The Austrian Hans Asperger was the first to work with the syndrome that now bears his surname in the mid-40s when
autism was considered a kind of “psychosis”. In his studies with children, the Austrian doctor realized two things: that they have a “strange” behavior, especially in terms of their social relationships and that they use a farfetched, somewhat irritating and inappropriate language for their age, I call these guys “little teachers” or even “natural little genius”. Not all scientists have agreed on what “being Asperger”
means. Some believe that it is simply a different way of seeing the world, a special type of personality. Others consider it a developmental disorder. El síndrome de Asperger es un trastorno neurobiológico caracterizado por problemas mentales y conductuales que forma parte de los trastornos del espectro autista. Su principal característica es la dificultad en la interacción social y en encontrar intereses. Pese a ello, no representa una limitante en la demostración de talento. El austríaco Hans Asperger fue el primero en trabajar con el síndrome que ahora lleva su apellido a mediados de los años
40, cuando el autismo se consideraba una especie de “psicosis”. En sus estudios con niños, el médico austríaco se dio cuenta de dos cosas: Que tienen un comportamiento “extraño”, sobre todo en cuanto a sus relaciones sociales y que usan un lenguaje rebuscado, un tanto irritante y poco apropiado para su edad, de ahí que llame a estos chicos “pequeños profesores” o incluso “geniecillos naturales”. No todos los científicos se han puesto de acuerdo sobre en qué consiste “ser Asperger”. Algunos creen que es sim-
plemente una forma distinta de ver el mundo, un tipo especial de personalidad. Otros consideran que es un trastorno del desarrollo. El síndrome es difícil de determinar, pues las personas que lo tienen pueden ocultarlo con su buena comunicación oral, un coeficiente intelectual dentro de lo normal e incluso, muchas veces, más alto de lo normal, y un estilo de vida solitario. A psychology in obesity can help achieve real and permanent change in eating habits, it can define the background of the condition and weight in a person.
By Pamela Escobar One of the aspects that most concern people are physical appearance and one way to improve it is to lose weight, for this, they use the help of surgical procedures or diets, but they do not know the background of the problem. For this you can get help from a psychologist in obesity, which is a mixture of mental processes that affect patients in the control of their weight, so said the graduate in Nutrition and Food Science Jessica Gama Meléndez. The nutritionist explained this as the set of both terms, that is, psychology is the science that studies the mental processes, sensations, perceptions and behavior of the human mind in the social and personal day life. “While the approach to obesity could be defined as the background to the condition of weight, how a person reaches a certain weight, why he came to this, the process, the feelings, motives, the way in which it is perceived, how can it not magnify its weight, its attitude, its damage in health, among other things”, explained the graduate of the Autonomous University of Tamaulipas. La psicología de la obesidad ayuda a lograr el cambio real y permanente en hábitos alimenticios, puede definir el trasfondo de la condición y peso en una persona. Por Pamela Escobar Uno de los aspectos que más preocupan a las personas es la apariencia física y una manera de mejorarla es el bajar de peso, para eso se ayudan de procesos quirúrgicos o dietas, pero desconocen el trasfondo del problema. Para ello se puede echar mano de la psicología de la obesidad, que es una mezcla de la procesos mentales que afectan a los pacientes en el control de su peso, así lo dijo la licenciada en Nutrición y Ciencia de los alimentos Jessica Gama Meléndez. La nutrióloga lo explicó como el conjunto de ambos términos, es decir, la psicología es la ciencia que estudia los procesos mentales, sensaciones, percepciones y comportamiento del ser humano en el ámbito social y personal. “Mientras que el enfoque hacia la obesidad se podría definir como el trasfondo de la condición del peso, el cómo una persona llega a cierto peso, por qué llegó a éste, el proceso, los sentimientos, motivos, la forma en cómo se percibe, cómo puede no magnificar su peso, su actitud, su daño en la salud, entre otras cosas”, detalló la egresada de la Universidad Autónoma de Tamaulipas. La especialista en Psicología de la Obesidad comentó además que la mayoría de las personas no se dan cuenta en qué punto llegan a aumentar su peso; hasta llegar al grado de mencionar: “lo máximo que había pesado”. “La psicología de la obesidad, no es resolver estas dudas, sino ayudarle como guía a la persona en hacer una introspección de sí misma, que encuentre las respuestas al cómo, por qué, cuándo subió de peso y así encontrar una solución”, agregó Gama Meléndez. De acuerdo con la doctora, integrante del Colegio de Nutriólogos de Tamaulipas, el respaldo de la psicología de la obesidad ayuda tratando la parte externa del cambio de hábitos alimenticios y el fondo del problema, enfrentando al paciente con su realidad personal. BENEFICIOS EN LA PSICOLOGÍA DE LA OBESIDAD
La integrante del Comité Multidisciplinario para la prevención de sobrepeso y obesidad en Reynosa, comentó que se tienen múltiples beneficios a utilizarla, como lograr el resultado deseado, personalizado y lograr adherencia en el cambio de alimen-
tación, convirtiéndolo en un cambio realmente permanente. “Es recomendable además de asesoría médica y no solamente alterna, debería ser parte fundamental del todo equipo multidisciplinario de médicos, para lograr el objetivo del cambio de vida de cada paciente”, dijo la nutrióloga. Precisó que es importante que sea utilizada por todas las personas con hábitos poco saludables de alimentación, siguiendo la terapia indicada se tenga o no obesidad. Consideró que la psicología aplicada en las cuestiones de salud y alimentación debería ser mucho más valorada y tomada en cuenta. “El uso de la psicología de la obesidad puede apoyar para otro tipo de padecimientos aparte del control de peso, lo anterior, al enfocarse en conocer el proceso mental del paciente; realizando un buen expediente, así como dar un diagnóstico certero y se enseñe a la persona la forma correcta del cambio de hábitos, sea cual sea el caso”, advirtió la especialista de la alimentación. “La mayoría de las personas tiene una idea equivocada del por qué han subido de

peso o de plano no tiene ni idea y ésta hermosa ciencia de la mente humana nos puede ayudar a dar en el clavo y así al fin lograr un equilibrio en nuestra salud con base en los hábitos alimenticios”, añadió la especialista.

By José Manuel Meza According to Erick Adrián Huesca Pérez, a nutritionist graduate from the Autonomous University of Nuevo León (UANL), vitamins are necessary for the body and are naturally present in many foods. When a variety of essential nutrients are not consumed, there are those who opt for food supplements and the number of people who use them is increasing. Nowadays the use of such products is mainly focused on the improvement of physical performance and its consumption is a very common practice in people who attend gyms, although they are also used by office workers, workers, students and children. However, the vast majority ingests them on the recommendation of their parents, friends, coaches and not by a specialist, which, according with Huesca Pérez can also cause health problems. “You need to keep control, and the person should not self-medicate either, because sometimes he or she does not require supplements and is overloading the liver”, he explained. Huesca Pérez remarked that the consumption of vitamins is only recommended when you have a lack of nutrients. “One of the benefits of supplements is that it can reduce the possibility of contracting diseases such as anemia. The consumption of iron and some other vitamins help to prevent such ailments”, he said. Because of the universal recommendation to eat fruits and vegetables, the nutritionist mentioned some that provide the number of minerals and vitamins that the body needs. “Apple, banana, tomato, spinach, but to consume the number of vitamins that a spinach brings you have to eat about eight or nine bouquets. So, when we are deficient in some vitamin, instead of resorting to feeding on so many spinach, a pill will be enough”, he said. He added that supplements containing B complex are recommended, which are generally sold in capsules at a cost accessible to anyone. And he said that one pill a day is more than enough.

