AOGIN VIETNAM . 2015 Dear colleagues, “The 2015 AOGIN Vietnam Meeting”, which is an international conference on genital cancer, is scheduled to take place in Ho Chi Minh City on September 25th and 26th, 2015 by HOGA and Tu Du Hospital, in cooperation with AOGIN organization.

The goal of “The 2015 AOGIN Vietnam Meeting” is as follows: 1. Share and discuss the latest research indings on gynecologic cancer in Vietnam and other Asian countries.

2. Discuss the application of these research indings in the prevention and treatment of gynecological cancer.

Speakers of this conference include foreign experts - members of AOGIN - and Vietnamese experts. The organizers would welcome all active participation of researchers and scientists from hospitals, centers and units to make the conference a real success. CURRENT STATUS OF CERVICAL CANCER CONTROL IN THE EMERGING ECONOMY COUNTRIES Neerja Bhatla

Professor, Department of Obstetrics & Gynaecology All India Institute of Medical Sciences New Delhi, India President, AOGIN (2014-16) Cervical cancer continues to be an important, preventable cause of mortality and morbidity in the emerging economy countries. It is now the fourth most common cancer among women in the world but is second after breast among women aged 15-44 years in most of the low- and middle-income countries (LMICs). For many decades, screening was the mainstay of cervical cancer prevention in the developed world, where repeated rounds of cytology-based screening (the Pap smear) was able to prevent about 70% of the mortality.

However, the LMICs faced severe challenges in implementation of this strategy. Lack of infrastructure, trained personnel and awareness were the main problems. Linkages between screening and treatment of detected cases were also difficult to establish.

Large scale trials of visual screening methods (VIA and VILI) showed that these simple methods had equal sensitivity to the Pap smear and VIA was incorporated into the national screening program as well as introduced in some pilot programs in several countries including Bangladesh, Thailand, Nepal and India. Yet screening coverage remains poor. The development of tests for HPV DNA detection was the next advance in cervical cancer screening. While this has now been shown to be the best test for cervical cancer detection, with the highest sensitivity and the least possibility of missing disease, and co-testing with Pap and HPV DNA in women over 30 years is now the preferred method in most guidelines. However, its expense has been the major barrier to wide implementation. It is hoped that there will soon be more affordable, rapid tests that will change the pattern of screening for cervical cancer. Also, the HPV test has the potential for self-collection of samples that can allow wider reach for even those women who are not coming to clinics for screening.

The development of the HPV vaccines opened a new possibility of primary prevention. Most of the emerging economy countries do have an effective system of immunization. There is also good acceptance of vaccination among the population. In recent years, the acceptance of the vaccine as a GAVIeligible vaccine and its pre-qualification by WHO led to its uptake in several national programs as well as in demonstration projects. The vaccine is in the national programs of over 60 countries, which target young teenage girls.

Notable in the region have been Bhutan and Malaysia, but there are many emerging economy countries like Panama, Republic of South Africa and Brazil where the HPV vaccine has been accepted with good coverage of nearly 90%.

The latest development has been the acceptance of the 2-dose regime by WHO for girls aged < 15 years at the time of vaccination. The two doses should be at least 180 days apart. For girls who are 15 years and above, or those who are immunocompromised, the standard 3-dose regimen is recommended. Large meta-analyses have shown the vaccines to be safe and effective with no serious adverse events that can be attributed to the vaccine.

Newer vaccines are in development and it is hoped that these will be less expensive and more widely implemented.

With a combination of vaccination of young girls and screening of women, a comprehensive cervical cancer prevention package can be developed that can eliminate this cancer in the foreseeable future. Cervical cancer prevention efforts should be combined with information on breast cancer prevention and preventive health checks for common non-communicable diseases like diabetes and hypertension to ensure that our women have a long and healthy life and play their role in society in the best ways possible.

HPV TYPE-DISTRIBUTION OF ENDOCERVICAL ADENOCARCINOMA IN CHINA: A HOSPITAL-BASED, MULTI-CENTER STUDY You-Lin Qiao, et al.

Background: Endocervical adenocarcinoma (CADC) is an important public health problem in China, however, data on the HPV type-distribution in CADC using highly sensitive HPV DNA detection methods is limited. This abstract reports preliminary results from two of seven sites in a large multicenter study. Objectives: To determine the HPV type-distribution of endocervical adenocarcinoma in China and the role of HPV in histopathological subcategories of CADC. Methods: A multi-center study is being conducted in seven regions of China spanning the national territory.

Paraffin-embedded blocks are being obtained from study hospitals containing tissue diagnosed locally as CADC. For cases from two sites (N=196) original histological diagnosis was validated by central review (CICAMS); in a subset of these (N=115) expert consensus review was carried out. Pathology review was supported by immunohistochemical staining using p16 and progesterone receptor. HPV DNA was detected using the sensitive SPF10 PCR*, with typing of HPV-positive specimens by reverse hybridization line probe assay. Additional HPV testing will be performed on HPV DNA negative cases. All multiple infections as well as single infections of special histological subtypes will be further investigated using Laser Capture Microdissection. Results: Altogether 1013 CADC cases have been collected. 196 cases with an average age of 45.0 years have been processed usingsandwich cutting, central histological review and HPV DNA testing. In addition, 115 of the 196 endocervical adenocarcinoma cases were validated by expert pathology review.

Fifty-seven cases (57/115, 50%) were diagnosed as true cervical adenocarcinoma (ADC-CX), 8/115 (7%) as endometrioid adenocarcinoma (ADC-ENDO), and 50 other types, respectively. HPV prevalence varied by subtype, as follows: 75.4% (43/57) in ADC-CX, 60% (3/5) in ADC-CC, 50% (4/8) in ADC-ENDO, 33.3% (1/3) in minimal deviation ADC (ADC-MIN), 80.5 (33/41) in unspecified (NOS) ADC, 0.0% (0/1) in Serous (SER)ADC and 90.9%(20/22) in adenosquamous carcinoma (ASC). HPV type-specific distribution was HPV16 (28.7%,33/115), HPV18 (35.7%, 41/115), HPV31 (2.6 %), HPV33 (1.7%), HPV45 (0.9%), HPV52 (4.3 %), HPV58 (0%), HPV59 (2.6%), HPV66 (0.9%) and HPV68 (0%), respectively. HPV16 and 18 are present in every case of multiple infections. Conclusion: Prevalence of HPV varies with CADC subtypes. HPV16 and 18 are the most prevalent HPV types in CADC.

SỰ PHÂN BỐ CÁC TYPE HPV TRONG NHỮNG TRƯỜNG HỢP ADENOCARCINOMA KÊNH CỔ TỬ CUNG Ở TRUNG QUỐC: MỘT NGHIÊN CỨU ĐA TRUNG TÂM, TẠI BỆNH VIỆN You-Lin Qiao, và cộng sự Tổng quan: Adenocarcinoma kênh cổ tử cung (CADC: endocervical adenocarcinoma) là một vấn đề sức khoẻ cộng đồng quan trọng tại Trung Quốc, tuy nhiên hiện các dữ liệu về sự phân bố các type HPV trong CADC bằng các phương pháp phát hiện DNA HPV còn hạn chế. Abstract này cung cấp một số kết quả ban đầu từ hai trong bảy trung tâm trong nghiên cứu đa trung tâm này.

