; ya que a esa edad es cuando les aplican el último refuerzo de la vacuna y con ello se está protegido para no sufrirla. Y por consiguiente no padecer la culebrilla.

“Para identificarlo, se empiezan a manifestar erupciones en el cuero cabelludo, posteriormente cara, tórax, y luego se extiende hacia la espalda y las extremidades, las erupciones en todo el cuerpo se encuentran en sus diferentes fases al mismo tiempo”, explicó el miembro del Colegio de Cirujanos de México.

Explicó que dicho padecimiento en los adultos nunca se presenta de forma moderada, por ello recomendó acudir de inmediato con el médico, ya que puede causar la muerte.

“En el caso de los enfermos que presentan la varicela de tipo grave y que no reciban tratamiento tienen un 7 por ciento de mortalidad, por ello, la peor consecuencia sucede con la contaminación de las erupciones por las bacterias en la piel, que provoca una infección severa y que puede causar el deceso”, dijo el doctor.

DURANTE EL EMBARAZO

Por otro lado el cirujano advirtió sobre los riesgos de padecerla durante el embarazo, ya que dijo, puede ser muy peligrosa y más para el producto; afectándolo a nivel encefálico, ocular, en sus extremidades, a nivel cardiaco y causar afectaciones congénitas importantes.

“Por eso cuando el producto tiene menos de tres meses o 20 semanas se recomienda hacer un aborto terapéutico debido a que el menor va a nacer con muchas alteraciones genéticas”, expresó el entrevistado.

Si la madre se niega, el especialista dijo que es un hecho que el producto al nacer presente alguna alteración; ya sea desde problemas mentales, cardiacos, brazos pequeños o sin manos.

“Si tiene más de tres meses de gestación no se le puede recetar un antiviral ya que ese tipo de medicamentos pasa la barrera placentaria y puede afectar al bebé”, afirmó.

The giant

Yaqui

It weighs more than a thousand tons, is the largest sculptural work in Latin America, and is in Sonora to pay tribute to the brave Yaqui tribe, the most emblematic of these lands.

The largest statue of Latin America is in the town of Loma de Guamúchil, in the municipality of Cajeme, Sonora, representing a Yaqui dancer who stands proud, in honor of that ethnic group and shows his face to the north where most of the Yaqui People has been establish.

The figure of one thousand 100 tons of weight, made of synthetic bronze, stands proud with its 33 meters high.

In the place there is a space for tourists and citizens who circulate on the international highway, it serves for a moment of rest and relaxation, especially to admire the colossus raised in honor of the Yaqui Tribes.

The tourist Parador is located on the federal highway Mexico 15, between the towns of Estación Corral and Loma de Guamúchil, both communities belong to the Yaqui Tribe group.

The idea, proposal, and concept of the project began in 2010 and it was in 2011 that it was competed on to obtain this work, carried out by the state government of Sonora.

Among the applicants to make it was a company from San Francisco, California but the proposal of the sculptor from Sonora, Marlon Balderrama Monge, who belongs to the fourth generation of an outstanding family of sculptors, was the winner.

“The proposal was to make an emblematic figure, to dignify the work of an upright Yaqui, which represents the pride, courage, perseverance of one of the most representative ethnic groups in Sonora”, says Balderrama Monge.

It is made of synthetic bronze, which is a mixture of metal and resin with reinforcing fiber and a structure of the lower part of reinforced concrete, and from the waist up is a steel structure and was made in the workshops of the family of Balderrama Monge, in Hermosillo, Sonora.

In the design and execution of the work, 20 people participated directly in the workshop, indirectly a team of 10 engineers were involved in what was the structure, calculation, and structural design.

It cost 20 million pesos and is three meters larger than the Corcovado Christ found in Brazil. The Statue of Liberty measures 46 meters plus its base, in steel.

Pesa más de mil toneladas, es la obra escultórica más grande de Latinoamérica, y está en Sonora para rendir tributo a la brava etnia Yaqui, la más emblemática de esas tierras.

La estatua más grande de Latinoamérica se encuentra en el poblado Loma de Guamúchil, en el municipio de Cajeme, Sonora, representa un danzante Yaqui que se yergue orgulloso, en honor de esa etnia y se muestra con su rostro hacia el norte dónde se encuentran la mayoría de los Pueblos Yaquis.

La figura de mil 100 toneladas de peso, elaborada en bronce sintético, se muestra altivo con sus 33 metros de altura.

En el lugar existe un espacio para los turistas y ciudadanos que circulan sobre la carretera internacional, sirve para un momento de descanso y relajación, sobre todo para admirar al coloso levantado en honor a los Yaquis.

El parador turístico se encuentra sobre la carretera federal México 15, entre los poblados Estación Corral y la Loma de

Guamúchil, ambas comunidades pertenecientes a la etnia Yaqui.

La idea, la propuesta y concepto del proyecto se inició en 2010 y fue en 2011 que se concursó para obtener dicha obra, realizada por el gobierno estatal sonorense.

Entre los postulantes para realizarlas estaba una compañía de San Francisco, California pero la propuesta del escultor sonorense Marlon Balderrama Monge, quien pertenece a la cuarta generación de una destacada familia de escultores, fue la ganadora.

“La propuesta era realizar una figura emblemática, dignificar la obra de un yaqui erguido, que representa el orgullo, la valentía, la perseverancia de una de las etnias más representativas de Sonora”, dice Balderrama Monge.