En el mercado existen productos para todo público que no solamente reponen una deficiencia de nutrientes, sino que contribuyen a un mayor rendimiento físico, a mejorar el aspecto de la piel, las uñas, estimular el apetito y ayudar a tener buena memoria, entre otros, pero ¿son realmente efectivos? Por José Manuel Meza De acuerdo con Erick Adrián Huesca Pérez, nutriólogo por la Universidad Autónoma de Nuevo León (UANL), las vitaminas son necesarias para el organismo y se encuentran presentes de forma natural en numerosos alimentos. Cuando no se consume una variedad de nutrientes esenciales hay quienes optan por los suplementos alimenticios y cada vez es mayor la cantidad de personas que los utiliza. Hoy en día el uso de tales productos está principalmente enfocado al mejoramiento del rendimiento físico y su consumo es una práctica muy común en personas que asisten a los gimnasios, aunque también los emplean oficinistas, obreros, estudiantes y niños. No obstante, la gran mayoría los ingiere por recomendación de sus padres, amigos, entrenadores y no por un especialista, lo cual, a consideración de Huesca Pérez también puede ocasionar problemas de salud. “Se necesita llevar un control. La persona no debe de automedicarse tampoco, porque a veces no requeriere de suplementos y le está dando una sobrecarga al hígado”, explicó. El nutriólogo, quien se desempeña en el Sector Salud, subrayó que la aceleración del metabolismo también puede originar daños, porque el paciente entra en un estrés metabólico y en lugar de ser saludable tomar vitaminas puede terminar siendo perjudicial. Por eso manifestó que para buscar un equilibrio se necesita acudir al nutriólogo, quien sugerirá la cantidad adecuada que una persona necesita. “El ingerir muchas vitaminas provoca que el paciente tenga hambre, porque le estimulan el apetito, entonces pueden caer en el error de estar consumiendo calorías de más que no necesita por el consumo de suplementos”, enfatizó.

CUANDO SÍ Y CUANDO NO TOMARLOS
Se define como suplementos a los productos que contienen ingredientes como vitaminas, minerales, aminoácidos y enzimas para complementar los componentes de una dieta. Sin embargo, para Huesca Pérez cuando alguien tiene una alimentación balanceada éstos no son necesarios. Aún así la proteína es el vitamínico que más demanda tiene en un 54.3 por ciento del mercado. De acuerdo con diversas encuestas de cada diez personas que la consumen sólo dos fueron recomendadas por un nutriólogo. Cabe decir que los suplementos son ofertados en el mercado en diferentes presentaciones y formas como tabletas, cápsulas, polvo, líquidos y barras energéticas, los cuales generalmente no entran en la categoría de fármacos. La gente los busca por su efectividad hacia ciertas funciones, pues hay estudios que demuestran que la creatina mejora significativamente la fuerza, el desarrollo muscular y el desempeño en actividades físicas de alta intensidad.
En el caso de la cafeína, se ha constatado que reduce el tiempo de reacción y retarda la fatiga en la práctica de deporte. No obstante, sus efectos biológicos pueden incrementar la tensión arterial y ser causa de infartos por su uso excesivo y prolongado. ‘SÍ SON EFECTIVOS, PERO CON MODERACIÓN’
El especialista Érick Adrián Huesca Pérez dijo que hay pacientes que tienen una deficiencia o una necesidad de nutrientes debido a una mala absorción, por lo que es necesario hacer uso de estos suplementos. “Sí ayudan porque es muy práctico consumirlos, pero obviamente es mucho mejor la alimentación orgánica, pero estos productos sí tienen una buena función en el organismo como auxiliar alimenticio”, mencionó. El entrevistado reconoció como un hecho que por el estilo de vida en las ciudades la gente ya no se alimenta igual que antes. Por Pamela Escobar De acuerdo con cifras de la Organización Mundial de la Salud (OMS), las infecciones respiratorias agudas y la enfermedad pulmonar obstructiva crónica, permanecerán dentro de los primeros lugares como causa de muerte en todo el mundo. Además de las muertes por dichos padecimientos se les suman la neumonía y asma, mismas que a largo plazo causarán complicaciones a nivel mundial, así lo advirtió también el otorrinolaringólogo, Rubén Heredia Garza. “Para el año 2050 la resistencia de las bacterias a los antibióticos va a matar más personas que el cáncer por el actual uso indiscriminado de los antibióticos para contrarrestar una gripe común” detalló el especialista en enfermedades respiratorias. El egresado de la Universidad Autónoma de Nuevo León, explicó que en el caso del catarro común no se debe de tomar antibiótico y menos si se padece alguna enfermedad crónico degenerativa como la diabetes o cáncer, tampoco deben tomarlo personas de la tercera edad pues se puede complicar y convertirse en una enfermedad infecciosa. Sin embargo, comentó que el 80 por ciento de las infecciones de vías aéreas superiores en los adultos son virales y autolimitadas como la rinosinusitis o también comúnmente conocida como gripe e influenza, mientras que el 20 por ciento restante son bacterianas. Mencionó que el otro 20 por ciento tiene virus y bacterias al mismo tiempo, mismas que inflaman las vías aéreas como las mucosas respiratorias y cuando eso sucede son más susceptibles a infectarse. “De las de tipo viral, 8 de cada 10 se curan al paso de 5 o 9 días de manera natural gracias a los mecanismos

Reconoció al Cabildo por haber aprobado el PIOP 2017 con el cual se desarrolla la pavimentación hidráulica de 3 mil 591.22 metros cuadrados de la calle José María Morelos, con una capa de concreto hidráulico de 15 centímetros de espesor. Para el resguardo de los peatones, la calle incluye 1,177.29 metros cuadrados de banquetas y 643.38 metros lineales de guarnición trapezoidal, además de la aplicación de 280.93.00 metros lineales de pintura de tráfico que delimitará los carriles de circulación vehicular y marcará los pasos peatonales. Serán instalados 13 postes de señalización vertical y también se hará la reposición de 10 luminarias LED, que ofrecen más intensidad lumínica y menor consumo de energía.
Para mejorar la eficiencia del sistema de drenaje sanitario en la calle Morelos, fueron desazolvados y renivelados cuatro pozos de visita y se instalaron 30 descargas sanitarias, se instalaron y renivelaron 93 tomas domiciliarias y también fueron renivelados 93 registros sanitarios del drenaje. De esta forma, se completa una obra integral de las más de 300 que el gobierno municipal que preside la doctora Maki Ortiz realiza por los sectores más necesitados de la ciudad.

Sit ... Stay ... PLAY: Pioneering dog-therapy program comforts patients. By Victoria Shelton Fort Worth, Tx. Cook Children’s has brought in visiting dogs for decades as a way to help patients and their families cope with their situations, but it was one of the first hospitals nationwide to realize the value in having dogs as badge-yielding staff members. “People wouldn’t have pets the way they do and treat them like family members if there wasn’t something to that”, said Facility Dog Program Coordinator Kizzy Marco. Just over three years ago, Ralph and Chanel became the first dogs to join Sit...Stay....PLAY, the facility dog program at Cook Children’s. “We are pioneering something really new”, said Marco. “The majority of hospitals in the country have animal-assisted therapy or dogs that visit routinely and that’s wonderful, but having them here as residents and as employees makes such a difference”. As facility dog program coordinator, a large portion of Marco’s job involves handling Ralph, answering referrals and working one-onone with patients and their families. “I feel like people moved mountains to make these dogs available for our patients and families”, said Marco. “I’ve always loved working at Cook Children’s, but that was the moment I realized how much this place cares, probably beyond words, about the emotional well-being of its patients. Of course health care exists to take care of people physically, but we also have a responsibility to take care of people emotionally and these dogs are doing that”. When Ralph comes in contact with a patient, the atmosphere changes immediately. “I’ve seen kids at their very sickest”, said Marco. “I’ve seen kids at the end of their lives light up to see Ralph”. Stephanie Myers witnessed the power of dog therapy first-hand when her 4-year-old daughter Baylee was introduced to Ralph after being diagnosed with stage 4 neuroblastoma. “She hadn’t walked or gotten out of bed in about a week”, said Myers. “Kizzy brought Ralph in and I saw a light in Baylee’s eyes I hadn’t seen in a while. She instantly wanted to sit up and then stand so she could pet him”.