Mục tiêu: Nhằm xác định sự phân bố các type HPV của CADC tại Trung Quốc và vai trò của HPV trong các subtype của CADC. Phương pháp: Một nghiên cứu đã trung tâm được thực hiện trong bảy khu vực của Trung Quốc, trải dài khắp lãnh thổ Trung Hoa. Các mẫu được niêm phong bằng paraffin lấy từ các bệnh viện tham gia nghiên cứu chứa các mỗ được chẩn đoán tại chỗ là CADC. Đối với những case tại hai trung tâm (N=196), chẩn đoán mô học ban đầu được kiểm định lại bởi CICAMS (nhóm kiểm định trung tâm); trong một nhóm nhỏ (N=115) của những trường hợp này, chúng tôi cũng tiến hành đánh giá lại những đồng thuận của chuyên gia. Kết quả bệnh học được kiểm tra lại bằng cách dùng phương pháp hoá mô miễn dịch để nhuộm p16 và receptor progesterone. Phát hiện DNA của HPV bằng phương pháp SPF10 PCR*, và những mẫu dương tính sẽ được định type bằng các test thích hợp. Các test HPV khác sẽ được thực hiện nếu test HPV DNA cho kết quả âm tính. Tất cả các trường hợp subtype mô học đặc biệt đều được khảo sát thêm, sử dụng phương pháp vi phẫu tích bằng laser (laser capture microdissection), dù là nhiễm đơn độc hoặc đồng nhiễm với các subtype khác.

Kết quả: Tổng cộng có 1013 case bị CADC đã được đưa vào mẫu nghiên cứu. 196 case với độ tuổi trung bình là 45.0 tuổi đã được xử lý bằng biện pháp cắt sandwich, khảo sát mô học và test HPV DNA. Ngoài ra, 115/196 trường hợp CADC đã được xác nhận lại kết quả bệnh học. 57 case (57/115, 50%) đã được chẩn đoán là adenocarcinoma cổ tử cung (ADC-CX), 8/115 (7%) là adenocarcinoma dạng lạc nội mạc (ADC-ENDO), và có 50 types khác. Tỷ lệ hiện mắc của HPV theo subtype như sau: 75.4% (43/57) là ADC-CX, 60% (3/5) là ADC-CC, 50% (4/8) là ADC-ENDO, 33.3% (1/3) là ADC sang thương tối thiểu (ADC-MIN), 80.5% (33/41) là ADC không đặc hiệu (unspecified ADC), 0.0% (0/1) là ADC thanh dịch (SER-ADC), và 90.0% (20/22) là ASC (adenosquamous carcinoma). Sự phân bố HPV theo type là HPV16 (28.7%, 33/115), HPV18 (35.7%, 41/115), HPV31 (2.6%), HPV33 (1.7%), HPV45 (0.9%), HPV52 (4.3%), HPV58 (0%), HPV59 (2.6%), HPV66 (0.9%), và HPV68 (0%).

HPV16 và 18 là hai tác nhân hiện điện trong mọi trường hợp bị nhiễm nhiều type HPV. Kết luận: Tỷ lệ hiện mắc của HPV thay đổi theo từng subtype của CADC. HPV16 và 18 là các type HPV thường gặp nhất trong CADC. 7

MANAGEMENT OF CERVICAL CANCER: WHAT’S NEW? Tran Dang Ngoc Linh

Cervical cancer is the fourth most common in women. It was estimated that there were 528000 new cases and 266000 deaths from cervical cancer worldwide in 2012.

Our presentation reviewed and updated current status of invasive cervical cancer treatment.

For early invasive cervical cancer stages (stage IB1 and IIA1), primary surgery with radical hysterectomy and pelvic lymph node dissection or definitive radiation therapy are equally effective, but differ in terms of morbidity and type of complications. Post operative adjuvant therapy is indicated for intermediate risk and high risk patients.

Adjuvant radiation therapy is indicated for intermediate risk cases whereas adjuvant concurrent chemoradiation is the treatment of choice for high risk cases. New more aggressive regimens are evaluated comprised of concurrent chemoradiation for intermediate risk cases and concurrent chemoradiation followed by chemotherapy for high risk cases. In contrary, fertility-preserving surgery consisting of radical trachelectomy or conization with/without chemotherapy can be offered to young patients with early-stage cervical cancer wishing to preserve their fertility.

For locally advanced stages (stage IB2, IIA2, IIB-IVA), concurrent chemoradiation with weekly Cisplatin is considered as standard treatment. New regimens with new chemotherapy agents, molecular targeted agents are being evaluated in many clinical trials. Adjuvant chemotherapy following chemoradiation for locally advanced cervical cancer, proved to be effective in phase II, are being evaluated in phase III clinical trial.

For metastatic, recurrent, persistent diseases, Cisplatin was proved as the most effective single agent but the efficacy was very limited. Median survival in such patients is only 7 months. Cisplatin-based doublet combination regimens, such as cisplatin– paclitaxel and cisplatin–topotecan, are more effective than Cisplatin as a single agent.

Median survival is approximately 10 months. Bevacizumab added to doublet combination was proved to increase median overall survival to 17 months. New targeted therapies of metastatic and recurrent cervical cancer, including therapeutic HPV vaccine, anti EGFR, anti angiogenesis,… are in translational researches.

In addition, advances in radiation therapy such as IMRT, IGRT, 3-D brachytherapy help to deliver higher dose to target volume whereas lower dose to organs at risk, increase local control and decrease radiation complications.

CẬP NHẬT XỬ TRÍ UNG THƯ CỔ TỬ CUNG Trần Đặng Ngọc Linh Ung thư cổ tử cung là ung thư thường gặp thứ tư ở phụ nữ. Ước tính năm 2012 trên toàn thế giới có 528000 ca mới mắc và 266000 ca tử vong do ung thư cổ tử cung.

Phần này tóm tắt, cập nhật điều trị ung thư cổ tử cung xâm lấn hiện nay. Đối với ung thư cổ tử cung xâm lấn sớm (giai đoạn IB1 và IIA1): phẫu trị đầu tiên bằng cắt tử cung tận gốc và nạo hạch chậu hay xạ trị triệt để có hiệu quả ngang nhau, nhưng có những biến chứng khác nhau. Xạ trị bổ túc sau mổ được chỉ định cho những bệnh nhân nguy cơ trung bình trong khi hóa xạ trị bổ túc là điều trị chọn lựa cho những bệnh nhân nguy cơ cao. Những phác đồ mạnh tay hơn đang được đánh giá bao gồm hóa xạ trị đồng thời bổ túc sau mổ cho các trường hợp nguy cơ trung bình và hóa trị củng cố thêm sau xạ trị đồng thời những trường hợp nguy cơ cao. Ngược lại, phẫu thuật bảo tồn bao gồm cắt cổ tử cung hay khoét chóp có kèm hay không kèm hóa trị được đề nghị cho bệnh nhân ung thư cổ tử cung trẻ tuổi muốn bảo tồn chức năng sinh sản. Đối với ung thư cổ tử cung giai đoạn tiến xa tại chỗ (giai đoạn IB2, IIA2, IIB-IVA): hóa xạ trị đồng thời với Cisplatin mỗi tuần được xem là điều trị chuẩn. Nhiều phác đồ hóa trị mới với các thuốc mới, thuốc nhắm trúng đích đang được đánh giá trong các thử nghiệm lâm sàng. Hóa trị bổ túc sau hóa xạ trị đồng thời đối với ung thư cổ tử cung giai đoạn tiến xa tại chỗ đã được chứng minh hiệu quả trong nghiên cứu pha II, đang được đánh giá trong thử nghiệm lâm sàng pha III. Đối với bệnh di căn, tái phát, tồn tại, Cisplatin là tác nhân đơn chất hiệu quả nhất nhưng hiệu quả cũng rất hạn chế. Trung vị thời gian sống còn ở những bệnh nhân này khi được điều trị bằng Cisplatin đơn chất chỉ khoảng 7 tháng. Phác đồ phối hợp 2 thuốc dựa trên Cisplatin, như cisplatin–paclitaxel và cisplatin–topotecan, hiệu quả cao hơn Cisplatin đơn chất nhưng trung vị thời gian sống còn chỉ khoảng 10 tháng. Thêm Bevacizumab vào phối hợp với 2 thuốc này đã được chứng minh là có thể tăng trung vị thời gian sống còn đến 17 tháng. Những liệu pháp nhắm trúng đích mới bao gồm liệu pháp vắc xin HPV điều trị, kháng EGFR, kháng sinh mạch… đang được nghiên cứu.

