The Fulton County Medical Center offers full-time cardiology services led by Michael T. Donahoe, MD, Jeffrey S. Mandak, M.D. F.A.C.C, and Dustin Pine, PC-C.

When Seeing a Cardiologist Is a Good Idea and Event recording: The patient wears a portable device for a specified period of time, which is then returned to the medical office for evaluation.

There are many instances in which visiting a heart doctor can be a good thing and even save your life. Chest pain, shortness of breath, and dizziness can be signs that a heart attack has happened or is about to happen. Heart arrhythmias can go unnoticed until there are more serious symptoms and even signs of heart failure. An expert in cardiopulmonary treatment can determine what tests or procedures are right for you. Technology is what makes our cardiopulmonary center in McConnellsburg, Pa., stand out. Department testing at our Fulton health clinic includes: • Electrocardiogram (EKG): Using patches and wires, and the machine they’re connected to, doctors can measure the electrical signals produced by the heart. • Echocardiogram (ECHO): Sound waves are used to show how the heart is functioning, and to produce an image that lets a cardiologist see heart valves and other structures. • Cardiac Stress Testing: An

Balance/Cardio - 2 p.m. A good total-body workout. Class begins with stretching and transitions into balance, which includes performing various poses. Participants have the option to use balance foams to challenge themselves more. The Cardio portion of class is done with handheld weights.

Yoga

Yoga celebrates the mindbody connection with a variety of practices that connect the breath with poses to improve strength and flexibility. It is accessible to all individuals, regardless of their current flexibility. There are a variety of practices that range from gentle to aerobic. Yoga can also improve balance and bone health to ensure a good quality of life during aging. Yoga is a living workout in that it offers new challenges as the individual’s fitness improves.

Pilates

Pilates offers similar health benefits as yoga, with an emphasis on strengthening the “powerhouse” muscles of the lower back, hips, and abdomen. Its goal is to improve balance and coordination using a series of controlled, non-jarring movements. Pilates is an excellent option for building core strength for improved back health and improving posture. It can also help reduce the chance of injury with better balance. It focuses on correct form rather the number of repetitions.

Zumba

Zumba classes bring energetic and musical elements to a workout program with lively Latin dance rhythms. The goal is to improve aerobic fitness and strength with a series of active dance moves and exercises. As an aerobic workout, it’s a great way to burn lots of calories in a short period of time while in a fun setting. The sense of community it

provides can increase motivation for sticking with an exercise plan for the long term.

Tai Chi

Like yoga and Pilates, tai chi is a slow, relaxing form of exercise that focuses on reaching a state of mental calm while building strength and coordination through a series of structured poses. It is a non-competitive workout with a variety of different styles. As a form of Chinese martial art, tai chi offers additional mental health benefits for relieving stress and anxiety as a type of moving meditation. It may even decrease the risk of falls in older individuals. FCMC will be offering a FREE Diabetes forum Nov. 13, 2020, at the Celebration Unlimited in Fort Littleton, Pa., from 7:30 a.m. to noon. Anyone newly diagnosed with diabetes is encouraged to come, as well as those interested in new tips and education.

General Diabetes Education

Computed Tomography (CT): CT uses x-rays to take pictures of sections of the body. A CT scan shows the body’s organs more clearly and in greater detail than regular x-rays. Fulton County Medical Center offers a 64-slice CT scanner which provides improved image quality and speed compared to conventional CT scanners. In addition, updated computer software has allowed for a reduction in the total dose of radiation you receive from an exam. CT scans help: • Find abnormalities in the body which may indicate disease • Determine how far a disease has spread • Show the effects of treatment and how your body is responding to it FCMC Diagnostic Imaging offers Low-Dose CT Lung Cancer Screenings for eligible candidates. Eligible candidates are between the ages of 55 and 80, have a history of heavy smoking, and smoke now or have quit within the past 15 years. Heavy smoking means you have a smoking history of 30 pack years or more. For example, a person could have a 30-pack year history by smoking 1 pack per day for 30 years or 2 packs per day for 15 years. Lung cancer screening looks for potentially cancerous spots in the lung called pulmonary nodules. Screening is best performed with a low-dose CT scan, which can detect nodules up to 10 times smaller than those that can be seen by x-ray. Lung Cancer Screenings: Why depend on luck to discover an early lung cancer? Routine mammography and colonoscopy save many lives by discovering breast and colon cancer at an early curable state; why not screen for lung cancer?