Está realizada en bronce sintético, que

es una mezcla de metal y resina con fibra de refuerzo y una estructura de la parte baja de concreto armado, y de la cintura hacia arriba es una estructura de acero y fue realizada en los talleres de la familia de Balderrama Monge, en Hermosillo, Sonora.

En el diseño y la ejecución de la obra participaron directamente 20 personas en taller, indirectamente un equipo de 10 ingenieros involucrados en lo que fue la estructura, cálculo y diseño estructural.

The Mayor said that with 383 works in one year, today, after the Wastewater Pumping Station (EBAR), number 14 and the school classroom, it has to inaugurate 17 streets paved with hydraulic concrete.

La presidenta municipal, Maki Ortiz, reconoció que “uno de los retos más grandes que tenemos es acabar con estas aguas negras en toda la ciudad; hemos hecho y estamos haciendo el plan de obras más grande en la historia de Reynosa”.

Dijo la alcaldesa que con 383 obras en un año, hoy, después de la Estación de Bombeo de Aguas Residuales, (EBAR), número 14 y del aula escolar, tiene que inaugurar 17 calles pavimentadas con concreto hidráulico.

El gerente técnico operativo de la Comapa, Jorge Herrera Bustamante, explicó que el organismo no es lucrativo, en esta administración no tuvo que esperar recursos de terceros

porque la alcaldesa Maki Ortiz lo invirtió y que la obra refleja la coordinación entre Comapa y municipio.

Los tres motores instalados en la EBAR 14 son de la mejor marca y calidad, tiene columnas y canastillas de acero inoxidable y esta obra beneficia a 21 mil 200 familias.

Participaron en la inauguración el secretario de Obras Públicas, Eduardo López Arias, los regidores David Jorge Aguilar Meraz, Alfonso Peña Rodríguez, María Elena Blanco Chávez, María Concepción Sánchez Garza, Anselmo Almaraz Salazar, la señora Elvira González Sánchez y vecinos del sector.

Previo a la inauguración de la EBAR en Villas de Imaq, la alcaldesa Maki Ortiz puso en marcha la primera etapa de la Estación de Bombeo de Aguas Residuales número 10, que beneficia a 250 mil habitantes del oriente de la ciudad.

La obra se construye con un presupuesto superior a los 64 millones de pesos y cuenta con un nuevo colector de aguas residuales de más de un metro de diámetro.

“Una obra tan grande que nos está haciendo la señora presidenta”, dijo la señora Alma Rosa Castillo Sánchez, residente de la colonia Nuevo Amanecer y beneficiaria de la rehabilitación del cárcamo.

“We thank you for coming to remember that here are the roots, the effort and the work of the inhabitants of those years”, she said.

Later Historian Martín Salinas Rivera gave a brief chronicle of the city.

The mayor said that remembering the story is to remember that Reynosa is a great city that has always received migrants.

“A group of men and women, knowing or not knowing, 269 years ago had the idea of settling here and I do not know if maybe they already dreamed of having Reynosa, as the most important city of the State of Tamaulipas”, she said.

An inclusive beach

These are the actions in which we have worked to comply with international standards for people with disabilities, some of them have already been implemented and others are in process:

A special floor was installed that allows you to get closer to the beach.

It was equipped with amphibious equipment that can work both on land and at sea and that is intended to facilitate access to the beach and its environment for

people with disabilities.

CUASTECOMATES Una playa incluyente Está ubicado en el municipio de Cihuatlán, Jalisco, es el primer pueblo y playa incluyente de México para personas con alguna discapacidad física pues cuenta con instalaciones, calles y servicios adecuados a los estándares mundiales en la materia, para que este tipo de personas y adultos mayores puedan disfrutar sin “obstáculos” este destino turístico.

Anclada dentro del corredor turístico Costa Alegre y más específicamente en Bahía de Navidad, Cuastecomates es un sitio con playas de oleaje tranquilo, donde se pueden realizar deportes acuáticos como el veleo, la natación, el buceo y el esnórquel.

Precisamente el oleaje tranquilo y la poca profundidad de sus aguas a lo largo de 150 metros caminando “mar adentro” sin que se rebase las rodillas de una persona adulta, es lo que ha permitido a la zona ser ideal para el proyecto.

Además de ser un magnifico lugar para el turista convencional, desde hace algunos años, las secretarías de turismo federal y estatal, pusieron en marcha acciones para convertir a Cuastecomates en una playa completamente accesible para personas con algún tipo de discapacidad o dificultad en movilidad debido a su edad.

En el mundo solamente existen nueve playas incluyentes.

De acuerdo con cifras del Consejo Estatal de Población, en México existen 6 millones de personas con algún tipo de discapacidad.

Este sitio está adaptado con señaléticas, accesos, vialidades, mobiliario urbano y anfibio, además de ser una playa cardioprotegida y autosustentable en atención médica y de urgencias.

Estas son las acciones en las que se ha trabajado para cumplir con los estándares internacionales para personas con discapacidad, algunas de ellas ya se han implementado y otras están en proceso: Adecuar la infraestructura y mobiliario urbano para que permita accesibilidad universal a personas con algún tipo de discapacidad.

Se instaló un piso especial que permite acercarse a la playa.

Se dotó con equipo anfibio que puede funcionar tanto en tierra como en mar y que está destinado a facilitar el acceso a la playa y su entorno a personas con discapacidad.

Las vialidades cuentan con banquetas sin desniveles, también existen cajones de estacionamiento especiales. Áreas para perros guías.