Ralph brought normalcy and softness to the situation with Baylee, as he does with so many others. “Anytime Baylee has needed a little push, Ralph has been there to guide her through it”. said Myers. “He is such an amazing therapy dog and has given our daughter strength and a smile when we thought she might be out”. “What our dogs do for patients is deep. A patient once vulnerably told me that he loved Ralph because he knew Ralph would never judge him. That says it all right there”, said Marco. Because of the large number of both inpatient and outpatient cases, therapy dogs are not able to sit on every child’s bed or walk the halls with them or get ready for surgery with them. However, many patients, families and staff members get the experience of seeing one of the dogs in passing walking down the hallway or in the cafeteria, which Marco considers a therapeutic encounter. “You don’t know what people are carrying emotionally when they’re walking through the hallways”, said Marco, “so adding this goofy, smiling, prancing, fluffy, handsome Golden Retriever to the environment is so important. If you never knew a single patient story, if you never knew how much of an impact these dogs make, just knowing that is huge”. When Ralph does have the opportunity to
work one-on-one with a patient, they form a bond so strong that every child thinks Ralph is their best friend. When they see him and start calling his name, his tail starts to wag, he squints his eyes and starts shaking around with excitement. “It’s almost like he remembers every kid he’s ever met”, said Marco. While the dogs are a friendly face for children at the medical center, they really do serve an important role in the physical and emotional well-being of these patients. Whether giving a dog a syringe of water mixed with juice to get a child to take medicine or taking him for a walk to get kids up and moving, these animals are trained to relate to the patients and their difficult experiences in a way that normalizes their environment and gives them something else to focus on. Using dogs as a means of reducing anxiety in patients is especially true for Child Advocacy Resources and Evaluation (CARE) Team, which assesses cases of child abuse. “When you’re talking about abuse and really difficult subjects with these kids, having a dog there can really ease their anxiety and help them voice what happened to them,” said Jamye Coffman, M.D., a child abuse pediatrician at Cook Children’s. “When you reduce the anxiety in a child, it reduces all the stress hormones so you don’t have any of the effects of stress on the child that can contribute to illness.” Dr. Coffman has handled Kitty since she came to Cook Children’s nearly a year and a-half ago. Kitty stays with the CARE Team and is trained to jump up on the couch with patients when they are being interviewed to help relax them and their parents, making the environment feel less like a doctor’s office and more like a home. But there is also an art to the things that dogs can do innately, the things that cannot be taught. Not long after Marco and Ralph began working as a team, they were asked to visit a patient with a severe developmental delay. Marco was admittedly nervous because this patient was unpredictable in her movements and she was not sure how Ralph would react. Without hesitation, Ralph walked up to the
patient, looked her in the eyes and put his head on her lap while he sat there and let her do whatever she wanted. “That’s the true gift of our dogs is that they don’t see any differences,” said Marco. “We know patients whose illness or treatments make them look or behave in a certain way. We know the ones that have especially great challenges, and hopefully it doesn’t alter how we interact with them, but Ralph doesn’t even know those things”. For that child and her family, interacting with Ralph was a completely normal experience and there was nothing atypical about the situation. “That’s an innate art that is something that we as a health system found a way to provide. It had never been provided to our patients and their families before”, said Marco. “It is very humbling to know that our dogs can make moments like that happen for families”. It’s important to remember that Ralph is only one dog out of five at Cook Children’s, making up only one-fifth of the program. Our most recent addition is a 2 year old Golden named Lulu who started working in inpatient psychiatry and partial hospitalization program just about a month ago. Every day these dogs have encounters like this one, and every day they are doing something to make the hospital a better place for patients and their families. The impact that these dogs have made and the moments they have shared with people in the time they have been at Cook Children’s is remarkable. “It is profound that they know and have the ability to mourn with people who are mourning, to celebrate with people who are celebrating and to play with people who are playing,” said Marco. “I think there is a bond or intuition that connects them to people exactly where they are, and to watch that is amazing”. Achalasia
A
rare disorder that hinders the transfer of food and fluids to the stomach is achalasia and the way to detect and correct it in time is through endoscopy. To this end, the first international course of the region was held in Reynosa, where topics such as endoscopic or mucosal dissection were addressed. Once we have detected gastric cancer or the incipient colon cancer in the patient, it is how to solve it, what are we going to do, since one thing is to detect it and the other is how we are going to treat it. For this, there are techniques by endoscopic routes that avoid an open surgery, or a laparoscopic one with which we can solve this type of diagnosis. That’s what we talked about in this course. The POEM O was also approached, that’s the endoscopic procedure to solve this relatively frequent problem in this country, the acalsia, which is in the final part of the esophagus where it joins the stomach, there is a muscle that functions as a valve, normally it is closed and when it enters the food it opens and allows it to pass, to later close again and in this way prevent stomach acid from returning to the esophagus. But in achalasia this muscle loses its function and remains contracted, it does not relax and then the food begins to accumulate in the esophagus and that brings as a disorder, weight loss of the patient and many other alterations. Until a few years ago these problems were solved by means of surgeries, but now we do it by means of endoscopies and that is what we call POEM, that is endoscopy or endoscopic oral myotomy; Incidentally, this is the central part of this course. Physicians from Argentina, Nicaragua, Costa Rica, Peru from Central and South America attended the courses. And
the doctor from Argentina asked us to go to their country to do this same course and replicate it with Dr. Michelle Kahaleh who, as well as being the course director, has been my mentor for the last three years as well as a great friend and excellent person. Also, Dr. Kahaleh is the chief of interventional endoscopy at a hospital in New York and recognized worldwide and together we replicated this course with the assistance of specialists from all over Argentina. It has been a great advantage recently to also be at the Juarez Hospital in Mexico City in the third endoscopy course of submucosal endoscopic dissection and POEM in one of the most important hospitals in the country and to attend as a teacher of this course. The prevention of colon cancer is a reality and is available in Reynosa at the Center of Gastroenterology and interventionist in the town. And is that every time there are more people under 45 who are diagnosed with colon or stomach cancer, so it would be highly recommended that anyone from 40 years, even if they have no history of family cancer or symptoms to take some time to do an endoscopy Many people for a long time have seen such procedure as a very aggressive and horrible, but we have really advanced so much in the technologies and medicines that we have now, that really does a colonoscopy procedure the patient feels absolutely nothing. La acalasia
U
n trastorno poco frecuente que dificulta el traspaso de la comida y los líquidos al estómago es la acalasia y la manera de detectarlo y corregirlo a tiempo es por medio de la endoscopia. Para ello se realizó el primer curso internacional de la región en Reynosa, en donde se abordaron temas como la disección endoscópica o mucosa. Una vez que ya detectamos el cáncer gástrico o el de colon incipiente en el paciente es cómo resolverlo, qué vamos hacer, ya que una cosa es detectarlo y la otra es cómo vamos a resolverlo. Para ello hay técnicas por vías endoscópicas que evitan una cirugía abierta, o una laparoscópica con las que podemos resolver este tipo de diagnósticos. De eso hablamos en este curso. También se abordó el POEM O, que un el procedimiento endoscópico para resolver dicho problema relativamente frecuente en este país, la acalsia, que es en la parte final del esófago donde se une con el estómago, ahí hay un músculo que funciona como válvula, normalmente está cerrado y cuando entra el alimento se abre y permite que pase, para posteriormente cerrarse de nuevo y de esa manera evitar que el ácido del estómago regrese hacia el esófago. Pero en la acalasia este musculo pierde su función y queda contraído, no se relaja y entonces se empieza a acumular el alimento en el esófago y eso trae como trastorno la pérdida de peso del paciente y muchas otras alteraciones. Hasta hace unos años estos problemas se resolvían por medio de cirugías, pero ahora lo hacemos por medio de endoscopias y a eso le llamamos POEM, es decir endoscopia o miotomía oral endoscópica; dicho sea de paso, la parte central de este curso. A dicho curso asistieron médicos de Argentina, Nicaragua, Costa Rica, Perú de centro y Sudamérica. Y la doctora de Ar-
gentina nos solicito ir a su país a realizar este mismo curso y replicarlo junto al doctor Michelle Kahaleh quien, además de ser director del curso, ha sido mi mentor durante los últimos tres años además de un gran amigo y excelente persona. Asimismo, el doctor Kahaleh es el jefe de endoscopia intervencionista de un hospital en Nueva York y reconocido a nivel mundial y yo junto a él replicamos dicho curso con la asistencia de especialistas de todo Argentina. Ha sido una gran ventaja recientemente estar también en el hospital Juárez de la Ciudad de México en el tercer curso de endoscopia de disección endoscópica submucosa y de POEM en uno de los hospitales más importantes del país y asistir como profesor de este curso. La prevención del cáncer de colon es una realidad y está disponible en Reynosa en el Centro de Gastroenterología e intervencionista de la localidad. Y es que cada vez son más las personas menores de 45 años que son diagnosticadas con cáncer de colon o estómago, por ello sería muy recomendable que cualquier persona a partir de los 40 años, aunque no tenga ningún antecedente de cáncer familiar o síntoma, se realice una endoscopia. Mucha gente durante mucho tiempo ha visto dicho procedimiento como uno muy agresivo y horrible, pero realmente hemos avanzado tanto en las tecnologías y medicamentos con los que contamos ahora, que realmente hacer un procedimiento de colonoscopia el paciente no siente absolutamente nada. Porque se aplica un sedante y cuando el paciente despierta ya terminamos con el procedimiento que fue indoloro para la persona y nos da la información que necesitamos precisamente para prevenir problemas a futuro y problemas serios que ponen en riesgo la vida del paciente.