Hơn nữa, những tiến bộ trong xạ trị như kỹ thuật xạ trị điều biến liều, xạ trị dưới hướng dẫn hình ảnh, xạ trị trong với CT mô phỏng (IMRT, IGRT, 3-D brachytherapy) giúp tăng liều xạ trị vào bướu trong khi giảm liều xạ vào mô lành, tăng kiểm soát tại chỗ và giảm biến chứng do xạ trị.

The condition that characterized by genital ulcers is the most common of skin lesions of the female reproductive system. Instead of the etiologic agents of sexually transmitted diseases such as herpes simples virus, syphilis, chancroid, Donovanosis , thera are other infectious or noninfectious conditions that are not sexually transmitted dieases .

The algorithm of genital ulcer management from the primary care providers to the specialists will be described. The treatment of these conditions will be updated with the guidelines of CDC June 2015.

Tác giả cập nhật điều trị các bệnh lý này theo hướng dẫn mới nhất của Trung tâm Kiểm soát bệnh của Mỹ tháng 6/2015.

TREATMENT OF CERVICAL INTRAEPITHELIAL NEOPLASIA AT CERVICAL TRANSFORMATION ZONE: THE BIOLOGICAL BASIS AND PRACTICE Tang-Yuan Chu, MD, PhD Department of Obstetrics and Gynecology, Center for Cervical Cancer Prevention, TzuChi Medical Center, TzuChi University, Hualien, Taiwan Cervical cancer is the only cancer that can be totally prevented by vaccination for human papillomavirus (HPV) and screening for the precancerous cervical intraepithelial neoplasia (CIN). The reason for this unrivalled opportunity can be attributed to the invariable origin of CIN at the squamous - columnar junction where the stem - cell based squamous metaplasia takes place, providing the biological stage for HPV transformation. This specific and invariable tissue - of - origin of CIN at an accessible location at the uterine cervix provide the unprecedented success in both screening and treatment of precancerous lesions. Two standard treatment methods can be used to treat CIN: Loop electro - excision procedure (LEEP) and cryotherapy. Both easy to learn and can be conducted at an out-patient services.

LEEP is the standard treatment for CIN to excise the lesion get tissue diagnosis.

Cryotherapy is especially suitable for low - resource settings due to it's ease of performance, minimal complications, and cost - effectiveness. This talk will introduce the cell- of - origin and the biological basis of CIN arisen in the cervical transformation zone and the principles, procedures and effectiveness of the two treatment options for CIN.

ĐIỀU TRỊ U TÂN SINH TRONG BIỂU MÔ CỔ TỬ CUNG TẠI VÙNG CHUYỂN TIẾP CỦA CỔ TỬ CUNG: NỀN TẢNG VỀ MẶT SINH HỌC VÀ THỰC HÀNH Tang-Yuan Chu, MD, PhD Ung thư cổ tử cung là loại ung thư duy nhất có thể được phòng ngừa hoàn toàn bằng vaccine đối với HPV và tầm soát đối với các sang thương u tân sinh trong biểu mô cổ tử cung tiền-ung-thư (CIN). Lý do giải thích cho cơ hội đặc biệt này có thể bắt nguồn từ nguồn gốc không đổi của CIN tại vùng chuyển tiếp tế bào vảy-tế bào trụ nơi hiện tượng chuyển sản của tế bào mầm bắt đầu diễn ra, và đây cũng là một giai đoạn trong lây nhiễm HPV. Hiện tượng đặc hiệu và hằng định này của CIN tại một vị trí có thể tiếp cận được (cổ tử cung) giúp cho tỷ lệ thành công cao chưa từng có trong tầm soát và điều trị các sang thương tiền-ung-thư. Hai biện pháp điều trị tiêu chuẩn đối với CIN là: Cắt bằng vòng đốt (LEEP: loop electro-excision procedure) và liệu pháp đông lạnh (cryotherapy). Cả hai thủ thuật đều dễ học và có thể thực hiện ngoại trú.

LEEP là biện pháp điều trị tiêu chuẩn đối với CIN nhằm lấy mẫu mô để làm chẩn đoán mô học. Liệu pháp đông lạnh đặc biệt thích hợp với những nơi ít điều kiện do dễ thực hiện, tỷ lệ tai biến tối thiểu, và hiệu quả chi phí. Bài thuyết trình này sẽ trình bày nguồn gốc tế bào và những kiến thức nền tảng về mặt sinh học của CIN phát triển tại vùng chuyển tiếp tại cổ tử cung cũng như các nguyên lý, thủ thuật và hiệu quả của hai biện pháp điều trị CIN.

CERVICAL CANCER PREVENTION AND CONTROL IN VIETNAM: EVIDENCE AND RESOURCE-BASED APPROACH Luu Thi Hong*, Nguyen Vu Quoc Huy** * Assoc.

Prof. Dr, Vice-Rector, Hue University of Medicine and Pharmacy; nvqhuy@huemed-univ.edu.vn While cervical cancer is a nearly preventable disease, it’s currently one among most frequent cancers that affect Vietnamese women. In 2012, there are 5,664 newly detected cases, its incidence is 13.6/100,000. One risk factor among the others leading to this status is the fact that women have not been systematically screened by using appropriate and easy-to-access screening test, and then can be treated timely and effectively as needed.

Conventional Pap’s smear or liquid-based cytology are screening methods for precancerous lesions and cervical cancer with proven diagnostic validity. Besides many advantages, this test requires a standardized process from sampling, processing the smear, interpretation of findings, and management of detected abnormalities, most of them are only available at the high-level health facilities.

Infection with high-risk Human Papillomavirus (HPV) has been confirmed as causal factor of cervical cancer. The HPV DNA test was initially deployed in major cities, in order to detect high-risk HPV infection for and appropriate management. Several large studies abroad have initially confirmed the value of HPV DNA testing as a primary screening test, which may allow an extension of screening intervals up to 3-5 years. In recent two decades, there are an increasing evidence from international as well as domestic studies in validity and widespread applicability of visual inspection with acetic acid (VIA).

In answering the need of health care providers on knowledge and skills providing cervical cancer screening, prevention and treatment services at different health care levels, the “Guidelines on screening and treatment of precancerous lesions for cervical cancer secondary prevention” has been developed and promulgated by Ministerial Decision Nr.: 1476 /QD-BYT on May 16th, 2011 - is complimentary to the National Guidelines on Reproductive Health Care Services, promulgated by Ministry of Health in 2009, for implementation of health care services related to cervical cancer screening, prevention and treatment for secondary prevention, integrated into provision of reproductive health care services.

Many international multicentre trials with large sample size recently shown that HPV DNA tests having diagnostic value not lower than those from cervical cytology in detecting CIN2 + lesions. Parallels to cytology, HPV testing has become the primary screening test for women age 30 and older and is on track to become an independent screening for women aged 25 and older.