DEXA Scan: Testing your bone density -- how strong your bones are -- is the only way to know for sure if you have osteoporosis. One common test doctors use is called dual energy X-ray absorptiometry (DXA or DEXA). DEXA scanning focuses on two main areas -- the hip and the spine. If you

Early Detection is Key

Early detection and diagnosis of a lung nodule, which may be cancerous, provides the best chance for a cure. Lung cancer has been one of the leading causes of cancer-related deaths in the United States, as well as in Fulton County. There is now clear evidence that screening people at high risk for lung cancer with a low-dose CT scan leads to 20% fewer deaths from this disease. At FCMC, we offer a low-dose screening CT scan to identify nodules or other findings suggestive of lung cancer which can then be managed by one of our specialists. What is a Lung Nodule? A pulmonary nodule is any “spot” in the lung that is less than 3 centimeters in diameter. Often these nodules cause no symptoms and therefore go undetected. The majority of nodules are found through testing performed for other reasons, such as x-rays or CT scans for pneumonia or abdominal pain. While most pulmonary nodules are benign and can come from an infection or scar tissue from old infection, some are early signs of lung cancer and require prompt evaluation. monary nodules by a team of skilled specialists, including primary-care physicians, thoracic surgeons, pulmonologists, interventional radiologists and a nurse practitioner. This team works together to: • Thoroughly evaluate each nodule • Develop a comprehensive individualized plan of care utilizing the most advanced technologies • Significantly reduce the time from detection to treatment. The FCMC Lung Cancer Screening Program recommends screening for the following individuals: • 55-79 who have smoked the equivalent of one pack daily for 30 years • 50-79 who have smoked the equivalent of one pack daily for 20 years and have one additional risk factor Additional risk factors are: Radon exposure, asbestos exposure, cancer history, strong family history of lung cancer, significant second-hand smoke exposure, COPD, Pulmonary Fibrosis. Lung cancer screenings are not for patients with active cough, hemoptysis, and shortness of breath. (These patients will require a regular CT scan of the chest.) Screenings are $99 and are NOT covered by insurance. Lung cancer screenings at FCMC require a physician order and a scheduled appointment. To schedule an appointment, please contact us: 717-785-6107 Magnetic Resonance Imaging (MRI): MRI is a non-invasive test that uses magnetic waves to take two- and three-dimensional pictures of the inside of the body. Our GE 1.5 Tesla MRI can take fast and accurate pictures of the body from all angles so the radiologist can look at body parts, organs and tissues from head to toe, allowing for more accurate diagnoses. We also offer a large bore opening which helps accommodate patients with a large or broad frame, as well as those with claustrophobia. Appointments are required. Nuclear medicine: Nuclear medicine is a medical specialty that uses radioactive tracers (radiopharmaceuticals) to assess bodily functions and to diagnose and treat disease. Specially designed cameras allow doctors to track the path of these radioactive tracers. Nuclear medicine services offered at FCMC include, but are not limited to, bone scans, cardiac stress testing, gallbladder scan, gastric emptying study, liver/spleen scan, lung VQ scan, renal scan, and thyroid uptake scan. Digital mammography produces high-quality images of the breast that can show cancer that may be too small for you or your doctor to feel. Digital mammograms also allow for more accurate diagnosis of breast cancer and easier access to your imaging results by your referring physician.

Ultrasound: Ultrasound imaging, also known as sonography, is the measure of echoes throughout the body in the form of sound waves. These high-frequency sound waves produce images of your body’s organs or the blood flow through your veins and arteries. Conventional ultra10

General x-ray: An x-ray (radiograph) is a noninvasive medical test that helps physicians diagnose and treat medical conditions. Imaging with x-rays involves exposing a part of the body to a small dose of ionizing radiation to produce pictures of the inside of the body. X-rays are the oldest and most frequently used form of medical imaging. Fluoroscopy is an imaging technique that uses x-rays to obtain real-time moving images of the internal structures of the body. Imaging of the upper and lower gastrointestinal tract, kidneys and bladder, and joints (arthroscopy) are offered at FCMC. Walk-ins are welcome for general x-ray services. No appointment is necessary. All other procedures require an appointment. Mammography: A mammogram is an x-ray of the breast tissue. Doctors use mammograms to determine changes in breast tissue or detect early stages of breast cancer. FCMC Women’s Imaging Department offers state-of-the-art digital mammography and 3D mammography (tomosynthesis). Both screening and diagnostic mammogram imaging are offered by appointment. We also have a breast health navigator to help guide patients through scheduling and treatment. Spa Days: Available one Saturday a month. Women’s Imaging in McConnellsburg, Pennsylvania Mammography: A mammogram is an x-ray of the breast tissue, which doctors use to determine changes in breast tissue or detect early stages of breast cancer. Treatment

