Required Immunizations

The information in this document is for general University Requirements. If you are in a program that requires clinical verifications, such as hospital rotations or internships, please refer to the information in the packet provided to you by your program. You must still complete the general University requirements. In accordance with New York State Law, all students attending classes must provide: 1. Proof of vaccination against or documented immunity to Measles, Mumps, Rubella (MMR) AND 2. Proof of vaccination against, documented immunity to, or declination of Meningitis. These requirements are satisfied by uploading and entering information in Medicat. Before proceeding, please verify that the below are all true. • My documents are signed/stamped by a healthcare provider and include my name and date of birth, as well as the provider’s name and office contact information. • My documents are in English, or have been officially translated into English. If officially translated, information on the translation company must be included. • I have the documents either on the same device I am using to access Medicat or printed out in front of me with a way of digitizing them, either by scanning or taking a photo of them. o Note: If you take a photo with an iPhone, turn off “Live” photos as the system cannot process them. Once you have the documents ready as described above, proceed to the next page (Entering the Dates of Immunizations). Please note that providing immunization information is a two-step process, and

not completing both steps will result in your information not being saved.

You may already have the information on your immunizations. If not, this information may have been provided by you previously to places such as a high school, pediatrician’s office, pharmacy chain, or local health department. If you do not have the records on-hand, reach out to these places first. If you have any issues locating, accessing, or uploading your immunization records, it is important to contact Student Health Services as soon as possible.
