Uploading Immunization Records

Required Immunizations

The information in this document is for general University Requirements. If you are in a program that requires clinical verifications, such as hospital rotations or internships, please refer to the information in the packet provided to you by your program. You must still complete the general University requirements.

In accordance with New York State Law, all students attending classes must provide: 1. Proof of vaccination against or documented immunity to Measles, Mumps, Rubella (MMR) AND 2. Proof of vaccination against, documented immunity to, or declination of Meningitis.

These requirements are satisfied by uploading and entering information in Medicat. Before proceeding, please verify that the below are all true.

• My documents are signed/stamped by a healthcare provider and include my name and date of birth, as well as the provider’s name and office contact information.

• My documents are in English, or have been officially translated into English. If officially translated, information on the translation company must be included.

• I have the documents either on the same device I am using to access Medicat or printed out in front of me with a way of digitizing them, either by scanning or taking a photo of them. o Note: If you take a photo with an iPhone, turn off “Live” photos as the system cannot process them.

Once you have the documents ready as described above, proceed to the next page (Entering the Dates of Immunizations). Please note that providing immunization information is a two-step process, and

not completing both steps will result in your information not being saved.

You may already have the information on your immunizations. If not, this information may have been provided by you previously to places such as a high school, pediatrician’s office, pharmacy chain, or local health department. If you do not have the records on-hand, reach out to these places first.

If you have any issues locating, accessing, or uploading your immunization records, it is important to contact Student Health Services as soon as possible. We will be happy to assist you in becoming compliant!

Entering the Date(s) of Immunization

1. Once you are logged in to Medicat, select “Immunizations” from the menu on the left. a. Mobile devices will only show the syringe icon.

2. If you received the MMR vaccine a. Click the “Required” tab i. Select “MMR Vaccine” 1. Input the date of your first and second MMR vaccine.

3. If you received MMR Titers a. Click the “Acceptable Alternatives” tab i. Select the Titer you received* ii. Enter the date and result 4. If you received the Meningitis vaccine a. Click the “Required” tab i. Select “Meningitis Vaccine or Declination” 1. Input the date of your most recent Meningitis vaccine.

5. If you will be declining the Meningitis vaccine a. Finish these steps, then submit the Meningitis Declination Form, located on the “Forms” tab. A guide is included after the next page.

6. Once all dates are entered, click “submit”. a. This will bring you to the document upload page, details of which are found on the next page of this document.

*Positive Titers for each of Measles, Mumps, and Rubella are required to be considered compliant. If one is not completed/provided, you will not be considered compliant.

Uploading Immunization Records

1. Follow the instructions on the previous page to enter the date(s) of your immunization(s).

2. Ensure an image of the immunization proof is on your device OR have the immunization record ready (mobile only).

3. On this page is a list of possible documents to upload, with an accompanying “Select File” button.

4. If you are a new student entering Hofstra for the first time, select: Immunization Record (New Students) 5. Click the “Select File” button next to the document you are uploading.

6. Within your device’s storage, locate and select the immunization record file. a. If you are using a mobile device, you will see an option to either select a file or take a picture.

7. The file name will appear in the place of “Select File” 8. Scroll to the bottom of the page and click “Upload” 9. Repeat the process for each additional page & document.

10.If you would like to view previously-uploaded files, scroll to the bottom of the page where you will see ‘Documents already on file”

Submitting a Meningitis Declination Form

*Only complete this form if you are declining the Meningitis vaccine* 1. Once you are logged in to Medicat, select “Forms” from the menu on the left. a. Mobile devices will only show the pencil icon.

2. On this page scroll down to “Meningitis Declination Form” 3. Review the provided information 4. Select “Decline the Immunization” 5. Ensure your first & last name are correct.

6. Click “Submit” 7. Once submitted, you will be considered compliant with the Meningitis requirement.

Viewing your Immunization Status

Immunization Status is applicable only to general University requirements and is not indicative of a status for clinical verifications.

1. Once you have input the date(s) and document(s) of your immunizations you can view your current immunization status.

2. Click on “Immunizations”.

3. You will see the below screen:

4. Your overall status is shown in the middle of the page and is based on the following criteria: a. Compliant, Verified i. You have submitted all required records and they have been verified by our team. b. Compliant, Not Verified i. You have submitted all required records but they have not been reviewed by our team. ii. No action is required. Unless we reach out to you, you are considered compliant. c. Not Compliant i. You have not submitted all required records.

5. On the right-hand side of the page is a listing of the specific requirements and their status. If any of these are not compliant (red) you are not considered compliant.