The doctor belonging to the American Academy of Dermatology said that there are many economic and good prices in Mexican natural medicine that work excellent in this condition. “When it is a light psoriasis that is common, we start with emollient creams, they can be with a little salicylate vaseline, natural remedies like oatmeal with a little honey, or oatmeal with coconut, salicylate cold cream or coal tar in small quantity, always protecting from the sun”, she explained. Debido al mal diagnóstico, muchas veces la psoriasis es tratada como un hongo, lo que provoca el cambio de la enfermedad, con complicaciones y muchos malestares para el paciente. Por Beatriz Flores La psoriasis es un padecimiento multifactorial que tiene que ver con la herencia, causas genéticas y el medio ambiente que se presenta cuando el sistema inmunológico se altera y se caracteriza por manchas rojas y escamosas. Las regiones más comunes en donde se presentan es en la cabeza o piel cabelluda y los codos, aunque también puede afectar la cara, rodillas, uñas de manos y pies y a las articulaciones, recibiendo ésta el nombre de artritis psoriásica. La dermatóloga María Lourdes Altamirano García, dijo que el estrés también puede ser un factor para su aparición y aumento y que se presenta más en época de invierno o en países fríos, sin embargo, en esta región está presente durante todo el año. “Es un padecimiento que se presenta a nivel mundial, en un tres a cinco por ciento de la población y además de las consecuencias físicas, también tiene sociales porque el paciente es discriminado por su aspecto”, comentó la doctora. NO ES UN HONGO
Altamirano García mencionó que es muy común que el padecimiento sea confundido con hongos, pero aunque sean similares en apariencia, no son iguales. Explicó que debido al uso de esteroides su apariencia puede cambiar y tener grandes complicaciones. “Cuando se ponen esteroides la psoriasis cambia. Por ejemplo, si son manchas rojas con escamas claras y brillantes como si fuera una escamita de concha brillante ya no la ves así, se ve una mancha roja y se puede confundir con otras cosas”, mencionó la doctora, de la Sociedad Mexicana de Dermatología. Dijo que un problema es que en ocasiones, el paciente no tiene comezón y al no presentar molestia deja pasar muchos años antes de acudir al especialista, le recetan tratamiento para hongo y aparecen las complicaciones. Mencionó que para un dermatólogo es fácil identificar la psoriasis debido a la historia clínica y aunque generalmente no se realizan exámenes, es recomendada una biopsia cuando el padecimiento luce fuera de lo normal. “El examen que yo pido es un exudado faríngeo pero no voy a buscar una infección, busco antígenos bacterianos, o sea, todas las bacterias que son normales para nosotros, pe-

ro para la gente que tiene psoriasis no porque le aumenta el crecimiento celular”, agregó. Advirtió que los focos infecciosos como esteptococo y estafilococo son factores desencadenantes, siendo el más frecuente en vías aéreas, oído, nariz y garganta y pulmones. “Cuando hay esto el sistema inmunológico se activa y provoca el crecimiento celular, si tu piel crece en tres semanas, con la psoriasis crece en tres o cinco días y a veces lo ven en los codos y piensan que es resequedad pero no es así”, comentó la integrante de la Sociedad de Cirugía Dermatológica. Pero no solamente se buscan infecciones en vías aéreas, también urinarias, amibas, parásitos, fiebre tifoidea o tifo, cualquier cosa que desequilibre al cuerpo o a las células. El estrés también influye pero no es la causa, aunque si influye y provoca que empeore su situación. “La psoriasis es un proceso inflamatorio de la piel porque hay antígenos en su superficie y al recibirlos empieza a inflamarse, crece como protegiéndose y esa reacción inflamatoria es lo que causa la psoriasis”, explicó. MEJOR LO NATURAL
La médico, perteneciente a la Academia Americana de Dermatología dijo que hay muchas cosas económicas y a buen precio en medicina natural mexicana que funcionan excelente
en este padecimiento. “Cuando es una psoriasis ligera que es la común, empezamos con cremas emolientes, pueden ser con un poco de vaselina salicilada, remedios naturales como avena con un poquito de miel, o avena con coco, cold cream salicilada o alquitrán de hulla en poca cantidad, protegiéndose siempre del sol”, detalló. Es común que se presenten en su consultorio pacientes desesperados por las molestias en su padecimiento luego de haber usado muchos esteroides y que se complicara. Las lesiones se ponen de color rojo, aparece la fiebre, dolores articulares y el paciente está delicado y ante esa situación deben utilizarse medicamentos antiinflamatorios que pueden ser esteroides o no, pero por corto plazo. “Los tratamientos de la psoriasis son de acuerdo a la extensión corporal, al grosor de la piel y de acuerdo a la escama, hay una escala que se llama PASI (Psoriasis area and severity index) y así es como se trata”, expresó. También se utilizan medicamentos que pueden ser tomados y se llaman retinoides, pero tanto en Estados Unidos como en México son muy costosos. “Cuando ya te dio la psoriasis con artritis, que se engarruñan las manos, que ya hay artritis deformante, se te alteran las piernas, todo está rojo y escamado nos vamos a los biológicos, son medicamentos derivados de células que atacan directamente a las células alteradas”, dijo la doctora. Entre los elementos naturales con uso medicinal se encuentra el aloe vera que se puede usar como hidratante y antiséptico. “La psoriasis se puede complicar con hongos y bacterias después de usar muchas cremas, sobre todo esteroides, pueden utilizar el aloe vera siempre y cuando no sean alérgicos porque puede haber quemaduras; también agua de manzanilla con un poco de vaselina o aceite de ricino o de coco como lubricantes”, explicó. Dio a conocer que el agua de mar tiene grandes beneficios, pues sus sales mejoran el crecimiento celular y ayudan a evitar problemas inflamatorios; otra opción es comprar sal de mar y agregarla a agua tibia. “Son cosas naturales que funcionan, hay para todos los pacientes, el problema es cuando ya está muy delicado”, expresó. ESTIGMA SOCIAL
La psoriasis de piel cabelluda, además de un
padecimiento de salud es un problema social, pues muchas personas son discriminadas por su apariencia. “Si tienes tu piel llena de costras con escama y tienes que trabajar en una empresa, se te cae la escama y no te dejan trabajar porque creen que es herpes, te discriminan, las personas son rechazadas a veces creen que es lepra o es algo contagioso, pero es una enfermedad inmunogenética”, dijo Lourdes Altamirano. La mala noticia es que se ha encontrado que quienes tienen psoriasis también presentan problemas metabólicos, hay tendencia a engordar por problemas hormonales, tienen antecedentes diabéticos y hay aumento de triglicéridos y colesterol. “La gente joven generalmente no tiene esos problemas pero ya en adultos de cuarenta o cincuenta años si, los hombres tienen tendencia a tener infartos si no se controlan, deben alimentarse bajos en grasa animal, pero si consumir grasas buenas, si comes mucho aceite de salmón eso ayuda, el aceite de pescado también, hay que eliminar el gluten como harinas, trigo, cebada, granos, etc”, comentó. Lo anterior evitará que aumente la psoriasis, y recomendó también llevar una alimentación sana con muchos antioxidantes, como lo son el tomate, los morrones, y derivados de la vitamina A. NO SE CURA, SE CONTROLA
Como conclusión, la doctora Altamirano dejó en claro que la psoriasis no se cura, pero si se controla, con lo que el paciente tendrá una buena calidad de vida. “El control falla cuando no sabemos que tenemos psoriasis, el paciente debe entender su enfermedad, no lo puedo curar, no le puedo prometer que en 10 años no se presente pero sí que se controle, que no tenga infecciones; debe entender que no es hongo”, precisó. Comentó que cada paciente es diferente pero no hay que tenerle miedo a la psoriasis. “Hay muchas variedades de psoriasis, puede presentarse en placas chicas, placas grandes, en todo el cuerpo, en partes geográficas, en puntos, hay muchas variedades y los tratamientos son largos”, explicó. hepatitis This is an inflammatory disease that affects the liver. There are different types and they can be contracted in different ways. By José Manuel Meza If talking about infections, hepatitis is one of the conditions that most fall into this category. According to the World Health Organization (WHO), more than 400 million people have been affected by this virus, while in Mexico each year there are about 25 thousand new cases, according to figures from the Ministry of Health in México. It is a disease that does not exempt anyone from suffering it and that can exponentially change the health of a person, which must be well taken care of, otherwise, it can cause death. “The most common hepatitis is A. It is spread even through the contact with the sweat of infected patients. Not say for sexual contact, because it is easier to spread. “Those who touch the silverware, (such as a spoon, a plate, a glass) are also susceptible to being infected because they touched those objects with sweat or saliva and the patient brings them to their mouths”, said Dr. Alfredo Balboa Bustamante. Balboa Bustamante asked not to take this virus lightly, because of the dangers that hepatitis implies, as the defenses collapse and if the patient is not treated can lead to more serious health problems or even cause death. “The vast majority of patients recover around a month and do not need much treatment. In fact, there is no procedure that is totally effective for the hepatitis virus”, he said. On the recommendation to consume sweets, he said that it was used in the past because the components of the liver that give energy are used up and preventively candy was given to those who suffered it, but said that this is a fallacy. “It is an endemic disease and has to be reported to the Health Agency at the time it is detected, to have control, because if there is an outbreak in a school it is because there is a source of hepatitis, that could be the water and the authorities have to take precautions”, said the doctor. Although there are more types of this disease, one of the most dangerous, he said, is hepatitis C. “This is almost exclusively contracted through sexual contact or blood transfusion. It never heals and destroys the liver as time goes by. It is in the long run and there is no specific treatment”, he added. Balboa Bustamante said that if a patient have hepatitis C, he may have vague symptoms, but can lead an almost normal life taking appropriate care. He recommended going to the doctor at the beginning, but in case general medicine can not do what it takes to help patients a lot, he mentioned that sometimes they have to be hospitalized to be treated by internists, who will provide some other drugs, to control the problem.

