Honor Code/Academic Honesty Procedure Flow Chart Instructor suspects a student has violated the Honor Code/Academic Honesty Policy (FPS #11 for Undergraduates and FPS #11G for Graduate Students).
Instructor initiates communication with the student about alleged violation.
If the instructor concludes no violation occurred, incident terminates.
If instructor concludes that a violation did occur, the instructor determines whether or not a penalty should be imposed; informs the student of the penalty; and submits a Violation of Academic Honesty Report form to the Provost and Dean of Students. All violations, regardless of whether a penalty has been imposed, must be reported.
The Dean of Students Office notifies the student that a violation form has been filed and offers the opportunity to meet and discuss the violation.
If the student believes the instructor is in error or that the penalty imposed is unreasonable he/she may initiate an appeal within seven days, by submitting a written appeal to the Provost's Office.
The Provost's Office will review the charges and speak with the student.
If the student chooses not to continue with the appeal, no further action is required.
If the student wants to continue the appeal, the Provost’s Office representative will empanel an ad hoc appeal committee whose membership will include 3 voting members from the Honor Board (a student, a faculty member, and an administrator) and 4 non-voting members (a representative from the Provost’s Office, Student Affairs, the Dean’s Office in the school or college where the violation was said to have occurred, and the Chair of the Department where the violation was said to have occurred).
The Ad Hoc Appeal Committee hears the case following procedures outlined in FPS 11/11G and renders a decision that is final and binding.