IV infusions are a reliable way of transferring important nutrients and medications into our patients quickly and easily, alleviating pain and other symptoms faster than oral or topical treatments. Fulton County Medical Center offers several types of IV treatments to accommodate the unique needs of each patient. Hydration therapy Hydration therapy can quickly re-introduce vital fluids with nutrients, including electrolytes, minerals or amino acids for dehydrated patients or those who can’t digest food and water independently. Injectable medications FCMC offers injectable medications of many different types for patients. Blood transfusions When patients have lost large amounts of blood, an IV is used to replace the lost blood with a donated substitute. Infusion therapy Infusion therapies administered through IV can carry medications, as well as natural supplements. Our clinic can provide the following: • Antibiotics • Iron products • Magnesium • Arthritis medications • IVIG

Peripherally inserted central catheters (PICC) A peripherally inserted central catheter is a thin, soft, flexible tube for IV administration inserted into a vein on the upper arm. Once inserted and secured, it’s possible to move and use your arm normally until treatment is complete. Insertion of midline catheters A midline catheter is used for veins larger than those used in a normal IV therapy treatment. Midlines are used for short treatment plans where patients are getting IV medications for 1-4 weeks. Phlebotomy from central lines Blood tests are a common method of diagnosing health issues in any part of the body. When patients need blood tests for medical diagnoses, Fulton County Medical Center can draw enough blood via a central IV line to send away for lab testing. Maintenance of vascular access devices Vascular access devices (VADs) are typically the ports or catheters used to allow frequent access to the veins for patients who get regular IV therapy or medication. Our staff can help patients keep these areas clean and clear for safe and continued use throughout their treatment. ■

The laboratory at Fulton County Medical Center (FCMC) is a state-ofthe-art computerized facility providing comprehensive clinical testing based on your physician’s orders. Staffed by certified medical technologists, technicians and phlebotomists, our lab provides microbiology, chemistry, hematology, urinalysis and blood bank services. Our professional staff is equipped to analyze all types of clinical specimens, including swabs, urine, blood, feces, sputum, cerebrospinal fluid and infected tissue. We perform complete blood counts (CBCs), blood films and other specialized tests, as well as toxicology and endocrinology analyses, and urinalyses to check for a wide range of disorders, from urinary tract infections (UTIs) to kidney disease to diabetes.

The History

Dr. MacKinlay saw Fulton County’s need for a hospital to be built in the 1950s, and a similar need arose to take care of the aged population

in the early 1970s. Just before Christmas in 1976, FCMC opened its extended care wing of the hospital. Nineteen residents made FCMC their home permanently. Nine of the 19 residents came from the former Decker Nursing Home. One year later, FCMC opened the intermediate care unit. FCMC could now offer three levels of care: acute care, intermediate care and skilled care. This would eventually make up the 57 beds that FCMC maintained until 2007 when they moved to their new facility and added an additional 10 beds for individuals needing longterm care. Today, FCMC Long Term Care is very much a vital part of the organization. Health care has dramatically changed since 1970 when the LTC unit was built; however, the quality of care, coupled with the family-like atmosphere, has remained unchanged. In 2014, the FCMC nursing home received a four-star rating from Medicare. gov and again in 2020.

The speech-language pathologists (also called speech therapists) at FCMC provide therapy for children and adults whose ability to communicate or swallow might have been affected by injury or illness. We pride ourselves in providing the most current treatment for individuals with communication impairments, cognitive-communication, swallowing or voice limitations. Our skilled speech-language pathologists provide the best care for your needs while remaining aware of the sensitivity that some of us feel regarding these issues. Modified Barium Swallow Study

MBS studies are available as on outpatient speech therapy participant when ordered by your physician. The purpose of the study is to identify the cause of difficulty or impairments with swallowing. Your study at FCMC will be completed in the radiology department, with the radiologist and speech and language pathologist present. Once the assessment is complete, the therapist will use that information in developing your continued treatment plan.