Los últimos años el flujo de turistas ha aumentado como un destino incluyente, recibiendo hasta mil 200 personas al día. La oferta hotelera es de 110 habitaciones, suficientes para los viajeros que acuden al lugar.

Gastroparesis

G

astroparesis (or delay in emptying the contents of the stomach into the small intestine) occurs when the stomach takes a longer time to empty its contents.

It is caused by a gastric motility problem (the movement of the stomach muscles is affected). For many people, it has been a problem for many years, but treatment can relieve symptoms and prevent complications.

In gastroparesis, the nerves or gastric muscles are damaged so that the motility becomes slower or stops completely.

As a result, food does not move properly to get out of the stomach. This delay in emptying can cause some or all of these symptoms: • Nausea and vomiting • Feeling full very soon after eating • Acidity or burning in the stomach • Abdominal pain • Abdominal distention or swelling • Weightloss • Malnutrition The causes of gastroparesis are diverse, such as: • Severe gastric diseases such as ulcers, lumps, narrowing of the pylorus • Diabetes • Gastric or intestinal surgery • Some diseases such as systemic scleroderma, Parkinson’s, among others The diagnosis of gastroparesis is made through a complete clinical history

and complementary studies such as Superior Digestive Endoscopy, which allows us to see the changes directly, as well as perform biopsies of the affected tissue and plan the most appropriate treatment.

The main objective of the treatment is to help control your problem. This may consist of one or more of the following: • Changes in feeding. Make adequate feeding schedules, avoid prolonged fasting. More snacks of less quantity each one. Avoid fats, excess fibers and carbohydrates. In some patients, it will be necessary to place percutaneous feeding tubes that avoid passage through the stomach and reach the small intestine directly.

• Medicines. In some cases, medications that accelerate gastric emptying will be necessary.

• Therapeutic Endoscopy. With the use of therapeutic or interventional endoscopy, surgeries are avoided, since transoral endoscopy is possible to perform procedures where the stomach outlet is opened and the food can continue its proper passage to the intestine. This is done on an outpatient basis in most cases.

It is important that the patient must continue to control gastroparesis, since everything is aimed at helping the gastric emptying.

Gastroparesia

L

a gastroparesia (o retardo en el vaciamiento del contenido del estómago al intestino delgado) ocurre cuando el estómago tarda un tiempo mayor en vaciar su contenido.

Es causado por un problema de motilidad gástrica (se encuentra afectado el movimiento de los músculos del estómago). Para muchas personas ha sido un problema de muchos años, pero el tratamiento puede aliviar los síntomas y prevenir las complicaciones.

En la gastroparesia, los nervios o los músculos gástricos están dañados con lo cual la motilidad se hace más lenta o se detiene completamente. En consecuencia, la comida no se mueve correctamente para salir del estómago. Este retraso en el vaciamiento puede causar alguno o todos estos síntomas: • Náusea y vómito • Sentirse lleno muy pronto después de comer • Acidez o ardor en el estómago • Dolor abdominal • Distención o hinchazón abdominal • Pérdida de peso • Desnutrición Las causas de la gastroparesia son diversas como: • Enfermedades gástricas severas como úlceras, tumoraciones, estrechez del píloro • Diabetes • Cirugía gástrica o intestinal • Algunas enfermedades como la esclerodermia sistémica, Parkinson, entre otros

El diagnóstico de la gastroparesia se realiza mediante una historia clínica completa y estudios complementarios como la Endoscopia Digestiva Superior, la que nos permite ver las alteraciones directamente, así como el realizar biopsias del tejido afectado y planear el tratamiento más adecuado.

El objetivo principal del tratamiento va encaminado a ayudar a controlar su problema. Este puede consistir en uno o más de los siguientes: • Cambios de alimentación. Hacer horarios de alimentación adecuados, evitar ayunos prolongados. Más colaciones de menor cantidad cada una. Evitar grasas, exceso de fibras y carbohidratos. En algunos pacientes será necesario colocar tubos de alimentación percutáneo que evite el paso por estómago y llegue directamente al intestino delgado.

• Medicamentos. En algunos casos será necesario medicamentos que aceleren el vaciamiento gástrico.

• Endoscopía Terapéutica. Con el recurso de la Endoscopia Terapéutica o intervencionista se evitan cirugías, ya que es posible por vía Endoscópica transoral realizar procedimientos donde se abre la salida del estómago y el alimento podrá continuar su paso adecuado al intestino. Esto se realiza de forma ambulatoria en la mayoría de los casos.

Es importante que el paciente deba continuar controlando la gastroparesia, ya que todo está dirigido a ayudar al vaciamiento gástrico.

intestine Correctly called Irritable Intestine Syndrome is a condition often confused with gastritis, esophagitis and other disorders of the digestive system. An expert talks about it.

By José Manuel Meza José Joaquín Villegas Villalobos is a doctor from the Universidad Autonoma de Mexico (UNAM). He is has 40 years as a specialist.

He trained in Tokyo, Japan, is a gastroenterologist expert in digestive and therapeutic endoscopy by the University of Showa in Yokohama.

Throughout his career he has seen countless cases related to problems of the digestive system. One of the most frequent is one that has to do with inflammation of the large intestine, popularly known as colitis.

“To all the patients who come and complain about their stomach, I have to ask them to point a finger at exactly where it hurts. If it is from the bellybutton to the top it is not colitis, but gastritis or some stomach condition, such as a gastric ulcer, duodenal ulcer and even a problem of esophagitis (reflux), which they called the disease of the century because it was very frequent due to overweightenfermedad diarreitica”, he mentioned.