Based on international and national evidence accumulated during the last 10 years and available resources and services providing capability at all levels of the health system in Vietnam and forecast trend in the next 10 years, the Ministry of Health is currently developing an National Action plan for cervical cancer prevention and control in Vietnam (2016-2025), while updating the current " Guidelines on cervical cancer prevention".

DỰ PHÒNG VÀ KIỂN SOÁT UNG THƯ CỔ TỬ CUNG TẠI VIỆT NAM: TIẾP CẬN DỰA TRÊN BẰNG CHỨNG VÀ NGUỒN LỰC Lưu Thị Hồng*, Nguyễn Vũ Quốc Huy** Mặc dù là bệnh có thể dự phòng và phát hiện sớm, hiện tại ung thư cổ tử cung vẫn là một trong những bệnh ung thư thường gặp nhất ở phụ nữ Việt Nam. Năm 2012, tại Việt Nam có 5.664 phụ nữ được chẩn đoán mắc mới ung thư cổ tử cung, tỷ lệ mắc mới ung thư cổ tử cung là 13,6/100.000 phụ nữ. Một trong những lý do dẫn đến tình trạng này là đa số phụ nữ chưa được sàng lọc định kỳ và có hệ thống để phát hiện sớm ung thư qua các xét nghiệm thích hợp, dễ tiếp cận; và khi phát hiện tổn thương tiền ung thư thì cũng chưa được điều trị kịp thời và hiệu quả.

Xét nghiệm tế bào cổ tử cung truyền thống hoặc tế bào trong dung dịch là các phương pháp sàng lọc tổn thương tiền ung thư và ung thư cổ tử cung với giá trị đã được chứng minh. Bên cạnh nhiều ưu điểm, xét nghiệm này đòi hỏi một quy trình chuẩn từ khâu lấy mẫu, xử lý tiêu bản, đọc và diễn giải kết quả đến xử trí các trường hợp có bất thường được phát hiện, đa số chỉ sẵn có ở các cơ sở y tế tuyến cao.

Nhiễm Human Papillomavirus (HPV) các týp nguy cơ cao đã được khẳng định là yếu tố nguyên nhân của ung thư cổ tử cung. Các test ADN HPV bước đầu đã được triển khai tại các thành phố lớn, nhằm phát hiện các trường hợp nhiễm HPV nguy cơ cao và có xử trí phù hợp. Một số nghiên cứu lớn ở nước ngoài đã bước đầu khẳng định giá trị của xét nghiệm ADN HPV như một test sàng lọc, có thể cho phép giãn khoảng thời gian giữa hai lần sàng lọc lên đến 3-5 năm. Trong hai thập kỷ gần đây, ngày càng có nhiều bằng chứng quốc tế cũng như trong nước về giá trị và khả năng ứng dụng trên diện rộng của phương pháp quan sát cổ tử cung sau acid acetic (visual inspection with acetic acid - VIA). Trong bối cảnh đó, trên cơ sở các nghiên cứu trong môi trường bệnh viện, cộng đồng và một dự án triển khai thử nghiệm tại Việt Nam trong khoảng thời gian 2006 – 2011 trong lĩnh vực sàng lọc ung thư cổ tử cung, “Hướng dẫn sàng lọc, điều trị tổn thương tiền ung thư để dự phòng thứ cấp ung thư cổ tử cung” do Bộ Y tế ban hành ngày 16.5.2011 đã đặt ra khung pháp lý và kỹ thuật cho việc triển khai công tác sàng lọc trên diện rộng.

Nhiều thử nghiệm quốc tế với cỡ mẫu lớn gần đây đã chỉ ra rằng xét nghiệm ADN HPV có giá trị chẩn đoán không thấp hơn tế bào cổ tử cung trong phát hiện các tổn thương CIN2+. Đồng thời với tế bào học, xét nghiệm HPV đã trở thành xét nghiệm sàng lọc sơ cấp dành cho phụ nữ độ tuổi 30 trở lên và đang trên đường trở thành một xét nghiệm sàng lọc độc lập cho các phụ nữ từ 25 tuổi trở lên.

Trên cơ sở các bằng chứng quốc tế và trong nước trong khoảng 10 năm gần đây, căn cứ vào nguồn lực và khả năng cung cấp dịch vụ tại các tuyến của hệ thống y tế Việt Nam và dự báo tiềm năng phát triển trong 10 năm sắp đến, Bộ Y tế đang xây dựng Kế hoạch hành động đự phòng và kiểm soát ung thư cổ tử cung Việt Nam giai đoạn 2016-2025, đồng thời tiến hành cập nhật “Hướng dẫn dự phòng và kiểm soát các ung thư phụ khoa”.

Management of post-LEEP with Positive Margins.

Loop electrosurgical excision procedure (LEEP) is an excisional procedure commonly used for CIN 2,3 lesions wherein a wire loop is utilized through which an electric current is passed at variable power settings. This may be a diagnostic and therapeutic approach that provides a specimen for histological examination.

CIN 1 has a 1% risk of progression towards invasive carcinoma, while CIN 2 and CIN 3 have 5% and 12% risk of progression, respectively. In a local study done at the Section of Gynecology Oncology-Philippine General Hospital, the cure rate of LEEP for CIN was 91% with a recurrence rate of 9%. The recurrence would appear in the first 2 years following treatment. These data are comparable with international statistics.

The presence of positive margins from LEEP specimen signifies presence of residual disease in the cervix and predicts the high likelihood of CIN recurrence. The extent of involvement of the endocervical margins and depth of invasion must be checked. The rate of recurrence rises to 27.5 - 37% if with positive margins.

Recurrence also rises with patients who are positive for the high-risk HPV types. HPV 16 was associated with the greatest tendency to persist and the highest probability to progress to invasive carcinoma when it persists. Co-testing with cervical cytology and HPV DNA test provides a sensitivity of 97% for detection of post-treatment CIN 2.

Following LEEP, the results may be categorized into three groups: CIN 1 with positive or negative margins and CIN 2,3 with negative margins, CIN 2,3 with positive margins, and unknown margin status (underwent cryotherapy or laser ablation). For the first group, cervical cytology or pap smear test at 6 months or HPV DNA test at 12 months may be viable options. Treatment depends on the cervical cytology result or positivity of the HPV DNA test. If the cervical cytology result is negative as well as the HPV DNA test, routine screening per age group is followed.

For patients with CIN 2, 3 with positive margins, three options are available: cervical cytology or pap smear test at 6 months with endocervical cytology, re-exicison (especially if invasion is suspected), or hysterectomy. Following re-excision or hysterectomy, management will depend whether there is persistent or recurrent CIN, and presence of microinvasion or invasive carcinoma. When deciding on re-excision or hysterectomy, the patients must be well-advised on the potential risks. Risks of repeat excision must be weighed against the potential risk of untreated CIN or the potential for

A SURVEY ON KNOWLEDGE, ATTITUTE AND PRACTICE (KAP) OF PARENTS WHOSE DAUGHTERS HAVING HPV VACCINATION IN HUNGVUONG HOSPITAL Huynh Xuan Nghiem Hung Vuong Hospital

Objectives: To investigate the KAP of parents whose daugters from 9 to 17 years old having HPV vaccination in Hung Vuong Hospital of Ho Chi Minh City.

Method: A descriptive study using a questionnaire was performed for participants (n= 5000) whose daughters coming to Hung Vuong Hospital for HPV consultation or vaccination (from August 2013 to March 2014).