hepatitis Es una enfermedad inflamatoria que afecta al hígado. Existen diferentes tipos y pueden contraerse de diferente forma. Por José Manuel Meza Si de infecciones se trata, la hepatitis es una de las afecciones que más entra en esta categoría. De acuerdo con la Organización Mundial de la Salud (OMS) más de 400 millones de personas se han visto afectadas por este virus, mientras que en México cada año se registran alrededor de 25 mil nuevos casos, según cifras de la Secretaría de Salud en México. Es una enfermedad que no exime a nadie de padecerla y que puede cambiar exponencialmente la salud de una persona, la cual debe ser bien atendida, de lo contrario puede causar la muerte. “La hepatitis más común es la A. Se contagia hasta por el contacto con sudor de pacientes infectados. Si existe contacto sexual, es más fácil la contaminación. “Aquellos que tocan los fómites, (como puede ser una cuchara, un plato, un vaso), también son susceptibles de ser contagiados porque tocaron con el sudor o con la saliva esos objetos y el otro paciente los lleva a la boca”, mencionó el doctor Alfredo Balboa Bustamante. El médico general por la Universidad Autónoma de Guadalajara (UAG) agregó que otra causa de infección de la hepatitis puede ser el contacto con heces contaminadas. “Es una enfermedad bastante contagiosa. En términos generales dura aproximadamente la inflamación como un mes y el síntoma principal es que se debilita el paciente y se pone amarillo de la piel. “Eso va disminuyendo conforme pasan los días. Hay que vigilar que las bilirrubinas se vayan bajando a niveles normales para que el paciente esté bien. No genera inmunidad, así que la persona puede ser contagiada en otra ocasión si vuelve a entrar en contacto con la hepatitis”, explicó. ENFERMEDAD RIESGOSA
Balboa Bustamante pidió no tomar este virus a la ligera, por los peligros que la hepatitis implica, ya que las defensas se desploman y si el enfermo no se atiende puede derivar en problemas de salud más graves o hasta causarle la muerte. “La inmensa mayoría de los pacientes se recupera como en el periodo de un mes y no necesita mucho tratamiento. De

hecho no existe un procedimiento que sea totalmente eficaz para el virus de la hepatitis”, dijo. Sobre la recomendación de consumir dulces, comentó que se acostumbraba en el pasado porque se agotan los componentes del hígado que dan energía y de forma preventiva se les daban caramelos a quienes la padecían, pero manifestó que eso es una falacia. “Es una enfermedad endémica y tiene que ser reportada a Salubridad en el momento que se detecta, para tener un control, porque si hay un brote en una escuela es porque existe una fuente de hepatitis que puede ser el agua y las autoridades tienen que tomar precauciones”, puntualizó este médico. Aunque existen más tipos de esta enfermedad, uno de los
más peligrosos, comentó, es la hepatitis C. “Ésta es casi exclusivamente contraída por contacto sexual o por transfusión sanguínea. Nunca se cura y va destruyendo al hígado conforme pasa el tiempo. Es a la larga y no hay un tratamiento específico”, añadió. Balboa Bustamante expresó que en la hepatitis C el paciente tiene síntomas vagos, pero puede llevar una vida casi normal llevando los cuidados pertinentes. Recomendó acudir en principio con el doctor, pero en caso de que la medicina general no pueda hacer lo conducente para ayudar a los pacientes mencionó que a veces tienen que ser hospitalizados para ser tratados por internistas, que le suministrarán algunos otros fármacos, para controlar el problema.

El abuso se comete por una persona que se encuentra en una posición de poder, lo cual le facilita ejercer presión sobre la víctima, quien por lo general es alguien quien está en un nivel inferior en sitios como la escuela, la oficina y en otros espacios. Por IRMA IDALIA CERDA Para prevenir el acoso y el abuso sexual es indispensable que las personas vivan su sexualidad de manera sana y responsable, pues hay que superar un problema sociocultural que está afectando las relaciones entre hombres y mujeres. Así lo afirmó la doctora Lourdes Mantecón Garza, quien se ha dedicado en los últimos años a investigar y difundir los temas relacionados con la sexualidad humana, y quien considera que es muy importante que se pongan en la mesa las dudas e inquietudes que se tienen al respecto. Desde hace varias semanas, salieron a la luz varios casos de personajes que han sido acusados de haber abusado de actrices que trabajaron con ellos en algunas películas o series. Al poco tiempo, se fueron destapando más casos no sólo en Hollywood, sino en otras ciudades de Estados Unidos y cada día más mujeres y hombres denunciaban que en el pasado habían sido víctimas de abuso sexual. Las celebridades hollywoodenses prendieron la mecha de una bomba cuya explosión alcanzó a otros países y en México también hablaron muchas de las víctimas que por años callaron situaciones de acoso y abuso sexual. La doctora Mantecón Garza sostiene que el abuso se comete por una persona que se encuentra en una posición de poder, lo cual le facilita ejercer presión sobre la víctima, quien por lo general es alguien quien está en un nivel inferior en sitios como la escuela, la oficina y en otros espacios. “¿Cómo vamos a definir cuándo es acoso? el acoso sucede cuando una persona está en posición de poder y te pide favores sexuales a cambio de lo que sea: puede ser dinero, puede ser un permiso, puede ser un aumento de salario o simplemente dejarte conservar tu empleo. “Porque si no acedes a tener sexo con esa persona de autoridad, pierdes tu trabajo. No necesariamente vas a obtener un beneficio, es simplemente el hecho de conservar tu trabajo”, señaló. Entre las faltas que puede cometer una persona en posición de poder en relación al aco-
so y al abuso mencionó que la seducción o enamoramiento también están en la lista, porque no necesariamente tienen que usar la fuerza física ni la violencia para tener relaciones sexuales con su alumna, subordinada o alguna persona de su familia, como puede ser prima, sobrina y en algunos casos su propia hija. “Si estás hablando de un maestro que por su posición de autoridad enamora a sus estudiantes, aunque no haya un intercambio de favores por las calificaciones, nada más el hecho de enamorarlas, ahí estamos hablando de acoso. “Y si llega a tener actividad sexual, las toca, se masturba en frente de ellas, o exhibe sus genitales frente a ellas, ya estamos hablando de abuso, porque no necesariamente el abuso habla de violencia, no necesariamente el abuso son tocamientos, pues son expresiones diferentes del mismo problema, una persona en posición de poder o autoridad, sobre otra”, indicó. Esta situación se genera regularmente en diversos sectores de la vida cotidiana, porque puede ser en el ámbito laboral, social, académico y familiar, en donde existan siempre las dos partes: el que ejerce la autoridad, ya sea un jefe, un maestro, padre o tío, sacerdote, por mencionar algunos y el que está en un nivel inferior: subordinado, estudiante o hija(o), sobrina(o), etcétera. “No hay una línea muy clara desde el punto de vista legal, sobre cuándo es acoso y cuándo es abuso, porque el abuso puede depender simplemente por tu edad o por tu estado mental.

