So says Dr Pieter van Heerden, Specialist in Orthodontics and Fellow of the World Federation of Orthodontists. Here he tells us why.

Marketeers are prone to calling all sorts of products a ‘game changer’ as this is what they’re always hoping they’ll turn out to be.

However, very few of those products ever manage to live up to the claim; the Carriere® Motion 3D™ Appliance is one of the few.

I have been an orthodontic practitioner for about 35 years, yet I would say that introducing the Carriere® Motion 3DTM system to my treatment modalities is one of the most dramatic changes I have ever made in my practice. It has made a major positive difference, and these are the reasons why.

The same general orthodontic principles in a new combination

In my opinion, the Carriere® Philosophy, on which the system is based, still works to the same general orthodontic principles that we all know and understand. It’s the combination of those principles that makes it very efficient and time-saving.

The Carriere® Philosophy uses fast, natural and gentle forces to correct Class II and Class III malocclusions at the start of treatment with the Motion 3DTM appliances, and I now use these extensively in my orthodontic practice.

The Motion 3DTM appliance rotates and uprights the maxillary first molars while distalising the posterior segment, which corrects the posterior occlusion. Mimicking a human hip ball-socket joint, the Motion 3DTM Appliance allows the first molar to rotate and upright to its natural finishing position, producing a continuous, natural, and uniform force for distal molar movement with controlled rotation and tipping correction. This approach ensures that you address the patient’s entire facial harmony.

It is a discreet appliance that is fitted from canine or premolar to molars and moves each posterior segment as a unit. Once the appliance is bonded, Carriere® Oral Elastics are attached from the mandibular molar to the hook of the maxillary cuspid of the Motion 3DTM Class II Appliance to activate the corrective forces.

Great for my practice

A great advantage of the Carriere® 3DTM Motion Appliance is that it accomplishes a lot of different corrections at the same time, including tipping the teeth, changing the occlusal plane, changing the muscle function and sorting out vertical misalignments.

Since there are no competing forces in the mouth from brackets or other appliances in this first stage, average Class II correction can be completed in about four months, which is quicker than other methods I have used. The time-saving element is a big advantage for my practice and means I can either fit in more treatments or use the time for other work.

It is also extremely easy to use, so much so that once I have planned the treatment, I can delegate the setting of the Motion 3DTM appliance to a colleague. A lot of thought has gone into the design which makes it straightforward to fit.

Patients love it

Of course, my patients love the time-saving aspect too as it’s natural to want to wear any orthodontic appliance for as short a time as possible. The feedback I receive, especially from those who have been treated with other modalities before, is that the Carriere Motion 3DTM appliances are much more comfortable to wear. This is due to the low, gentle force it applies and the smooth, low-profile design.

My patients also like how discreet the appliances are and that once they are in a Class I framework, often clear aligners can be used for the second part of the treatment.

Last but not least, it means we can reduce the likelihood of tooth extractions or other more invasive measures, which other treatment modalities sometimes require.

For all of these reasons, I now regularly use the Carriere® Motion 3DTM system, which is exclusively available through Henry Schein Dental, in my orthodontic practice. I have spoken many times to its inventor, Dr Luis Carrière, and attended some of his training courses. I can honestly say that Dr Carrière and his innovative system have changed the way I practise.

In certain cases, a virtual consult can serve the full purpose of an in practice visit once a patient has had their first treatment in practice with a dentist. Appropriate records should be kept of all patient contacts, including care management and onward referrals. This will be essential for contract management calculations for 2020/21.

If you have an intraoral scanner or camera, start taking scans of ALL your patients each time you see them. This way you can build up a bank of images of their teeth which will be of great help when you are practising teledentistry. You can also add xrays, and extraoral photographs to their notes.