Results: Cervical cancer knowledge was rather high; approximately 65%. Knowledge of and attitude towards vaccination against cervical cancer caused by HPV virus were as follows: good, 78.8%; satisfactory, 17.5%; poor, 3.7%. Furthermore, 27.4% participants thought that after being vaccinated against HPV, they would be completely protected from cervical cancer; 93.3% worried about HPV infection, but the vaccine price was deemed expensive by 68.2% of participants.

Conclusion: The rates of participants who have adequate knowledge of and right attitudes towards vaccination against cervical cancer caused by HPV virus in are rather high in this study, especially for participants who have been living in Ho Chi Minh city.

Further attempts of health education and propagation are needed to prevent customersâ&#x20AC;&#x2122; misunderstanding after vaccination that they will be protected completely from cervical cancer.

KHẢO SÁT KIẾN THỨC, THÁI ĐỘ, THỰC HÀNH CỦA CHA MẸ CÓ CON GÁI CHỦNG NGỪA HPVTẠI BỆNH VIỆN HÙNG VƯƠNG Huỳnh Xuân Nghiêm Đây là một nghiên cứu khảo sát kiến thức thái độ và hành vi của phụ huynh các bé gái 9– 17 tuổi về vắc xin ngừa HPV tại Bệnh viện Hùng Vương TP.HCM từ tháng 08/201303/2014.

Phương pháp: Nghiên cứu mô tả, sử dụng khảo sát cộng đồng (cỡ mẫu n = 5000) với bảng câu hỏi có cấu trúc.

Kết quả: Tỉ lệ đối tượng nghiên cứu (ĐTNC) có kiến thức chung về ung thư cổ tử cung tương đối cao đa số >65% và thái độ về chủng ngừa ung thư cổ tử cung do HPV gây ra được đánh giá tốt là 78,8%, kiến thức và thái độ khá là 17,5%, và kém là 3,7%. Chỉ có 27,4 % khách hàng nghĩ rằng chủng HPV là có thể ngừa ung thư cổ tử cung hoàn toàn.

Và 93,3% khách hàng sợ bị lây nhiễm HPV. Có 68,2% khách hàng không tiêm phòng HPV vì giá quá cao.

Kết luận: Tỉ lệ có kiến thức và thái độ đúng về chủng ngừa ung thư cổ tử cung do HPV gây ra khi đến chủng ngừa HPV tại Bệnh viện Hùng Vương tương đối cao. Điều này nói lên sự quan tâm về sức khỏe bản thân và kiến thức về HPV của người dân đến khám là tốt. Nhưng đòi hỏi cần phải đẩy mạnh công tác truyền thông để khách hàng không chủ quan sau khi chủng ngừa HPV cho rằng sẽ ngừa ung thư cổ tử cung hoàn toàn .

THE MANAGEMENT OF AGUS SMEARS AND CERVIX GLANDULAR LESIONS Neerja Bhatla, Prerana Singh

Glandular abnormalities are present in approximately 0.1-2.1% of cervical cytology samples. Atypical glandular cells (AGC) are seen most commonly in women aged 40 years or older. The risk of malignancy in women with AGC increases with age.

Malignancy is diagnosed in approximately 15% of women with AGC who are 50 years or older. AGC are associated with premalignant or malignant disease in approximately 30% of cases.

In the Bethesda system, glandular abnormalities are classified as follows: 

Atypical glandular cells (endocervical, endometrial or not otherwise specified NOS) – Replaces the previous term AGUS



Atypical glandular cells, favour neoplasia (endocervical, endometrial or not otherwise specified – NOS)



Adenocarcinoma in Situ (AIS)



Adenocarcinom

Glandular abnormalities reported on Pap smear may be associated with high-grade squamous lesions, adenocarcinoma in situ (AIS) or invasive cancer. Because of the spectrum of neoplasia linked to atypical glandular cells, evaluation must include colposcopy, endocervical evaluation and endometrial evaluation. Colposcopy of glandular lesions may be more challenging than evaluation of squamous lesions.

Endometrial sampling is recommended in all women aged ≥ 35years and for younger women at risk for endometrial neoplasia (risk factors/ symptoms are present). HPV testing is not necessary if it has been done previously because colposcopy and endometrial sampling are required regardless of HPV results. For pregnant women with AGC, endocervical sampling with a curette and endometrial sampling should not be performed, as there is a risk of disturbing the pregnancy; however, the endocervical canal may be sampled gently with a cytobrush.

Positive findings are treated as appropriate for the histologic diagnosis. If cervical biopsy or ECC identify cervical intraepithelial neoplasia (CIN) of grade 1,2 or 3, AIS, or microinvasion, further evaluation by cold knife conisation (CKC) is indicated. Loop 30

electrosurgical excision procedure (LEEP) may not be adequate to sample the entire endocervical canal and glandular lesions are often multifocal. Top-hat excision by LEEP is inferior to CKC for pathologic reporting. Further management is to be planned depending on the final diagnosis, patientâ&#x20AC;&#x2122;s age and parity and desire for future fertility, Patients with CIN 1-3 or AIS may be placed on follow-up.

Rarely atypical glandular cells on cervical cytology are associated with a primary tumor at sites other than cervix or uterus. Evaluation for disease at other sites is required for selected women who have negative finding despite comprehensive evaluation (i.e., colposcopy, endometrial sampling, conisation). This includes women with cytologic findings associated with high risk of malignancy. These patients should be evaluated for primary or metastatic disease involving the fallopian tube, ovary, and other pelvic and abdominal organs. The first-line study is a transvaginal ultrasound. Women with an adnexal mass should be further evaluated for ovarian or tubal cancer. Women with no adnexal mass should be evaluated for colon or other intra-abdominal malignancy with colonoscopy and abdominal computed tomography or magnetic resonance imaging.

Thus a diagnosis of atypical glandular cells is a more suspicious one with possible origin from a variety of organs and should be followed by a thorough work-up and complete management plan.

The Papanicolaou smear, or the conventional Pap smear as it is commonly known, has been the cornerstone of one of the most successful cancer screening programmes in history.

Since its introduction in the 1940s, it has been credited with helping to halve the cervical cancer incidence and death rates especially in countries with established cervical cancer screening programmes.

The introduction of new technology in the form of liquid based cytology has slowly but surely resulted in the decline of the conventional Pap smear.

In many countries, LBC has all but replaced conventional cytology.

Australia is amongst the last few countries with established screening programmes, that still predominantly uses the conventional Pap smear.

This however is about to change in light of newer molecular tests for HPV and the introduction of a National HPV vaccination programme that vaccinates both girls and boys of school age in Australia.

The radical changes will occur with the implementation of the Renewal of the Cervical Screening Programme in May 2017.

In anticipation of, and to inform the preparations needed to meet this challenge, the Victorian Cytology Service (VCS) Inc and Cancer Council NSW, have embarked on a randomised control trial of >120,000 women to study the use of primary HPV screening vs cytology in vaccinated and un-vaccinated women.

An LBC specimen will form the basis for this study as it also will in the Renewal Programme for cervical cancer screening in Australia.

This will be the death knell for the conventional Pap smear in Australia.

LBC TRONG TẦM SOÁT UNG THƯ CỔ TỬ CUNG: CHẶNG ĐƯỜNG PHÍA TRƯỚC Siew Chin Lian, MBBS, FRCPA Test Papanicolaou smear, còn gọi là test Pap smear truyền thống, đã từng là một công cụ chính yếu của những chương trình tầm soát ung thư cổ tử cung thành công nhất trong lịch sử. Từ khi ra đời những năm 1940s, Pap smear được cho rằng đã giúp giảm một nửa số trường hợp mới mắc của ung thư cổ tử cung và tử suất, đặc biệt ở những quốc gia có chương trình tầm soát ung thư cổ tử cung hiệu quả.