Villegas Villalobos indicated that when patients complain of colitis they usually point below the umbilicus and that has to do with the large intestine since usually the thin intestine presents very few pathologies.

“In fact, the correct thing is Irritable Bowel Syndrome, although people call it colitis, chronic colitis, nervous colitis and spastic colitis. When we speak of acute colitis it is usually translated by diarrheic disease, where the stools are diminished in consistency. tu intestino Correctamente llamado Síndrome de Intestino Irritable, es un padecimiento muchas veces confundido con la gastritis, esofagitis y otros trastornos del aparato digestivo. Un experto habla al respecto.

Por José Manuel Meza José Joaquín Villegas Villalobos es médico por la Universidad Nacional Autónoma de México (UNAM). Está por cumplir 40 años como especialista.

Se entrenó en Tokio, Japón, es gastroenterólogo experto en endoscopía digestiva y terapéutica por la Universidad de Showa en Yokohama.

A lo largo de su carrera ha visto un sinnúmero de casos relacionados con problemas del aparato digestivo. Uno de las más frecuentes es aquel que tiene que ver con la inflamación del intestino grueso, conocida popularmente como colitis.

“A todos los pacientes que llegan y se quejan del estómago les tengo que pedir que me señalen con un dedo dónde exactamente les duele. Si es del ombligo para arriba no es colitis, sino gastritis o algún padecimiento del estómago, como una úlcera gástrica, úlcera duodenal y hasta un problema de esofagitis (reflujo), que le llamaron la enfermedad del siglo, porque es muy frecuente por el sobrepeso”, mencionó.

Villegas Villalobos indicó que cuando los pacientes se quejan de colitis le señalan por lo general por abajo del ombligo y eso tiene que ver con el intestino grueso, ya que por lo regular el delgado presenta muy pocas patologías.

“De hecho lo correcto es Síndrome de Intestino Irritable, aunque la gente le llama colitis, colitis crónica, colitis nerviosa y colitis espástica. Cuando hablamos de colitis aguda generalmente se traduce por enfermedad diarréitica, donde las evacuaciones están disminuidas de consistencia. Se puede asociar con fiebre, dolor de cabeza,

dolor de cuerpo”, alertó.

COLITIS AFECTA MÁS A MUJERES

Cuando el paciente es mayor de 40 años y es del sexo femenino, el síndrome del intestino irritable ocupa un número predominante de la consulta diaria, según manifestó este gastroenterólogo.

“Hasta recientemente se le asocia con una infección patógena, o cuando el paciente ha recibido una serie de tratamientos, si es mujer por infecciones cérvicovaginales o cérvicourinarias.

“Lo que sucede es que se altera la flora bacteriana y sí, el antibiótico acaba con las células patógenas, pero también barre con las bacterias que normalmente están en el intestino, la flora bacteriana normal y se producen cambios”, agregó.

Y dijo también que el origen del Síndrome de Intestino Irritable tiene que ver con la cuestión alimentaria, la comida chatarra como las hamburguesas, las pizzas, las papas fritas y las bebidas gaseosas.

“Pero desde el momento que es un tras-

torno funcional se relaciona además con la estabilidad emocional. Si es una paciente muy aprensiva, preocupona, pues eso influye para que ella se sienta peor”, expresó.

PONER ATENCIÓN

No obstante, para el especialista, hay otros aspectos que no deben pasar desapercibidos, ya que advirtió que en los 30 últimos centímetros del recto es donde se asienta el 50 por ciento de los tumores cancerosos de todo el intestino.

“De acuerdo con la Asociación Americana de Endoscopía Gastrointestinal, de la cual yo soy miembro internacional, ha establecido que a partir de los 40 años, todo mundo, hombres y mujeres tenemos un 7 por ciento de probabilidades de desarrollar cáncer de colon.

“Como el factor principal para esta enfermedad es la edad, entonces se sabe con seguridad que cada 10 años ese 7 por ciento se va a duplicar, así que a los 50 años todos los individuos tenemos un 14 por ciento de probabilidad y a los 60 hasta el 28 por ciento, por lo tanto desde los 40 años deben hacerse una revisión del intestino grueso, pero a los 50, si no se la han hecho, es prioritario una endoscopía”, aconsejó.

Villegas Villalobos explicó que la ventaja es que mediante ese procedimiento se pueden detectar otras patologías, que no tengan que ver con el Síndrome de Intestino Irritable, como los divertículos, los cuales son defectos en la pared del intestino grueso.

“Si la persona es constipada y come comidas erróneas o bebidas gaseosas, o pizzas, o legumbres con frijol, lenteja o repollo, estos globitos se van a extender, inflar y causar molestia.

“Típicamente los divertículos están del lado izquierdo del colon, abajo, por arriba de la ingle, es el sitio más común, pero pueden situarse en todo el intestino. Aquí, también del lado izquierdo, es el sitio más frecuente para que se acumule la presión del gas antes de desalojarse o de las evacuaciones”, detalló.

SÍNTOMAS Y ALGO MÁS

El entrevistado puso énfasis en que cuando existe una colitis crónica, como la gente lo conoce, ésta puede dividirse en tres tipos.

“Uno es cuando el paciente que cursa con constipación o estreñimiento hace hasta los tres, cuatro o cinco días. El otro es en el que predomina la diarrea y es un paciente con colitis nerviosa y hace cuatro o cinco veces al día. En tanto que hay tipos que incluyen los dos y se alternan”, enumeró.

