Anestadent 4% w/v and 1:000,000 solution for injection (Articaine hydrochloride/ Adrenaline (Epinephrine) tartrate). Composition: Articaine hydrochloride 40.00 mg/ mL; Adrenaline tartrate 0.01 mg/mL, Sodium metabisulphite (E223) 0.5 mg/mL. Therapeutic Indications: Local or local-regional anaesthesia in dentistry and dental surgical procedures in patients. Not to be used in children aged below 4 years. Avoid use during pregnancy, unless strictly necessary. Seek advice if breastfeeding. Dosage and Administration: Adult – Use the smallest volume of solution: 1 to 3 cartridges per session, depending on indication, health, age and weight. Do not exceed 7 mg per kilogram of body weight. Children >4 yrs - The mean dose in mg should be calculated as follows: Child’s weight (in kilograms) x 1.33. The maximum dose is 5 mg per kilogram of body weight. Elderly - Reduce to one-half the dose used for adults. Local or regional intraoral submucosal injection. The injection rate must not exceed 1 mL of solution per minute. This product is essentially ‘sodium- free’. Contra-indications, special warnings and precautions for use: Hypersensitivity to other local anaesthetics (amide) or any component of this product. Risk of local necrosis in hypertensive or diabetic subjects. Risk of self-biting under anaesthesia such as lips, cheeks, mucosa, and tongue. Caution and monitoring should be increased in cases of severe alterations of renal and hepatic
function, cardiovascular disease, abnormalities of cardiac conduction, blood coagulation disturbances, patients with cholinesterase deficiency, epilepsy; hyperthyroidism; diabetes mellitus. In the case of serious liver failure, it may be necessary to reduce the dose. Ability to drive and use machines: The dentist will evaluate the patients’ ability to drive and use machines after surgery. Interactions: Caution for use with halogenated volatile anaesthetics, antidepressants, Non-selective MAOIs. Use only with strict medical supervision with Selective MAOIs, Non-selective beta-blockers, Guanethidine and related medicinal products (anti glaucoma agents). If the combination cannot be avoided, use smaller doses of sympathomimetics (adrenaline) with precaution. Hypersensitivity, over-dosage or an incorrect injection may result in lesions to nerves, hyposensitivity and alterations of taste, excitatory or depressant manifestations of the CNS or cardio-vascular system, respiratory and allergic reactions or in patients with particular conditions (see SMPC). Sodium metabisulphite can cause severe allergic reaction and bronchospasm. If this medicine is accidently injected into blood vessels it can cause lack of blood flow and then tissue necrosis.