Kỹ thuật mới dựa trên lấy tế bào nhờ dung dịch lỏng (LBC: liquid-based cytology) đã từ tử thay thế và làm giảm vai trò của Pap smear truyền thống. Tại nhiều quốc gia, LBC đã thay thế hoàn toàn Pap smear truyền thống. Úc là một trong những nước cuối cùng thiết lập chương trình tầm soát ung thư cổ tử cung, và hiện vẫn sử dụng nhiều phương pháp Pap smear truyền thống. Tuy nhiên, điều này đang thay đổi, và chuyển qua sử dụng các test sinh học phân tử để phát hiện HPV, và giới thiệu chương trình tiêm vaccine quốc gia đối với HPV cho cả trẻ gái và trẻ trai ở độ tuổi đi học tại Úc. Những thay đổi triệt để này sẽ được tiến hành vào tháng 5/2017, với chương trình Cải tiến Tầm soát Ung thư Cổ tử cung. Nhằm chuẩn bị trước cho những thay đổi này, công ty VCS (Victorian Cytology Service Inc) và hội Ung thư NSW, đã tiến hành thử nghiệm ngẫu nhiên có nhóm chứng trên >120,000 phụ nữ nhằm nghiên cứu khả năng tầm soát HPV nguyên phát, và kết quả tế bào học ở nhóm phụ nữ được tiêm vaccine và nhóm chưa được tiêm vaccine.

Một mẫu bệnh phẩm được lấy với kỹ thuật LBC sẽ được sử dụng trong thử nghiệm này, và kỹ thuật này cũng sẽ được dùng trong chương trình Cải tiến Tầm soát Ung thư Cổ tử cung tại Úc. Đây sẽ là tiếng chuông báo tử đối với kỹ thuật Pap smear truyền thống tại Úc.

Pap test is the common method for cervical screening in Vietnam. Since more than a decade, HPV DNA testing was progressively used in medical settings. We first developed a PCR-RDB (Reverse Dot Blot) method for HPV genotyping which contributed to the realization of some epidemiological studies on HPV. Nevertheless, PCR-RDB is not an appropriate tool for routine diagnosis, due to low sensitivity, risk of cross-contamination and

unnecessary HPV types detected. A real-time PCR

indentifying common high-risk HPV types based on the recommendations of medical organizations worldwide was subsequently developed. This method was sensitive, cross-contamination free and detected the most significant high-risk HPV types.

However, infection by high risk HPV types does not always progress to cancer. Pap test shows cell transformation but can be subject to unclear results. Therefore, different biomarkers were proposed to identify women with HPV positive, abnormal or unclear Pap test, who have high probability to undergo cervical cancer progress. We developed immunocytochemistry method and ELISA based on a monoclonal antibody against p16INK4A, a cellular protein overexpressed in cervical cancer cells. These methods were tested in cervical cells from swabs and liquid-cytology from some gynecological hospitals in Ho Chi Minh City, and showed potential for application in diagnosis and prognosis of cervical cancer.

MỘT SỐ XÉT NGHIỆM HPV VÀ CHỈ THỊ SINH HỌC CHO UNG THƯ CỔ TỬ CUNG ĐANG SỬ DỤNG VÀ CÓ TIỀM NĂNG Ở VIỆT NAM Hồ Huỳnh Thùy Dương Ở Việt Nam, Pap test được sử dụng để sàng lọc ung thư cổ tử cung. Từ hơn mười năm trở lại đây, xét nghiệm HPV DNA dần được triển khai tại nhiều cơ sở y tế. Phương pháp PCR-RDB (Reverse Dot Blot) mà chúng tôi phát triển trong thời kỳ đầu đã đóng góp vào một số công trình dịch tễ học về nhiễm HPV. Tuy nhiên, PCR-RDB không phù hợp cho chẩn đoán thường quy do độ nhạy thấp, dễ ngoại nhiễm, phát hiện nhiều týp HPV thừa. Do đó, phương pháp real-time PCR phát hiện chỉ các týp HPV “nguy cơ cao” phổ biến, theo khuyến cáo của nhiều tổ chức y tế trên thế giới được phát triển sau đó.

Phương pháp này có độ nhạy cao, tránh được ngoại nhiễm, phát hiện các týp gây bệnh chủ yếu. Tuy nhiên, sự nhiễm ngay cả các týp HPV “nguy cơ cao” cũng không luôn dẫn đến ung thư. Xét nghiệm Pap cho thấy được sự chuyển dạng tế bào nhưng có thể cho kết quả không rõ ràng. Do đó, một số chỉ thị sinh học đã được đề xuất để nhận diện những trường hợp nhiễm HPV, có kết quả xét nghiệm tế bào học bất thường hoặc không rõ ràng, và có khả năng diễn tiến thành ung thư. Chúng tôi phát triển phương pháp hóa mô miễn dịch và ELISA dựa trên kháng thể đơn dòng kháng với một protein tế bào người là p16INK4A , có hàm lượng tăng cao ở tế bào ung thư.

Các phương pháp này đã được thử nghiệm trên các mẫu phết và dịch tế bào cổ tử cung từ một số bệnh viện phụ sản thành phố Hồ Chí Minh và cho thấy có tiềm năng ứng dụng trong tương lai.

Objective: We conducted phase I clinical trial of a novel therapeutic HPV DNA vaccine, GX-188, which encodes engineered HPV16/18 E6 and E7 antigens, to see the complications and preliminary therapeutic effects.

Methods: GX188 DNA vaccine was administered by in vivo electroporation (GX188E) in nine subjects who had a biopsy-proven cervical intraepithelial neoplasia grade 3 (CIN3, severe dysplasia or carcinoma in situ) with HPV 16/18. After injecting three times(0,4,12weeks) with 1mg, 2mg and 4mg of GX188 in each group of three patients, all patients were followed up with colposcopic biopsy and HPV DNA test after two months from the last injection.

Results: Seven (77.8%) of nine patients showed complete regression from CIN3 to cervicitis on colposcopic biopsy. On the HPV DNA test, type 16 and 18 disappeared in seven(77.8%) patients. Two had a persistent HPV infection and CIN3 lesion. All patients immunized with GX188E had elevated T-cell responses to HPV16/18 E6 and E7 peptide pools as detected by IFN-g enzyme-linked immunosorbent spot (ELISPOT) compared to pre-vaccination baseline levels. The injection-site pain was tolerable with mean VAS score of 1.9 ten minutes after injection, and none of the patients presented significant adverse events.

Conclusion: These results demonstrate that a novel therapeutic HPV DNA vaccine, GX-188, is highly effective for the treatment of CIN III with a high rate of clearance of HPV infection and even can be safely used.

Vaccine DNA trị liệu HPV 16/18, GX-188, mang lại tỷ lệ sạch nhiễm HPV cao, và cũng làm thoái triển CIN III Young Chul Sung PhD1, Hyun Tak Jin PhD1, Tae Jin Kim MD, PhD2, Soo YoungHur MD, PhD3, Jong Sup Park MD, PhD3 Mục tiêu: Chúng tôi thực hiện thử nghiệm lâm sàng phase I của một vaccine trị liệu DNA mới (GX-188), được tạo bởi kháng nguyên HPV 16/18 E6 và E7, nhằm khảo sát các bién chứng và tác dụng trị liệu ban đầu.