Sin embargo, una vez que se hace la colonoscopía se descarta que haya otras patologías. Si el paciente tiene 40, 50 o más, lo mayormente delicado es asegurarse que no exista una enfermedad maligna.

“La maldición del cáncer es que cuando comienza no se siente nada. No se tienen molestias, pero en mi especialidad, en la gastro, hay dos signos de alarma. Cuan-

do se pierde peso inexplicablemente no es porque esté a dieta, en un régimen de ejercicio o deprimido. No es ni siquiera porque esté enamorado.

“Cuando no existe un resultado que justifique una pérdida de peso es un signo de alarma. Y el otro signo muy importante es cuando el paciente sangra, que puede ser roja, fresca; poco oscura, como el vino tinto; pero cuando es negra como el chapopote y tiene un olor muy fétido, eso traduce que la sangre viene desde el estómago y ya viajó siete metros de intestino delgado, antes de ser desalojada.

“Esas son señales de que posiblemente exista una enfermedad avanzada. Por eso de rutina, de forma preventiva se recomienda hacer una colonoscopía, como las mujeres se hacen la mastografía cada dos años.

El estudio basta con realizarlo cada cinco años, pero es conveniente a partir de los 40

y obligatorio después de los 50”, recalcó Villegas Villalobos.

El costo en promedio de este estudio es de 8 mil pesos, que vienen siendo poco menos de 400 dólares.

“Se toman imágenes y si vemos alguna lesión, úlcera o una pequeña tumefacción que parezca sospechosa se toman biopsias.

Esas lesiones pequeñas se llaman pólipos, que pueden ser uno o dos centímetros. Generalmente son benignos, pero cuando es del tamaño de una cereza o aceituna, se sabe que a partir de seis o 12 años ese pólipo se va a transformar en cáncer. El cáncer no aparece de la noche a la mañana y es muy importante tomar en cuenta que las hemo-

rroides jamás se convierten en cáncer, porque hay muchos pacientes que se confunden”, alertó.

Y en relación a la colitis, este especialista dijo que la padece del 30 al cincuenta por ciento de la población.

“La principal causa son los trastornos funcionales, la mala dieta y la ansiedad. La inestabilidad emocional, el estrés, eso tiene que ver, la falta de sufientes líquidos en tu dieta; la ausencia de ejercicio, el sedentarismo.

“Yo como especialista haría muy mal en recetar al paciente sin saber primero qué es lo que tiene. Primero hay que saber que no se trate de algo de peligro o de cuidado, por lo menos”, mencionó.

Por Pamela Escobar Cifras médicas alertan sobre las elevadas cifras de las muertes maternas en el mundo por embarazo ectópico, un padecimiento que, de acuerdo con el ginecólogo Ulises Guerrero Cuevas, es un embarazo fuera de la matriz.

“Este tipo de gestación se pueden localizar en diferentes partes fuera de la matriz, desde el cuello de la misma, hasta en ovarios, abdomen o trompas. Este último, el más frecuente, justo en la región más amplia de la trompa uterina, por el espacio y cavidad que hay”, explicó el médico.

Agregó que no hay síntomas, como tal, ya que las pacientes no presentan ninguno que pudiera alertar sobre el hecho de que el embarazo se esté gestando fuera de la matriz.

Pues todos los síntomas que se presentan son los de un embarazo normal, desde que la prueba sale positiva “El único detalle que cambia es la cantidad de fracción beta, denominada así a la hormona que produce el embarazo. Nosotros tenemos que ver dentro de la matriz un saco y medir arriba de mil unidades de esta hormona, pero si no vemos ese saco y la paciente tiene más de las mil unidades, debemos de sospechar de que se trata de un embarazo ectópico”, explicó el doctor.

Advirtió que este tipo de embarazos no llegan a término por la falta de viabilidad, además de todos los riesgos que enfrenta la madre, y explicó que hay de dos tipos: el embarazo ectópico y el embarazo ectópico roto.

“El denominado roto es el más peligroso, pues amenaza la vida de la gestante y es considerada una de las principales causas de muerte materna en Reynosa, si la paciente no lo detecta a tiempo mediante el control prenatal, el embarazo sigue creciendo en una ubicación que no cumple las características para ello, por eso se rompe y sangra en abundancia y puede llevar a la muerte de la paciente”, advirtió el ginecólogo.

Detalló por otro lado que aunque no hay síntomas premonitorios específicos, el método para detectarlo es un ultrasonido vía vaginal; además la paciente puede sufrir una hemorragia al presentar un embarazo ectópico roto y permanecer en estado crítico.

“En algunos casos cuando la madre no

Dr. Ulises Guerrero Cuevas se da cuenta que padece un embarazo de ese tipo, puede llegar a presentar un dolor súbito a nivel del abdomen, y que a su vez pueden llevar al desmayo, a un efecto choque, todo ocasionado por la pérdida de sangre tras la hemorragia”, explicó el especialista.

FACTORES DE RIESGO

Quienes tienen más riesgo de sufrir este padecimiento, es quien haya tenido un embarazo ectópico previo, el tabaquismo, infecciones vaginales que se vuelven pélvicas, es decir, una infección vaginal no bien atendida y que sube hacia la matriz o bien, pacientes que se recanalizaron de trompas tras una salpingoclasia.

En el caso de este último, es el de mayor peligro.