“Es decir, que tenga algún retraso mental, porque en este caso puede ser mayor de edad, pero no está consciente en sus facultades mentales, para aceptar o rechazar alguna actividad sexual”, añadió. Dijo que el acoso se puede dar de diferentes maneras, ya que el acosador o abusador acorrala o amenaza a su víctima, pero muchas veces utiliza la seducción o el enamoramiento para lograr sus objetivos. “Pero la otra persona no sabe que está siendo seducida, entonces pareciera que actúa voluntariamente. Me ha tocado recibir (en el consultorio) a personas adultas que tienen actividad sexual con personas muy jóvenes y no estoy hablando de pedofilia. “Y lo que argumentan los adultos es que no están cometiendo ninguna agresión porque fue con el consentimiento de la otra persona, y esto significa el no reconocer que están abusando de la inocencia de la otra persona. “Ellos sí saben qué están haciendo, mientras que la otra parte aún no tiene la suficiente madurez para entender que fueron seducidos por esa figura de autoridad, por eso ante la ley se protege al vulnerable y se habla de abuso”, afirmó la especialista. HABLAR DEL TEMA
Lourdes Mantecón Garza apuntó que al momento en que salieron a la luz todas las denuncias y acusaciones, se puso el tema en la mesa, y que se debe seguir discutiendo al respec-
to, pues es la única manera de que este problema se pueda enfrentar. Agregó que se trata de un problema sociocultural, en el que por lo general se coloca a la mujer como la culpable de lo que sucede siempre, porque en la mayoría de los casos, las víctimas son señaladas por ser ellas las que provocaron la situación. “Este es un problema sociocultural, porque tanto hombres como mujeres hacemos eso de culpar a las víctimas, al hacer suposiciones como: ‘¿por qué andaba sola en la calle ?, ¿qué andaba haciendo a esas horas de la noche?, Es culpa de ella, para qué tomaba’. Siempre es culpa de la víctima y esa manera de pensar es lo que tenemos que cambiar”, aseguró. Para la autora de Sexualidad Sana, información médica para adultos, hay que aprovechar esta coyuntura que permitió que el tema del acoso se extendiera en las redes sociales y se convirtiera en un “trending topic” para tratar de arreglar el asunto de la sexualidad entre hombres y mujeres en esta sociedad. Propuso que se hable del tema, hablar de sexualidad, ya que hay que reconocer que somos una especie sexuada, por lo que debemos aprender a vivir con la sexualidad de una manera sana y responsable. “Yo estoy a favor de la educación; es importante que primero reconozcamos el problema, educar a nuestros hombres, pero primero tenemos que educarnos a nosotras mismas como mujeres. “Tenemos un problema sociocultural porque esta esta idea de que el hombre es el que tiene que venir a conquistar a la mujer y la palabra conquista es clave en esto, porque quienes son acosadores o abusadores, no les interesa el sexo por el placer del sexo, a ellos les interesa el placer de dominar a su víctima a través del poder”, finalizó. What is keratoconus?
T
he cornea is the transparent window in the form of a dome in the front of the eye and is the one that focuses light on the eye. Keratoconus is when the cornea becomes thin and pops-up like a cone. The change of shape of the cornea causes the light rays to be out of focus. As a result, vision is blurry and distorted, making daily tasks such as reading and driving difficult. WHAT ARE THE SYMPTOMS OF KERATOCONUS?
Usually, keratoconus affects both eyes and can generate a different vision in them. The symptoms may be different in each eye and vary over time. In the early stage, the symptoms of keratoconus can be: • Mild blurred vision • Slightly distorted vision • Greater sensitivity to light and glare • Redness or swelling of the eyes In the more advanced stages, the symptoms of keratoconus can be: • Blurred and distorted vision • Greater myopia or astigmatism • Not being able to wear contact lenses. It is possible that they no longer fit properly and feel uncomfortable. Keratoconus usually takes years to move to the advanced stage. However, in some people, it can get worse quickly. HOW IS KERATOCONUS DIAGNOSED?
Keratoconus can be diagnosed with a routine eye exam. The ophthalmologist will examine