Phương pháp: Vaccine DNA GX188 được sử dụng ở 09 đối tượng được chẩn đoán CIN3 kèm theo nhiễm HPV 16/18 dựa trên kết quả sinh thiết (CIN3, nghịch sản nặng hoặc carcinoma tại chỗ). Sau khi tiêm 3 lần (0, 4, 12 tuần) với liều 1 mg, 2 mg và 4 mg GX188 ở mỗi nhóm 3 bệnh nhân, tất cả bệnh nhân đều được theo dõi bằng sinh thiết qua soi cổ tử cung và test HPV DNA sau 2 tháng kể từ lần tiêm vaccine cuối cùng.

Kết quả: 7/9 đối tượng (77.8%) cho thấy sự thoái triển hoàn toàn từ CIN3 xuống còn viêm cổ tử cung trên kết quả sinh thiết qua soi cổ tử cung. Trong test HPV DNA, HPV type 16 và 18 biến mất trong 7 đối tượng (77.8%). 2 đối tượng có nhiễm HPV và sang thương CIN3 kéo dài. Tất cả bệnh nhân được tiêm GX188E đều tăng đáp ứng miễn dịch do tế bào T đối với HPV 16/18 E6 và E7, được phát hiện bởi test ELISPOT (IFN-g enzyme-linked immunosorbent spot) khi so sánh với mức baseline trước khi chích vaccine. Đau tại nơi tiêm vaccine không nghiêm trọng, điểm VAS là 1.9 tại thời điểm 10 phút sau tiêm, và không có bệnh nhân nào xuất hiện các phản ứng nguy hiểm sau khi tiêm vaccine.

Kết luận: Những kết quả trên cho thấy một vaccine DNA mới dùng trị liệu HPV, GX188, đạt hiệu quả cao trong điều trị CIN III với một tỷ lệ sạch nhiễm HPV cao, và có thể sử dụng an toàn.

Objectives: We study the prevalence of HPV and subtype in squamous cell carcinoma of the penis and the relationship between HPV infection and epidemiologic, clinical, pathological factors.

Material and methods: Investigations were carried out on 48 penile carcinoma patients treated at HCM city Oncology Hospital, from 08/2009 to 03/2010. The technique of PCR is used to detect the presence of HPV on penile spiecemen.

Results: HPV is detected in 16,7% of the tumors, with HPV 16 being 75%. The prevalence of urban patients in group HPV (+) is 50% where as that in group HPV (-) is 17.5%. The tumor and lymph nodes size in group HPV (+) are 2.9 cm and 0.6 cm ( 4.4cm and 1.1 cm in group HPV (-) ) Conclusion: the prevalence of HPV infection in penile cancer patient is relatively small with HPV 16 being predominant. The size of tumor and lymphe nodes of infected patients are smaller than the one of non infected patients.

NHIỄM HPV VÀ UNG THƯ DƯƠNG VẬT Phạm Đức Nhật Minh, và cộng sự Mục tiêu: xác định tỷ lệ nhiễm HPV và sự phân bố các týp trên BN UTDV cũng như khảo sát mối liên quan giữa tình trạng nhiễm HPV với các yếu tố dịch tễ, lâm sàng, bệnh học trên bệnh nhân ung thư dương vật Đối tượng phương pháp: 48 trường hợp nghiên cứu tiền cứu ung thư dương vật từ 08/2009 đến 03/2010, chẩn đoán dựa vào kỹ thuật PCR mẫu mô bệnh phẩm sang thương dương vật Kết quả: tỷ lệ nhiễm HPV là 16,7%, trong đó típ 16 chiếm 75%. Ở nhóm bệnh nhân HPV(+), tỷ lệ sống tại thành phố là 50% trong khi ở nhóm bệnh nhân HPV (-) chỉ có 17,5%. Kích thước bướu và kích thước hạch trung bình ở nhóm HPV (+) là 2,9 cm và 0,6 cm trong khi ở nhóm HPV (-) là 4,4 cm và 1,1 cm.

Kết luận: tỷ lệ nhiễm HPV ở bệnh nhân ung thư dương vật tương đối thấp, trong đó típ 16 chiếm đa số. Cervical cytology test has been successfully used for cervical cancer screening program leading to significant reduction in the incidence of invasive cervical cancer.

The Bethesda System is the most commonly adopted cytology reporting system. The 2014 version is now available. There are minimal changes to the terminology. The 2tiered classification scheme of low grade and high grade squamous intraepithelial lesions (LSIL, HSIL) is maintained. For squamous lesions with features that lie between LSIL and HSIL, they can be interpreted as atypical squamous cells (ASC) cannot rule out HSIL (ASC-H) in addition to LSIL. The other change regards reporting of benignappearing endometrial cells, which is now recommended for women ď&#x201A;ł45, to improve the predictive value for endometrial hyperplasia/carcinoma.

New technical platforms have been developed to improve the efficiency of screening.

(1) Liquid based cytology to improve the quality of the sample for evaluation and provide convenient and reliable sample material for ancillary tests. (2) High risk human papilloma virus (HR-HPV) molecular tests to detect most important risk factor for cervical cancer and increase sensitivity of screening. (3) Computerized imaging equipment for automated detection of abnormal cells. (4) Detection of molecular markers such as p16INK4A, Ki67, topoisomerase II-alpha, minichromosome maintenance protein-2, HPV L1 capsid, HPV E6 and E7 HPV mRNA transcripts assay, methylation status of candidate tumour suppressor genes and cellular proteins overexpressed in HPV-infected cells to increase specificity of cancer cell identification.

In particular, various HR-HPV detection and/or genotyping tests have been developed.

Such HPV tests are useful for triage of cervical cancer screening, triage of ASC of undetermined significance (ASC-US) and evaluate risk of recurrence of cervical diseases. Health professionals should be aware of the psychosocial impact of HPV test results. Low-risk HPV tests should not be used. HR-HPV tests are not indicated for women < 25 or as reflex test after Pap test results of HSIL. One should bear in mind that quality control of all laboratory tests is of utmost importance in reliable screening. Laboratory accreditation should be encouraged. Most importantly, continuation of cervical cancer screening is necessary even in the post HPV vaccine era until all females can be vaccinated by multivalent vaccine.

Hệ thống phân loại Bethesda là hệ thống được sử dụng rộng rãi nhất. Version 2014 đã được phổ biến. Có rất ít thay đổi về cách sử dụng thuật ngữ. Cách phân loại 2 lớp gồm các sang thương tế bào lát trong biểu mô grade thấp và grade cao (LSIL, HSIL) vẫn được duy trì. Đối với các sang thương tế bào vảy với các đặc điểm nằm giữa LSIL và HSIL, chúng sẽ được diễn giải là ASC (tế bào vảy không điển hình), không thể loại trừ HSIL (ASC-H) kèm theo LSIL. Các tế bào nội mạc tử cung bình thường hiện tại được khuyến cáo ghi nhận trong báo cáo đối với những trường hợp phụ nữ 45 tuổi, nhằm cải thiện giá trị tiên đoán cho (tình trạng) tăng sinh/carcinoma nội mạc tử cung. Các kỹ thuật mới đã được phát triển nhằm cải thiện hiệu quả của các biện pháp tầm soát. 91) Test tế bào học dựa trên chất lỏng (liquid-based cytology) nhằm cải thiện chất lượng của mẫu để cho quá trình đánh giá tốt hơn, và cung cấp những mẫu đạt tiêu chuẩn để làm các test khác. (2) Test phát hiện HPV nguy cơ cao (HR-HPV: high-risk HPV) nhằm phát hiện các yếu tố nguy cơ quan trọng nhất trong ung thư cổ tử cung và tăng độ nhậy của việc tầm soát. (3) Phát triển các hệ thống hình ảnh học để có thể phát hiện tử động các tế bào bất thường. (4) Phát hiện các marker phân tử vs. p16INK4A, Ki67, topoisomerase II-alpha, minichromosome maintenance protein-2, HPV L1 capsid, các test phát hiện mRNA HPV E6 và E7, tình trạng methylation (methyl hoá) của các gene ức chế u và các protein trong tế bào có biểu hiện quá mức ở những tế bào đã nhiễm HPV nhằm gia tăng độ đặc hiệu của quá trình phát hiện các tế bào ung thư. Đặc biệt, nhiều test giúp phát hiện và định kiểu gene của HR-HPV đã được phát triển.