Además mencionó Guerrero Cuevas que es un padecimiento que no se puede prevenir, solo evitar los factores de riesgo antes mencionado. En el caso de las infecciones vaginales, detectar si dichas infecciones son recurrentes y acudir al ginecólogo para tratarlas a tiempo y de esa manera evitar que se vuelvan pélvicas. En caso de fumar, redecir el consumo de tabaco.

“No existe un grupo etario que se pueda decir sea más susceptible a padecerlo. El factor de riesgo está generalizado para cualquier mujer que busque un embarazo, pues la edad no es una característica distintiva en los casos de embarazos ectópicos”, comentó Guerrero Cuevas.

CONSECUENCIAS

Las consecuencias son principalmente para la madre, dijo el médico pues el embarazo no llega a término, por el lugar donde se desarrolla y el embarazo ectópico es también una de las principales causas de muerte para la madre por la pérdida de sangre.

“La gestante también puede perder el ovario, si el embarazo se ubica en esa parte del área reproductiva de la mujer, o la trompa uterina, una de las más graves si no se detecta a tiempo y posterior a eso se rompe y sangra”, explicó el entrevistado.

Agregó que también lo pueden sufrir quienes se embarazan por vía inseminación artificial o por el uso de técnicas de fertilidad.

TRATAMIENTOS

El tratamiento ideal para el embarazo ectópico actualmente es por vía laparoscópica, detalló el médico, y se realiza por medio de tres incisiones muy pequeñas de mínima invasión y secciona la ubicación del embarazo.

“Actualmente es el método ideal por que causamos menos daño en las estructuras de reproducción femeninas como lo son los ovarios el útero y las trompas. Claro, siempre y cuando no esté roto. De no ser así, el tratamiento a seguir, es por medio de laparotomía exploradora”, explicó el doctor.

Añadió que laparotomía es abrir el vientre bajo de la paciente y localizar el foco sangrante, ya que en esos casos corre riesgo la vida de la paciente , pues se encuentra ines-

The town of Nuevo Progreso offered a colorful party for the thousands of visitors who participated in Tourist Day .

By José Manuel Meza Nuevo Progreso, Tamaulipas Every March 21 marks the birthday of President Benito Juárez, but specifically, this same day a community on the border of Tamaulipas, Mexico, celebrates one of its most important festivities.

It is offered to foreign tourists who are already returning to the north of the continent as the freezing climates of winter dissipate and spring begins.

This is Nuevo Progreso, a friendly community that year after year receives a huge number of vacationers who settle in South Texas, many of whom cross the border and visit this place to enjoy its folklore, gastronomy and to access the various health services offered, since it is also considered the “Medical Tourism Capital of Mexico”.

This time the municipal government of Rio Bravo, which is responsible for the administration of this town, held a lively party for the ‘winter Texans’.

They danced, ate, bought handicrafts, and were mainly entertained with the hospitality of the inhabitants of this Mexican corner, its

The mayor, Juan Diego Guajardo, who was accompanied by his wife, Rosalba Viera, said that over four decades this celebration has served to strengthen the ties of friendship and trade with the United States.

Later they were distinguished as Miss and Mr. Tourist 2018, the citizens Kristi Collier and the northern music singer Rosendo Cantú, respectively, who were given “la cuera” typical dress of the State of Tamaulipas.

Also crowned with the traditional charro hat to the king and queen of the ‘winter Texans’ Bob and Jody Ferns. The celebration was a success and many foreign and national tourists enjoyed a cordial and entertaining way, extending their hands so that next year they will visit Nuevo Progreso again.

‘winter texans’ El pueblo de Nuevo Progreso ofreció una colorida fiesta para los miles de visitantes que participaron en el Día del Turista.

Por José Manuel Meza Nuevo Progreso, Tamaulipas Cada 21 de marzo se conmemora el natalicio del presidente Benito Juárez, pero en específico, este mismo día una comunidad en la frontera de Tamaulipas, México, celebra una de sus fiestas más importantes.

Es ofrecida a los turistas extranjeros que ya se regresan al norte del continente al disiparse los climas gélidos del invierno y dar comienzo la primavera.

Se trata de Nuevo Progreso, una amigable comunidad que año con año recibe una enorme cantidad de vacacionistas que se asientan en el sur de Texas, muchos de los cuales cruzan la frontera y visitan este lugar para disfrutar de su folklore, gastronomía y para acceder a los diversos servicios de salud que ofrece, ya que también es considerado la “Capital del Turismo Médico de México”.

Bailaron, comieron, compraron artesanías, y principalmente fueron agasajados con la

Viera, expresó que a lo largo de cuatro décadas este festejo ha servido para fortalecer los lazos de amistad y comerciales con Estados Unidos.

Como parte de las actividades, se realizó en el límite internacional el tradicional saludo y cambio de banderas por representantes de las comunidades de Nuevo Progreso y Progreso, Texas.

Posteriormente fueron distinguidos como la señorita y el señor turista 2018, los ciudadanos Kristi Collier y el cantante de música norteña Rosendo Cantú, respectivamente, a quienes se les entregó “la cuera”, típica vestimenta tamaulipeca.

Asimismo se coronó con el tradicional sombrero charro a los reyes de los ‘winter texans’ Bob y Jody Ferns. La celebración fue todo un éxito y muchos turistas extranjeros y nacionales disfrutaron de manera cordial y amena, extendiéndoles la mano para que el próximo año vuelvan a visitar Nuevo Progreso.

Hernias of various types are a common condition that should be addressed and treated by a specialist ... before it becomes a severe problem and requires emergency surgery.