Neuromuscular Electrical Stimulation (Vital Stim)

This treatment intervention is utilized for patients who have trouble swallowing (dysphagia). It uses small electrical currents to stimulate the muscles responsible for swallowing, while speech-language pathologists help patients re-educate their muscles through rehabilitation therapy. Swing Bed Program REHABILITATION SERVICES

When needed, our team of physical, occupational, and speech therapists will complete a comprehensive evaluation and create an individualized treatment plan for you based on your needs which could include: strength and balance training, mobility, ADL’s (dressing, bathing, kitchen activities), swallowing difficulties and/or memory deficits. What is Swing Bed? Swing bed is a team used to describe a federal program that allows FCMC, as well as other rural hospitals the ability to keep you in the hospital longer when your doctor feels you are not ready for discharge. This allows you to receive needed therapy and/or nursing care to help you prepare for home. Who is eligible for Swing Bed? Three areas you need to consider for eligibility: • In most cases you must have a three day hospital stay before consideration for the swing bed program (stay can be at FCMC or another hospital) • Pre-approval by your insurance company • You must have a qualifying illness/surgery Common Reasons for Swing Bed • Postsurgical recovery • Fracture Recovery • Neurologic Progression needing therapy • Comprehensive Medication Management • IV therapy/Central Line/PICC • New feeding issues (PEG tube placement or TPN) • Pain management • Diabetic Management

tive Sleep Apnea Syndrome, or OSA. Sleep apnea is a condition where a patient will stop breathing repeatedly during periods of sleep, resulting in fragmented unrefreshing sleep. Sleep apnea is usually associated with loud snoring or snorting. Jerking of arms and legs may be noticed by a bed partner and the patient may awaken short of breath or with heart pounding. Patients with sleep apnea are often unaware of their problem and have frequent arousals/awakenings caused by the pauses in breathing. Untreated sleep apnea usually results in excessive daytime sleepiness and may contribute to high blood pressure, cardiac problems, depression and sexual dysfunction. In addition to obstructive sleep apnea, sleep-disordered breathing may also be related to central or mixed apneas upper respiratory resistance syndrome or asthma-related sleep breathing problems. Surgical Services The Surgical Services Team at FCMC is committed to providing a safe and pleasant experience for all of our surgical patients. Our Operating and Procedure Rooms are both equipped with state-of-the-art technology, including High Definition Video Laparoscopy and Endoscopy Units, LED overhead lighting, Intra-operative Radiography (x-ray) and patient warming devices. Ophthalmology Dr. Stephen Facchina began doing cataract removal surgery here at FCMC in 2006. He operates on residents from Fulton County and the surrounding area and has an office based in Greencastle. Dr. Facchina also performs laser eye surgery for the treatment of glaucoma at FCMC. Cardiovascular Dr. Jeffrey Mandak has been serving patients in Fulton County since 1997. He is here Monday-Friday (except Thursdays) for consults and appointments. He also performs Transesophageal Echocardiograms and Cardioversions with the assistance of the Surgical Staff.

Podiatry Dr. Todd Harrison has been meeting podiatry needs of patients in Fulton County since 1993. He is part of the podiatry group, Foot and Ankle Specialists of the Mid-Atlantic, along with Dr. Melody Stouder (top left), Dr. Gregory Smith (bottom left) and Dr. Adam Grahn (bottom right). Podiatry Surgery is currently being performed at FCMC on rotating days of the month and appointments with surgeons are available at the Specialty Services Department. Procedures include (but are not limited to) hammertoe repair, bunion repair, foot and ankle reconstructive surgery, fracture repair, correction of Charcot’s Deformity, amputation and excision of lesions.