Những test HPV như thế sẽ mang lại nhiều lợi ích trong việc xác định mức độ ưu tiên trong điều trị đối với tầm soát ung thư cổ tử cung, hoặc những case bị ASC-US (ASC of undetermined significance), và đánh giá nguy cơ tái phát của các bệnh lý cổ tử cung. Các chuyên gia y tế nên nhận thức được mứ độ ảnh hưởng về mặt tâm lý-xã hội của kết quả các test HPV. Các test HPV để phát hiện các type HPV nguy cơ thấp không nên sử dụng. Các test HR-HPV không được chỉ định ở phụ nữ <25 tuổi hoặc sau khi có kết quả Pap test là HSIL. Chúng ta nên nhớ rằng kiểm soát chất lượng trong tất cả các test trong lab là điều quan trọng nhất nếu muốn tầm soát có hiệu quả. Chúng ta nên có chương trình và cấp chứng nhận chất lượng cho mỗi phòng lab. Điều quan trọng nhất là cần tiếp tục tầm soát ung thư cổ tử cung sau thời kỳ đã sử dụng HPV vaccine cho tới khi tất cả mọi phụ nữ đều được chích vaccine đa type (mulivalen vaccine).

CYTOLOGY AND COLPOSCOPY â&#x20AC;&#x201C; CAN THEY DO WITHOUT EACH OTHER IN CERVICAL CANCER SCREENING ?

Department of Obstetrics and Gynecology Faculty of Medicine University of Indonesia Cipto Mangunkusumo Hospital, Jakarta, Indonesia Cytologic smears were known as a screening test for preinvasive lesions, which, if treated, would be prevented from developing into invasive cancer. Credit for its conception and development goes to George N. Papanicolaou, who first reported in 1928 that malignant cells from the cervix can be identified in vaginal smears. Later, in collaboration with the gynecologist Herbert Traut, Papanicolaou published detailed descriptions of preinvasive cervical lesions. Then, the test which is well known as the Pap test or Pap smear has been used widely in the world for the prevention of cervical cancer.

The introduction of the regular Pap smear as a screening tool for cervical cancer has markedly decreased the number of deaths from cervical cancer. This screening method is known to have a high rate of false-negative results; therefore, serial examinations are necessary for optimal sensitivity. The sensitivity of the routine Pap smear is further increased with the addition of colposcopy to cytologic screening.

Colposcopy is a diagnostic method useful for the diagnosis and evaluation of cervical intraepithelial neoplasia and preclinical invasive cancer after an abnormal cytologic smears result. It allows magnified visualization of the site where cervical carcinogenesis occurs. It enables taking directed biopsy and in delineating the extent of lesions on the cervix in screen-positive women. It also helps in directing treatments such as cryotherapy and loop electrosurgical excision procedure for cervical intraepithelial neoplasia. Topical application of saline, acetic acid, and iodine solutions helps identify biopsy sites. Other indications for colposcopy include visible cervical abnormalities, evidence of clinical HPV infection, and follow-up of prior cervical treatment. Unfortunately, both Pap smear and colposcopy are not widely available and not widely practiced in many developing countries where a high incidence of cervical cancer is observed. There are some options in cervical cancer screening method now, with or without cytology and/or colposcopy after the introduction of VIA (visual inspection with acetic acid application) and HPV test.

Some cervical cancer prevention programs will or already been switch from cytology to carcinogenic HPV test-based screening, so there will be some clinical implications of that switch on the performance and workload of colposcopy.

Độ nhạy của Pap smear thường quy được tăng lên khi kết hợp với so cổ tử cung (colposcopy).

Soi cổ tử cung là biện pháp chẩn đoán hữu hiệu để chẩn đoán và đánh giá các sang thương CIN và ung thư xâm lấn tiền-lâm-sàng sau khi có kết quả tế bào smear bất thường. MANAGEMENT OF RECURRENT CIN AFTER LEEP OR CRYOTHERAPY Efren J. Domingo, MD.,PhD.

The overall cure rate (defined as absence of persistent or recurrent CIN) for CIN is 8488.3% with the use of cryotherapy. However, the recurrence rates of CIN 1, CIN 2-3 and all CIN are greater with cryotherapy compared with LEEP with an odds ratio of 2.14 for all CIN. Following treatment with LEEP, the average recurrence rate for CIN 2-3 is 9.1% and most recurrences occur in the first 2 to 3 years. This rises to 27.5-37% recurrence rate if there are positive margins.

The decision regarding treatment options will depend on the degree of dysplasia, extent of the disease, patient age and desires regarding preserving fertility. For patients with persistent or recurrent CIN 1, either ablative of excisional methods may be employed.

Excision procedure is preferred if there is inadequate or unsatisfactory colposcopy, there are findings of CIN 2 or higher on endocervical curettage, and for previously treated patients. Once the result of the repeat excisional procedure shows a higher grade of CIN or is identified at the margins of an excisional procedure, three options include cervical cytology and endocervical curettage at 4-6 months, repeat excision and/or hysterectomy if repeat excision is not feasible. A repeat diagnostic excisional procedure or hysterectomy is acceptable for women with a histologic diagnosis of recurrent or persistent CIN 2 or 3.

Ablation procedure is not recommended for CIN 2, CIN 3, CIN 2-3 with inadequate colposcopy, and endocervical sampling positive with CIN 2, 3. Observation of CIN 2 or CIN 3 with sequential cytology and colposcopy is likewise unacceptable except in pregnant women and young women.

Included in the management of recurrent CIN is the use of human papilloma virus (HPV) DNA test. Persistent human papilloma virus (HPV) infection has been shown to increase the risk of developing cervical cancer following 2 years after treatment compared with women who have no history of CIN. This is most especially true for patients positive for HPV 16. Women identified with the high-risk HPV DNA are recommended to have closer surveillance.

All patients must be advised on the risk of recurrence of CIN especially for the high-risk individuals, and the importance of close follow-up even after treatment.

Quan sát CIN 2 hoặc CIN 3 với nhiều mẫu tế bào và soi cổ tử cung cũng không thích hợp trừ những trường hợp thai phụ và phụ nữ trẻ.

Quá trình điều trị của CIN tái phát bao gồm thực hiện test DNA phát hiện HPV. Nhiễm HPV kéo dài được chứng minh làm tăng nguy cơ bị ung thư cổ tử cung sau điều trị 2 năm so với những phụ nữ không có tiền căn CIN. Điều này đặc biệt đúng khi bệnh nhân dương tính với HPV 16. Những phụ nữ nhiễm HPV nguy cơ cao được khuyến cáo nên được theo dõi sát sao hơn.
