STERILISATION TESTING SOLUTIONS The most common autoclave type used in Dental is a B class autoclave, these machines are quite complex and require frequent monitoring and testing to ensure they are performing correctly.

These tests must be carried out prior to the first cycle to ensure reprocessed instruments are sterilised.

Leak rate test this is a mechanical test built into the machine and assesses the seals and vacuum of the steriliser.

Air removal and steam penetration test Depending on your load and instruments being sterilised whether they are porous such as gauze/cotton rolls or for hollow/solid instruments such as triplex tips/handpieces/instruments will determine the type of test you need to complete.

Porous loads The Bowie & Dick type tests ensures that air is properly removed from the chamber and, as a result, that porous loads are fully penetrated with steam (e.g. surgical gowns, gauze, wraps).

Non-porous loads a process challenge device (aka Helix test) is to be used for solid and hollow instruments such as handpieces, triplex tips and solid instruments. This test is to be performed daily using a B class cycle.

WIPE SOLUTIONS Understanding your wipe down protocols Since Covid-19 there has been more talk about the additional precautions we need to adhere to, in particular around environmental cleaning for treating moderate to high risk Covid-19 patients.

Here is a brief explanation of cleaning VS disinfecting and how to apply this thinking to standard and transmission-based precautions.

Cleaning is the removal of soil and contaminants from surfaces, whereas disinfection relates to the inactivation of pathogens by use of a disinfectant.

Standard Precautions Standard Precautions are the primary strategy to reduce the transmission of infectious agents with all patients, at every interaction to stop infectious agents spreading via contact, droplet or air. This includes environmental cleaning using a neutral ph detergent. Neutral pH detergents are best for environmental cleaning because they are less likely than acid or alkaline detergents to damage metals such a stainless steel or to cause skin irritation. Please refer to the ADA Guidelines of Infection Control for more information.

Transmission Based Precautions Transmission based precautions are implemented where standard precautions alone may be insufficient to prevent further infection. Transmission based precautions are applied in addition to standard precautions to target infective agents based on their mode of transmission. This includes a further step in the environmental cleaning regime where a disinfectant step is required after the initial cleaning step. A neutral ph detergent followed by a disinfectant is sufficient or a 2 in 1 cleaner and disinfect agent can be used but you must follow the two step process of an initial wipe to clean followed by a second wipe to disinfect.

Transmission based precautions should be applied (risk-based) to patients suspected or confirmed to be infected with agents transmitted by the contact, droplet or airborne routes. The agents of most concern to dental practise are respiratory viruses. Non-urgent treatment should be delayed or postponed. All patients prior to appointment should be screened to establish. If patients require urgent care, transmission-based precautions must be followed. Please refer to the ADA Guidelines of Infection Control for more information.

It’s important to note there are several types of disinfectants available with different intended purposes. Just because a product is listed as a Hospital Grade disinfectant does not necessarily mean you can use on your dental chair. It’s classification could be for hard surfaces only and intended use for bench tops but not medical devices such as dental chairs.

Please always check with your Equipment manufacturer on their approved disinfectants list and intended purpose for use and refer to the ADA Guidelines for Infection Control for a more detailed overview of the above.

The issue of waterline management has come to prominence with dental clinics closing down and then reopening because of various restrictions to practice from the COVID-19 pandemic.

Stagnation of water inside the tubing of a dental chair which has been shut down is a major problem because waterborne organisms such as Legionella overgrow readily whenever water sources become stagnant. In addition to Legionella, many other bacteria and protozoa that may be found in reticulated water will also overgrow, forming a thick biofilm which can completely occlude the lumen of the tubing, and prevent the subsequent flow of water.

MASK SOLUTIONS Which Mask & When to Use? Level 1 Not widely used in dental due to the creation of aerosols and risks with blood/body fluid exposure.

Only typically recommended for zero risk procedures e.g. inserting mouthguard or removable prosthetics.

Level 2 The most common option for use in dental settings. Level 2 masks provide fluid resistance and block particle sizes we commonly encounter during routine treatment. Level 2 masks can be 95% effective at blocking for example influenza particles, they must be fitted correctly though.

Level 3 These masks provide a higher fluid resistance and are meant for use in procedures where the risk to blood/body fluid is increased. Procedures could be surgical procedures, use of ultrasonic scaler for prolonged period or major trauma.

Respirators P2/N95/D95 – needed when treating patients that require airborne precautions. These require to be fit checked and tested in order to be effective. There are also other additional requirements required for airborne precautions.