By Fernanda Alvarado Monterrey, N.L. Hernias are a common affliction in people of all ages and the scarce information that we have about them are the causes that the people that sufferers from them put in risk their health when not attending with a specialist.

In Mexico, more than 10 percent of the population has some type of a hernia and they appear more frequently in people between 30 and 60 years old.

Dr. Jorge Ballí Martínez explained that the gastroenterologist is the specialist indicated to treat this condition.

In an interview, the specialist said that hernias without treatment are like “time bombs” that usually get complicated and require emergency surgery.

4What are hernias?

The hernias are a protrusion of the content of the abdomen that comes out of its place, it can be said that it is a small ball, medium or large, that springs from a weakness of the abdominal wall.

4What types of hernias are there?

There are four types, the most common is an umbilical hernia that goes around the bellybutton.

The second is an inguinal hernia, which is commonly seen in men, is located in the groin area and is very difficult for patients to detect at the beginning. They are caused by the weakness of the inguinoscrotal wall. In this type of hernias, fat content is produced and later the intestines go to the front, near the testicle or the scrotum of the patient.

The third type of a hernia is that of Spiegel, they are not very common and appear at the junction of the lateral edge of the rectus abdominis muscle.

The last type is Post-incisional hernias, they are a protrusion that comes out after a surgical intervention. In the scar of the operation can leave a hernia that is very dangerous for the person who has it.

4How can someone get a hernia?

There are many reasons why they can be produced but we have to go back to the synthesis of collagen because it is what gives us strength in the abdominal wall.

Inheritance is one of the main reasons for

this condition. In many of the cases, the patient’s family has, like him, a deficiency in the synthesis of collagen.

Physical activity that requires a lot of effort, can contribute to these hernias, that are caused by the increase in intra-abdominal pressure.

4Who is more likely to suffer from hernias?

As previously mentioned, inheritance is an important factor for the appearance of hernias, but the most prone are: athletes, work-

ers (who perform constant heavy physical effort), pregnant women due to the pressure exerted by the product during pregnancy and last, the people who have been operated on.

It is necessary to include those who suffer from constipation problems because at the time of evacuating they push a lot and that effort goes directly to the abdomen that can end up developing hernias. Also, those who have problems urinating, either because the prostate is swollen or because they have had problems with their bladder, may develop an umbilical or an inguinal hernia depending on where their weakness is.

Others are those who suffer from a chronic cough because they make a lot of effort in the stomach every time they cough. Smokers often do so and inadvertently develop the problem.

4Is it true that babies who are allowed to cry for a long time, can herniate?

It’s a total myth, there is no scientific article that proves that. If there are babies that can get to have hernias but for other reasons, like heredity or because they were born prematurely.

4What are the indicators or warning signs that a hernia is growing?

When a hernia starts, some people show a cramp or pain in the inguinal region, by the navel or near the testicle in the case of men.

Another symptom is to feel a ball that is growing in the body and that is totally diminished when we are lying down.

4Can the growth of a hernia be stop wearing sashes?

Yes, you can stop the growth, but they will not heal or close a hernia, the problem is that the sashes are annoying and uncomfortable and hardly anyone can use it every day throughout the day.

4How large can a hernia grow?

It can have a giant size, so much that the intestine and the complete colon fit in a hernia. This type is very difficult to deal with.

We must prevent them from reaching that size and attack them in time.

I had a patient that I successfully operated on, that had an inguinal hernia, that covered his genitals due to its large size.

4How can we prevent a hernia?

Sometimes you can prevent it, if we put on a belt to perform physical activities of great effort, by not smoking, if we are not constipated. This way we can prevent them from appearing on our body, although this does not guarantee anything.

4Can a person live normally with a hernia?

There are people who know that they have a hernia and prefer not to attend to it, they do their daily activities and of course they can live for many years, however, it is like having a time bomb inside the body because the intestine is stuck in that hernia, it is prone to get strangle and become a problem.

Sooner or later the patient would need and emergency surgery which is a more risky operation than having it attended in time.

There are people who may lose part of their intestines or suffer from peritonitis due to not being treated.

4What is the indicated treatment for hernias?

For each type of a hernia, what must be done is to repair the wall, a mesh is placed to restore that abdominal wall, the hole of a hernia must be covered.

Years ago an open surgery with suture was performed, but the recurrence rate of this operation was up to 48%, the patients returned,because the would appear again.

Currently, we enter with mini paroscopy (mini surgery), it is done with small instruments no larger than 5 millimeters and then we put a mesh accepted by the body, made of a plastic material that reinforces that wall and covers the space.

Bomba de tiempo Las hernias de diversos tipos son un padecimiento común al que debe ponérsele atención y atender con un especialista... antes que se convierta en un problema severo y requiera una intervención quirúrgica de emergencia.

Por Fernanda Alvarado Monterrey, N.L. Las hernias son un padecimiento común en personas de todas las edades y la escasa información que se tiene del tema provoca que quienes las padecen pongan en riesgo su salud al no atenderse con un especialista.

En México más del 10 por ciento de la población presenta algún tipo de hernia y aparecen con mayor frecuencia en personas de entre 30 y 60 años.

El doctor Jorge Ballí Martínez, explicó que el gastroenterólogo es el especialista indicado en atender este padecimiento.

En entrevista, el especialista dijo que las hernias sin atenderse son como “bombas de tiempo” que generalmente se complican y hacen necesaria una intervención quirúrgica de emergencia.

4¿Qué son las hernias?