General Surgery Fulton County Medical Center Chief of Surgery, Dr. Karl Riggle (of Tristate Surgeons) has been serving patients in Fulton County for over 25 years. Dr. Riggle is a Board Certified General Surgeon and sees patients for general surgery and vascular needs by appointment every Tuesday in the Specialty Services Department. He also performs general surgery procedures and provides consultation on Emergency Room and Acute Care patients. Dr. Nicole DeBolt joined FCMC in February 2019. She is a Board Certified General Surgeon who is at FCMC seeing patients and performing general surgery procedures Monday through Friday. She also provides consultation services to hospital and emergency room patients as needed. Dr. DeBolt is available by appointment through Fulton Surgical Services at 717-485-7333. General Surgeries performed at FCMC include (but not limited to), gallbladder removal, appendix removal, colonoscopy, hernia repair, gastric endoscopy, excision of lesions and lipomas, hemorrhoid banding or excision, access port placement and biopsies. Orthopedics Dr. Matthew Espenshade, Espenshade Orthopedic Surgeon of Arlington Orthopedics out of Harrisburg, has enjoyed seeing patients for orthopedic needs at FCMC since 2012. He is available for appointments every Wednesday and Thursday in the Specialty Services Department. Some of the surgeries Dr. Espenshade performs at FCMC include shoulder and knee arthroscopies, rotator cuff repairs, carpal tunnel repairs, trigger finger repairs, procedures to help with arthritis and fracture repairs. Plastic Surgery Dr. Mark Boland, Boland plastic surgeon, began seeing patients at FCMC in 1987. He addresses multiple skin problems with a focus on cancerous and pre-cancerous lesions. Dr. Boland is available by appointment in the Specialty Services Department on Wednesday, Thursday and Friday twice a month. He performs plastic surgery cases at FCMC as needed. Thoracic Surgery Dr. Troy Moritz, Moritz DO has been seeing patients at FCMC since 2014. He is a thoracic surgeon who specializes in chest surgery. He performs some related diagnostic procedures at FCMC surgical services. He is here seeing patients every fourth Friday. Urology Dr. Jose Prudencio of Urology of Central PA (UCPA) has been seeing patients at FCMC since 2016. Dr. Prudencio sees patients at FCMC two Fridays a month and has clinical interests in prostate cancer, incontinence and kidney stones. He performs surgery at FCMC, including (not limited to) vasectomies, ureteral stenting, cystoscopy, hydrocelectomy, transrectal prostate biopsy with ultrasound and gold marker placement for radiation therapy.

Anesthesia Services Toby McCarty, McCarty Certified Registered Nurse Anesthetist, has been providing anesthesia services to patients at FCMC since 2010. His services include providing surgical care before, during and after surgical procedures, as well as steroid epidural injections and nerve blocks for pain control.

Wound Care Services

Our wound center at Fulton County Medical Center (FCMC) offers a variety of specialized services to meet our patients’ wound care needs. We work closely with Team Home Health and can provide nutritional interventions to treat the whole individual. For more serious wounds, Hyperbaric oxygen therapy (HBOT) is a medical treatment approved by the Undersea & Hyperbaric Medicine Society and involves breathing 100% oxygen while enclosed in a Hyperbaric oxygen chamber. The relaxing treatment can be completed while patients listen to music or watch movies and can be effective for a range of medical conditions and wounds. Chronic refractory osteomyelitis Chronic refractory osteomyelitis is an inflammation of bone tissue caused by an infection in the bloodstream or near the affected bone(s). Diabetic ulcers About 15 percent of diabetic patients develop ulcers, an open sore or wound on the feet that can become seriously infected without treatment. Enhancement of healing in selected problem wounds Hyperbaric oxygen therapy can speed up the healing time in problem wounds that might not heal on their own and would take a significant amount of time. Carbon monoxide poisoning Carbon monoxide poisoning occurs when the oxygen in your red blood cells is replaced by carbon monoxide in environments where there is not enough natural oxygen to breathe. Acute traumatic ischemia Acute traumatic ischemia is a technical term for traumatic injuries like a crush injury, compartment syndrome and others. 30

Crush injuries Crush injuries can happen at home, at work, in car accidents, and any time part of your body is crushed between objects or moving parts. Arterial wounds When soft tissue in the body doesn’t receive adequate blood flow, arteries are at risk for developing ulcers or becoming damaged. These injuries are very slow to heal without additional support from treatments like HBOT. Delayed radiation injury Even when radiation is effective at eliminating cancer cells, the side effects can create complications and injuries of their own to treat separately. Soft tissue radionecrosis Radiation that causes soft tissue death from damaged blood vessels is called soft tissue radionecrosis (STRN). Bony necrosis (osteoradionecrosis) Radiation that causes bone death from damaged blood vessels is called bony necrosis.