¿Qué es el queratocono?
L
a córnea es la ventana transparente en forma de cúpula en la parte delantera del ojo y es la que enfoca la luz en el ojo. El queratocono es cuando la córnea se adelgaza y sobresale como un cono. El cambio de forma de la córnea hace que los rayos de luz se desenfoquen. Como resultado, la visión es borrosa y distorsionada, lo que dificulta las tareas diarias como leer y conducir. ¿CUÁLES SON LOS SÍNTOMAS DEL QUERATOCONO?
Por lo general, el queratocono afecta a ambos ojos y puede generar una visión diferente en ellos. Los síntomas pueden ser diferentes en cada ojo y variar con el paso del tiempo. En la etapa temprana, los síntomas del queratocono pueden ser: • Visión borrosa leve • Visión ligeramente distorsionada • Mayor sensibilidad a la luz y deslumbramiento • Enrojecimiento o hinchazón en los ojos En las etapas más avanzadas, los síntomas del queratocono pueden ser: • Visión más borrosa y distorsionada • Mayor miopía o astigmatismo • No poder usar lentes de contacto. Es posible que ya no se ajusten adecuadamente y se sientan incómodos. • El queratocono generalmente toma años para pasar a la etapa avanzada. Sin embargo, en algunas personas puede empeorar rápidamente. ¿CÓMO SE DIAGNOSTICA EL QUERATOCONO?
El queratocono puede diagnosticarse con un examen ocular de rutina. El oftalmólogo examinará la
córnea y medirá su curvatura, esto ayudará a mostrar si hay un cambio en su forma. También puede generar un mapa de la superficie de la córnea con una computadora especializada la cual mostrará una imagen detallada del estado de la superficie de la córnea. ¿CÓMO SE TRATA EL QUERATOCONO?
El tratamiento del queratocono depende de los síntomas. Cuando los síntomas son leves, la visión puede corregirse con anteojos, más adelante puede ser necesario usar lentes de contacto rígidos para disminuir la distorsión visual entre otros métodos como lo son: Intacs: Pequeño dispositivo curvo colocado quirúrgicamente en la córnea que ayuda a aplanar la curva de la córnea para mejorar la visión. Reticulación del colágeno: Se usa una luz especial UV y gotas oculares para fortalecer las fibras de colágeno de la córnea, lo que ayuda a aplanar o endurecer la córnea y evitar que se siga abultando. Trasplante de córnea: cuando tienen síntomas severos puede recomendarse un trasplante de córnea. Esto consiste en reemplazar toda o parte de la córnea enferma con un tejido de córnea saludable de un donante. Por Pamela Escobar Para lograr el control familiar existe el método anticonceptivo llamado salpingoclasia, mismo que es considerado como definitivo para la mujer que ya no quiere tener más descendencia, comentó el especialista en ginecología y obstetricia Constantino Castillo Palacios. El egresado de la Universidad de Monterrey (UDEM), agregó que al igual que éste, existen otros métodos anticonceptivos para control familiar, unos son a corto plazo, otros a mediano plazo y la salpingoclasia que es definitivo. “Se procede tras una anestesia regional; es decir del abdomen y piernas, dura alrededor de media hora y es un procedimiento muy sencillo en el que la paciente está despierta y consciente; se hace una incisión de máximo un centímetro y la recuperación es muy rápida”, explicó el integrante del Colegio de Ginecología y Obstetricia en Reynosa. Detalló que es diferente de la salpingoclasia tradicional, donde se realiza una cirugía de más de 15 centímetros ligando ambas trompas uterinas, para que ya no pase el óvulo desde el ovario hasta la matriz y evitar la concepción. Actualmente se puede acceder a dicho método de manera más sencilla, aunque en ambos casos exista o no un alumbramiento. “La salpingoclasia por medio de laparoscopia es considerada como una cirugía de mínima invasión, por lo que el riesgo es muy poco. De acuerdo con el especialista la decisión de practicarse la salpingoclasia es siempre de la paciente, no importa la edad o etapa de la vida en la que se encuentre, sólo que a su parecer ya haya completado su familia. “Actualmente realizarla por laparoscopia, es lo más nuevo en ginecología, se emplea una pequeña cámara, sin necesidad de hacer una herida grande y la recuperación es más rápida”, comentó el especialista. Añadió que es el método anticonceptivo más efectivo para la no concepción ya que además de ser definitivo también es reversible por medio de una cirugía similar en la cual se puede volver a recanalizar las trompas y conseguir la permeabilidad de los tubos de la matriz para que pueda recobrar su funcionalidad. “La cirugía para revertir la salpingoclasia se puede realizar en cualquier momento, preferentemente después de los primeros cinco años. Pero si no se da marcha atrás, es considerado el mejor y definitivo método para no tener más hijos”, señaló el médico. Afirmó que las pacientes que son sometidas a la salpingoclasia no sufren ninguna alteración hormonal, ya que únicamente se manipula con el conducto de la trompa uterina, no el ovario que es donde se producen las hormonas del cuerpo de la mujer. “De esa manera tampoco se altera la producción o circulación de la sangre hacia los ovarios, por lo que no deben registrarse cambios hormonales de ningún tipo y en cuanto al costo, es un procedimiento tan accesible como una cesárea”, expresó el ginecólogo. Eliacib Leija Garza, director de Medio Ambiente, informó que la doctora Maki Ortiz está desarrollando un programa de sanidad integral para lograr tener un municipio limpio y aseguró que. “Vamos a empezar a sancionar, en enero ya la empezaremos a aplicar, es algo que requerimos, que se tiene que hacer para po-

La ‘pastilla azul’ puede tener repercusiones en hombres como sangrado nasal, dolor en el pecho, baja presión y peor aún, ceguera. Los mitos y realidades de la disfunción eréctil en la voz de un especialista.
Por José Manuel Meza Librado Cárdenas Muñoz es médico desde hace 52 años. Se graduó con mención honorífica de la Universidad de Nuevo León, antes de que fuera autónoma. Cursó la especialidad en urología en el Instituto Mexicano del Seguro Social (IMSS). A lo largo de varias décadas ha visto llegar a su consultorio muchos pacientes con diversos problemas, muchos de los cuales son de tipo urinario, de las glándulas suprarrenales y también del aparato reproductor masculino, como la disfunción eréctil. Particularmente en el caso de las personas que sufren impotencia sexual, el experto mencionó que la gran mayoría busca solucionarlo automedicándose y, sin saberlo, puede hacerle daño a su organismo. En 1998 la farmacéutica estadounidense Pfizer lanzó al mercado uno de sus mejores éxitos de ventas: la Viagra, una pastilla con la que ha logrado facturar por año hasta 2 mil millones de dólares. Sin embargo, los efectos de la pastilla azul no son del todo positivos y en algunos casos presenta secuelas muy desagradables. “La fórmula es el Sildenafil. Tengo un sentimiento con ese tipo de laboratorio, que fue el que descubrió tal sustancia. Experimentalmente se estaba investigando un producto para la hipertensión arterial y ellos observaron que los monos a los que se les estaba administrando andaban con el pene erecto. “Descubrieron que servía para la disfunción eréctil y dio resultado, pero el medicamento primero se dio a conocer en los medios de comunicación, dándole publicidad y no se consideró a los especialistas. No se le presentó primero al urólogo y nos hicieron a un lado. “Puede dar buenos resultados en lo que a erección se refiere, pero tiene sus efectos desagradables. Lesión a la retina, la persona puede quedar ciega, bajarle la presión y otros efectos”, detalló Cárdenas Muñoz.

A decir de este médico después aparecieron otros productos mejores que ese, como el Cialis y Levitra, pero aclara que todos provocan efectos secundarios y deben ser administrados por un experto. Algunas farmacias comercializan Viagra y los productos de la competencia por 50 pesos el frasco y prácticamente cualquier persona puede tener acceso a ellos, lo cual puede tener consecuencias muy nocivas para la salud. “No se consigue con receta. Aquí en México la gente va a la farmacia de enfrente, pide Viagra y se lo venden. Cuando está muy caro hay otros genéricos que también lo comercializan”, dijo. El doctor Cárdenas Muñoz explicó que para atender la impotencia sexual existen tres opciones principales: la primera son los medicamentos y cuando éstos no dan resultado al paciente se le indica la bomba de vacío, que produce una erección. “Ese aparato no tiene ningún efecto secundario, porque el pene se mete en un tubo que hace vacío y los cuerpos cavernosos se llenan de sangre. Luego se baja una liga en
la base para que no se vacíen. Esa liga puede permanecer ahí 30 minutos. “Después hay que retirarla, esperar unos 15 minutos y puede ponerse otra vez, pero con media hora es más que suficiente para practicar un acto sexual. La bomba es uno de los mejores procedimientos, pues el varón tiene sensibilidad y eyaculación”, comentó el urólogo. LA OPERACIÓN ES OPCIÓN
Librado Cárdenas Muñoz también explicó que las personas con impotencia sexual no deben tomar Viagra debido a efectos perniciosos o simplemente porque no les funciona pueden ser candidatos a una prótesis, las cuales son sofisticadas. Se colocan dos tubos en los cuerpos cavernosos. Se aprieta a un lado y hay una erección, mientras que del otro se baja. Sin embargo, subrayó, hay que encontrar la causa de la disfunción, porque hay casos en los que el paciente, por ignorancia, acude a tratamientos que pueden resultar contraproducentes. “Aquí conmigo han venido pacientes que fueron a otras partes como Boston Medical Group y tuvieron una mala experiencia. Lo
que ellos hacen es ponerles una inyección en los cuerpos cavernosos: Caverject, que les produce una erección; no obstante, han llegado personas que se sometieron a ese procedimiento y traen priapismo. “Eso quiere decir que después de la inyección hay una erección prolongada que no se baja, se sigue manteniendo, pasan las horas y continúa igual; tres, cuatro, seis, diez, 12 horas y lo mismo. “Tan erecto se pone que a veces el glande se pone en contacto con el ombligo, por la erección tremenda y dolorosa. Pasan 24 horas y no se baja y hay que operar para sacar la sangre acumulada. Hay varias técnicas quirúrgicas para resolver eso”, mencionó. El especialista hizo la recomendación de no ponerse tal inyección. “Un paciente que presenta priapismo ese ya queda impotente. Si estaba semi–impotente cuando le pusieron la inyección ahora sí va a quedar impotente definitivamente. “Todo el que llega ahí se la ponen y sale encantado porque tuvo erección, pero no a todos les viene después el estado de flacidez, sino que persiste erecto y tienen que buscar auxilio”, aclaró. –¿Qué pasa si el pene se mantiene así?, ¿puede reventar un vaso sanguíneo? “Se necrosa. Hay que operar y no lo soportan porque es muy doloroso”, alertó. Por lo que ante la incapacidad repetida de lograr mantener una erección lo suficientemente firme como para lograr una relación sexual satisfactoria, lo mejor, especificó el doctor Cárdenas Muñoz es acudir primero al urólogo, antes de ir a la farmacia por Viagra o la inyección Caverject. “Todos las medicinas que el médico receta tienen dos aspectos. Uno para aliviar los síntomas y el otro. los efectos secundarios. “Por ejemplo, una aspirina te quita el dolor de cabeza, pero produce gastritis, te desgasta el estómago. De la misma manera esto del Viagra tiene un lado bueno y uno malo y la gente debe de saberlo”, puntualizó el especialista.