Las hernias son una protrusión del contenido del abdomen que se sale de su lugar, se puede decir que es una bola chica, mediana o grande, que brota por una debilidad de la pared abdominal.

4¿Qué tipos de hernias existen?

Hay cuatro tipos, la más común es la hernia umbilical que sale alrededor del ombligo.

La segunda es la hernia inguinal, que

se presenta comúnmente en hombres, se ubica en la zona de las ingles y son muy difíciles de detectar por los pacientes al principio. Son causadas por la debilidad de la pared inguinoescrotal. En este tipo de hernias se produce contenido de grasa y posteriormente los intestinos se van para el frente, cerca del testículo o el escroto del paciente.

El tercer tipo de hernia es la de Spiegel, no son muy comunes y aparecen en la unión del borde lateral del músculo recto del abdomen.

El último tipo son las hernias Postincisionales, son una protrusión que sale posterior a una intervención quirúrgica. En la cicatriz de la operación puede salir una hernia que resulta muy peligrosa para quien la padece.

4¿Cómo se produce una hernia?

Existen muchas razones por las cuales se puedan producir pero hay que remontarnos a la síntesis de colágeno porque es el que nos da la fortaleza en la pared abdominal.

La herencia es una de las principales razones de este padecimiento. En muchos de los casos, la familia del paciente tiene al igual que él, una deficiencia en la síntesis de colágeno.

La actividad física que requiere de mucho esfuerzo, puede contribuir para que estas hernias aparezcan, son causadas por el aumento de la presión intraabdominal.

4¿Quiénes son más propensos a padecer hernias?

4¿Es cierto que a los bebés que se les deja llorar por mucho tiempo, pueden herniarse?

Es un mito totalmente, no hay ningún artículo científico que pruebe eso. Sí hay bebés que pueden llegar a tener hernias pero por otras razones, como herencia o porque nacieron prematuros.

4¿Cuáles son los indicadores o focos de alerta que muestran que una hernia está creciendo?

Cuando inicia una hernia, algunas personas manifiestan un calambre o dolor en la región inguinal, por el ombligo o cerca del testículo en el caso de los hombres.

Otro síntoma es sentir una bola que está creciendo en el cuerpo y que disminuye totalmente cuando estamos acostados.

4¿Se puede evitar el crecimiento de una hernia con fajas?

Sí, se puede detener el crecimiento, pero no van a curar ni a cerrar la hernia, el problema es, que las fajas son molestas e incómodas y difícilmente alguien puede utilizarla todos los días durante todo el día.

4¿A qué tamaño puede llegar a crecer

una hernia?

Puede llegar a tener un tamaño gigante, tanto que el intestino y el colón completo quepan en la hernia. Este tipo es muy difícil de tratar. Hay que evitar que lleguen a ese tamaño y atacarlas a tiempo.

En algunas ocasiones se puede prevenir si nosotros nos ponemos faja para realizar actividades físicas de mucho esfuerzo, si no fumamos, si no somos estreñidos. Así podemos evitar que salgan en nuestro cuerpo, aunque esto no garantiza nada.

4¿Una persona puede vivir normalmente con una hernia?

Hay personas que saben que tienen una hernia y prefieren no atendérsela, hacen sus actividades cotidianas y claro que pueden vivir muchos años, sin embargo, es como tener una bomba de tiempo dentro del cuerpo porque al estar el intestino metido en esa hernia, está propenso a que se estrangule y se tenga un problema.

Tarde o temprano tendrá que ser operado de emergencia, es una operación mucho más riesgosa que habérsela atendido a tiempo. Hay personas que pueden llegar a perder parte su intestino o sufrir de una peritonitis por no atenderse.

4¿Cuál es el tratamiento indicado para las hernias?

Para cada tipo de hernia lo que hay que ha-

cer es reparar la pared, se coloca una malla para que se restituya esa pared abdominal, el hueco de la hernia hay que taparlo.

Años atrás se realizaba una cirugía abierta con sutura, pero el índice de recurrencia de esta operación era de hasta el 48 por ciento, a los pacientes les volvía a salir la hernia.

Actualmente, entramos con mini paroscopía (mini cirugía), se realiza con instrumentos pequeños no mayores a 5 milímetros y luego colocamos una malla aceptada por el cuerpo, hecha de un material tipo plástico que refuerza esa pared y tapa el espacio.

4¿Cuál es el índice de recurrencia de los pacientes que son operados?

En mi caso, el índice de recurrencia es de 0.2 por ciento al 0.5 por ciento. Disminuye drásticamente la incidencia con estos nuevos materiales que utilizamos hoy en día para quitar una hernia.

4¿Qué cuidados se deben tomar posteriores a la cirugía?

Colocar una faja para que la protrusión vaya tomando su forma normal en el cuerpo.

No, los pacientes que creen que los remedios caseros ayudarán a evitar el crecimiento de la hernia están perdiendo su tiempo, lo que deben hacer es atenderse con un especialista y realizar la paroscopía para acabar con el problema.

4¿Cuál es el tiempo de recuperación, posterior a la operación de la hernia?

La recuperación es muy rápida, a comparación de la cirugía abierta hay mucho menos dolor y ese mismo día el paciente anda de pie, caminando y comiendo. Ese mismo día pueden irse a su casa a descansar o pueden quedarse en el hospital una noche si así lo desean.

Los que trabajan en oficina pueden regresar a trabajar al tercer o cuarto día. Los pacientes que realizan labores pesadas, les recomiendo más tiempo de reposo en casa, de 15 días a 30 días de recuperación.