La temporada de los Winter Texans 2017– 2018 llegó y paulatinamente comienza a verse una mayor afluencia de visitantes que cruzan hacia el lado mexicano. Nuevo Progreso, una localidad conocida como la “capital del turismo médico”, es de los sitios que más arribo de extranjeros tiene en esta temporada, en la que recibe gente de Estados Unidos y Canadá. La gran mayoría evita pasar el duro invierno en el norte del continente y se establece durante varios meses en el sur de Texas. Por José Manuel Meza Nuevo Progreso, Tam. La familia Stovall es originaria de Chicago, Illinois. Llegó a la región del sur del país hace unos días para pasar momentos menos fríos, ya que en el norte el invierno es muy crudo. Por primera ocasión sus siete integrantes cruzaron a Nuevo Progreso, Tamaulipas, un poblado que colinda con Progreso, Texas. Ellos dijeron que aman la cultura mexicana y que se sienten cómodos aquí. Aprovecharon para tomar sombra, beber algunas ‘margaritas’ y cervezas, y por supuesto, entre risas tomarse la foto del recuerdo. Todos coincidieron en que los precios que han hallado en este lugar son muy accesibles para cualquier persona que quiera disfrutar de una buena bebida, comida o de surtir sus medicamentos. Les platicaron que también para atenderse cuestiones médicas es muy barato y el pueblo tiene especialistas certificados. El médico cirujano dentista Tito Antonio Pérez Trejo habla de la importancia del hábito en la higiene bucal para mantener una buena dentadura y revertir problemas con la salud de las piezas dentales. Por José Manuel Meza Nuevo Progreso, Tamaulipas De los dientes hay tantos temas que se pueden mencionar, pero en uno que es tan simple y principal, el de la limpieza, hay que estar insistiendo constantemente. Para este doctor originario del puerto de Tampico, pero radicado desde hace ocho años en la frontera, la higiene es la base de cualquier tratamiento. “Si tú no tienes una higiene adecuada, se puede causar una infección o inflamación. Periodontitis, que es la inflamación de la encía y después la gingivitis, que es más crónico y afecta no nada más a la encía, sino que se compromete el hueso y por ende la movilidad dental se hace presente”, alertó. Por eso reconoció que no está de más la tan trillada recomendación de lavarse los dientes tres veces al día y también acudir al dentista para realizarse una limpieza, ya que el cepillo no llega donde debe y no quita algunos minerales que se van pegando al esmalte. Mencionó que los alimentos más dañinos son todos aquellos relacionados con lo dulce y también los ácidos que causan descalcificaciones. “Los cítricos, el limón es uno de los principales y las personas que consumen mucho medicamento también sufren descalcificación severa. “Eso se puede combatir mediante la remineralización. Para ello tenemos unas pastas dentales que se deben usar una vez en la noche, después de haber cenado y cepillar su boca, como cuando ya no se va a comer ni tomar agua. Simplemente se va a usar la pasta como tipo crema, que estará durante ocho horas mientras dormimos suministrando calcio y sustancias que ayudan a revertir ciertas caries incipientes”, comentó Pérez Trejo. DENTADURAS QUE SORPRENDEN
De acuerdo con este médico cirujano dentista por la Universidad Autónoma de Tamaulipas (UAT) y actualmente estudiante de especialidad en Ortodoncia por el Instituto Profesional Odontológico Contemporáneo (IPOC), cada organismo es distinto y no todas las piezas tienen la misma resistencia. En su experiencia relató que le ha tocado mirar desde dentaduras muy descompuestas en jóvenes hasta aquellas excelentemente conservadas en ancianos. “Hay personas con sus dientes completos sin ninguna amalgama, de 80 años, a las que les he hecho limpieza. Probablemente tienen muy buena genética. Son algunos cinco casos los que he atendido yo. “Y por el otro lado he atendido jóvenes que, por cuestiones de medicamentos, o que están metidos en drogas pierden la dentadura. Me ha tocado hacer tratamiento de extracciones y poner placa a los 30 años de edad”, comparó. Por ello, Tito Antonio Pérez Trejo resaltó la importancia de estar bien informados acerca del cuidado de los dientes y tomar cartas en el asunto.

“Las drogas tienen sintéticos y cosas que hacen mal a la dentadura, porque son muy ácidas y todo eso descalcifica y se va perdiendo la protección, que es el esmalte”, agregó el cirujano que atiende en Nuevo Progreso, Tamaulipas. –¿Y quienes toman agua de limón? “Pues si está muy ácida sí afecta, y si está muy dulce también, por lo que lo recomendable es mejor utilizar un chicle que no tenga azúcar, como algo preventivo. Se recomienda usar todo lo que disponga de recaldent. Algunas gomas de mascar lo tienen. “Después siguen unos de cuatro pastillas que no tienen azúcar, sino un sustituto llamado Xilitol, que ayuda a producir más saliva y obtienes una limpieza”, dijo. –A veces es cuestión de ignorancia o desidia, pero la limpieza no es tan cara. “Relativamente no es caro. En Nuevo Progreso cuesta 30 dólares y en otras partes varía. Anda entre 300 y 500 pesos. También depende la calidad de la limpieza que hagan. Las aplicaciones de flour refuerzan a los niños. “Para los adultos es el recalcificante, el cual no se consigue en ninguna tienda, más que en lugares específicos. Aquí en el consultorio tenemos porque lo solicitamos al depósito cuando compramos el material y pueden acudir con nosotros”, mencionó el entrevistado. NO TOMARLO A LA LIGERA
Además Pérez Trejo aceptó que siempre es importante tener un buen diagnóstico, lo cual le ayudará al paciente a cuidar mejor su dentadura y presentar menores problemas propios del desgaste. “Si es cuestión dental general, suponiendo que existe un dolor, se puede ir con el dentista, pero si específicamente se quie-
re un tratamiento se recomienda ir con el ortodoncista, que hará un diagnóstico por medio de una radiografía panorámica y lateral de cráneo, donde va a elaborar un diagnóstico de cefalometría, con lo que se toman modelos de estudio y se llega a un análisis para ver la discrepancia dentaria y ósea. “También vamos a analizar con las fotografías de una manera más específica lo que tenemos que hacer en ortodoncia. Ahí más que nada podemos ver si se puede lograr por medio de ortodoncia o si el paciente es quirúrgico. Si hay que pasarlos directamente con un maxilofacial o se necesita prepararlo. Ya después se manda a hacer una cirugía”, explicó. Reiteró el dentista que la educación dental es importante y lo principal para cualquier tratamiento. “Por ejemplo, si te van a poner un puente y los dientes que van a trabajar tienen mucha movilidad causada por la enfermedad, pues ese tratamiento no va a tener éxito. Primero tenemos que regresarle la salud al paciente mediante el cuidado del tejido, del hueso. “Una de las especialidades que está descuidada porque la gente no lo conoce es el periodoncista, el especialista de las encías. Normalmente le dices a los pacientes y no lo conocen, pero lo más importante es la educación y prevención, que en México no la tenemos. “Como relativamente no es muy caro el tratamiento dental no lo vemos de esa manera. En otros países todos los productos que usan de prevención son más caros, pero a la larga tienen un mayor beneficio”, comparó el doctor Tito Antonio Pérez Trejo.
